Thank you for this important video! I am almost 74. I was post menopause by age 42. I was on HRT for probably 15 years. I decided I had been on it long enough as I was reading all the misinformation and health scares at the time and was worried about my overall health. After I went off of it is when I ended up with osteoporosis, Afib, a debilitating rotator cuff injury, arthritis and fainting spells. And of course vaginal issues. I really wanted to go back on HRT but my GYN told me that because I had gone off of it, because of my age I couldn't go back on it. I am very interested in starting on topical estrogen for my face and hands and have high hopes that my GYN will OK a prescription for me. I just thought someone would be interested in my HRT journey. It was a blessing and kept me young and I shouldn't have gone off of it.
I’m 76 and was recently re-started on HRT by a functional medicine GYN. I was off of it for 4-5 years after losing access to a practitioner, then the pandemic. I’m now on compounded bio identical estrogen, oral progesterone and several nutritional supports (for things like low B-12). This type of HRT has been recognized as safe and beneficial to protect cardiac function, muscle mass and bone integrity, as well as restore better cognitive function, sleep and more. I had to find, and pay, this person privately and will be looking in the future for someone who accepts insurance to continue this protocol. I was on HRT from 45-52 and got off due to my sister getting breast cancer and her telling me I shouldn't be on it! I felt like I was aging too fast, arthritis, joint pain, depression, weight gain, just overall felt bad. So I just started again at 62. The research on breast cancer was flawed, although I think there was a slight risk with oral estrogen (I use bioidentical cream now and a progestrerone pill). To me it's better for my overall health, and I want to try and avoid the dementia my mom has. Your brain needs estrogen or it shrinks! I go to Dr. Dian Ginsburg - she is a virtual Ob/gyn who only does menopause medicine now. You should check her out. Luckily there are many ob/gyn's switching to this practice so we can get better information. Many I follow say you should be on estrogen for life and I believe them! I will continue to have my yearly mamogram and check for breast changes of course. Dr. Ginsberg said put a little of the cream under your eyes and on your crows feet, forehead wrinkles and hairline. Then half is vaginal and half is on alternating thighs.

I"m 65 and use topical estrogen. I've been using tretinoin since I have been 42 and had a deep C02 done when I was 59, and that really helped a lot. When I turned 64, I didn't recognize the person in the mirror anymore. Topical estrogen has helped. I would recommend it. Thanks for address this. Very few dermatologists address this problem. We all get there some day. My mom passed away at 93 and she was on hormone therapy and looked very good in her senior years.
How do you use it? because I have tried mixing my Retin A & Estradiol cream for my face & neck to firm and where I may see some moisturization I have not seen any firming! How do you apply it?
Just wanted to add that post menopause you can actually have low testosterone, too! As women we actually need testosterone (just much less than men). My mom suffered with debilitating anxiety and irritability for 2 years (which we all certainly noticed, just didn’t know the cause) before she went to a hormone specialist who told her she had low testosterone and she began supplementing. Within weeks she noticed a huge difference in her emotional state
Interesting. I would think that lack of progesterone or maybe estrogen would make someone irritable but I associate testosterone more with motivation, but who knows! Glad she is feeling better. It's incredible how much our hormones affect our moods and "personalities".

I’m a 45-year old, medium fair-skinned Asian woman. For the last three months, I’ve used Musely’s Aging Repair Cream for my face in the morning ( 0.3% estriol and 1% vitamin C) and the Estrogen Boost in the evening (0.4% estriol, 0.1% estradiol, 20% progesterone; note the progesterone) and had fantastic outcomes so far. Overall, my face and body looked de-aged 10 years. The most dramatic change I’ve ever seen tbh, and I already had fairly good skin (like most of us here, I have a regular routine of 1% retinol, vitamin C, sunscreen, BHA/AHA exfoliant). The loss of elasticity, plumpness, and “bounce” of my skin was something I wanted to keep (or at least have high standards regarding) and was looking for something to address that. IMHO, it’s not wrinkles per se that age us but skin texture and tone changes (AKA crepey-ness and age spots). Because the creams are a hormonal treatment and I’m already in my mid-forties heading into perimenopause, I had the extra benefits of having more energy and sex drive than I had since my early 30s. Wowowow muscle tone improvement overall. The brain fog was real and much improved too. I sleep so much better. All cherries on top! I definitely say: give topical estrogen a shot. The dosing is low and I experienced very mild side effects (initial site irritation and mild breast tenderness that eventually dissipated; so far, no pigmentation issues either). You’re only applying the hormonal cream to your face? and yet you’re seeing improvements in your body muscle tone, a greater sex drive, sleeping better and less brain fog(?). This means ‘it is systemic’ and not just local. Have you discussed all the other benefits with your prescriber? And if so, what did they say?
@@Tica5012it’s the estrogen boost cream with the estrodial and progesterone that’s giving her all those additional benefits. She likely uses that one on the arm or belly (maybe her face) and it does have systemic effects because of the higher blend of hormones. The aging repair cream that she uses on her face in the morning (I use this one as well) is really localized because it’s a much lower amount of estriol and doesn’t have the other hormones. Just my take on her comment being a long time musely customer.
Be aware if you have pets. We had recently gotten a puppy when I started using my topical estrogen (bioidentical) cream on my inner thigh at night. The puppy did not sleep with us but would come in and cuddle in the morning. Several months later, our vet noticed the puppy had an enlarged vulva and larger than normal teats. She the asked me if I was taking topical estrogen. Apparently, she knew the topical estrogen can transfer to pets. I immediately replaced the topical estrogen to troches and the dog returned to normal. It's amazing to me how powerful the estrogen transfer must be after a night of sleep and the dog having no direct contact with my thighs. There is plenty of research on this if you check, but I had no idea prior to my experience.
This is so helpful. I don't usually pick up my dog until after my estradiol and testosterone dries but I will be more careful. I think I may have to switch to a patch. Tried the troche, made me nauseated and bloated as I was probably swallowing some of the estradiol and testosterone.
I’m in HRT for last 2 years. I’m 47! It has totally been a game changer for me! I had debilitating side effects of estrogen loss & my patch & progesterone took them away! I feel almost normal now.
@@Kne2450 weight gain of 20 lbs in a couple months, when before I’d always maintained my weight. Depression, not sleeping or waking at 3 am and not able to go back to sleep, migraines that lasted a week, itchy dry skin, feelings of bugs crawling all over my skin, crying all the time for anything, feeling terrible with flu like symptoms most of the month. No hot flashes but I had cold flashes so bad that my skin would itch and I would scratch and make myself bleed over nothing, joint pain, no energy, no desire to workout (which I’ve always done) I was a completely different person and hated myself and even was contemplating suicide. My Dr put me on an anti depressant which didn’t help my menopause symptoms. I started watching videos on UA-cam: Dr Mary Claire Haver, others, and educated myself on what was essentially happening to my body. It was peri menopause and I’d started having symptoms around 42. My periods which had been very light before that were non existent now, and I was having awful Vaginal dryness that it actually hurt to walk. Everyone’s side effects can be different so I had to just explain everything I was going through to an Endocrinologist who knew based on my age 44 that I was going through estrogen withdrawal basically. I got the hormone replacement I needed and they help a lot. I feel 80% like myself.

