I’m so glad you mentioned LOCAL anesthesia. I was reccommended surgery due to the fat protrusion. The eye bags are genetic and my ophthalmologist said that I’m not a candidate for fillers cause I already have a VP shunt . Thank you for making me feel comfort in a blepharoplasty

Hi @trueunion, I don't recommend at home thermal devices because people using them are not trained to limit heat, exposure, nor do they know about the healing time needed between sessions. Hi Dr. Prasad, I am in my early 50s... when I was in my early 30s, I had a moderate tear trough deformity, which I was able to have corrected with filler. I now have mild to moderate eye fat herniation and can have it corrected if I get filler in my cheeks, unfortunately, it takes 4 - 5 syringes of filler to give me adequate volume to correct the herniations and tear trough. My question is, if I have surgery to remove the herniated fat, can some be repositioned to also take care of the tear trough issue? Also, I am assuming even with a facelift, I would have to continue to obtain filler for my cheeks or a cheek implant - the issue now is cost due to the amount of filler I now need for correction. Dr. Prasad: If the main cause is fat herniation (prolapsed fat), why is fat removed rather than the surgeon "pushing" it back into place? I would view this conceptually as slightly different from traditional fat repositioning, but perhaps what I am proposing is simply not possible/advisable? Thank you.
Hi Michael Hilson, Orbital fat prolapsing also occurs because of tissue changes that once held the fat, so it isn't practical to push it back into place as weakened tissue can also cause the fat to prolapse again. I hope this clarifies things for you.

Your surgeon should be able to locate your existing filler, but you should let them know about it prior to the procedure so they can be planned around, without having to dissolve the filler. Prolapsed orbital fat that causes eye bags is different from cheek fat, and the effects of your filler, so your surgeon should be able to plan around the existing filler volume. Hi Dodie, Lower eyelid surgery can be minimally invasive, so you'd be surprised at how easy it is on the patient. Being minimally invasive with transconjunctival blepharoplasty is more of a technical challenge to the surgeon, and becomes routine with experience, but most eyelid surgeons aren't trained in the technique.

Hi Gen T, The results of lower eyelid surgery can last many years, and if fat were to prolapse again, it would be new orbital fat, not the same fat that was treated. While filler may or may not be the right treatment for you, permanent fillers are very prone to permanent complications due to migration and foreign body reactions, while temporary hyaluronic acid fillers are much safer because hyaluronic acid is already naturally present in the body, and are safely metabolized with time. I had 3 sessions of plexr done. I wonder what his thoughts are on this procedure. Not worth the money in my opinion, but PRP did work. I should have just done surgery though.

What happens to the skin after the fat pads are taking out? I do see some people who had the procedure done and then they end up having more wrinkles where the bags once were. Are there any treatments for that? Otherwise it would be like choosing between the lesser of 2 evils lol

@AmiyaPrasadMD From all my reading, it seems that Hashimoto’s or certain hypothyroids could attribute to these under eye bags. I also read that some doctors refuse to do this procedure on patients with hypothyroid. Has this been an issue in your practice? Is there both fat and protein fluids filling the under eye space? I want this but confused if I should expect bags to come back since I take thyroid meds.

Hi Mandy S, Tear trough implants require surgical placement, so they are more involved than injectable tear trough fillers. While silicone tear trough implants can be a long-term treatment for under eye hollowness, many people don't find them comfortable, and softer hyaluronic acid fillers as a more comfortable. Tear trough implants are not permanent, as facial dimensions do get smaller due to aging, so eventually they won't fit, and will need replacement. However, I would have to diagnose if the eye bags are due to your allergies, or to prolapsed orbital fat. Allergies causing eye bags are treated differently. Eye bags that are always visible are likely to be caused by lower eyelid fat prolapse, as explained in the video. I only treat eye bags that are caused by prolapsed orbital fat, as I can sculpt and reduce the fat with surgery, or camouflage it in milder cases with filler. Hi@@pranav7484, I only treat puffiness that visible all the time because it is caused by orbital fat that pushed forward, causing eye bags. If your puffiness comes and goes during the day, then this is likely an allergy, so you need to see an allergist for allergy testing. I hope this helps you.

Hi, There is no external incision in transconjunctival blepharoplasty by definition. It is highly unlikely a revision is needed because results can be seen while still in the operating room, but generally revisions are not done until at least 6 months after the initial surgery.

Hi, Sorry, I misunderstood your question. Yes, the incision through the conjunctiva can be used again, and a revision can be done in as soon as 1 month after the initial surgery, depending on the degree of post-operative swelling from the previous surgery. I hope this clarifies things for you.

Dr, why is application of filler something that you strongly recommend when there is some hollowness to be restored? A patient would have to return to get filler about every two years. I thought the benefit of getting surgery was to have something permanent done.
Hi Patricia Lopez, I usually recommend surgery for eye bags as results can last long-term, but it's the adjacent upper cheek hollowing where I recommend fillers. Facial implants do last longer, but people often find them uncomfortable, and they are not permanent because the face loses volume due to aging, so eventually they won't fit. Hi@@patricialopez5358, The art of lower eyelid surgery is leaving a little fat behind to prevent hollowing, and for a nice, continuous contour. With experience, I've actually consistently avoided removing too much fat in lower eyelid surgery. Lower blepharoplasty actually ends at the tear trough, with the eyelid cheek junction below it, so it shouldn't be affected by surgery. The lack of cheek projection at the eyelid-cheek junction is usually due to a genetic volume deficiency, or a volume loss due to aging. I hope this clarifies things for you.

