Jeremy Piven looks the same no matter what year it is. Interesting cut scene with Trejo, confirms how and why Neil asks him "Why you do it?" later on in the theatrical version.
I think this scene was better left cut, but it does add context. I always felt like Neil’s attitude towards the doc when taking his shirt was a bit rough but a result of him just having had enough of everything that already happened, but he’s paying him more than enough to buy a new shirt so who cares if Neil is rough around the edges. Here we see that the doctor is actually extorting him based on the situation so I can see why Neil would be like fine take your money but I’m taking your shirt. It’s actually kind of funny in this context now that I think about it. Honestly a lot of theatrical releases sometimes suffer from deleted scenes (lord of the rings comes to mind), but Heat is a great example where they made cuts in a logical and appropriate way to maintain the film’s flow.
I like the cut of this scene that was in the movie better (no shakedown) because Dr. Ari wouldn't be stupidly renegotiating his deal with Nate, which is $15K cash per. For that he treats bullet wounds, doesn't report them as required by law, and gets more work next time a crew member gets wounded. He's rocking the boat here, and anybody smart enough to earn an MD would know better than to do that with Nate, Neil or associates.
Why the hell wasn't this included??? could've became a classic line, and it beats the idea of neil leaving chris with some doctor hoping to keep his mouth shut just cause they paid him.
So is Neil basically saying instead of double the original $15 large, he'll pay him four times ($60000) on the understanding that should the doc roll over he's a dead man?

JC Schwartz if watched the movie this was after the bank shoot out and Neil had blood on his shirt as was helping Chris to get away after Chris was shot. He was wanting a clean shirt so doesn’t get noticed. The corrupt doctor was helping get Chris fixed up after being shot.