I started topical estrogen about 2 months ago and have felt like im back to being more like myself! Its amazing and i had no idea how much better I'd feel! Im 67 and this stuff is wonderful!!!
My mom have had a total hysterectomy (severe endometriosis) when she was 28 (now 56). She was prescribed since 28 a estrogen gel to apply on her lower arms. With this she avoided all the menopose symptoms and she is still on it. Her skin is indeed very good looking too. Barely have wrinkles and she never been into creams or any kind of skincare routine and she never stepped into a beauty clinic. Just showering, using a washcloth and soap for face and body. On the other hand, my grandma and my mom always wore sunscreens since the 90s for outside doors activities. So I imagine that helped a lot for their skin preservation.
Anyone honestly worried about breast cancer who is still drinking alcohol, smokes and is overweight/diabetic, is totally missing the boat wrt estrogen. The studies do not show a statistically significant increase in breast cancer. Yet smoking, being overweight, and alcohol all increase the risk in statistical ways.
@@healershealth5818 I had breast cancer and I'm on HRT. I had to fight with my oncologist, but I convinced her. I was having serious mental health problems due to the decline of estrogen.
@@healershealth5818 if you have been diagnosed with hormone positive breast cancer you cannot take hormone replacements. You are placed on a hormone blocker. My oncologist did tell me last month that the topical estrogen cream would be ok but I think I'll pass for now.
I am 54, post-menopausal now for about 2 years. I am on the estradiol patch, prometrium, testosterone and vaginal estradiol . I also use the vaginal estradiol on my face and neck as well. I use the vaginal estradiol cream on my face in the morning since I use Tretinoin at night. It takes a good 6 months before you start seeing some changes. It is somewhat drying and not cosmetically elegant but it’s cheaper than the compounded estriol creams and I put a moisturizing cream on top and then sunscreen. One small study comparing the estradiol cream to the compounded estriol cream showed the same skin improvement results in both groups. And, I did check with my OBGYN and she approved. By the way, LOVE your Prequel Vitamin C! New holy grail status!
You and I are on pretty much the same regime only I'm a bit older (58). Is there a reason not to use the estrogen with the tret? I usually do my tret and serums and then later, after it's had time to soak in, I go in with the estrogen cream (usually when I brush my teeth right before bed). I definitely have noticed a difference since starting the estrogen on face and neck.
@@Cherylebreaux Hi, from what I understand you can use Tretinoin and estrogen at night. The recommendation from Alloy website for their M4 estriol cream is estrogen first then tret. I would assume it’s the same for the estradiol cream. I separate them AM/PM because they both tend to be a little drying on my skin. If you are not experiencing any irritation and you are seeing results using them both at night, I think your routine is great.
@@reginarizer5797 Yes, I checked with my OBGYN. It’s not an elegant formula and can be drying but I put a moisturizer on top and it’s fine. The vaginal estradiol cream is stronger than the estriol cream that is made for the face so keep in mind if you are prone to melasma it can worsen it. I have not noticed any issues with melasma on my skin. Just make sure to use sunscreen everyday. If you are prone to melasma the compounded estriol cream might be a better fit.

I am 63 and have been on HRT for almost 3 years. I have been using Estradiol for several years but it wasn’t enough for my symptoms….foggy brain, lack of energy, hot flashes, weight gain, none existent sex drive. Now I am on Biodenticals….”Progesterone, Pregnenolone, DHEA and a second Estradiol cream ( inside wrist 2x day). Blood test shows I don’t need to be on Testosterone because I work out intensely with heavy weights 3/4 times a week. If you cannot find a doctor who will listen and take care of you find a nurse practitioner. I found mine in a weight loss/hormone replacement clinic. Life is GOOD!

I really want to see the medical community significantly increase research concerning menopause and treatments. HRT is ideally started during menopause and is studied safe used for 5 years a test 4 genetic mutations that make women way more susceptible to breast cancer should be done first & yearly mammograms should not be missed I’m on bio identical progesterone it’s amazing

Topical estradiol cream (Ovestin) absolutely works mixed into your usual moisturizer. I’m 61 I am not a candidate for systemic HRT unfortunately. But the topical cream is a total game changer. I’m so happy .
100%. There is a neglect that seems to happen. Not one doctor gave me information or told me what was happening. I suffered from severe menopause and was never told it could be mitigated. Most doctors don’t deserve our trust. Also, most doctors are prescribers, not healers.
Yes I started estrogen cream on face about a month ago. I’m 56 and have been on HRT for about 5 years. The estrogen cream has made a noticeable difference in my skin clarity and suppleness. And it was noticeable within 2 weeks if not earlier. I love it. I haven’t had side effects from estrogen cream at this point but I did up the tretinoin use at night which did irritate my skin. I have been on bioidentical hormone replacement therapy since age 46. I blasted through perimenopause and it was awful and compounded by the fact that it was a challenge finding a doctor to acknowledge and address it. I am 71 and found my people! I’m 55 in perimenopause. A few months ago the symptoms started really bad: hot flashes, brain fog, weight gain and mood swings. I started with natural supplements that helped but then I switched to HRT thanks to Dr Marie Claire Harver. What a game changer! The alloy cream is estriol wic is the weakest form of estrogen. That’s bunk! I want estradiol for my face! Im currently using my doctor prescribed Premarin crem but it’s it a lot of other weird hours hormone in it.

I was put on hrt at 55. Game changer. My skin is finally getting back to what is was. Not a cure all, still hard to maintain moisture, but the acne finally stopped. So yea estrogen depletion is real. By replacing it, I can attest, game changer
I'm 46 and on the estrogen patch and yes, I noticed my cystic acne has all but vanished, knock on wood. I still get small breakouts, and I do think I am getting very small brown patches due to the estrogen, but I'll take it! I had horrible inflamed acne in my 30s and 40s that mauled my face, and it's so frustrating to know now that I should have at least investigated hormone treatment options, e.g. the pill. I feel like for a long time there was so much scaremongering, especially with HRT, so I suppose I should feel lucky that at least now there seems to be a more enlightened, evidence-based approach, at least where I am in the UK.

Yea that only goes so far. And you need to it nightly for probably 4 months. Then go back again. I do agree it's great for inflammation and joints etc. But it doesn't get down to the crux of the matter. Which is the fiberblsst cells. What feeds them????
I’ve been using facial topical estrogen for a month now. I’ve noticed some improvement in my skin elasticity and firmness. It’s not a huge improvement but enough that I want to continue. The improvement is most obvious is photos, especially around the jowls. I’ve not experienced any negative effects
I am on a low doe of HRT because I had polyps in my uterus which I was told was caused by too much estrogen. I have been using vagafem for years with no problems. I just started using estrogen on my face so I haven’t noticed improvement yet but I am hopeful. I am thrilled that us older women are finally being taken care of as it has been discovered what an important part all these hormones play in our bodies! Thank you for this explanation!
Thank you so much for this video! I'm in perimenopause and have done quite a bit of research. I get my hormones tested every 3-4 months (on my own through Private MD Labs) to make sure they're balanced and my last test showed I have no hormones left. The skin sagging is real :/ I've learned the key to avoiding cancers is balancing estrogen and progesterone. I use a natural progesterone cream just so I can sleep. Without it my peri-caused insomnia is awful. I also take 5 mg of DHEA and even though that is a low dose, its helped tremendously with my energy and strength. My doctor and the articles I've read said that DHEA raises testosterone and in women that raises estrogen. However, with all of that I still have vaginal atrophy and after this video will be talking to my doctor about estrogen cream. I just had polyps removed from my uterus and cervix-benign thankfully-and wanted to wait on adding estrogen until they were removed. Ladies, get your hormones tested please! Regardless of what your doctor says. I was told my symptoms of erratic periods was normal because I'm in peri. Before my last pap appointment I tested my hormones, which is how my doctor realized something was wrong. I had no hormones but had a cycle, which was caused by uterine and cervical polyps. A lot of docs don't even order hormone testing. They just tell you you're in perimenopause. Benign polyps can become malignant so please do your due diligence with your health :)

