Retired RN. 48 years in the profession. Majority of my career in the operating room. Ortho, Neuro, General, Plastics and so forth. Finally, a Dr on the “net” speaking the truth about our aging faces. Do your homework and find a good Plastic Surgeon in your area. All the lotions and potions and those crazy heat devices will not solve the eventual loss of elasticity, bone loss, fat loss and sagging of your face. I do wish I could have had the opportunity to work with Dr Kopelman. From watching some of his videos I would say he would have been excellent to work with and an extremely good Teacher. I learned so much in those 48 years in nursing. Every day was a new learning opportunity. But I don’t miss being On Call 😉
Love your honesty and knowledge. There are very few Dermatologists channels on you tube not selling theIs services ,devices or their own product line! You sir are a gem!❤
Thank you so much for your honesty. I have an appointment for Sculptra to help with jowls and wrinkles on side of mouth. I think I should be canceling that appointment.
I appreciate your honesty and your perspective. I have heard numerous dermatologists say that the biostimulator, Sculptra, can lift the face (obviously not like surgery) by stimulating collagen production and increasing volume in the mid-face.
❤Educational for all! Marketing is of dishonesty, money makers and criminal, Knowledge is power to prevent all that! Great tutorial! Thank you for sharing your professional knowledge talent ! 🌹🌹🌹🌹🌹
@drkopelman Yes, I am. I also enjoyed hearing your opinion on exosomes and I'm 100% behind you in all of your statements there too. The reason I'm agreeing with you so much is because I've tried most of the esthetic treatments and did my own research therefore I'm speaking from experience. I appreciate you spitting the truth Doc! ❤️
Dr Kopelman your channel is consistently an engaging plastic surgery TED talk! Thanks for discussing these topics in the context of evidence-based medicine. I've just had a DPFL & NL after years of HA fillers - best decision .....HA fillers over 45: a bit like "swallowing the spider to catch the fly"....
How does Dr. Kopelman feel about the Mini Face LIft, not an actual face lift where they lift the skin over the face? My Doc recommended a mini face lift j
@@louisedery1049 "Minilifts" sound good but if that means the surgeon is going to trim a little skin and not address the deeper structures it will be doubtful how long the benefit will last
Thanks for saying it like it is! There’s a dermatologist in Newport Beach, extremely expensive, with her u tube channel touting her lasers, etc. saying this is how she looks so young. I’ve watched her old videos and am positive she’s had a neck lift. People are throwing her thousands so they can look as good as her and it’s never going to happen. People do your research and only watch reputable channels like this one for information.
Dr Kopelman, what do you think about cryolipolysis? It seems early for the facelift in the early 40x, but those jowls and double chin are killing me 😅 (and I am not fat or something)
Cryolipolysis can be effective for reducing fat under the chin, but I’m not a big fan of it for addressing jowls. Jowls are often more about skin laxity and sagging rather than fat, so treatments like skin tightening (radiofrequency) or even subtle fillers for contouring tend to work better.
Sculptra works excellent! In temples, mid cheek. It fills in depressions in soft tissue. It stimulates collagen production. It works best to blanket the entire face evenly
HIFU, or High-Intensity Focused Ultrasound, is a non-invasive treatment that I don't often recommend for skin tightening. It works by delivering focused ultrasound energy deep into the skin, which might feel like tiny shocks. This energy heats the deeper layers, which stimulates your skin to produce more collagen-an important protein that keeps your skin firm and tight. While it’s different from other devices that simply use heat, HIFU specifically targets areas under the skin to naturally encourage tightening over time.
As a facial plastic surgeon, I can tell you that while red light therapy can help improve skin texture and stimulate collagen, it's not effective for lifting saggy jowls in a noticeable way. For more pronounced sagging, the only reliable and lasting solution is a facelift. This procedure directly addresses loose skin and underlying tissues, providing a significant and long-term improvement that non-invasive treatments like red light therapy simply can't achieve. If you're looking for a more dramatic result, a facelift would be the best approach.
I wish that was the magic solution. If you want easy things to start doing my biggest recommendation is wearing sunscreen daily and staying out of the sun ☀️
There is a chance that the Botox will droop the corner of your mouth so i would not recommend Botox in that area and it would have to be repeated every few months
A facelift can help solve many but not all the signs of aging of our face. Still important to explore fat injections and laser treatments to address volume loss and changes we all experience of our skin.
I don't understand the use of Dr. Amir Karam, who thinks like you, in fact he is against techniques, such as suspension threads, against radiofrequency which causes deep damage, he is a supporter of lymphing, which he invented, of restoration vertical, which unlike normal facelifts, addresses the problem in the deeper layers of the dermis, to simultaneously obtain a repositioning of the lost volumes caused by tissue laxity, turning the clock back in time, without ever alter the person, distorting their features, but ensuring that they can regain the face they had years before. It is obvious that facing a facelift at 70 years old will not bring the same results as a 50 year old, but for some they feel satisfied in regaining volume on a tired face, a more toned neck, no more bags above and below the eyes, and even if they don't return at 30... that's okay. I really admire Dr.'s work. Amir Karam, just the distance in km makes me worry, but from the many people I've seen, he's the only one I'd let touch. Without taking anything away from his long career.
I'm 75 and have been staying out of the sun since I had sun poisoning that kept me bedridden for 4 days when I was 21. I think it paid off. People think my husband and I are about the same age. I am 10 years older.
I agree sun exposure is the BIGGEST skin ager. I'm 57 and can easily pass for early 40s and I've used sun screen my whole life! EXCEPT on my hands and the difference between my face and hands is very dramatic!! 😮😢 I do a lot of driving and now have brown spots etc on my hands. I'm thinking of getting Intense Pulse Light treatments. I've seen the before and after pictures on line and they are pretty amazing! 😊 I never wear shorts and the skin on my legs is very youthful. Smoking is bad for the skin (and everything else!!!). I agree with this doctor about too much alcohol and lack of sleep have a bad effect. I have see great results with micro needling. But I would worry about infection.
Ten years ago had laser and lost facial fat. Since then, when bring it up during consultations in PS or Derm office (and state I don’t want laser treatment) staff will try tell me lasers don’t destroyed facial fat. Unbelievable. Ty Dr. Kopelman for being a doctor with high standards and being well informed regarding lasers. They may work well on younger patients, but for the 50+, I’d stay far away.
I completely understand your concern, and you're not alone in experiencing facial fat loss after laser treatments. While lasers can be effective for many patients, especially younger ones, they do carry the risk of fat loss, particularly in more mature skin.
I have had a facelift two years ago. I did not have many wrinkles, I only did it for my jowls. For about 5 weeks I was happy with the results, but after that the jowls came back along with the marionette lines. For all that money! I was very disappointed
You were only happy while the swelling lasted. Swelling is very misleading, indeed. I saw the return of my submental laxity six weeks later. Now, nine months later, I see the jowls trying to come back.
Did you have a Deep Plane facelift or the regular facelift? My understanding the Deep Plane facelift with a necklift is the optimum procedure for the jowls and face to last 10-15 years. No drinking, smoking and limit the sun.
Ive come to learn ALL smokers have underlying depression, anxiety, PTSD. There is not a smoker in the world that does not suffer from some form of depression or mental/emotional disability. Not everyone realizes this, and I wish there was a solution to the root of this ugly habit. I've been able to quit alcohol, drugs, but for some reason I increased my habit of smoking! Sometimes I wonder if its possible to eliminate all negative habits if you suffer from depression, anxiety, or or other mental health problems.
Where did you get your data on every smoker in the world? Some of them are just idiots who started smoking because there friends did it . Either way, it's not an excuse.
filler migrates making you look awful and older.......cog threads...laennec....exosomes and micro needling is fantastic if you want to see a real difference without having expensive surgery which is only for rich women

