I got 2 injections of PRP in my hip for labral tear. First injection helped and was sore for 2-3 days then gradually walking more and more. The second injection I got severe pain and swelling that increased and by day 5 landed me in the ER. No one has been able to tell me what caused that. They did MRI and cultures and thankfully no infection came up
This is the second one of your videos I've watched about after-care of an injection - cortisone, I think. With the first one I was surprised at the details and realized that was no simple shot. But it was helpful after my own (shoulder) injection, because I didn't get that level of instruction from my doc. I wasn't surprised to find the same level of detail here for a different kind of treatment. Even though I haven't had PRP I am thinking it might a consideration later on (for a different joint!) so it's good to know what to expect. Thank you.

I have been suffering from a rt knee pain for about a year and just recently diagnosed with knee osteoarthritis. The pain and the restrictions on my mobility has really affected me both mentally and emotionally. Both OTC and prescriptionN NSAIDs have not been very helpful. You’re going to read a lot of things about PRP online. I am very honest with my patients that this is not 100% effective in all patients. Patient selection is key. I tell people it is neither snake oil, nor the elixir of life, but sits inbetween. The vast majority of my patients that I perform this treatment on do show levels of considerable improvement. But this is after many years of experience, and knowing which patients would likely benefit from the procedure. Achilles tendonapathy can be extremely difficult to treat. However, I find that tendon injuries tend to respond better to PRP then osteoarthritis or joint issues.

I go to the VA .. and i take a lot of medicine.. I just happened to check my medications 1 by 1 myself.. and about 2/3s had a negative reaction with prp.. but I was only told about the pain reliving meds . Anxiety and sinus meds also were said to have a negative affect. Also for my PRP to my hip.. this doctor went in straight through my hip through all of the muscles and things.. the pain kicked in about 3 hours later. And it was terrible.. I could barely breathe when I had to walk.. the pain was a solid 8 for nearly 20 hours.. I had the shakes it was so bad..
Wondering if your doc used ultrasound to guide the injection? I've heard the VA's really support and pay for this b/c it helps get troops active. I wish our health care agencies would cover the cost. Saving my pennies!
Same experience…had MRI guided high dose PRP to my hip and groin for tendon tear, scar tissue and bone spur. It all went well during procedure but the next day I was writhing in excruciating pain. It was like continuous spasms of stabbing pain for two days. I thought I would pass out. Even the strong pain med I was given didn’t help much. By third day, pain was back to tolerable.

Within the last few weeks, I’ve had PRP (double spin) on the Achilles, knees, elbows, shoulders (I do MMA so you can imagine). The golfers elbow seems to have improved, but nothing so far with the rest. I can do as many rounds as I want so why not. But I’m a little less hopeful than I was initially.
Where did they inject you? I had one the other day for the first time. For starters, they withdrew 15ccs of fluid off of my vastus lateralis just above the knee. Then they injected me in a similar nearby area with my PRP. They didn't inject me in the actual meniscus tear like I thought they were going to🤷♀️
I might have done a little too much of the exercises my doctor supplied, after the injection. I now have more pain in my knee joint, than before the procedure. (osteoarthritis) My question is I hope I did not “ruin” any of the PRP treatment? And I assume I should quiet the joint down and rest before continuing the simple leg movements?

Could you please advise whether it is advisable to do knee exercises to distribute the PRP injection inside the joint right after the injection for a medial meniscus tear? One orthopedic surgeon told me to sit on a high table and swing my knee back and forth five times. However, another orthopedic surgeon said that this should not be done and that the knee should be kept still to ensure that the injection remains in the area of the lateral meniscus, as otherwise, with the swinging movements of the knee, the plasma will spread throughout the joint instead of staying strictly in the area of the lateral meniscus where the injury occurred.
I honestly believe that even if you stay still the injection will be moving around the knee joint. I advise people to gently move their knees in the days following the injection to distribute the PRP and improve blood flow to the joint.
@@douss11 It’s been just over 1 month. Pain is still there, but not as much. There is some things I can do than before, like brush my teeth. Shaving is still a issue. My specific issue is turning my wrist a certain way. I was told I have multiple tears in my shoulder, 50%. I believe I caused this injury doing pull-ups, palms facing each other. Bringing my arm toward my shoulder in that same position at rest, or wrist turned a little more clockwise hurts still. Any sudden, fast twitch movement, it’s a very intense pain. I was told not to do anything until my next doctor visit which is in a few weeks. No therapy, or nothing. I think my progress will be slow. Hopefully it’s slow, than surgery. I’ll give an update after the visit on what’s next.
yes I ask my patients to avoid anti-inflammatories, because it will block the PRP process. We want a certain degree of inflammation during the treatment.

