Total T not the most important marker. Also SHBG, Free T and symptoms. 450 can be low for a guy that was 1200 in his 20's and 30's. 450 can be high for a guy who has never been over 450. The more complicated issues is receptor count. All clinic should be considering all factors.

I am a 70 year old retired Orthopaedic surgeon with symptomatic low testosterone. Through a referral from a friend I contacted Hone Health for online evaluation. I have been in treatment for a year. I am quite happy with the results. I was seen by a physician initially and every 3 months since. She took a thorough history, counseled me about risks and ordered and reviewed with me appropriate lab tests. My dosage has been adjusted downward and upward based on lab results to keep my testosterone in range.
It is true that they don’t target a value considered normal for my age, but rather a value appropriate for a younger man and, after all, isn’t that the point for a guy my age? I feel, look, think and act act like a middle aged guy rather than like my feeble old friends.
Admittedly, my situation is different from a much younger guy. I am not concerned about fertility and is too late for me to die young.
That said, TRT has been a great thing for me.

    • @billphister
      @billphister 10 місяців тому +1

Absolutely agree. The whole "normal for your age" thing is ridiculous. What matters is the level that alleviates your symptoms. It may be "normal for my age" to have cataracts, but that doesn't mean that I have to accept being blind! We take measures to address the problem. I just don't get why it's any different with testosterone.

I get testosterone shots at a local clinic. But in all honesty, I’ve never seen a doctor there either. Only a nurse practitioner, and even then she’s not the one who administers the shots. She’s the one I check in every three months when I get blood work done.
But I do feel totally confident in her ability, and confident in the clinic itself.

I have been getting TRT at a local "male clinic" (a regional chain) for the past 2 1/2 years, overseen by a PA. I also take clomid and anastrozole to balance the effects of the TRT on fertility (not that it matters at my age) and my estradiol level. I get monthly blood work as part of my subscription fee, including a PSA test, so I am essentially on active surveillance for my prostate as a bonus.
When I started TRT, my testosterone was about 300 ng/dl. It is now in the 600-700 range. I feel great, and my libido is significantly improved. I also take low dose (5 mg) tadalafil daily for BPH symptoms and erectile function enhancement. The whole regime works like a charm.
As other guys have said, they can't find a urologist willing to prescribe testosterone...so these clinics are clearly addressing needs of patients like me, and my experience has significantly improved my quality of life. No offense to Dr. Malik, whose work here proves she cares about these things, but I have not been well served by most of the urologists I have seen over the years...though I think I have a good one now. I see him this Friday...

The biggest problem with the range the medical field uses is that at 35 yrs old 350 is not normal. The range needs to be re-done, and age needs to be considered. Testosterone needs to be considered on a patient by patient basis.

Is it? YES. If u do your research. Remember, you are your most important advocate. Do the legwork. Stop relying on a Dr to be a “know it all.”
I’ve been on TRT for 2.5 years. I started with a local place paying way too much $. Switched to online to save some $$. Then over a year later I found another local dr that cost even less $. The script cost me about $22/month. That’s cash price. Ins did not cover. Ins only covered Dr visit and bloodwork.

After a Dr Phil episode a out Testosterone, I told my psychiatrist about it ( yes, psychiatrist) who tested my levels 3 weeks in a row. Then my pcp prescribed my androgel, didn't work so they gave me injections, and boy, what a diff!!! They never told me about infertility though ( luckily I already had my daughter then). Hey Dr I have a question. It’s weird but I shot the shot. Recently, had a yearly check up. They took my blood and urine test. Everything came out good and they haven’t pointed anything out. They said I’m healthy. I’m glad I am because im only 20 years old. But during night I feel like I have to go pee before I go sleep then 3 minutes later i gtg again and maybe one more time but no I don’t wake up at night or I don’t have problems going during day time. Do you think you could respond to me in the coming. Thank you Dr

What is the difference between say testosterone supplied by a clinic ... and an anabolic steroid, such as Dianabol? I mean if the clinics are not doing due diligence and making sure levels are correct, regular blood tests etc. Aren't they just pushing steroids? But under a medical licence. Or is testosterone replacement completely different? Both seem to have a number of side effects.

Govts should facilitate authorised outlets for supply of testosterone, meeting essential requirements for the benefit of the male population in their respective countries. By doing so, they favour their female population too by affording them a more satisfying sex-life.

She had good information, but showed a strong bias in her reporting by throwing Physician Assistants and Nurse Practitioners under the bus. Some of the best medical care I've and my friends have received has been from very knowledgeable NPs and PAs. All day long I'd rather have a good NP or PA than a poor MD and there seem to be many poor MDs. With that as backdrop, my wife gets her HRT from an online service (Athena Midi health)....its been comprehensive, thoughtful, one on one and very affordable. I've been very impressed. So we all know there are fly by night Testosterone replacement online services but I'd hope there are some good thoughtful guideline practicing ones for men's health just like there are in women's health. But we don't know which ones they tested, but the whole tone seemed to have an agenda, so I wouldn't be surprised if they did not seek out and test the ones with better reputations. Too many MD's seemed threatened by patients seeking solutions to their health problems, which is so ironic in a day when the medical establishment barely gives MD's (and PA/NPs) more that 10 minutes to figure out a persons complex medical issue.

I guess the question is would the results from the online clinics be different from in-person visits? Meaning do the in-person clinics provide the same patients with the same options?

The reason these clinics have exploded is because the medical/insurance industry set the level at 300 discounting everything else, they won't even check free test, SBGH, LH, FSH etc... I have 325, at 270 I can compete as a woman in sports now. I ask my doctor for more complete tests and he said, that is a lifestyle issue and I'm not that kind of doctor. So now I'm looking at going to a clinic...

There was a company pushing their testosterone boost online so I tried the trial. I had to take saliva samples and mail them in & do questionnaires blah blah blah.
So at the end they said my testosterone levels were something like 100.
I was almost 50 and said well that can't be good. I had a normal blood draw scheduled already but asked if they could include a testosterone screen.
Well wouldn't you know the results were somewhere in the low 530 range. Can you tell I don't know how they score it?
Anyway my Dr said my testosterone was fine & if I wanted to add a supplement that was fine, but to be careful and also that I didn't need to. So we discussed my options & what I hoped to gain from it. I'm 54 now, the only supplements I take are vitamins, Super Beta Prostate & A quality Horny goatweed daily.
Honestly I have the same erection now as I had when I was 20 so I guess I'm lucky.
I originally thought I had dysfunction when I was married. We eventually made it to marriage counseling which is where the therapist told both of us my man parts were fine since I had no problem masterbating and that I just didn't like my wife and she had zero sexual prowess. Obviously that was a symptom of a bad marriage but the supplements, exercise, a clear head and the RIGHT WOMAN basically fixed it.
I think a woman can do more damage mentally to a man than most ppl realize, men too but we're talking dysfunction here.
Don't bate or not so much. Be a happier person and find the right partner.

@@johnnysquiretube I went to two doctors before finding one. When I asked them about bc for women they said, "oh yeah, all day." I later found out they were simply fearful of treating men due to controlled substances. The Dr I found works with lots of athletes at all ages. Glad I was referred to him. Two ladies in my girl got test pellets put in by their OBGYN. Both have had great results with it after 6 months.