I've used topical estrogen for a year without any issues, and I have seen an improvement in my fine lines. I always use SPF and I am very happy with the results.
I was put on estradiol gel tabs in May. By July, I was off of them. I continued with testosterone and progesterone but nothing compares to estradiol. I had to research for myself why my lymph nodes under both arms ached 24 hrs a day. Why I had breast pain. I have a hormone specialist and I’m not sure if she understands that taking estradiol orally converts to estrone which is inflammatory or she didn’t think to tell me at one of my many visits to find out what was wrong with me -that cost hundreds of dollars every appt about this detail. It’s drs like you on UA-cam that I am learning from because i HAVE to. I asked for the patch after 2 months of estradiol replacement and WOW. I am watching my skin rejuvenate again. No lymph node pain. No breast pain. Just the benefits.
I did not know this oral conversion to.estrone Must be due to.having to go thru the liver. Been on estradiol for 15 years. last year, due to the crazy increased cost of estradiol.patch, I started oral estradiol. Does not work as well as the patch so I was happy recently to discover a more reasonable price for the patch which I just started again today.
this explains it! i have to take hormone blockers to prevent my metastatic stage 4 breast cancer cells from eating their favorite thing that keeps them thriving and that is my hormone ESTROGEN! This explains why i looked far younger a year ago i was 47 when diagnosed but looked 37 until AFTER nearly a year of taking letrozole and zoladex injections that one is a hormone blocker that prevents estrogen absorption in the body and the other shuts down my ovaries to prevent further production of estrogen. It was within less than a year that i now look older than my age. People were asking me what is it that is making me look years younger than was i really was prior to my treatments, now people are asking me why do i look so old. No that is not them being mean, they were worried about my overall health as they didn't know about me having stage 4 cancer. Even with stage 4 cancer i had for well over a year and did not know it, i still looked way younger in years so it is not the cancer that is causing my aging in this case, though it made me feel old as dirt and dead on my feet. I have the slow growing kind of cancer that is why it didn't kill me right away at stage 4 but the fact that my body is being starved from producing and absorbing estrogen is causing me to age rapidly facially.
My heart goes out to you and others for dealing with the double whammy of cancer plus having to take estrogen blockers. Having experienced significant symptoms associated with menopause and finally getting treated with HRT and feeling normal again, I can’t imagine dealing with what you had to experience….So thank you for sharing, I’m glad you are ok, and I’m sorry you have had to deal with all that!
Love this presentation. HRT is the bomb and at 65 I have been using it since my early 50's. I added Testosterone a year ago. Game Changer! We need our hormones to be our best and we have estrogen receptors throughout all systems in our bodies for a reason. Unfortunately, the focus has only been on reproduction for so long. Too many millions of women have suffered unnecessarily for too long. So glad to see things are changing and it is being talked about studied, and prescribed. So important! Lots to learn on this topic, and so much fear and misinformation, it is great to have another sound voice on this topic. Find providers who are educated about the benefits and understand all the nuances. It is complicated, but the bottom line is we need our hormones and they benefit your health your entire lifespan.
Great topic. I'm 65, but at 42, I was diagnosed with Premature Ovarian Failure. No symptoms, my periods just stopped. I was put on the Estradiol patch and progesterone. At 48, I was taken off of the hormones. I went on Estrace Vaginal Cream, and have been on it twice a week, since then, and have had no negative side effects. I'm definitely going to ask my gyno if I can apply to my face. I must admit, that I've tried it a few times, but wasn't sure if I should be doing so. I have no risk factors such as family history of breast cancer. I've also been on topical Tretinoin since my 20s. Wow, what a great combination the two would make. Your gyno won't know. Look at the % as the vaginal cream is normally 1%. You want 0.01 %. I use the 1% but just a smidge of cream, about the size of a grain of rice.

I wish they knew more back then. I would have fought to stay on in. But, via a heart monitor, it was discovered that I went into non sustained v-tach, and threw me off of the transdermal estrogen patch. Ugh.@ammonitelight
@@reneklein6049 With any hormone, it is better to use daily, so that it is not the case of one day there is a hormone and the next day there isn't. I have been researching this subject for 18 years. It would be way better to use a smaller amount every day. 😁
This video was informative from the dermatology point of view, thank you. I did listen to this entire video and did rewind to make sure I heard correctly. The study that showed that women on HRT were at an increased risk of breast cancer was the WHI, this was over 20 years ago. The data was misinterpreted (absolute risk v relative risk). The arm of the women taking estrogen alone actually did NOT have increased breast cancer. Also the women took Premarin, a oral form of estrogen and synthetic progesterone. Women today are given transdermal estrogen and bioidentical progesterone if they have a uterus. It is stated that the women does need to make this decision with her oncologist and gyn but listening to this information is instigating fear over facts. Definitely every women needs to make this decision based on her history but again the information needs to be current and accurate.
Dr. Ellis actually says the same thing you did, but you're making it sound like she was saying the opposite. She said the exact things you did about systemic versus topical estrogen, and bioidentical progesterone. And that the statement of increased risk might be correct in a vacuum but there are other factors involved that actually make the risk not as a parent. It seems you stopped listening after the sentence she said about increased risk of cancer. Why people are liking your comment I have no idea.
@saltycrunch Dr. Ellis does state there's more to determining the actual risk of breast cancer, but in the video, there's a text box pop up thing (whatever you call it) where it says "People on hormone replacement therapy are at higher risk of breast cancer". And then around 12:15, she states again that estrogen causes a small increase in breast cancer. An entire generation of women have needlessly suffered from no HRT based on this WHI study that was misinterpreted. I'm one of them ❤

I’m currently doing an ivf cycle where I was temporarily put into menopause to treat endometriosis. Let me tell ya! Estrogen does so much. My skin was dull and I developed wrinkles. A week back on estrogen I felt and looked like a new person.
This was excellent, thank you. Very well presented and so helpful. I have topical estrogen for applying to the opening of the urethra to help with stress incontinence, I didn’t like the pessaries. I’m also on HRT but thrilled that I can use the cream on my hands, face and neck. My hands have hardly any fat and are by far the oldest looking part of my body! My lower pelvic physio at the hospital told me during an exam that she can tell I’m on HRT. A slightly awkward compliment but good to know!
I was hoping that someone would address incontinence. Thank you. I have been on HRT for 27 years now (the patch). I also have a bladder condition called interstitial cystitis which causes feelings of urgency as well as urine leakage. About a year ago, I started on additional topical estrogen applied vaginally and around the urethra. It has made a huge difference. Sometimes systemic estrogen is just not enough. I would encourage any woman with bladder issues to speak to her gynecologist about topical estrogen, as well as personal benefits and risks. For me, it's been a quality of life saver.
I was anorexic for 2 years of my life from 34-36. My skin looked horrible as I was protein and estrogen deficient. I was 93 pounds and skeletal thin. Now at 36 1/2 and at 135 pounds with a regular menstrual cycle - my skin is desperately trying to catch up.
My ND prescribed a topical HRT cream that includes various forms of estrogen as well as testosterone that I've been applying to my inner thigh for a few months now. It's made a huge difference in alleviating my pervasive post-menopausal symptoms (the hot flashes never went away for me until treatment), but I never considered what it could do for my face. I have had to have my urethane stretched 3 times . United Healthcare refused to pay for the $500 estrogen cream. But ecoli is just fine for us to pay for because at 56 and full hysterectomy by age 32..everything is atrophied. Not to mention my weight gain and mood. Hugs friend!

This information is so valuable! Thank you so much for making a deep dive into this topic. As a post menopausal woman that has suffered from the symptoms you mentioned, I am super interested but still haven't made my mind around it. Personally, at the moment I am not convinced about using this cream topically on my face for aesthetic purposes only. After almost 5 years of starting this process, I discovered that this new stage of our lives (at least in my case) required more than 'just' taking HRT, which has proven to be so beneficial for me, but also lifestyle changes, from exercise to nutrition, skin care, hair care and so on... It takes some work (and years) in getting to know this new body, but if you learn how to give it what it needs you can get great results out of those changes, from the outside and inside as well. What I mean is that this cream could be an option but it is not the only answer. At least that is the case for me now.
Everything I have learned indicates that taking hrt (estrogen) does not increase a woman's risk of breast cancer. A good resource is a book titled Estrogen Matters.
It does increase it by 1 in 1000 to exact. Where people say no increase at all they are basing that on the studies with CEE and hardly anyone would use Premarin now due to ethical concerns for horses.