Thats amazing. Some people notice benefits much sooner than others. I've had people tell me they feel good 1-2 weeks after, and others say it takes 3 months. The usual however is 8-12 weeks.
I recently had one ultrasound guided PRP injected into my right posterior tibial tendon after 3.5 years of unsuccessful treatment with conventional therapies (brace, rest, physical therapies). PRP injections helped my hip arthritis and labral tear 2 years ago. Really helped but now the hip pain has returned 24 months later. The injections into both hips were extremely painful, Do u think I can find a doctor to give me general anesthesia? I can’t take the pain of deep injections again but I benefitted from the PRP injections. Comments?
It's not routine to do these injections under GA, at least not commonly in Canada. It's not usual for people to experience much pain with the injection, rather I find pain can get worse afterwards for a few days.
I am going in for a consult this afternoon.. I've had medial epicondylitis for 2 yrs - two. 6-week rounds of PT, 2 steroid injections (2 years apart), and the pain is worse than it's ever been. I've been reading about PRP vs. PRP+stem cells (and, perhaps, adipose tissue?) But I recently listened to a video of a Dr. talking about multiple studies that show PRP ($) is just as effective as PRP+Stem cells ($$$). What is your experience/opinion?

While the research is not clear, Plavix tends to be more of an issue because it affects platelet function. Theoretically, if we’re looking at platelets, and you are on a medication that affects platelet function, it can affect the quality of PRP. This would have to be an individual discussion with your provider. Well, it may not be an absolute contraindication, each provider will have their own thoughts about this.
It’s not a strength so much as the number of cc of final product put into the knee. Another consideration if there is a single or double spin system that was concentrate the blood more.

2 weeks ago had a tenex procedure on medial side of elbow for scar tissue and calcium deposits. One week ago I had PRP on lateral side for torn tendon. The doctor also injected the PRP in the joint as he indicated I had a lot of arthritis/inflammation there. The swelling and pain has subsided though it is still tender. Hot tub acceptable after 2 weeks since PRP? He indicated 6-8 weeks before I can get back to weights and golf because of injecting into the joint as well as the tendon? Sound accurate? Appreciate the kind words. Yes I have. I do them quite a lot for hip OA and even just some hip pain due to mild arthritis or hypermobillity/labral tears.
I have had 4 PRP injections since 2022 in the shoulder for impingement/bursitis, rotator cuff supraspinatus & infraspinatus tendinopathy, articular surface fraying and glenohumeral OA. It helps for a while but the pain always comes back. My question is; Does PRP help regenerate? I am on antibiotics for urology issue for months, will taking antibiotics interfere with the PRP?
PRP can regenerate tendon, but it does not regenerate cartilage, it can merely stabilize it. That is why you are likely seeing pain return. However, tendons can be regenerated/healed with PRP and in those cases only a few treatments (or one) may be required.
It can be done. Likely will be addressing more of the underlying osteo. It will not be effective for treating the inflammatory RA (which usually needs oral pain meds/biologics to do so). However, I have treated patients with RA with PRP.

I work with the physiotherapist to help patients develop a specific exercise prescription. I would encourage you to reach out to your provider to see who they recommend.
You will need an assessment first to determine if you are a candidate for injections. Not all thumb and shoulder pain may require this. A history and physical exam will need to be conducted for your personal concerns.
Excellent video. I had my PRP into my knee joint and my doc gave me no aftercare notes or instruction. I am 4 days in a feeling no pain at the injection site, can I get back into my regular workout routine and golf? I am still unclear even after watching the video on whether this could damage the results of the PRP.
This is an individualized question based on your pre-injection issues and how severely injured the tendon was prior. My personal recommendations for my patients vary case by case.

