  • Strengthening your core may not always be good for your back pain. Surprised to hear this? It is very much true! In fact at times, factors depending, it can contribute to back pain instead of giving you relief. What you need to understand here is that, it is not the exercises itself which cause the damage, but the strategy behind doing them. Confused?     Dear Ryan, let me start by saying that I'm a physiotherapist myself in Belgium. Since a couple of days I've been going through a substantial amount of your video"s now on this channel. The reason is that for almost 4 years I struggle with low back pain and Sciatic like pain in the leg. I woke up with this complaints after a day of biking on a bike which didn't fit me at all (it was from my father in law). So the next day I felt a shooting pain through my front thigh and my groin when I lifted my knee to dry it off after showering. Since that moment I had pain when walking or climbing stairs. Only sitting down made the pain go away. The evolution of all this led to a feeling of instability and spasm of my piriformis muscle and the other pelvic muscles. X-Ray and MRI revealed nothing. Doctors couldn't diagnose the problem. It's only since a week or two that I realise I suffer from a ligament injury of the SI joint. Luckily I found your channel. and working on my core (adressing these muscles is probably easier for me because of my knowledge of anatomy and my general knowledge on this matter). Your analysis of the origin of most low back pain is a real eye opener, especially the content here in the video above about resetting the whole neuromuscular system instead of purely trying to strenghten the core muscles without first correcting the imbalance of the system is mindblowing, even to me as a physiotherapist. Your theory and solutions for this neuromuscular imbalance and lack of connection of the core muscles with the muscles that create movement of the limbs SHOULD BE TEACHED WORLDWIDE TO EVERY MEDICINE AND PHYSIOTHERAPY STUDENT!
    My deepest respect for your work! I started to train myself for two weeks now with this knowledge in mind (I also use the SI belt) and I already I feel improvement. Also your comparison of the SI ligament healing with other ligament injuries has been a real eyeopener for me. I now realise why some forms of stretching I did made the problem worse. We found it deeply insightful, and we're thrilled to hear that you have successfully targeted your abdominal muscles and are feeling as if they are the driving force behind the movement. This is precisely what we hope to achieve, and your assessment is absolutely correct. By engaging your abdominals correctly, you would experience less torque on the left SI joint generated by the iliopsoas. It can be extremely difficult to arrive at a diagnosis when the SI joint is involved, as lower back pain and SI joint dysfunction share many commonalities.
      We're so happy that you never gave up hope for a solution and that led you to Dr. Ryan's teachings. We agree with you; looking at the body as a whole and digging deeper into the root cause of pain and dysfunction is often overlooked, and many modalities hyperfocus on treating the symptom, pain, rather than the cause. We're so glad that you agree with Dr. Ryan's approach and philosophy regarding infant development and SI joint dysfunction stemming from ligament laxity. You're not alone in this, and we find it astounding that we often treat SI joint dysfunction by attempting to mobilize a joint that, by its very design, is meant to stabilize the pelvis.

    This is very timely for me, and gives me some pointers to move forward.
    I signed up for 2 months (up to & including Module 9) of the Core Balance Training, and worked faithfully through the first several modules - but increasingly found it was getting beyond my capacity. Including the bit about going for a 30 minute walk every day!
    I started this program with no back pain, but severe hip arthritis and some corresponding muscle imbalances from compensating for lack of hip articulation, and also from too much sitting. I have some pain in my legs which seriously limits my walking, and even standing to some degree. It's hard to tell what is because of the hip arthritis (there is some real inflammation around my hip joints) and what is due to muscle & fascia issues - but both do apply, I know.
    I found many of the exercises for developing core awareness very helpful, and this has given me back some movement function that was deteriorating. However, I did develop some lower back pain that had not been there before (especially with that floor front swimming exercise - the front anchor challenge), and this was a bit of a deterrent!
    However, that pain is clearly not around the joints so should be fixable, with a bit of work on muscle balancing.
    I guess I need to go back through my notes and try to figure out which parts I need to spend more time on...

      Reassessing and going back through your notes, as well as taking the time to identify specific areas that may need more attention, is a great approach. If you can take it slow and recruit the intrinsic stability muscles, you can change your relationship with the deep core.

    Such good information. Only the people that experienced debilitating back pain for a prolonged amount of time will really understand this and a lot of the other info you put out.

    Don't know where to begin, my lower back is a problem, my knees, my neck and my shoulders. It seems like there is now return to a better day, can't find the right help where i live, also i have develop anxiety, feel like i'm caught😖

      Am badly off almost like you but me also the ribs of the right. But I advise you to pray more as well you can go and see a rheumatologist

    Repeated Dead Bug exercise, crunch, cycling in supine position ( which may tightened abdominal Core muscles) may be one of main cause of Posterior Pelvic Tilt/ Flat back syndrome?

Repeatedly performing exercises that involve abdominal muscle engagement, such as the Dead Bug exercise, crunches, and cycling in a supine position, may contribute to muscle imbalances and potentially affect pelvic alignment. However, it's important to note that the relationship between these exercises and the development of Posterior Pelvic Tilt or Flat Back Syndrome is complex and can be influenced by various factors. Overemphasis on abdominal exercises without adequate attention to the muscles on the posterior chain (lower back, glutes, hamstrings) can create muscle imbalances. Imbalances between the anterior (front) and posterior (back) muscles of the core can influence pelvic tilt, which may, in turn, possibly affect the spine. I hope this is helpful.     As for me the pain is complex. Right leg around the curf muscle through the knee, sacroiliac joint, right ribs, rt shoulder pain, right neck pain bellow the ear and right headache through the mastoid bone. The pain cannot be relieved by any pain killer
