      If knee pain is truly due to RA I would first recommend speaking with a rheum to address the underlying disease process as oral agents may be required.

    This is the second one of your videos I've watched about after-care of an injection - cortisone, I think. With the first one I was surprised at the details and realized that was no simple shot. But it was helpful after my own (shoulder) injection, because I didn't get that level of instruction from my doc. I wasn't surprised to find the same level of detail here for a different kind of treatment. Even though I haven't had PRP I am thinking it might a consideration later on (for a different joint!) so it's good to know what to expect. Thank you.

      For thumb arthritis would you recommend the push splint over the full thumb spica. Why would you choose one over the other?

      @@bobmini9740great question. I really recommend patients going to a physio/sports med/ortho clinic or bracing clinic to purchase. They tend to have a better selection and can properly fit you. When you buy online you may not be getting the right product for you.

      @@LockhartFamily-z5n If you just have CMC arthritis, I would start with the push splint since its more minimal and can be quite effective. If you find that you need more support afterwards, then I would move to a thumb spica splint.

      This injection can be done for nerve pain in the back. However, in my personal experience, I find that back injections may not be as effective as those done in other parts of the body. Unfortunately, back in nerve pain is multifactorial and because of this a simple injection may not address everything

      To confirm what is being injected, you would just need to check the product box. This will give you the formulation being used.

      Depending on the type, size, and location of the tear PRP does have the potential of helping the tear scar down. However, if there are significant mechanical symptoms, in those cases, the meniscus is truly getting stuck as the knee is moving and surgery may be required.

    Wonderful videos! Thank you for making the time to produce them. There are many HA options available and one question not addressed is which has proven to be the most effective. Synthetic vs. avian, Synvisc, Synvic One, Supartz, Orthovisc, Hyalgan, Euflexa and in combination with Cortisone or not would be quite beneficial to patients considering viscosupplementation for Chondromalacia patella.       That’s a really good question. And it’s really hard to discuss the differences between each brand because some of the ones that you offered aren’t available in all countries. Many physicians have their preferences as to which ones work better. Synthetic versus avian is another consideration. Many people prefer the synthetic versions, as they’re vegan, friendly, and also less irritating. When it comes to brands, you have to look at the molecular weight of the structure. Too high or too low of a molecular weight can also be irritating to a joint. The other thing to consider is the disease process in the joint. The more advanced a disease, the less effective the injections can become. There is some evidence that combining them with Cortizone increases their effectiveness. There’s actually one formulation available in Canada, which has a premix with this. However, the take home point is that there is high variability, and if you speak to each company, they will tell you why their product is the best.

      Im not fully understanding the question. Usually lidocaine and depomedrol can be separately injected, however combination formulations may exsist.

      I don’t personally do that. You can stack PRP and HA in a short time period between each other. I would prefer to do one injection at a time.

    I had PRP today on my left knee and it was really painful, I cried a lot, the Doctor said I’m only allowed to take Tylenol for pain and to assist to my PT appointment tomorrow even if it hurts too much :(

    Hi Dr Sonam , my mom is 87 yrs old and had severe OA of the both knee joints with degenerated medial menisci ( KL classification grade 4 ) having pain in the knees while walking . Will PRP injection helpful to reduce the pain ?

      PRP is less commonly indicated in KL grade 4. Its more effective in mild OA, even moderate OA. However, in bone on bone, it is really a shot in the dark, and not indicated. At that point one would be thinking about surgical options.

    Good videos and expertise.....I got my injections (one each knee ) 2 months ago and they are working just ok. I have also just started hyaluronic acid topical cream applications as my home remedy. Your thoughts on if this will help supplement the injections? I am doing 2 applications a day for a week, one week off and another week of 2 a day. I just finished the first week.

      Ha topical cream will likely be beneficial for the skin but unlikely penetrate the joint.

    Great Channel ! Yes I have a question. I just got a steroid shot into the joint at the base of the thumb .I searched for information on the steroid shot and this channel dealt with the before , after and the Why of this treatment. Very valuable content! My question comes from something my doctor said.* Disclaimer *I know you can't not speak for my or all doctors and honesty the shot hurt, and I was pretty distracted, so I did not respond and ask him if this was possible at the time. He seemed to imply that there is the possibility that this shot will not be needed and may have something to do with the progression of the disease is there some feature in Osteoarthritis where the pain receptors "quiet down" and that the disease may still be present, but the pain stops?

    Just back from meeting with doctor. Considering PRP for tennis elbow. Pain is not that bad but know something is off. There is a tear. He is saying he will give me OxyContin which seems like a strong addictive pain reliever for this. Thoughts?

      Its not uncommon to give a small dose of high strength pain relievers for a few days. Many patients may not require it, but tendon PRP can be painful which is why its nice to send people home with something incase you need it. However, if you don't require it then thats great as well. Patients just cannot take NSAIDS which is why this is offered.

      It can be done. Likely will be addressing more of the underlying osteo. It will not be effective for treating the inflammatory RA (which usually needs oral pain meds/biologics to do so). However, I have treated patients with RA with PRP.

    I have had 4 PRP injections since 2022 in the shoulder for impingement/bursitis, rotator cuff supraspinatus & infraspinatus tendinopathy, articular surface fraying and glenohumeral OA. It helps for a while but the pain always comes back. My question is; Does PRP help regenerate? I am on antibiotics for urology issue for months, will taking antibiotics interfere with the PRP?

      PRP can regenerate tendon, but it does not regenerate cartilage, it can merely stabilize it. That is why you are likely seeing pain return. However, tendons can be regenerated/healed with PRP and in those cases only a few treatments (or one) may be required.

    Loved this video. I recently injured a muscle in my hip flexor (tear) from barrel riding in surfing. And well leg day, back flips flat and a few other things, which I didn’t let my body recover properly. Hoping it is a grade 1-2… love the insight on this video cheers.

      It can be, some do need repeats as well. I tend to evaluate this on a case by case basis.

      This will not treat the underlying cause of RA. Usually this addresses the local cartilage damage. If you have some OA in the joint, then yes.

      I recommend a minimum of 8 weeks to have the cortisone washed out from the area before doing PRP

    Thank you for your advice. Today, I just had PRP injections in my left knee for torn meniscus and arthritis and my post op direction was consistent with yours EXCEPT ONE thing: you recommend against any heat but my Dr encourages it via hot moist towels or heating pad on the joint. Can you please provide any more insight on your recommendation? Thanks again!

      Heat is likely okay. Ice is the big thing you want to avoid since it’s anti inflammatory and we don’t want that.

    I’m a person who consumes alcohol regularly. So after my first PRP injection to my shoulder I was told no alcohol by my doctor. But I wish to know for how many days do I resist alcohol after injection and also my regular exercise routine?

      Alcohol makes your blood thinner and induces even more inflammation. This is why we ask patients to avoid it prior and a short period of time after. The other thing is alcohol honestly has no positive effect on the body and is really slightly toxic at any dose, but that is another issue/concern entirely.

    Thanks for the great information! I had a PRP shot in my hip for arthritis 4 days ago. I am a dance teacher and was hoping to do some stretches and PT starting next week and then to go back to teaching after 10 days. Will I ruin the effect of the shot if I start exercising too soon? How do I know when it’s ok to do more? Thanks!