Dr. Ellis, Huberman lab recently did an excellent episode about peri-/full menopause with Dr. Mary Claire Haver and discussed several of these topics! She interrupts him a lot;), but I think it’s because she’s excited. She’s produced a lot of content about HRT and estrogen topicals. Nice to see another well-respected doctor validate her ideas because I wasn’t sure how seriously I should take her interpretations and data.
I haven’t listened to this but in regards to her interrupting him, I have listened to lots of podcasts on various subjects and I find that quite a lot of the hosts of the podcasts, when they have a special guest on they talk too much and I often get frustrated and think to myself, why have they bothered to invite a specialist on their show if they are not going to shut up and listen to them?? I’m not saying that is the case with the one you are talking about but if it is it might not actually be helpful for me to listen to it! 😂😂 But thank you for the recommendation I will look up the Huberman lab UA-cam channel and have a look for it.

I'm a BC survivor of 8 years and started estradiol a couple months ago on my face and neck. Dr. Ellis just reminded to do our hands as well. It hasn't hurt my complexion, so I'll continue it to see what the future holds. I also just had my first round of Sculptra, so can't wait to see what that does too. I'm getting two more treatments in the coming months.
There is a study much earlier than the ones you site that showed Estrogen alone doesn’t help your skin . Progesterone is required. During pregnancy both estrogen and progesterone elevates dramatically giving your skin that so called “glow”. Progesterone in particular elevates more and provides skin elasticity. I put both on my skin. As a clinical scientist I can monitor the effects.
@@lorisykes252 there have been multiple studies that demonstrate the benefit of estrogen alone, but that’s not to say that progesterone doesn’t a play a part, it’s just not as well explored.
@@glancycorner7425Not quite accurate. 1) Women with uteruses MUST take progesterone if they’re on systemic estrogen to minimize the risk of endometrial cancer. 2) Women without uteruses do not have to take progesterone while on estrogen to prevent endometrial cancer; however, they may still get some benefits with progesterone as it can mitigate sleep issues and anxiety.
I once started BCPs just before going on a cruise. I had very clear, even skin and no history of melasma. I went out on deck one day, came back, looked in the mirror and had brown patches on my cheeks and upper lips and have had it ever since. Literally in one afternoon. I’m 57, started using HRT in April. After 3 months, I noticed my face had increased volume, and I look less tired, when I’m not tired. I can imagine topical estrogen would be great for places where women have extra concern.

Great informative video. Yes, i have been using topical Estradiol cream prescribed by my doctor on my face for the better of six months. I haven't had any issues with side effects. I have seen good results, most particularly in areas where the skin is thinner, around the eyes, and neck areas. I've also been using it on the upper inside of my arms with great improvement in thickness as well. I do use it along with Retin A and other skincare. I'm quite happy with the results. Thank you 😊

My menopause doc prescribed the amount she tested my body to need, I use a tiny bit under eyes, a little vaginally and the rest on my thigh. I am a 64-year-old woman who has been on topical compounded estrogen and progesterone creams for 15 years with the guidance of my doctors. It has been life-changing in terms of quality of life and menopausal symptoms. Both my internist and gynecologist agree that I should stay on these creams For the rest of my life if I want to. I apply the estrogen in the morning to the inside of one arm and the progesterone in the evening on the inside of the other arm. Thank you! I am getting toward the end of my first M4 Rx through Alloy. I do feel like my skin looks better now, but I also use a red light mask, the Nira and Nuface, not to mention Tretinoin. So - it’s a new part of my arsenal and I am impressed so far but need to give it more time. BTW I just turned 60 and am past menopause

I was in full blown menopause at 44. Went on HRT and what a difference. I went back to “normal”. After 20+ years on it , I was diagnosed with breast cancer. Breast cancer does not run in my family. I do not have the BRACA gene. I attribute it to the HRT. Be careful when playing with fire. I didn’t take the HRT for my skin, as these younger generations are so incredibly obsessed with, I took it because I needed sleep because I was working .. working in a career with already tough hours as an international flight attendant. It never occurred to me that I was putting myself in jeopardy until it was too late. Please be wise. CANCER IS NOT FUN!
I'm so sorry you went through that! If it makes you feel better, based on the bulk of data, it wasn't the HRT that triggered the breast cancer: it was likely age-related. Older menopausal women, typically with the very lowest levels of estrogen, have the highest breast cancer rates, rather than younger women who have much higher levels of estrogen. So, you didn't do anything wrong- using HRT was most likely not the cause. The book Estrogen Matters, by medical oncologist Avrum Bluming, is really informative and eye opening about what the data actually shows about the safety of estrogen and HRT. Corinne Menn, OBGYN, herself a breast cancer survivor, is also an excellent resource.
I had a partial hysterectomy at 38 and at 48 I have been experiencing hot flashes for the last year. I will be talking with my Ob/Gyn at my next visit about HRT as I have lost a lot of fullness from my face in the last 2 to 3 years. Luckily I had a round face well into adulthood so I look like I lost a few pounds but I will be proactive.
I think an important note is you primarily need progesterone with estrogen when taking the pill because estrogen alone will result in an over growth of the uterine lining. I would think that’s where the cancer risk is rooted.
Exactly. We need more studies to understand if certain topical estrogen formulations, when applied locally, lead to enough systemic estrogen absorption to cause that issue.
@@DrSamEllis oh I think they already know that topically it won’t do that. It’s just a risk for systemic estrogen when at the age that you’re taking the pill. I think it’s different when you’re older and able to take other types of HRT.

I’ve been on the hrt patch and oral progesterone. I got CO2 laser. Just life changing. Melasma spots are gone. I sleep like a baby. Hot flashes are gone and I started using weights 2x a week with a trainer via FaceTime. I feel 22 at 60, and I’m glad I did it.
PLEASE tell me how I can get my hands on that estrogen gel you show. I'm 78 years old. Was on compounded estrogens for several years. Lost my doctor. (He lost his license for prescribing hormones and billing insurance companies. Long story). So I went to a regular doctor and asked for a prescription for estrogen and progesterone and she laughed me out of the office. Literally. She said who would prescribe hormones for a 70 year old woman. That was eight years ago. She was older, I thought she'd understand, but no, she was sure it would kill me. So, I found a way to get Estrodial gel from overseas illegally and I bought 3 tubes. I use bio identical progesterone cream from over the counter sources. Now the estrogen gel is almost gone and I don't know what to do. My overseas source has dried up. Doctors won't prescribe it. Interesting -- the last time I saw my primary doctor, who I wouldn't dare tell I use the stuff, he was in shock about my age. He asked me what do I do, I look no more than 40! I told him I take B vitamins. Why can't they just make it so a woman can decide whether she wants to take something. It's not Fentanyl.
There is a dr in Texas who has videos on youtube. Dr. Shawn Tassone. He does tele medicine and, depending on what state you are in, can and will prescribe hormones.

Did not put the dots together! Can you have Angiomas & thin broken blood vessels safely removed? Women using bio identical hormone replacement therapy are not at greater risk for breast cancer or any other cancer associated with hormones. This is the research! as well, women who have had hormone related breast cancers can do bio identical hormone replacement. This also is the research that has been done.

I’m on HRT patch . I also use estrogen cream 2 x a week with a steroid cream on the outside of my vagina for Lichen Sclerosus . I have been on this regiment for 2 years now and I’m 59 . The best decision I ever made for my health. I recently started using it on my face a few nights a week and my neck has improved a lot . I’m excited about the use of estrogen cream applied to the face .
Seemingly overnight as soon as I started skipping periods my skin began to sag and get crepey all over my body. Tests showed I still make estrogen but perhaps it’s started to decline rapidly. I have no other symptoms just the skin changes. It’s just awful. I always looked a decade or more younger before this. Now looking into HRT.
My personal experience: even a tiny amount of estrogen on the face is felt systemically and needs to be balanced by progesterone to feel balanced. Notably breast tenderness.