Thank you for posting these videos, as they are truly educational! Would PRP likely reduce further damage and pain associated with hallux limitus / rigidus? Currently I have a painful grinding / grating sensation and limited mobility in my hallux joint. My podiatrist has suggested a cortisone injection and eventual fusing of the joint, to address arthritis pain, but I do not want to lose the mobility in my foot (as I am very active, climbing ladders, swimming, hiking, etc.). I feel that the grating / grinding sensation could be caused by a bone spur (or something similar in the joint) which, if removed, might reduce further damage to the joint, and allowing PRP to be more effective. Would this combination of surgery and PRP likely provide the best long-term option for active use of the joint?

Any anti-inflammatory supplement can theoretically affect the quality of PRP. How much, its hard to say. If you want to play it safe, its worth avoiding before and after.

Thank you! This is a difficult question to answer without knowing age, the type of tear, and the mechanism of injury. Usually smaller tears in the vascular portions of the meniscus and younger patients tend to heal and settle down quickly. On the other hand, bucket handle tears and those tears in older patients with OA may be less inclined to settle down with conservative treatment. The other thing to consider is if someone has mechanical symptoms, like locking, catching, and buckling. In some cases, those symptoms may require surgery to settle down. On average about 70% of meniscus injuries can settle down on their own, but like I said MRI findings, and patient demographics are really important.
@@SonamMD I am 48/M with a very active lifestyle. The (to be confirmed by MRI) tear limited the range of motion on my right knee towards both ends by a little - maybe 5 degree or so, however it didn't cause any locking catching type of thing. But it is painful, especially after longer runs, turns during a run, standing up after sitting for a while. I'm considering PRP injection, your video is great, useful and easy to understand.
I’d start with PRP personally. Both options have their benefits. Sometimes my patients want to use their drug coverage so we start with HA. But I do prefer to start with PRP

Can be done. As long as their platelet and fibrin counts are within range. Have to do a full medication review prior to considering an injection in this patient population.
I have meniscus tear on both knees, was doing alot of rehab with so so response.. I have prp scheduled next week. I have been struggling with not having my strength flexibility and range of motion...wanted to know realistic expectations.
Good luck with your procedure. Definitely speak with your Doctor Who is performing the procedure, they can discuss realistic expectations with you in accordance to your pre-procedure exam and history.

Hi Dr Sonam , my mom is 87 yrs old and had severe OA of the both knee joints with degenerated medial menisci ( KL classification grade 4 ) having pain in the knees while walking . Will PRP injection helpful to reduce the pain ?
PRP is less commonly indicated in KL grade 4. Its more effective in mild OA, even moderate OA. However, in bone on bone, it is really a shot in the dark, and not indicated. At that point one would be thinking about surgical options.
Adductors can be tricky but yes prp can also be considered in this area. It’s extremely important to make sure you work on an adductor strengthening program as this can be an area easily reinjured
Husband had first of 3 injections this one was the anti-inflammatory one and my husband is in more pain now than he was before... it has been one week and he can hardly walk.
Hang in there. The area has purposefully been inflamed with platelets. Do NOT take meds. Pain will go away in a few days. PRP really worked for me. Hang in there. No meds or you will ruin the treatment!

Just back from meeting with doctor. Considering PRP for tennis elbow. Pain is not that bad but know something is off. There is a tear. He is saying he will give me OxyContin which seems like a strong addictive pain reliever for this. Thoughts?
Its not uncommon to give a small dose of high strength pain relievers for a few days. Many patients may not require it, but tendon PRP can be painful which is why its nice to send people home with something incase you need it. However, if you don't require it then thats great as well. Patients just cannot take NSAIDS which is why this is offered.

