Taking Medikinet XL for ADHD the first time - 30mg VLOG - Day 1

Hi there!
I'm Emily, I'm 32 and I had a late diagnosis of Inattentive ADD (a type of ADHD) just two months after my 30th birthday. Finding out I was neurodivergent was scary and exciting, but it's been a pleasure to share my ongoing medication journey with my subscribers over the past two years.
In this video, I begin a new form of medical treatment for my ADHD, taking my first dose of Medikinet XL (starting dosage 30mg). MedikinetXL is a prolonged/slow/long release stimulant medication (its scientific name is methylphenidate!) used to treat attention deficit hyperactivity disorder (ADHD). This medication is sometimes called Ritalin - particularly in the US. This video shows my first day on the drug and highlights some of its effects (and side effects!) on my mind and body.
I will be continuing a vlog showing my experience with Medikinet as I test out my current dose and through the titration period.
Whether you're considering starting medication, have already begun treatment, are curious about ADHD and its links to depression, or are simply looking for some reassurance that you're not alone, this video is for you.

      Sick of these replies, do your really think anyone thinks it's totally "free", of course we all know it's from tax money, we don't think it's magic fairy dust paying for it. It's "free at the point of use", that's what people mean, if you understand that, then you'll understand what people really mean when they say it's "free".

    I struggle with everything every day. Tomorrow I have to start taking contramyl which is the same as concerta. Kind of nervous and excited to see if it help me actually do anything. Everything takes so much energy to just start doing and I struggle so much with fatigue constantly. Hopefully I can get some stuff done and finished in my life!

    I myself take the medicine medikinet aka retalin 80 mg every day but the effect seems to quickly decrease after you have taken the first pill. But I would not be able to do without my medicine today.

    I’m in the inattentive camp too, finding it really hard to go anywhere but have just started titration so I’m hoping this helps. I’ve been prescribed Elvanse but not sure if Concerta maybe better but worried if I change they may not let me back onto the Elvanse 😣

    Keep posting videos, it is all really useful to see how others are getting along with finding the right medication. I was diagnosed in October last year at 52 years old... I had often thought that I did not quite get why other people seemed to behave strangely, it made me very interested in learning more about psychology... But of course I was assuming that my grey matter between my ears was just the same as everyone else's.. At the time of my diagnosis, Elvanse was recommended but was having availability issues, so I was put onto Concerta.. initially on 27mg, within an hour for the first couple of days, I suddenly felt like I had woken up from the best nights sleep ever, rather than my usual groggy until midday, and for the rest of the day I was driving a completely different brain than my usual one... No more "spinning plates" (thoughts of everything I needed to remember to do), and no more getting angry with anyone who interrupted me when I was concentrating. Then it seemed to not being having the same effect, so I stepped upto 36mg of Concerta, and the benefits returned.... But a month later I was going to step up to 54mg, but nowhere had concerta... So I had to get a prescription made out for the generic Methylphenidate MR.... Even though they say that the generic is identical to the brand name version (concerta) I persisted but did not find it delivered quite as well... I even tried Elvanse, but found I did not feel any benefit, but it was a low 30mg dose. Over the next few months, I received a different brand of Methylphenidate every month, occasionally getting Concerta, and finding it worked and delivered better for me. But the last couple of months, getting Methylphenidate has been getting harder and harder, I had to make a chart to keep track of what Pharmacies I had already called and eventually managed to get Concerta (I would have taken anything I could have got) This month tho, I called over 80 Pharmacies trying to get Methylphenidate with no joy within a 50 mile radius... But a few Pharmacies advised that getting 40mg Elvanse was a lot easier.... So today was my first day of 40mg Elvanse... Not sure if it is working, but I seem to have an OK and productive day. I watched this video because I have been told that Medikinet MR 20mg is also pretty easy to get at the moment, and my doctor has recommended I might want to either take two 20mg of Medikinet MR together in the morning, or take one in the morning and one mid afternoon... So I might try that next month, depending on what availability is like on everything else. The problem is, over the last year, I have been on so many different brands and drug types due to issues with availability that it feels like I am never going to properly fine-tune through titration and find out what works best for me.. I am really lucky to have a good private psychiatrist who is just a phone call away if I need to get another prescription made out when I hit problems with supply... But having said that, it would be nice to get my theraputic levels fine tuned, and then see if I can get my GP to take on a shared care agreement with my Psychiatrist so I can just get a repeat prescription (if the supply issues ever improve to the point of being able to have a regular repeating prescription that can get filled!)