Thank you for this important video! I am almost 74. I was post menopause by age 42. I was on HRT for probably 15 years. I decided I had been on it long enough as I was reading all the misinformation and health scares at the time and was worried about my overall health. After I went off of it is when I ended up with osteoporosis, Afib, a debilitating rotator cuff injury, arthritis and fainting spells. And of course vaginal issues. I really wanted to go back on HRT but my GYN told me that because I had gone off of it, because of my age I couldn't go back on it. I am very interested in starting on topical estrogen for my face and hands and have high hopes that my GYN will OK a prescription for me. I just thought someone would be interested in my HRT journey. It was a blessing and kept me young and I shouldn't have gone off of it.
I’m 76 and was recently re-started on HRT by a functional medicine GYN. I was off of it for 4-5 years after losing access to a practitioner, then the pandemic. I’m now on compounded bio identical estrogen, oral progesterone and several nutritional supports (for things like low B-12). This type of HRT has been recognized as safe and beneficial to protect cardiac function, muscle mass and bone integrity, as well as restore better cognitive function, sleep and more. I had to find, and pay, this person privately and will be looking in the future for someone who accepts insurance to continue this protocol.

I was on HRT from 45-52 and got off due to my sister getting breast cancer and her telling me I shouldn't be on it! I felt like I was aging too fast, arthritis, joint pain, depression, weight gain, just overall felt bad. So I just started again at 62. The research on breast cancer was flawed, although I think there was a slight risk with oral estrogen (I use bioidentical cream now and a progestrerone pill). To me it's better for my overall health, and I want to try and avoid the dementia my mom has. Your brain needs estrogen or it shrinks! I go to Dr. Dian Ginsburg - she is a virtual Ob/gyn who only does menopause medicine now. You should check her out. Luckily there are many ob/gyn's switching to this practice so we can get better information. Many I follow say you should be on estrogen for life and I believe them! I will continue to have my yearly mamogram and check for breast changes of course. Dr. Ginsberg said put a little of the cream under your eyes and on your crows feet, forehead wrinkles and hairline. Then half is vaginal and half is on alternating thighs.

I"m 65 and use topical estrogen. I've been using tretinoin since I have been 42 and had a deep C02 done when I was 59, and that really helped a lot. When I turned 64, I didn't recognize the person in the mirror anymore. Topical estrogen has helped. I would recommend it. Thanks for address this. Very few dermatologists address this problem. We all get there some day. My mom passed away at 93 and she was on hormone therapy and looked very good in her senior years.
I hope she was healthier due to o estrogen rather than spending half her life diving into bad health.

Just wanted to add that post menopause you can actually have low testosterone, too! As women we actually need testosterone (just much less than men). My mom suffered with debilitating anxiety and irritability for 2 years (which we all certainly noticed, just didn’t know the cause) before she went to a hormone specialist who told her she had low testosterone and she began supplementing. Within weeks she noticed a huge difference in her emotional state
Interesting. I would think that lack of progesterone or maybe estrogen would make someone irritable but I associate testosterone more with motivation, but who knows! Glad she is feeling better. It's incredible how much our hormones affect our moods and "personalities".

I’m a 45-year old, medium fair-skinned Asian woman. For the last three months, I’ve used Musely’s Aging Repair Cream for my face in the morning ( 0.3% estriol and 1% vitamin C) and the Estrogen Boost in the evening (0.4% estriol, 0.1% estradiol, 20% progesterone; note the progesterone) and had fantastic outcomes so far. Overall, my face and body looked de-aged 10 years. The most dramatic change I’ve ever seen tbh, and I already had fairly good skin (like most of us here, I have a regular routine of 1% retinol, vitamin C, sunscreen, BHA/AHA exfoliant).
The loss of elasticity, plumpness, and “bounce” of my skin was something I wanted to keep (or at least have high standards regarding) and was looking for something to address that. IMHO, it’s not wrinkles per se that age us but skin texture and tone changes (AKA crepey-ness and age spots). She likely uses that one on the arm or belly (maybe her face) and it does have systemic effects because of the higher blend of hormones. The aging repair cream that she uses on her face in the morning (I use this one as well) is really localized because it’s a much lower amount of estriol and doesn’t have the other hormones. Just my take on her comment being a long time musely customer.

Be aware if you have pets. We had recently gotten a puppy when I started using my topical estrogen (bioidentical) cream on my inner thigh at night. The puppy did not sleep with us but would come in and cuddle in the morning. Several months later, our vet noticed the puppy had an enlarged vulva and larger than normal teats. She the asked me if I was taking topical estrogen. Apparently, she knew the topical estrogen can transfer to pets. I immediately replaced the topical estrogen to troches and the dog returned to normal. It's amazing to me how powerful the estrogen transfer must be after a night of sleep and the dog having no direct contact with my thighs. There is plenty of research on this if you check, but I had no idea prior to my experience.

This is so helpful. I don't usually pick up my dog until after my estradiol and testosterone dries but I will be more careful. I think I may have to switch to a patch. Tried the troche, made me nauseated and bloated as I was probably swallowing some of the estradiol and testosterone.

Depression, not sleeping or waking at 3 am and not able to go back to sleep, migraines that lasted a week, itchy dry skin, feelings of bugs crawling all over my skin, crying all the time for anything, feeling terrible with flu like symptoms most of the month. No hot flashes but I had cold flashes so bad that my skin would itch and I would scratch and make myself bleed over nothing, joint pain, no energy, no desire to workout (which I’ve always done) I was a completely different person and hated myself and even was contemplating suicide. My Dr put me on an anti depressant which didn’t help my menopause symptoms. I started watching videos on UA-cam: Dr Mary Claire Haver, others, and educated myself on what was essentially happening to my body. It was peri menopause and I’d started having symptoms around 42. My periods which had been very light before that were non existent now, and I was having awful
Vaginal dryness that it actually hurt to walk. Everyone’s side effects can be different so I had to just explain everything I was going through to an Endocrinologist who knew based on my age 44 that I was going through estrogen withdrawal basically. I got the hormone replacement I needed and they help a lot. I feel 80% like myself.
Im 47 and still on birth control pill. I am sure this has prevented me from dealing with the crap. Plan on transitioning to HRT at 50

My mom have had a total hysterectomy (severe endometriosis) when she was 28 (now 56). She was prescribed since 28 a estrogen gel to apply on her lower arms. With this she avoided all the menopose symptoms and she is still on it. Her skin is indeed very good looking too. Barely have wrinkles and she never been into creams or any kind of skincare routine and she never stepped into a beauty clinic. Just showering, using a washcloth and soap for face and body. On the other hand, my grandma and my mom always wore sunscreens since the 90s for outside doors activities. So I imagine that helped a lot for their skin preservation.

Anyone honestly worried about breast cancer who is still drinking alcohol, smokes and is overweight/diabetic, is totally missing the boat wrt estrogen. The studies do not show a statistically significant increase in breast cancer. Yet smoking, being overweight, and alcohol all increase the risk in statistical ways.

Totally true. Estrogen has been show to decrease risk of breast cancer, dementia, osteoporosis, heart disease and diabetes.

@@healershealth5818 I had breast cancer and I'm on HRT. I had to fight with my oncologist, but I convinced her. I was having serious mental health problems due to the decline of estrogen.
@@healershealth5818 if you have been diagnosed with hormone positive breast cancer you cannot take hormone replacements. You are placed on a hormone blocker. My oncologist did tell me last month that the topical estrogen cream would be ok but I think I'll pass for now.
I am 54, post-menopausal now for about 2 years. I am on the estradiol patch, prometrium, testosterone and vaginal estradiol . I also use the vaginal estradiol on my face and neck as well. I use the vaginal estradiol cream on my face in the morning since I use Tretinoin at night. It takes a good 6 months before you start seeing some changes. It is somewhat drying and not cosmetically elegant but it’s cheaper than the compounded estriol creams and I put a moisturizing cream on top and then sunscreen. One small study comparing the estradiol cream to the compounded estriol cream showed the same skin improvement results in both groups. And, I did check with my OBGYN and she approved. By the way, LOVE your Prequel Vitamin C! New holy grail status!
You and I are on pretty much the same regime only I'm a bit older (58). Is there a reason not to use the estrogen with the tret? I usually do my tret and serums and then later, after it's had time to soak in, I go in with the estrogen cream (usually when I brush my teeth right before bed). I definitely have noticed a difference since starting the estrogen on face and neck.
@@Cherylebreaux Hi, from what I understand you can use Tretinoin and estrogen at night. The recommendation from Alloy website for their M4 estriol cream is estrogen first then tret. I would assume it’s the same for the estradiol cream. I separate them AM/PM because they both tend to be a little drying on my skin. If you are not experiencing any irritation and you are seeing results using them both at night, I think your routine is great.

