      Cold emailing. Just go for it. Reach out to faculty, fellows, residents via email, LinkedIn, Twitter. Walk up to ppl in conferences. Be respectful, fairly direct if you have a question or request instead of beating around the bush, and dont be pushy, give them time to respond

    Salam Omar, i just did some rotations in the states recently and my pereceptors were happy to write LORs for the match but i have a concern as i have to do a one year of internship after graduation to get my liscnse so i will end up applying to match 2026 not match 2025. Do you recommend buy a token next year make an eras number and ask my attendings to upload the letters then ask them the following year if they can update the letters date or just ask them to upload the letters when I actually apply. I wish i could do my electives near to the time of applying to the match but unfortunately interns are no longer consider "students" to be eligible for hands on / subIs experiences and i prefered to do subIs instead of observerships later. What are you gonna do if you were in my position?

      I think it's a good decision to do subi instead of observership. I think people approach this in either way as you described, either buying a token this year and having them upload the letters or having them upload the letters next year. I think either way works, the advantage of buying a token having them uploaded is that you will enter the right the letter up and upload it to ERAS versus you reaching out to them next year following up on your letter and they may or may not be able to respond.

      @@mohammadmansour4482 in my perception what matters more than the date of the letter is the date of the experience. If you did a USCE a year ago and even if you get an updated letter with a more recent date, the USC still happened a year ago which is what is the more important factor

    Do gaps matter after internship? Like career break, travelling, family commitment?

      Everyone has their unique circumstances, and everyone's application is unique. The impact of a gap on a candidate who has a prolific application strong in all domains may not be as much as someone with so called "red flags" eg attempt in Steps, no USCE, etc. In the end, if the gap was absolutely necessary and important to you, you should not feel bad about taking it. Family should ideally always come first. For the programs that like your application and extend you interviews, you will have the opportunity to elaborate more on the reasons for the gap in the interview as well. Otherwise, I have seen especially AMGs take time after med school for further degrees eg MPH, MBA, or research, or for personal reasons. IMGs dont generally enjoy the same level of flexibility and accommodation.

      @@seymabayram5510 congratulations! Different programs have different trip attendance policies. For our program the trip attendance is separate from our vacation. To get reimbursed for trip related expenses you would still need to apply for approval through your program / GME. Additionally, to ensure The trip does not impact your rotation you would need to also get approval from your program and discuss with your chiefs. Usually arranging coverage on outpatient rotations is easier than inpatient. So bottom line is, plan early and give your program and chiefs an early notice (several months prior if possible). If the conference is later in the year, the program I even be able to switch you from an inpatient rotation to an outpatient rotation if given an early enough notice. If the conference is on a weekend, arranging coverage while you are away won't be an issue but it would still be advisable to apply for approval so that you can get reimbursed for the trip expenses.

      Publications or poster/oral presentations from anywhere count and can be added to your ERAS CV. If you were working in a lab or did a research fellowship/internship you can also include it in your research experience on your CV

    Thanks a lot for your insightful sharing! May I ask that does experience of PGY training at hometown helps applying U.S. residency or marks as any positive signal in one’s profile as an IMG? In my hometown, we are required to obtain 2 years of PGY training after graduation and before applying for each residency program. I am struggling in either to choose enter my hometown PGY first for more hand-on and primary care experience or directly go for USCE and research (if I go for PGY first I will still pursue USCE and research after then). Is it a good idea, or it’s just wasting one’s YOG? Do you have any suggestion (I’m aiming for internal medicine)? Again thanks a lot!

      Thanks for the excellent question Josh. My recommendation journalists always to avoid additional years after graduation if possible. If possible, I recommend always applying directly to the match unless taking a gap year will profoundly improve your application. I understand that for some people applying directly to the match may not be possible due to financial reasons as well. So I'd say, if you are in a position to skip those additional years of training back home and directly pursue your US residency journey, that may be a reasonable approach.

      Thanks for the question. It definitely depends as Mayo has multiple campuses and many different specialties and training programs. I can speak for IM residency at Mayo Arizona and I can say that the work life balance is great and our 4+4 schedule system (4 weeks inpatient and 4 weeks outpatient) works nicely.

    Great video, I don’t know whether is it possible( in my curriculum) to do electives. So, will it be possible to match even in 2026 or 2027 seasons into internal medicine without electives and only with telerotation? Please let me know.

      It certainly is possible, particularly if there are other strong factors in the application that may help cushion the negative impact of not having USCE, for eg excellent USMLE scores, strong research profile. Any USCE is better than no USCE, so a telerotation is better than no rotation, and a telerotation would help you get US LORs.