    It gets so hard sometimes especially being undiagnosed now before people come at me, ive been experiancing symptoms since i was 12 im 19 now my dad has it and ive done research after research, im currently only NOW getting a diagnosis due to the fact i no longer will need to be reffered by school (which is so stupid in how that is done) Because im undiagnosed it gets so hard people never understanding it Especially the emotional dysregulation aspect of it that can fully make me feel depressed and symptoms of depression. Adhd untreated like mine has left me to do dangerous habits like bad eating ones (to gain control and a dopamine rush) and even addiction. It gets so hard, and honestly i think from so many years of being misunderstood in my own home has really damaged me mentally, suicidal ideation in adhd is no joke it gets so bad. If you are a parent with a child with adhd, LISTEN TO THEM Love their interests, be interested in what they have to offer, love the little facts they tell you, and dont get mad when their emotions may seem alot. It was these things that my parent neglected that left me struggling to hold on, but hey im trying. ❤

    ADHD makes everything overwhelming and impossible if you have it bad… so depression is a consequence naturally. When I first started adhd medication I cried, like the first few days, because it was such an unbelievable miracle. I felt much calmer, my obsessive thoughts went away and a rash I had on my ankle for 30 years disappeared 😂 literally an unbelievable miracle! I began to Make and keep friends and get invited for the first time in my whole life!!! The most miraculous life-saving drug on the planet for me. It let me HAVE a life-the life I never had that everyone takes for granted 😊❤

Cause as it seems, 95% of people who get the diagnosis in their hands, are sent in the ADHD world without a map or dictionary to navigate through it.     Update for anyone interested: Clinical Partners prices as of Sept 2024 are now £859 for a private assessment. The NHS RTC waiting list with them is 7-10 months. I have already been to Clinical Partners for my ASD assessment this year (had to wait 5 months on NHS RTC with them) and decided to go back to them after looking around for my ADHD assessment, which I started the ball rolling on this week.. Privately they are more expensive then others, however those others do NOT use Psychiatrists in the assessment, where as Clinical Partners do. I had to debate paying for the quickness (having things 'pending' is a huge stressor for me).. and as I already was on the NHS waiting list for a year. Though I decided to use RTC as if it came back negative I would of cried at the seeming 'waste' of £895!

    I dropped out of my teaching degree as I was given no instruction on lesson planning and was paralised when it came to creating content, i had to do lesson plans across a range of difficulties for kids with different abilities but wasnt allowed to see which kids had difficulties, I had to lesson plan at school but wasnt allowed to lo onto the school internet. The woman resopnsible for mentoring me had me in for a few weeks and punted me off to the guy who had been teaching a year. transgenders got to have their own bathrooms at the expense of the disabled.

    This video has been very helpful. I connect with your story. I started an adhd traeatment three months ago. In three weeks of adhd medication, I have felt a lot better than 15 years with antidepressants. I hope this continues giving me this peace, like you.       Sometimes ADHD meds can be very relaxing for people with ADHD. You may have been focusing more than you realise which can be more tiring. Let your provider know. Maybe you would be better on a different med if you don't see any positives.

      I read lot of negative experiences with Medikinet...look at pharmacokinetics of Ritalin IR/LA and Medikinet/Concerta...you'll see why it hit different. For me Provigil was very good, clear calm head..but it's expensive AF!

    I’m late to the party, but I find this funny. I was diagnosed with ADHD through Clinical Partners, and when it was time to get medication, I had to be re-referred for another assessment on the NHS just to receive treatment. This happened a few months after the documentary aired, and my doctors were skeptical. What amused me was how thorough the Clinical Partners assessment was-they interviewed my partner and two family members, and the assessment itself took two hours. A month later, I received a detailed report from them. In contrast, my NHS diagnosis took only 35 minutes, and I was given medication the same day. I felt that the lack of care from the NHS was shocking, and it’s been a terrible experience since. I wish Clinical Partners could have handled my medication titration. The NHS failed me miserably in terms of assessment and treatment. If I want an appointment with the psychiatrist or even just to ask a question, I have to wait 2-3 weeks for a response. This has been a nightmare while dealing with the side effects of stimulants.

    Btw: every meds blog you have uploaded has you saying you have a bit of a headache and assuming maybe you are dehydrated. I believe its the medication but you also said you are prone to migraines so maybe consider treating that.