@@reginarizer5797 Yes, I checked with my OBGYN. It’s not an elegant formula and can be drying but I put a moisturizer on top and it’s fine. The vaginal estradiol cream is stronger than the estriol cream that is made for the face so keep in mind if you are prone to melasma it can worsen it. I have not noticed any issues with melasma on my skin. Just make sure to use sunscreen everyday. If you are prone to melasma the compounded estriol cream might be a better fit.

I have been using Estradiol for several years but it wasn’t enough for my symptoms….foggy brain, lack of energy, hot flashes, weight gain, none existent sex drive. Now I am on Biodenticals….”Progesterone, Pregnenolone, DHEA and a second Estradiol cream ( inside wrist 2x day). Blood test shows I don’t need to be on Testosterone because I work out intensely with heavy weights 3/4 times a week. If you cannot find a doctor who will listen and take care of you find a nurse practitioner. I found mine in a weight loss/hormone replacement clinic. Life is GOOD!

I really want to see the medical community significantly increase research concerning menopause and treatments. Topical estradiol cream (Ovestin) absolutely works mixed into your usual moisturizer. I’m 61 I am not a candidate for systemic HRT unfortunately. But the topical cream is a total game changer. I’m so happy .

Yes I started estrogen cream on face about a month ago. I’m 56 and have been on HRT for about 5 years. The estrogen cream has made a noticeable difference in my skin clarity and suppleness. And it was noticeable within 2 weeks if not earlier. I love it. I haven’t had side effects from estrogen cream at this point but I did up the tretinoin use at night which did irritate my skin. I have been on bioidentical hormone replacement therapy since age 46. I blasted through perimenopause and it was awful and compounded by the fact that it was a challenge finding a doctor to acknowledge and address it. I am 71 and found my people! I’m 55 in perimenopause. A few months ago the symptoms started really bad: hot flashes, brain fog, weight gain and mood swings. The alloy cream is estriol wic is the weakest form of estrogen. That’s bunk! I want estradiol for my face! Im currently using my doctor prescribed Premarin crem but it’s it a lot of other weird hours hormone in it.

I was put on hrt at 55. Game changer. My skin is finally getting back to what is was. Not a cure all, still hard to maintain moisture, but the acne finally stopped. So yea estrogen depletion is real. By replacing it, I can attest, game changer
I'm 46 and on the estrogen patch and yes, I noticed my cystic acne has all but vanished, knock on wood. I still get small breakouts, and I do think I am getting very small brown patches due to the estrogen, but I'll take it! I had horrible inflamed acne in my 30s and 40s that mauled my face, and it's so frustrating to know now that I should have at least investigated hormone treatment options, e.g. the pill. I feel like for a long time there was so much scaremongering, especially with HRT, so I suppose I should feel lucky that at least now there seems to be a more enlightened, evidence-based approach, at least where I am in the UK.

I’ve been using facial topical estrogen for a month now. I’ve noticed some improvement in my skin elasticity and firmness. It’s not a huge improvement but enough that I want to continue. The improvement is most obvious is photos, especially around the jowls. I’ve not experienced any negative effects

I am on a low doe of HRT because I had polyps in my uterus which I was told was caused by too much estrogen. I have been using vagafem for years with no problems. I just started using estrogen on my face so I haven’t noticed improvement yet but I am hopeful. I am thrilled that us older women are finally being taken care of as it has been discovered what an important part all these hormones play in our bodies! Thank you for this explanation!

Thank you so much for this video! I'm in perimenopause and have done quite a bit of research. I get my hormones tested every 3-4 months (on my own through Private MD Labs) to make sure they're balanced and my last test showed I have no hormones left. The skin sagging is real :/ I've learned the key to avoiding cancers is balancing estrogen and progesterone. I use a natural progesterone cream just so I can sleep. Without it my peri-caused insomnia is awful. I also take 5 mg of DHEA and even though that is a low dose, its helped tremendously with my energy and strength. My doctor and the articles I've read said that DHEA raises testosterone and in women that raises estrogen. However, with all of that I still have vaginal atrophy and after this video will be talking to my doctor about estrogen cream. I just had polyps removed from my uterus and cervix-benign thankfully-and wanted to wait on adding estrogen until they were removed.
Ladies, get your hormones tested please! Regardless of what your doctor says. I was told my symptoms of erratic periods was normal because I'm in peri. Before my last pap appointment I tested my hormones, which is how my doctor realized something was wrong. I had no hormones but had a cycle, which was caused by uterine and cervical polyps. A lot of docs don't even order hormone testing. They just tell you you're in perimenopause. Benign polyps can become malignant so please do your due diligence with your health :)

I've used topical estrogen for a year without any issues, and I have seen an improvement in my fine lines. I always use SPF and I am very happy with the results.
I was put on estradiol gel tabs in May. By July, I was off of them. I continued with testosterone and progesterone but nothing compares to estradiol. I had to research for myself why my lymph nodes under both arms ached 24 hrs a day. Why I had breast pain. I have a hormone specialist and I’m not sure if she understands that taking estradiol orally converts to estrone which is inflammatory or she didn’t think to tell me at one of my many visits to find out what was wrong with me -that cost hundreds of dollars every appt about this detail. It’s drs like you on UA-cam that I am learning from because i HAVE to. I asked for the patch after 2 months of estradiol replacement and WOW. I am watching my skin rejuvenate again. No lymph node pain. No breast pain. Just the benefits.

this explains it! i have to take hormone blockers to prevent my metastatic stage 4 breast cancer cells from eating their favorite thing that keeps them thriving and that is my hormone ESTROGEN! This explains why i looked far younger a year ago i was 47 when diagnosed but looked 37 until AFTER nearly a year of taking letrozole and zoladex injections that one is a hormone blocker that prevents estrogen absorption in the body and the other shuts down my ovaries to prevent further production of estrogen. It was within less than a year that i now look older than my age. People were asking me what is it that is making me look years younger than was i really was prior to my treatments, now people are asking me why do i look so old. No that is not them being mean, they were worried about my overall health as they didn't know about me having stage 4 cancer. Even with stage 4 cancer i had for well over a year and did not know it, i still looked way younger in years so it is not the cancer that is causing my aging in this case, though it made me feel old as dirt and dead on my feet. I have the slow growing kind of cancer that is why it didn't kill me right away at stage 4 but the fact that my body is being starved from producing and absorbing estrogen is causing me to age rapidly facially.
My heart goes out to you and others for dealing with the double whammy of cancer plus having to take estrogen blockers. Having experienced significant symptoms associated with menopause and finally getting treated with HRT and feeling normal again, I can’t imagine dealing with what you had to experience….So thank you for sharing, I’m glad you are ok, and I’m sorry you have had to deal with all that!