Hi Dr. Prasad, I am in my early 50s... when I was in my early 30s, I had a moderate tear trough deformity, which I was able to have corrected with filler. I now have mild to moderate eye fat herniation and can have it corrected if I get filler in my cheeks, unfortunately, it takes 4 - 5 syringes of filler to give me adequate volume to correct the herniations and tear trough. My question is, if I have surgery to remove the herniated fat, can some be repositioned to also take care of the tear trough issue? Also, I am assuming even with a facelift, I would have to continue to obtain filler for my cheeks or a cheek implant - the issue now is cost due to the amount of filler I now need for correction. I hope this makes sense and thank you so much - your work is beautiful!

Dr. Prasad: If the main cause is fat herniation (prolapsed fat), why is fat removed rather than the surgeon "pushing" it back into place? Your surgeon should be able to locate your existing filler, but you should let them know about it prior to the procedure so they can be planned around, without having to dissolve the filler. Prolapsed orbital fat that causes eye bags is different from cheek fat, and the effects of your filler, so your surgeon should be able to plan around the existing filler volume. Lower eyelid surgery can be minimally invasive, so you'd be surprised at how easy it is on the patient. Being minimally invasive with transconjunctival blepharoplasty is more of a technical challenge to the surgeon, and becomes routine with experience, but most eyelid surgeons aren't trained in the technique.

The results of lower eyelid surgery can last many years, and if fat were to prolapse again, it would be new orbital fat, not the same fat that was treated. A little fat should be left behind after surgery to prevent a hollow appearance, but the fat left will not cause eye bags again. I hope this clarifies things for you.

If you had a hyaluronic acid filler, it is possible that the filler became encapsulated, preventing normal metabolization, so it remained. In these cases, the filler can still be dissolved if the injection reaches the encapsulated filler. I hope this clarifies things for you.

It's not clear if you have hollowness, or discoloration since your doctor is recommending radiofrequency. While filler may or may not be the right treatment for you, permanent fillers are very prone to permanent complications due to migration and foreign body reactions, while temporary hyaluronic acid fillers are much safer because hyaluronic acid is already naturally present in the body, and are safely metabolized with time. PRP, Plexr, or laser treatment are not appropriate treatments for puffy eye bags, but they can help improve the quality of lower eyelid skin to treat issues like wrinkles, and dark under eye circles. Eye bags that are visible all the time are caused by prolapsed orbital fat, and ideally surgery is needed to sculpt and remove them, while some slight to moderate eye bags can be temporarily camouflaged using filler. This procedure is less prone to complications since there is no external incision, or skin removal, and so results look completely natural, but fewer surgeons practice this technique. Tear trough implants require surgical placement, so they are more involved than injectable tear trough fillers. While silicone tear trough implants can be a long-term treatment for under eye hollowness, many people don't find them comfortable, and softer hyaluronic acid fillers as a more comfortable. Tear trough implants are not permanent, as facial dimensions do get smaller due to aging, so eventually they won't fit, and will need replacement. Sorry to hear that, but genetic eye bags can indeed be visible from a young age. However, I would have to diagnose if the eye bags are due to your allergies, or to prolapsed orbital fat. Allergies causing eye bags are treated differently. I only treat puffiness that visible all the time because it is caused by orbital fat that pushed forward, causing eye bags. If your puffiness comes and goes during the day, then this is likely an allergy, so you need to see an allergist for allergy testing. I hope this helps you.

There is no external incision in transconjunctival blepharoplasty by definition. It is highly unlikely a revision is needed because results can be seen while still in the operating room, but generally revisions are not done until at least 6 months after the initial surgery.

Dr, why is application of filler something that you strongly recommend when there is some hollowness to be restored? A patient would have to return to get filler about every two years. I thought the benefit of getting surgery was to have something permanent done.

I usually recommend surgery for eye bags as results can last long-term, but it's the adjacent upper cheek hollowing where I recommend fillers. Facial implants do last longer, but people often find them uncomfortable, and they are not permanent because the face loses volume due to aging, so eventually they won't fit. The art of lower eyelid surgery is leaving a little fat behind to prevent hollowing, and for a nice, continuous contour. With experience, I've actually consistently avoided removing too much fat in lower eyelid surgery. Lower blepharoplasty actually ends at the tear trough, with the eyelid cheek junction below it, so it shouldn't be affected by surgery. The lack of cheek projection at the eyelid-cheek junction is usually due to a genetic volume deficiency, or a volume loss due to aging. I hope this clarifies things for you.

I would have to examine if you have eye bags also due to lower eyelid fat prolapse. I only treat the fat that causes eye bags with transconjunctival blepharoplasty, which is featured in the video, and is done without any external incision on the eyelid skin, and no skin removal so results look natural, and complications due to skin removal, or complications due to incisions on the eyelid skin are completely avoided. Thanks for the compliment! Eye bags can be genetic, so they can occur in younger people. While there are special circumstances where younger teenagers can be operated for severe eye bags, I generally recommend patients are at least adults of 18 or older. I hope this clarifies things for you.