    I've been watching all of your journey, literally binge watching all your videos today from your first day to this... My prescription comes tomorrow and I'm going to start on 18mg as well, I know it's going to be a big journey for me as well and I'm both excited and scared because I will know how it feels when life is in autopilot instead of manual mode and I am scared of the fact that once that happens I can't unknow it anymore even if eventually I stop being on meds.

    I started methyphenidate in april and take Medikinet xl 30mg (was originally perscribed Ritalin but that was unavailable...), I often feel a bit off after two hours (fast heart-rate, sometimes tremors, and thirst..), I read today that Concerta and other brands are roughly 30% fast release and 70% extended release whereas Medikinet is 50% of each ! Don't quote me on it just in case but was hoping to talk to my psychiatrist about changing brands as that may explain the side effects.. Looks like the shortage is gonna be a problem though... 😭

    Wow, after binge watching literally all your videos before my first (unsuccessful) titration on Concerta and now you've uploaded your own second titration vlogs just as I begin my journey again too (on Elvanse though). Thank you for being so open in sharing your experiences, Emily. It's comforting to know that you're not the only one doing this 'figuring out how to navigate life with ADHD' thing. I'm hyperfixating on all content ADHD rn, so I'll make sure not to miss a new upload if and when it comes :)

    Happy to se you back! I watched a lot of your videos when i got my diagnosis and started concerta. I was so nervous when i started medication and your videos helped a lot. Cool to see you try medikinet because I switched to it from concerta and it has been much better for me :)

    Glad to see you're back! I was on the Concerta (18mg) for a while but I had to get off that because it was actually making me really *angry*! I was focusing a lot better while on it and getting a lot done, but it was really making me feel "wired" all the time and generally physically overstimulated and I was just snapping at everyone (it was a particularly stressful/frustrating period at work so that didn't help).... Did you notice anything like that with your Concerta (I think you were on a much higher dose too?)? I got off that and was put on 5mg Adderall (which is about equivalent to half the Concerta dose I was taking - there isn't a 9mg Concerta unfortuntately) and this is working out much better. The focus/stimulation is a lot more subtle and I feel I have to take another one around lunchtime to get me through the day but I'm not tense or wired or angry anymore so I think that's definitely better. I'm also on a high fibre diet now and noticed that the fibre gets in the way of absorbing the medication, so i'm taking the adderall about an hour before my all-bran breakfast and it's having an effect now (I was taking it at the same time before and wondering why it didn't seem to be working) so watch out for that!

      Glad to be back :) That's so interesting about the Concerta making you feel angry: I can't say I experienced the same effect though. I generally find that it makes me feel less overwhelmed and calmer in stressful situations. Everyone is different though! I'm glad the Adderall is working for better you :) Noted about the fibre - a great tip! Thank you x

      @@adhdemily yeah it took a while to realise what was going on. People at work were noticing a change - better focus and productivity but also more arguments and venting, which was not good! In hindsight I think it was because the dose was just too high and it was making me feel "wired" and "on edge" all of the time (higher temperature , elevated heartrate etc). And annoyingly concerta doesn't come in a smaller dose so I had to switch to Adderall because that did. 10mg Adderall felt like too much, but 5mg seems to work for me and it's been much better since!

    The trembles may just be that you're getting 30mg methylphenidate over 8 hours vs 12 and it'll peak much more suddenly. If you don't get on with medikinet XL, Equasym is over 8 hours but with a more similar release profile to Concerta etc.

    It’s so reassuring hearing someone else that didn’t feel Concerta 18mg made a huge change I’m currently going through my titration and 18mg doesn’t feel like it’s doing anything and I was getting in my head that it was me.     It's crazy how much the cost of a private diagnosis can differ from country to country. In Germany, we have a similar problem with ADHD diagnoses in adults, with really long waiting lists for public health insurance. For my private diagnosis, I paid 215€, which is about £180, and each follow-up appointment was around £85. For the medication itself (I take 36mg Concerta), I pay 53€, with an additional 5€ fee because ADHD medications are heavily regulated, resulting in higher administration costs for pharmacies. These relatively low costs made it possible for me to get a private diagnosis despite being a student with little disposable income. Once you have a diagnosis and a treatment plan, you can immediately switch to a public doctor who can treat you for "free." However, I'm still waiting for the appointment because most psychiatrists have so many patients that the earliest available slots are usually after 2-3 months.

    As someone who was recently diagnosed with ADHD-I, and now moving into teaching as a second career... Phew. I was really struggling in my office job so hoping the structure and hard deadlines in teaching will help me. Any tips?