I have been experiencing intense lower back pain for the past one year. I have a bulging disc with thecal sac indentation at L2/L3 and L3/L4 and herniated disc at L4/L5. I had an MIS discectomy at L3/L4 and L4/L5 and the pain at those levels seems to have subsided. However, I still have excruciating pain as well as muscle inflammation at L2/L3, which is worsened by sitting, all of which is on the left side. I have started doing softwave therapy but not sure how much that is helping me, and PT/acupuncture didn't really seem to help me at all. I am scheduled for intradiscal PRP next week, so I am hoping I can get some much needed pain relief. Have you seen significant improvements with patients that have bulging discs with thecal sac indentation?

Make SURE it is a 30 ml tube. Make sure you are in PT while healing. I have excellent success with this method. Jeffrey Peng is great here

Anxiety and sinus meds also were said to have a negative affect. Also for my PRP to my hip.. this doctor went in straight through my hip through all of the muscles and things.. the pain kicked in about 3 hours later. And it was terrible.. I could barely breathe when I had to walk.. the pain was a solid 8 for nearly 20 hours.. I had the shakes it was so bad..
Wondering if your doc used ultrasound to guide the injection? I've heard the VA's really support and pay for this b/c it helps get troops active. I wish our health care agencies would cover the cost. Saving my pennies!

Same experience…had MRI guided high dose PRP to my hip and groin for tendon tear, scar tissue and bone spur. It all went well during procedure but the next day I was writhing in excruciating pain. It was like continuous spasms of stabbing pain for two days. I thought I would pass out. Even the strong pain med I was given didn’t help much. By third day, pain was back to tolerable.
Appointment at 2:00 pm today for PRP for hip!

Heat is likely okay. Ice is the big thing you want to avoid since it’s anti inflammatory and we don’t want that.
Within the last few weeks, I’ve had PRP (double spin) on the Achilles, knees, elbows, shoulders (I do MMA so you can imagine). The golfers elbow seems to have improved, but nothing so far with the rest. I can do as many rounds as I want so why not. But I’m a little less hopeful than I was initially.

Where did they inject you? I had one the other day for the first time. For starters, they withdrew 15ccs of fluid off of my vastus lateralis just above the knee. Then they injected me in a similar nearby area with my PRP. They didn't inject me in the actual meniscus tear like I thought they were going to🤷♀️

I might have done a little too much of the exercises my doctor supplied, after the injection. I now have more pain in my knee joint, than before the procedure. (osteoarthritis) My question is I hope I did not “ruin” any of the PRP treatment? And I assume I should quiet the joint down and rest before continuing the simple leg movements?

Could you please advise whether it is advisable to do knee exercises to distribute the PRP injection inside the joint right after the injection for a medial meniscus tear? One orthopedic surgeon told me to sit on a high table and swing my knee back and forth five times. However, another orthopedic surgeon said that this should not be done and that the knee should be kept still to ensure that the injection remains in the area of the lateral meniscus, as otherwise, with the swinging movements of the knee, the plasma will spread throughout the joint instead of staying strictly in the area of the lateral meniscus where the injury occurred.
I honestly believe that even if you stay still the injection will be moving around the knee joint. I advise people to gently move their knees in the days following the injection to distribute the PRP and improve blood flow to the joint.
I have an appointment tomorrow for my shoulder. I’ll try to remember to give an update.

@@douss11 It’s been just over 1 month. Pain is still there, but not as much. There is some things I can do than before, like brush my teeth. Shaving is still a issue. My specific issue is turning my wrist a certain way. I was told I have multiple tears in my shoulder, 50%. I believe I caused this injury doing pull-ups, palms facing each other. Bringing my arm toward my shoulder in that same position at rest, or wrist turned a little more clockwise hurts still. Any sudden, fast twitch movement, it’s a very intense pain. I was told not to do anything until my next doctor visit which is in a few weeks. No therapy, or nothing. I think my progress will be slow. Hopefully it’s slow, than surgery. I’ll give an update after the visit on what’s next.

I had a PRP injection in my hip 7 days ago for gluteal tendonopathy. I mostly rested and it started feeling good so I played a hockey game tonight. My hip felt better than it has in years and years. Is this normal?
Thats amazing. Some people notice benefits much sooner than others. I've had people tell me they feel good 1-2 weeks after, and others say it takes 3 months. The usual however is 8-12 weeks.

