You're welcome, Brianna Hall! I do mention darker skin in my videos as it's more prone to hyperpigmentation with thermal treatments, and also more prone to keloid scarring in surgical procedures. It does take experience with ethnic skin to treat a diverse range of patients, and I have darker skin, too!

I inquired with a surgeon about bilateral lower lid blepharoplasty. She told me does not make the incision below the eye, she cuts a lateral incision on the outer corner of the eye. Have you heard of this approach?

Hi Dr. Prasad, Thank you for your excellent videos. I am 57, regularly use Retin A and other actives, but still losing the battle of under eye wrinkles. I use Botox for crows, but have very deep wrinkles that are starting to extend down my cheeks when I smile. When I'm not smiling, I have fine lines criss crossing the area. These become pronounced if I wear makeup. I do not have under eye bags or dark circles, and just barely volume loss in tear trough. I'm considering Fractional laser but very scared of fat loss. I would come to see you but I am in California. What do you think of a deep or very deep chemical peel to treat under eye wrinkles? Later followed by PRF in the area? The rest of my face does not have much wrinkling, and I eat right and stay in shape. While eye bag removal surgery can last decades, even a lifetime in some people, there is no guarantee that new prolapsed orbital fat can recur, causing new eye bags. Hi Craig Anthony, I most often perform transconjunctival blepharoplasty, with non-surgical treatment to improve under eye wrinkles. I only perform skin pinch blepharoplasties on rare occasions when the lower eyelid skin is folding over itself. Surgical treatment of under eye wrinkles, and the transcutaneous method, are the most common causes of lower eyelid surgery complications, so to avoid complications is to avoid these procedures. Dear Doctor Amiya Prahad, I am very stressed out and do hope that you can help me out with this eye problem. I've had an upper eyelid Surgery for 40 days so far,everything is going ok (just a bit swollen) until yesterday. I boiled a pot of water and lemongrass. I later ( 5 minutes after it is boiled) steamed my face with it for about 5 minutes. Initially I think steaming doesn't matter with my eyelid, but when I have it finished, my upper eyelids started getting red (just a bit, no pain) but the awful thing is my eyelid is not swollen as it was 5 minutes before. I waited until the next day, and the situation still the same, the swollen has gone. My concern is that if it gonna affect the process of healing of my eyelids, I afraid that there is no any change anymore with my eyelid from now on. ( I am sorry as my English is not good). Hi Hằng Đặng, While this shouldn't be a problem, there is a possibility of oils and fertilizer in the lemongrass causing irritation. It has been quite sometime since your surgery, so they won't likely get into the incision, but if you are experiencing discomfort, you can follow up with your surgeon. I hope this helps you.

I suggest you look for a surgeon who performs transconjunctival blepharoplasty, which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Hello Dr Prasad! I find your chanell very professional. I have a question. Maybe you will be so kind and help me somehow. I am 34 and I had fat grafting 5 month ago. I had a really bad swelling problems with lower and upper eyelid. When it finally came back to normal I found my skin less firmer, with more wrinkles than it was before the procedure. I watched your video on allergies to fillers, I want to try this out and went to a local med spa in the Portland, Oregon area and they didn't offer any patch allergy tests, is there any other way I can get tested for an allergy to the numbing and the filler before actually getting this procedure done? A friend had erbium laser about 10 years ago. It was a wound and took about a week to heal. It removed the upper layer of skin. But you video says no down time.
Hi minky cat, Erbium laser can be fully ablative, which needs downtime; fractional or partially ablative, which needs less downtime; or non-ablative, which needs no downtime. It all depends on what the patient needs to reach their aesthetic goal, the temperature setting, the wavelength, time of exposure, etc. as laser treatments are customized, and the expertise in using the laser as a tool depends on the experience of the doctor. Hi Any, Unintended fat loss is determined by the practitioner who may overheat or overexpose using the device, but not the device itself. I don't use high frequency ultrasound devices as they heat more suddenly instead of gradually like radiofrequency or laser devices. Radiofrequency can improve fine lines under the eyes, in the right patient, but a physical examination is needed for me to determine what is the best treatment for your situation. Hi Dontrel Dontrel, I do perform eyelash transplants, but it is quite a the commitment for the patient. Since donor hair is taken from the back of the head, once transplanted, these hairs do grow long and need to be trimmed. Their may be some styling needed as well for the lash to curl, depending on the natural curl of the graft. Also, there is a high risk that these transplanted lashes may also be pulled out again due to trichotillomania. I hope this clarifies things for you.