Dear Dr. Kopelman, thanks for telling the truth: nothing beats plastic surgery.
I am glad I can give you the real rundown on how to best approach jowls

Sculptra cannot lift sagging jowls only surgery can do that.
Thank you Dr. for your honest advice, there is so much misleading information on the Internet...all very confusing.
Would you consider Endolift as a facelift procedure for treating jowls? Also, would Sculptra improve the jawline?
Really enjoy your videos. Could you offer some recommendations for crepey underarm skin after menopause?

Thank you for being so honest. Does red light therapy also cause facial fat loss?
I wouldn’t be too concerned about red light therapy and facial atrophy

I’ve watched her! I agree, and too much laser work..
Thanks for your honesty.
Appreciate your input!! Thanks!❤
Any time!
Thank you. Helpful.
What about a neck lift? When I pull the skin on my neck under the jaw and tighten it it gets rid of the sagging jowl?
This was so informative,Dr. K. There are SO many UA-camrs presenting options and now we know are a waste of time and $
Hi thank you for this video, question , can Microcurrent build up muscle tissues after losing weight?
Microcurrent they say it won't atrophy your fat cells, of course no heat but it is a temporary effect is this true? Interesting video thank you!
Microcurrent purely temporary effects
❤ thank you!
Thank you for telling the truth, so many lie to take your money
I’d love to know about these at home red light therapy devices what you think 🤔

Did your hair turn white overnight? I swear only a few months ago it was brown!
I had a Hair Transplant during this period of time. My hair was white because I could not dye it.
@@drkopelman Oh, that explains it!
can you let me know about Hifu ? for tightening its not heating device but , giving stimulation by little skocks giving to skin which is control

No, I think it is pretty useless. I did it for 2 years, no results. Just another lie.

thank you
So is plastic surgery the only way to treat jowls?
Yes
yes
Thank you Dr
Where are you?
NYC and NJ 😀I do virtual appointments also