I recently had one ultrasound guided PRP injected into my right posterior tibial tendon after 3.5 years of unsuccessful treatment with conventional therapies (brace, rest, physical therapies). It’s been 4 weeks and I am not experiencing any relief from the procedure, any suggestions. My condition is bi-lateral and therefore may require injections into the left tendon

It’s not a typical treatment done for CRPS. This is a complex neurological hypersensitivity and needs to be treated as such.

PRP injections helped my hip arthritis and labral tear 2 years ago. Really helped but now the hip pain has returned 24 months later. The injections into both hips were extremely painful, Do u think I can find a doctor to give me general anesthesia? I can’t take the pain of deep injections again but I benefitted from the PRP injections. Comments?
It's not routine to do these injections under GA, at least not commonly in Canada. It's not usual for people to experience much pain with the injection, rather I find pain can get worse afterwards for a few days.
I am going in for a consult this afternoon.. I've had medial epicondylitis for 2 yrs - two. 6-week rounds of PT, 2 steroid injections (2 years apart), and the pain is worse than it's ever been. I've been reading about PRP vs. PRP+stem cells (and, perhaps, adipose tissue?) But I recently listened to a video of a Dr. talking about multiple studies that show PRP ($) is just as effective as PRP+Stem cells ($$$). What is your experience/opinion?

Yes it can. It’s actually quite a common reason will we treat with prp. These areas can be quite tricky to treat as a whole.

While this good be done, this would need to be at the recommendation of your surgeon. Some are okay with PRP after surgery, others avoid it.

While the research is not clear, Plavix tends to be more of an issue because it affects platelet function. Theoretically, if we’re looking at platelets, and you are on a medication that affects platelet function, it can affect the quality of PRP. This would have to be an individual discussion with your provider. Well, it may not be an absolute contraindication, each provider will have their own thoughts about this.

It’s not a strength so much as the number of cc of final product put into the knee. Another consideration if there is a single or double spin system that was concentrate the blood more.

2 weeks ago had a tenex procedure on medial side of elbow for scar tissue and calcium deposits. One week ago I had PRP on lateral side for torn tendon. The doctor also injected the PRP in the joint as he indicated I had a lot of arthritis/inflammation there. The swelling and pain has subsided though it is still tender. Hot tub acceptable after 2 weeks since PRP? He indicated 6-8 weeks before I can get back to weights and golf because of injecting into the joint as well as the tendon? Sound accurate? I have had 4 PRP injections since 2022 in the shoulder for impingement/bursitis, rotator cuff supraspinatus & infraspinatus tendinopathy, articular surface fraying and glenohumeral OA. It helps for a while but the pain always comes back. My question is; Does PRP help regenerate? I am on antibiotics for urology issue for months, will taking antibiotics interfere with the PRP?
PRP can regenerate tendon, but it does not regenerate cartilage, it can merely stabilize it. That is why you are likely seeing pain return. However, tendons can be regenerated/healed with PRP and in those cases only a few treatments (or one) may be required.

It can be done. Likely will be addressing more of the underlying osteo. It will not be effective for treating the inflammatory RA (which usually needs oral pain meds/biologics to do so). However, I have treated patients with RA with PRP.

You will need an assessment first to determine if you are a candidate for injections. Not all thumb and shoulder pain may require this. A history and physical exam will need to be conducted for your personal concerns.
Excellent video. I had my PRP into my knee joint and my doc gave me no aftercare notes or instruction. I am 4 days in a feeling no pain at the injection site, can I get back into my regular workout routine and golf? I am still unclear even after watching the video on whether this could damage the results of the PRP.
This is an individualized question based on your pre-injection issues and how severely injured the tendon was prior. My personal recommendations for my patients vary case by case.

Would PRP likely reduce further damage and pain associated with hallux limitus / rigidus?
Currently I have a painful grinding / grating sensation and limited mobility in my hallux joint. My podiatrist has suggested a cortisone injection and eventual fusing of the joint, to address arthritis pain, but I do not want to lose the mobility in my foot (as I am very active, climbing ladders, swimming, hiking, etc.). I feel that the grating / grinding sensation could be caused by a bone spur (or something similar in the joint) which, if removed, might reduce further damage to the joint, and allowing PRP to be more effective.
Would this combination of surgery and PRP likely provide the best long-term option for active use of the joint?