Love this presentation. HRT is the bomb and at 65 I have been using it since my early 50's. I added Testosterone a year ago. Game Changer! We need our hormones to be our best and we have estrogen receptors throughout all systems in our bodies for a reason. Unfortunately, the focus has only been on reproduction for so long. Too many millions of women have suffered unnecessarily for too long. So glad to see things are changing and it is being talked about studied, and prescribed. So important! Lots to learn on this topic, and so much fear and misinformation, it is great to have another sound voice on this topic. Find providers who are educated about the benefits and understand all the nuances. It is complicated, but the bottom line is we need our hormones and they benefit your health your entire lifespan.
Great topic. I'm 65, but at 42, I was diagnosed with Premature Ovarian Failure. No symptoms, my periods just stopped. I was put on the Estradiol patch and progesterone. At 48, I was taken off of the hormones. I went on Estrace Vaginal Cream, and have been on it twice a week, since then, and have had no negative side effects. I'm definitely going to ask my gyno if I can apply to my face. I must admit, that I've tried it a few times, but wasn't sure if I should be doing so. I have no risk factors such as family history of breast cancer.
I've also been on topical Tretinoin since my 20s. Wow, what a great combination the two would make.

Your gyno won't know. Look at the % as the vaginal cream is normally 1%. You want 0.01 %. I use the 1% but just a smidge of cream, about the size of a grain of rice.

I wish they knew more back then. I would have fought to stay on in. But, via a heart monitor, it was discovered that I went into non sustained v-tach, and threw me off of the transdermal estrogen patch. Ugh.@ammonitelight

I did listen to this entire video and did rewind to make sure I heard correctly. The study that showed that women on HRT were at an increased risk of breast cancer was the WHI, this was over 20 years ago. The data was misinterpreted (absolute risk v relative risk). The arm of the women taking estrogen alone actually did NOT have increased breast cancer. Also the women took Premarin, a oral form of estrogen and synthetic progesterone. Women today are given transdermal estrogen and bioidentical progesterone if they have a uterus.
It is stated that the women does need to make this decision with her oncologist and gyn but listening to this information is instigating fear over facts.
Definitely every women needs to make this decision based on her history but again the information needs to be current and accurate.

I am referring to the comment that Dr. Ellis made that women taking HRT or MPT have an increased risk of breast cancer.
Some women are still prescribed oral estrogen for HRT, so considering the risks for cancer and blood clots is still potentially important.
Dr. Ellis actually says the same thing you did, but you're making it sound like she was saying the opposite. She said the exact things you did about systemic versus topical estrogen, and bioidentical progesterone. And that the statement of increased risk might be correct in a vacuum but there are other factors involved that actually make the risk not as a parent.
It seems you stopped listening after the sentence she said about increased risk of cancer. Why people are liking your comment I have no idea.
@saltycrunch Dr. Ellis does state there's more to determining the actual risk of breast cancer, but in the video, there's a text box pop up thing (whatever you call it) where it says "People on hormone replacement therapy are at higher risk of breast cancer". And then around 12:15, she states again that estrogen causes a small increase in breast cancer. An entire generation of women have needlessly suffered from no HRT based on this WHI study that was misinterpreted. I'm one of them ❤

I have topical estrogen for applying to the opening of the urethra to help with stress incontinence, I didn’t like the pessaries. I’m also on HRT but thrilled that I can use the cream on my hands, face and neck. My hands have hardly any fat and are by far the oldest looking part of my body! My lower pelvic physio at the hospital told me during an exam that she can tell I’m on HRT. A slightly awkward compliment but good to know!
I was hoping that someone would address incontinence. Thank you. I have been on HRT for 27 years now (the patch). I also have a bladder condition called interstitial cystitis which causes feelings of urgency as well as urine leakage. About a year ago, I started on additional topical estrogen applied vaginally and around the urethra. It has made a huge difference. Sometimes systemic estrogen is just not enough. I would encourage any woman with bladder issues to speak to her gynecologist about topical estrogen, as well as personal benefits and risks. For me, it's been a quality of life saver.

I was anorexic for 2 years of my life from 34-36. My skin looked horrible as I was protein and estrogen deficient. I was 93 pounds and skeletal thin. Now at 36 1/2 and at 135 pounds with a regular menstrual cycle - my skin is desperately trying to catch up.

My ND prescribed a topical HRT cream that includes various forms of estrogen as well as testosterone that I've been applying to my inner thigh for a few months now. It's made a huge difference in alleviating my pervasive post-menopausal symptoms (the hot flashes never went away for me until treatment), but I never considered what it could do for my face. I have seen some improvements in my facial skin, but I honestly thought it was because of my focus on skincare. Now I'm rethinking a lot of things.
I have been using topical and systemic forms of HRT. I love the way the topical on my face has made a noticeable effect. I am 57.

I am 47 and have been suffering for the last 4 years with perimenopause. Severe weight gain with no change in lifestyle. Went from a size 4 to 14! Extra dey skin especially on my face. Bloated stomach, so much I looked 9 months pregnant. Brain fog, can't hold a thought. Severe Hot Flashes constantly! Can't sleep at night, up until 4 or 5 am. Night sweats/hot flashes wake me up when I do sleep. I am just miserable! My doctor won't prescribe any HRT says it's too soon. Why do I have to suffer for years!
My husband's friend went to his Doctor a month ago for high blood pressure, and being over weight. His doctor said he wanted to check his hormones. Turns out HE is on MALE MENOPAUSE. Went on hormone treatment asap, no questions asked, no doctor or insurance saying no.....
Yet here I am withy blood work proving I am in peri menopause and I can't receive treatment.
At this point I really do believe that the medical system hates women and doesn't care at all

I have had to have my urethane stretched 3 times . United Healthcare refused to pay for the $500 estrogen cream. But ecoli is just fine for us to pay for because at 56 and full hysterectomy by age 32..everything is atrophied. Not to mention my weight gain and mood. Hugs friend!
Get a better doctor. Many docs will prescribe HRT in perimenopause.

This information is so valuable! Thank you so much for making a deep dive into this topic. As a post menopausal woman that has suffered from the symptoms you mentioned, I am super interested but still haven't made my mind around it. Personally, at the moment I am not convinced about using this cream topically on my face for aesthetic purposes only. After almost 5 years of starting this process, I discovered that this new stage of our lives (at least in my case) required more than 'just' taking HRT, which has proven to be so beneficial for me, but also lifestyle changes, from exercise to nutrition, skin care, hair care and so on... It takes some work (and years) in getting to know this new body, but if you learn how to give it what it needs you can get great results out of those changes, from the outside and inside as well.
What I mean is that this cream could be an option but it is not the only answer. At least that is the case for me now.
Everything I have learned indicates that taking hrt (estrogen) does not increase a woman's risk of breast cancer. A good resource is a book titled Estrogen Matters.
It does increase it by 1 in 1000 to exact. Where people say no increase at all they are basing that on the studies with CEE and hardly anyone would use Premarin now due to ethical concerns for horses.
The problem is the majority of the medical community is not up to date and believe the flawed study that causes them to deny HRT to so many.
Correct. Taking HRT does not increase the risk of breast cancer. There’s a lot of misinformation out there.

I have been using the estradiol gel over the front of my body nightly as instructed but never knew that it could be used on my face and hands! How interesting. I will give this a try.

Dr. Ellis, Huberman lab recently did an excellent episode about peri-/full menopause with Dr. Mary Claire Haver and discussed several of these topics! She interrupts him a lot;), but I think it’s because she’s excited. She’s produced a lot of content about HRT and estrogen topicals. Nice to see another well-respected doctor validate her ideas because I wasn’t sure how seriously I should take her interpretations and data.

I haven’t listened to this but in regards to her interrupting him, I have listened to lots of podcasts on various subjects and I find that quite a lot of the hosts of the podcasts, when they have a special guest on they talk too much and I often get frustrated and think to myself, why have they bothered to invite a specialist on their show if they are not going to shut up and listen to them?? I’m not saying that is the case with the one you are talking about but if it is it might not actually be helpful for me to listen to it! 😂😂

I'm a BC survivor of 8 years and started estradiol a couple months ago on my face and neck. Dr. Ellis just reminded to do our hands as well. It hasn't hurt my complexion, so I'll continue it to see what the future holds. I also just had my first round of Sculptra, so can't wait to see what that does too. I'm getting two more treatments in the coming months.