I don't perform transplants using body hair because you will have to consider that donor area will be scarred from the extraction process, and these areas cannot be sutured and camouflaged as effectively as the donor hair at the back of the head. I had upper eye lid surgery and it really made below my eyes stressed from its very firm appearance. It's been about 9 months and not a whole lot better. However, you may have since experienced aging changes to your lower eyelids, or just noticed them more now since you have been tracking the progress of your upper eyelid procedure. I have a had all this fillers, fractional co2, chemical peel but NOTHING has helped at all and I look at 40 worse than people with 70. Surgery seems the last option. I had NO difference at all

Hi Dr. I have had two quite prominent lines under each eye since I was a kid. (after seeing pictures, I am pretty sure they are Dennie Morgan folds). I am turning 20 soon and I feel like the lines make me look a lot older than the people around me. I have been treating allergies, but I used to scratch my eyes a lot as a kid so maybe that made it worse. Hi hedda ds, Sorry to hear about your problem. Surgery is most likely not recommended, unless your aggressive eye scratching caused damage. Treatment is not usually recommended for Dennie Morgan lines. If it really bothers you, you do need a physical examination to see what can be done in your case, or if no treatment is recommended at all. Lower eyelid skin should not be surgically removed as it will change your eye shape, and can lead to severe complications like irritation from eye exposure, and not being able to close your eyes fully. Hi zibtihaj, The frozen expression from Botox is due to using too much material, but it doesn't happen much when done by an experienced practitioner. The reason why frozen-looking results are common is that many practitioners apply Botox, including non-physicians like medical assistants and even salon workers. The effects of Botox cannot be reversed, but do wear off in 4-6 months. I avoid frozen-looking results by administering Botox conservatively in the initial session, and then seeing patient again in follow up two weeks later. If a little more enhancement is needed, I do it then. It's always better to be conservative at first, and more later if needed, than it is to add too much, and be stuck with the frozen look until it wears off in 6 months. Keep in mind that some people your age do have genetic lines, but they still look young because they have youthful facial volume. I inquired with a surgeon about bilateral lower lid blepharoplasty. She told me does not make the incision below the eye, she cuts a lateral incision on the outer corner of the eye. Have you heard of this approach?

There are variations of blepharoplasty to enhance the appearance of the eyes, but it isn't clear what exactly you are looking to treat. Accessing the fat pockets below the eyes that causes eye bags would be limited with an incision at the outer corners alone. I hope this clarifies things for you.

Thank you for your excellent videos. I am 57, regularly use Retin A and other actives, but still losing the battle of under eye wrinkles. I use Botox for crows, but have very deep wrinkles that are starting to extend down my cheeks when I smile. When I'm not smiling, I have fine lines criss crossing the area. These become pronounced if I wear makeup. I do not have under eye bags or dark circles, and just barely volume loss in tear trough. I'm considering Fractional laser but very scared of fat loss. I would come to see you but I am in California. What do you think of a deep or very deep chemical peel to treat under eye wrinkles? Later followed by PRF in the area? The rest of my face does not have much wrinkling, and I eat right and stay in shape. These under eye wrinkles are really aging me I would do anything to treat them, that would not cause more harm than good. Thanks so much in advance

It is possible to perform lower eyelid surgery in teenagers in some special cases, but I normally perform this surgery in adults in their early 20s when the face has stopped growing. While eye bag removal surgery can last decades, even a lifetime in some people, there is no guarantee that new prolapsed orbital fat can recur, causing new eye bags. I most often perform transconjunctival blepharoplasty, with non-surgical treatment to improve under eye wrinkles. I only perform skin pinch blepharoplasties on rare occasions when the lower eyelid skin is folding over itself. Surgical treatment of under eye wrinkles, and the transcutaneous method, are the most common causes of lower eyelid surgery complications, so to avoid complications is to avoid these procedures. I am very stressed out and do hope that you can help me out with this eye problem. I've had an upper eyelid Surgery for 40 days so far,everything is going ok (just a bit swollen) until yesterday. I boiled a pot of water and lemongrass. I later ( 5 minutes after it is boiled) steamed my face with it for about 5 minutes. Initially I think steaming doesn't matter with my eyelid, but when I have it finished, my upper eyelids started getting red (just a bit, no pain) but the awful thing is my eyelid is not swollen as it was 5 minutes before. I waited until the next day, and the situation still the same, the swollen has gone. My concern is that if it gonna affect the process of healing of my eyelids, I afraid that there is no any change anymore with my eyelid from now on. ( I am sorry as my English is not good).
I am very sad and worried about it, so I am very grateful if you can answer this for me.

While this shouldn't be a problem, there is a possibility of oils and fertilizer in the lemongrass causing irritation. It has been quite sometime since your surgery, so they won't likely get into the incision, but if you are experiencing discomfort, you can follow up with your surgeon. I hope this helps you.

Fibroblast recruitment is part of PRP treatment, as indicated in your question from another video. I generally don't perform thread lifting, but more advanced surgeries such as treating lower eyelid retraction may need some type of anchoring. Although I know surgeons in California, we all have different practice focuses, and aesthetic styles. I suggest you look for a surgeon who performs transconjunctival blepharoplasty, which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality. I hope this helps you.