Any anti-inflammatory supplement can theoretically affect the quality of PRP. How much, its hard to say. If you want to play it safe, its worth avoiding before and after.

Thank you! This is a difficult question to answer without knowing age, the type of tear, and the mechanism of injury. Usually smaller tears in the vascular portions of the meniscus and younger patients tend to heal and settle down quickly. On the other hand, bucket handle tears and those tears in older patients with OA may be less inclined to settle down with conservative treatment. The other thing to consider is if someone has mechanical symptoms, like locking, catching, and buckling. In some cases, those symptoms may require surgery to settle down. On average about 70% of meniscus injuries can settle down on their own, but like I said MRI findings, and patient demographics are really important.

@@SonamMD I am 48/M with a very active lifestyle. The (to be confirmed by MRI) tear limited the range of motion on my right knee towards both ends by a little - maybe 5 degree or so, however it didn't cause any locking catching type of thing. But it is painful, especially after longer runs, turns during a run, standing up after sitting for a while.

I’d start with PRP personally. Both options have their benefits. Sometimes my patients want to use their drug coverage so we start with HA. But I do prefer to start with PRP

Can be done. As long as their platelet and fibrin counts are within range. Have to do a full medication review prior to considering an injection in this patient population.
I have meniscus tear on both knees, was doing alot of rehab with so so response.. I have prp scheduled next week. I have been struggling with not having my strength flexibility and range of motion...wanted to know realistic expectations.

Theoretically, I'd advise against cold plunges for 4-5 days because of its anti-inflammatory effects (not just infection risk).

I had PRP today on my left knee and it was really painful, I cried a lot, the Doctor said I’m only allowed to take Tylenol for pain and to assist to my PT appointment tomorrow even if it hurts too much :(

This will not treat the underlying cause of RA. Usually this addresses the local cartilage damage. If you have some OA in the joint, then yes.

PRP is less commonly indicated in KL grade 4. Its more effective in mild OA, even moderate OA. However, in bone on bone, it is really a shot in the dark, and not indicated. At that point one would be thinking about surgical options.
Dose this help with a adductor longus strain grade 2
Adductors can be tricky but yes prp can also be considered in this area. It’s extremely important to make sure you work on an adductor strengthening program as this can be an area easily reinjured
Husband had first of 3 injections this one was the anti-inflammatory one and my husband is in more pain now than he was before... it has been one week and he can hardly walk.
Hang in there. The area has purposefully been inflamed with platelets. Do NOT take meds. Pain will go away in a few days. PRP really worked for me. Hang in there. No meds or you will ruin the treatment!

Just back from meeting with doctor. Considering PRP for tennis elbow. Pain is not that bad but know something is off. There is a tear. He is saying he will give me OxyContin which seems like a strong addictive pain reliever for this. Thoughts?
Its not uncommon to give a small dose of high strength pain relievers for a few days. Many patients may not require it, but tendon PRP can be painful which is why its nice to send people home with something incase you need it. However, if you don't require it then thats great as well. Patients just cannot take NSAIDS which is why this is offered.

Yes. But I will space them out a few weeks apart. I will not do them in the same session.

Yeup I have done it in this area, but it can be quite painful I do warn people. Results are pretty promising as well.
I’m having PRP tomorrow for hip labrum tear, when can I resume running again?
I have been experiencing intense lower back pain for the past one year. I have a bulging disc with thecal sac indentation at L2/L3 and L3/L4 and herniated disc at L4/L5. I had an MIS discectomy at L3/L4 and L4/L5 and the pain at those levels seems to have subsided. However, I still have excruciating pain as well as muscle inflammation at L2/L3, which is worsened by sitting, all of which is on the left side. I have started doing softwave therapy but not sure how much that is helping me, and PT/acupuncture didn't really seem to help me at all. I am scheduled for intradiscal PRP next week, so I am hoping I can get some much needed pain relief. Have you seen significant improvements with patients that have bulging discs with thecal sac indentation?