There is a study much earlier than the ones you site that showed Estrogen alone doesn’t help your skin . Progesterone is required. During pregnancy both estrogen and progesterone elevates dramatically giving your skin that so called “glow”. Progesterone in particular elevates more and provides skin elasticity. I put both on my skin. As a clinical scientist I can monitor the effects.

Progesterone is not for women who have had total hysterectomies.
@@lorisykes252 there have been multiple studies that demonstrate the benefit of estrogen alone, but that’s not to say that progesterone doesn’t a play a part, it’s just not as well explored.
@@glancycorner7425Not quite accurate. 1) Women with uteruses MUST take progesterone if they’re on systemic estrogen to minimize the risk of endometrial cancer. 2) Women without uteruses do not have to take progesterone while on estrogen to prevent endometrial cancer; however, they may still get some benefits with progesterone as it can mitigate sleep issues and anxiety.
I once started BCPs just before going on a cruise. I had very clear, even skin and no history of melasma. I went out on deck one day, came back, looked in the mirror and had brown patches on my cheeks and upper lips and have had it ever since. Literally in one afternoon. Something to consider before any hormonal therapy. If only my doctor had informed me of the possibility. Thank you for this in depth video!

I’m 57, started using HRT in April. After 3 months, I noticed my face had increased volume, and I look less tired, when I’m not tired. I can imagine topical estrogen would be great for places where women have extra concern.

The night sweats are brutal. Great informative video. Yes, i have been using topical Estradiol cream prescribed by my doctor on my face for the better of six months. I haven't had any issues with side effects. I have seen good results, most particularly in areas where the skin is thinner, around the eyes, and neck areas. I've also been using it on the upper inside of my arms with great improvement in thickness as well. I do use it along with Retin A and other skincare. I'm quite happy with the results. Thank you 😊

Also, how can topical estrogen be incorporated into a skincare routine? What skincare ingredients can it be used with or layered with or shouldn’t be used with? I'm wondering that too - I'm thinking any estrogen on your skin becomes systemic so I hope people are taking this into account when they start using it on your face. My menopause doc prescribed the amount she tested my body to need, I use a tiny bit under eyes, a little vaginally and the rest on my thigh. I'm wondering if people may be using too much if they use a lot for their face and neck?

I am a 64-year-old woman who has been on topical compounded estrogen and progesterone creams for 15 years with the guidance of my doctors. It has been life-changing in terms of quality of life and menopausal symptoms. Both my internist and gynecologist agree that I should stay on these creams For the rest of my life if I want to. I apply the estrogen in the morning to the inside of one arm and the progesterone in the evening on the inside of the other arm. I’m wondering if I can use a dab of the same estrogen cream on my face. thank you for this informative video! I’ve been wondering about this for a while now.

Would love a deeper dive into this topic and suggestions you might have for skin and hair products if you choose not to go the estrogen cream/HRT route. My hair goes from feeling like straw for a few days to shedding like crazy to back to normal all within a few weeks. My skin is combination but now to the extreme resulting in crazy hormonal acne and eczema around my eyes. None of my makeup or skincare is working for me the way it used to. 😭Thank you!!!

I watched a video a few weeks ago, from a dermatologist, who stated that you can apply both. You should apply the estrogen cream first, wait a few minutes, and then apply the tretinoin.

I was in full blown menopause at 44. Went on HRT and what a difference. I went back to “normal”. After 20+ years on it , I was diagnosed with breast cancer. Breast cancer does not run in my family. I do not have the BRACA gene. I attribute it to the HRT. Be careful when playing with fire.
I didn’t take the HRT for my skin, as these younger generations are so incredibly obsessed with, I took it because I needed sleep because I was working .. working in a career with already tough hours as an international flight attendant. It never occurred to me that I was putting myself in jeopardy until it was too late. Please be wise. CANCER IS NOT FUN!

If it makes you feel better, based on the bulk of data, it wasn't the HRT that triggered the breast cancer: it was likely age-related. Older menopausal women, typically with the very lowest levels of estrogen, have the highest breast cancer rates, rather than younger women who have much higher levels of estrogen. So, you didn't do anything wrong- using HRT was most likely not the cause.
The book Estrogen Matters, by medical oncologist Avrum Bluming, is really informative and eye opening about what the data actually shows about the safety of estrogen and HRT. Corinne Menn, OBGYN, herself a breast cancer survivor, is also an excellent resource.

I had breast cancer at age 42 , like you breast cancer doesn't run in my family. According to my oncologist the cancer was not driven by hormones.

I had a partial hysterectomy at 38 and at 48 I have been experiencing hot flashes for the last year. I will be talking with my Ob/Gyn at my next visit about HRT as I have lost a lot of fullness from my face in the last 2 to 3 years. Luckily I had a round face well into adulthood so I look like I lost a few pounds but I will be proactive.
I think an important note is you primarily need progesterone with estrogen when taking the pill because estrogen alone will result in an over growth of the uterine lining. I would think that’s where the cancer risk is rooted.
Exactly. We need more studies to understand if certain topical estrogen formulations, when applied locally, lead to enough systemic estrogen absorption to cause that issue.
@@DrSamEllis oh I think they already know that topically it won’t do that. It’s just a risk for systemic estrogen when at the age that you’re taking the pill. I think it’s different when you’re older and able to take other types of HRT.

I’ve been on the hrt patch and oral progesterone. I got CO2 laser. Just life changing. Melasma spots are gone. I sleep like a baby. Hot flashes are gone and I started using weights 2x a week with a trainer via FaceTime. I feel 22 at 60, and I’m glad I did it.
PLEASE tell me how I can get my hands on that estrogen gel you show. I'm 78 years old. Was on compounded estrogens for several years. Lost my doctor. (He lost his license for prescribing hormones and billing insurance companies. Long story). So I went to a regular doctor and asked for a prescription for estrogen and progesterone and she laughed me out of the office. Literally. She said who would prescribe hormones for a 70 year old woman. That was eight years ago. She was older, I thought she'd understand, but no, she was sure it would kill me. So, I found a way to get Estrodial gel from overseas illegally and I bought 3 tubes. I use bio identical progesterone cream from over the counter sources. Now the estrogen gel is almost gone and I don't know what to do. My overseas source has dried up. Doctors won't prescribe it. Interesting -- the last time I saw my primary doctor, who I wouldn't dare tell I use the stuff, he was in shock about my age. He asked me what do I do, I look no more than 40! I told him I take B vitamins. Why can't they just make it so a woman can decide whether she wants to take something. It's not Fentanyl.
There is a dr in Texas who has videos on youtube. Dr. Shawn Tassone. He does tele medicine and, depending on what state you are in, can and will prescribe hormones.

Did not put the dots together! Can you have Angiomas & thin broken blood vessels safely removed? I tried having blood vessels - very very thin veins removed last year but the treatment made it worse not better - any help on this wonderful new aging symtom would be greatly appreciated - I am on tretinion not sure if this has any impact on the treatment.

Women using bio identical hormone replacement therapy are not at greater risk for breast cancer or any other cancer associated with hormones. This is the research! as well, women who have had hormone related breast cancers can do bio identical hormone replacement. This also is the research that has been done.

Perhaps it would be useful to speak about the gut-skin axis. I.e. the impact of poor digestive health, impaired gut barrier or imbalanced in gut microbiome and the effects on the skin

I’m on HRT patch . I also use estrogen cream 2 x a week with a steroid cream on the outside of my vagina for Lichen Sclerosus . I have been on this regiment for 2 years now and I’m 59 . The best decision I ever made for my health. I recently started using it on my face a few nights a week and my neck has improved a lot . I’m excited about the use of estrogen cream applied to the face .
Seemingly overnight as soon as I started skipping periods my skin began to sag and get crepey all over my body. Tests showed I still make estrogen but perhaps it’s started to decline rapidly. I have no other symptoms just the skin changes. It’s just awful. I always looked a decade or more younger before this. Now looking into HRT.
My personal experience: even a tiny amount of estrogen on the face is felt systemically and needs to be balanced by progesterone to feel balanced. Notably breast tenderness.