I had a really bad swelling problems with lower and upper eyelid. When it finally came back to normal I found my skin less firmer, with more wrinkles than it was before the procedure. Eye bags that are always visible, and not temporarily caused by allergies, sinus issues, or salt intake, are due to prolapsed orbital fat underneath the skin, and is not a skin issue. I watched your video on allergies to fillers, I want to try this out and went to a local med spa in the Portland, Oregon area and they didn't offer any patch allergy tests, is there any other way I can get tested for an allergy to the numbing and the filler before actually getting this procedure done? Hyaluronic acid fillers are labeled as hypoallergenic since hyaluronic acid occurs naturally in the body to help retain moisture and lubricate the joints. A friend had erbium laser about 10 years ago. It was a wound and took about a week to heal. It removed the upper layer of skin. But you video says no down time.

Erbium laser can be fully ablative, which needs downtime; fractional or partially ablative, which needs less downtime; or non-ablative, which needs no downtime. It all depends on what the patient needs to reach their aesthetic goal, the temperature setting, the wavelength, time of exposure, etc. as laser treatments are customized, and the expertise in using the laser as a tool depends on the experience of the doctor. Unintended fat loss is determined by the practitioner who may overheat or overexpose using the device, but not the device itself. I don't use high frequency ultrasound devices as they heat more suddenly instead of gradually like radiofrequency or laser devices. Radiofrequency can improve fine lines under the eyes, in the right patient, but a physical examination is needed for me to determine what is the best treatment for your situation. I do perform eyelash transplants, but it is quite a the commitment for the patient. Since donor hair is taken from the back of the head, once transplanted, these hairs do grow long and need to be trimmed. Their may be some styling needed as well for the lash to curl, depending on the natural curl of the graft. Also, there is a high risk that these transplanted lashes may also be pulled out again due to trichotillomania. I hope this clarifies things for you.

I don't perform transplants using body hair because you will have to consider that donor area will be scarred from the extraction process, and these areas cannot be sutured and camouflaged as effectively as the donor hair at the back of the head. Also consider that the difference in the texture of hair of the donor site as actual eyelashes. I hope this clarifies things for you.

I'm not interested in taking any hairs from my head, because trichotillomania has already weakened it. So I'm looking forward to pubic hair or leg hair. I'm can't wait super excited about it. I had upper eye lid surgery and it really made below my eyes stressed from its very firm appearance. It's been about 9 months and not a whole lot better. I believe my lower eye lids took a beating with the upper surgery with bruising, fluids and the simple stress-now I feel I need the lowers done. Have you seen this before happen-is it the risk???

Upper eyelid surgery should not have an effect on your lower eyelids. However, you may have since experienced aging changes to your lower eyelids, or just noticed them more now since you have been tracking the progress of your upper eyelid procedure. I hope you understand that your lower eyelid, and skin type would need to be examined before I make specific treatment recommendations. Wrinkle treatments featured in the videos such as lasers can cause pigmentation issues in darker Indian skin types like mine, so an examination is needed especially for darker ethnic skin types. Sorry to hear about your problem. Complications from lower eyelid skin removal are very common, so I only recommend surgery in the rare cases that eyelid skin is folding over itself. Non-surgical skin tightening treatments such as lasers usually require more than one treatment session to avoid overexposure, and overheating. Hi Dr. I have had two quite prominent lines under each eye since I was a kid. (after seeing pictures, I am pretty sure they are Dennie Morgan folds). I am turning 20 soon and I feel like the lines make me look a lot older than the people around me. I have been treating allergies, but I used to scratch my eyes a lot as a kid so maybe that made it worse. Do you think these lines could be treated by some laser treatments or is surgery the best solution?

Sorry to hear about your problem. Surgery is most likely not recommended, unless your aggressive eye scratching caused damage. Treatment is not usually recommended for Dennie Morgan lines. If it really bothers you, you do need a physical examination to see what can be done in your case, or if no treatment is recommended at all. Lower eyelid skin should not be surgically removed as it will change your eye shape, and can lead to severe complications like irritation from eye exposure, and not being able to close your eyes fully. The frozen expression from Botox is due to using too much material, but it doesn't happen much when done by an experienced practitioner. The reason why frozen-looking results are common is that many practitioners apply Botox, including non-physicians like medical assistants and even salon workers. The effects of Botox cannot be reversed, but do wear off in 4-6 months. I avoid frozen-looking results by administering Botox conservatively in the initial session, and then seeing patient again in follow up two weeks later. If a little more enhancement is needed, I do it then. It's always better to be conservative at first, and more later if needed, than it is to add too much, and be stuck with the frozen look until it wears off in 6 months. Sorry to hear about your problem. Lines under the eyes at your age are usually genetic, and not treatable as they are not due to diminished skin quality. Keep in mind that some people your age do have genetic lines, but they still look young because they have youthful facial volume. If you didn't have these lines when you were a teenager, you may consider seeing a local dermatologist for an examination of your skin for any potential issues. I hope this helps you.
