Are Avoidant Personality Disorder and Social Anxiety Disorder Different?

  • This video answers the question: Are avoidant personality disorder and social anxiety disorder actually different disorders? There's a debate in the mental health treatment community about these two disorders and sometimes it's really not a matter of how are they different it really, it is a matter of people that believe that one of these must not exist. I think the one that gets targeted more often, of course, is avoidant personality. In previous videos have also discussed how we have to use the research sometimes on social anxiety disorder to draw inferences about avoidant personality, because there is not a lot of research on avoidant personality disorder. This just speaks to the same challenge: are these two disorders really separate? I'm going to revisit this question but from a slightly different angle. I'm not going to go through the item-level analysis of the symptoms like I've done before, but rather give an overview of these two disorders and an overview of where we are in terms of our understanding. With social anxiety disorder there are many symptom criteria like anxiety and avoidance in social situations. With social anxiety disorder this may be specific, there's even a specifier “performance only” that's for performing or speaking in public. The idea here is that social anxiety disorder is not conceptualized as the manifestation of extreme personality traits, rather the symptoms of this disorder can come and go. They're not really necessarily stable over time as I indicated. We could think of a short version to remember what social anxiety disorder is all about is really something like “performance in social situations.” Avoidant personality disorder is conceptualized as more stable, it's a maladaptive pattern like we see with all personality disorders. The pattern here is one of social inhibition, so we see a lot of different emotions and feelings associated with social situations like a fear of being rejected, criticized, embarrassed, feelings of inadequacy, and being inept or inferior. This is quite similar to social anxiety disorder, but from a different angle. If we're trying to sum up what avoidant personality disorder is, we would say there's difficulties here in forming relationships. We could easily remember the main difference between these disorders as forming versus performing, difficulty in forming relationships with avoidant personality disorder and difficulty performing in social situations with social anxiety disorder.

    I have avoidant personality disorder, although on my UA-cam channel I talk about social anxiety because that’s what I was officially diagnosed with.
    It’s exhausting to live with, constantly running and avoiding. Nearly impossible to make myself open to connecting and impossible being vulnerable around people. The social anxiety aspect of AVPD is never ending no matter if it’s friends or close family. I feel afraid of being seen especially my face ( could be symptoms of facial dysmorphia, because of my fear of my severe anxiety seeming so transparent in my facial expressions) so often times I strategize how to avoid making contact with others

    I got misdiagnosised as APD and SAD after an appointment with a SAS doctor after 2 years of waiting lists; and then that ruined the next 2 years of my life, it gave the nurses an excuse when i said "that's not what i think, you are putting words into my mouth" when I said "CBT wasn't working" they said i was lieing beacuse im being avoidant...that I wasn't allowing the treatment to work, they blocked my referrals for second opinion. This conflict cause me so much selfdoubt I was going to kill myself, all of which re-enforced thier belief in a PD. Medics have a duty to look deeper into your patients and listen to them instead and of giving surface symptom diagnosises. The more i read and study the more I believe PDs may not exist... Maybe they do, but too many people are given them at first instance instead of last instance; after all other conditions are ruled out. I went to see a private doctor and got a ADHD diagnosis... hyperactivity can be inside the brain, you can be a very shy and quiet person (something no GP tells you)... The correct drugs helped my social anxiety immediately, I could go out and deal with my low self-esteem, thoughts didn't get suck in my head, I stopped be bothered as much by people and cars, I was talkative instead of mute... It's like I have armour instead of being naked.
    If you get a PD diagnosis, please check that CPTSD, ASD, ADHD, physical causes etc... are ruled out. Most medics have misconceptions about neurological conditions, and lack knowledge.

    I like it very much you made an update about this topic. It's great Dr. Grande to see you making so much videos about mental health disorders and PD's, with each time a specific question and what research has been done in this matter and what we can learn from this.
    You have a gift to tell actually very much interesting knowledge about a 'difficult' topic in less then 10 minutes. I'm so much pleased with your UA-cam channel.

    Social anxiety I think is being awkward and nervous talking to others.
    Avoidant is being terrified to the point of hiding behind a tree if you hear a car coming when you are taking out the trash, if you are delivering mail and there are people outside, pulling up to mailbox in a way so that they can't see your face, scanning a parking lot at grocery store to see if you recognize any cars and leaving without going in if you do, trying to avoid salespeople who ask if they can help you and leaving store if they ask you more than once

    Thanks for discussing social anxiety in your recent videos. I know someone who has social anxiety disorder and the person’s behavior confuses me and hinders the individual in different ways. These videos are enlightening.

    Thanks, Dr. Grande, for the very interesting video. The text summary you provided is also quite good, and it is even complementary to the video (not merely its textual rendition). I hope that by making these videos (which has to be a lot of work, judging by their high quality), you do not only educate us, interested viewers, but that you also gain some benefit for yourself as a clinician: I notice that when I have to explain anything nontrivial or highly specialized to lay persons, I always gain some better understanding of the subject myself; I hope you have a similar experience with regard to the topics you describe in your videos. As for the avoidant personality, I wonder how often you or your colleagues encounter such people in real life clinical practice? In our "Just do it!" age, it seems that neurotic individuals (and without predominant aggression) have almost disappeared.

      You're welcome! I have found I learn so much that can be applied in practice by making all of these videos. Even with a topic I know well, I almost always learn many new things from the literature I review. As far as avoidant personality disorder, I have seen it in practice several times, but not nearly as much as the Cluster B personality disorders.

    I have social anxiety which i had really bad along with agrophobia for many years (i went out only when really necessary like picking up my medication but it had to be done a certain way & not by myself)
    I have been trying to work on it because of guilt for my kids mostly which pushes me, but due to depression & mobility issues its not easy.
    Some days i can go out & appear to some that I'm okish but most of time i avoid going out or let anyone in.
    Is it possible for it to come and go at different levels.
    I'm never sure if I'm getting over it or if its the other issues i have thats causing it

    My last psychiatrist said I have schizoaffective bi polar, adhd, asperger's. So I am getting a second opinion. Imo I have aspergers, schizoid and avoidive personality disorder, social anxiety, mild depression and general anxiety disorder.

    I believe avoidant personality disorder is its own disorder I am diagnosed with social anxiety disorder but have struggled socially since the age of 11 and I am now 19 and can’t form any new relationships I am struggling with this soo much and I wish there was more help I have just got out of a psychiatric hospital today from a suicide attempt and the main thing making me depressed is my fear of socialising and loneliness

      The loneliness is super hard and I'm married with young children. I'm 24 and only talk to my husband and mom. I'm too afraid to reach out to anyone or make friends.

    Because I do have social phobia, we feel stress and expectant fear in being social at being ‘discovered’ as inferior, rejected😂
