    Very nice video. Many great tips & details. I just wished that it had elaborated more on USCE for graduates. Even for observerships, It's extremely difficult nowadays to get one at a hospital without strong connections.

      Thanks for the great feedback. It certainly has been difficult to arrange USCE due to COVID. Generally the formula still applies to graduates - private agencies, connections and reaching out to hospitals for opportunities. I have had several friends who were able to get USCE at smaller private centers and those one month observerships developed into something much bigger and more beneficial.

    Amazing and thanks for the tips Dr Omar! one question please , what should be done first ? Research or USCE ? and do you have any idea about Jons Hopkins “ Research electives “ its 9-week duration ,500$ and you apply throw their website, is this elective worth it ? ( I'm a 3rd year / total 5 Year Program )

      Thanks for your kind words Anas! What comes first will depend on how early in med school you start this journey. When we say "research" broadly, that will start day 1 of med school and you'll have the chance to work on research projects and publish papers at your med school. But for "US Research experience" that will happen either during your vacations in med school or after medschool. USCE generally happens toward the end of med school and for many IMGs in their last year of med school. Regarding the Hopkins program, it really depends what you're looking for and what other students say about the program. The Hopkins name is good, price seems reasonable and if there's a chance of being abstracts and publications out of it, might be a good experience

    hello omar, Can u please share how can we draft an LOR if an attending asks to write it ourselves. What points should one cover while writing an LOR? Are there any samples for LORs?

    Hi Mayo clinic allows only one location to apply for. Which one accepts IMGs more for the electives?(phoenix and Minnesota)

    Do you have emails or links so how can we apply for observership in mayo clinic .

      For Mayo electives, you'll need to apply through the VSLO portal if your med school has access to it. For observerships, I'm not aware of a specific email but the process of getting an observership would be reaching out to faculty in the specialty of your interest and exploring the possibility of the faculty member sponsoring you for the observership.

    Most of the electives are restricted due to covid.we are not sure when they gonna resume.And if only they open up ,it wont be benefited to those who are starting internship from 2022 january and not taken step1 or graduating this year.so weather the rotation provided by agency like AMO and others worth doing ??But some of the applicant complaint that they do a fraud and its quiet difficult.Can you name some of trustable agencies for hand on rotation??

      That's a great question and definitely a concern and doubt many applicants have. My rule of thumb is that any USCE is better than no USCE. Obviously the priority should be applying for the top elective programs in academic University hospitals. If for whatever reason those are not available for example coz of COVID-19, aim for smaller hospitals and medical centers. If those are not available at all, aim for private clinics and try to reach out to physicians in private practice to try and arrange those. If that's not possible either, then you could consider the private companies that offer electives such as the one you mentioned above in addition to other agencies like USMLE Sarthi, ACE, etc. I suggest you be careful with those as you mentioned reviews can be mixed. This is because student experience heavily depends on which preceptor they end up with. Try to look around for genuine student feedback and assess that in the context of the cost. I wouldn't go as far as calling them fraud but a lot of students are able to get letters and match with the help of those electives. Finally The last resort would be doing daily rotations because I have heard of students being able to get letters from these experiences and successfully match. Hope this helps

      @@MlekArt Thanks for your question. Boston Children's is one of the best pediatrics hospital in the country and a letter of recommendation from a rotation there would regardless be of value. Now obviously this clinical experience and the letter may not be as directly relevant if you are applying to IM. But I would argue it would still be a very good experience to have on your CV. Especially if you're interested in cardiology, the pediatric cardiology experience is totally relevant when we think about adult congenital heart disease. For example, in our cardiology fellowship program, we get to do a rotation at the Phoenix children's hospital with pediatric cardiology because it is certainly very relevant to us. So I would say if you're struggling to find high quality IM rotations, this one is worth doing. Good luck!

      Hey Latif. That has been a focus of a lot of discussion. We can't say for sure what impact it would have on IMGs until we actually see it in action in the Match season next year. The general expectation is that CK will become more important since it is still score based. And generally there is a push for programs to more holistically review applications, paying attention to all aspects of an applicant's CV. Another understanding is that med school reputation will become more important, where applicants from certain med schools that are better known to programs will be more favored. The good news is that this new pass/fail rule affects everyone the same, and we're yet to see how ERAS will handle the situation where some people have a score based result and some people have a pass/fail for their Step 1.

    And one more queries i had to ask you that
    I am basically from Nepal studying in government medical college in Bangladesh.So weather i have to apply for US visa and all the malpractice, health insurance,HIPAA certificate from my home country or I can apply from the Bangladesh where i am doing my graduation.I am sort of confused with all this.

      I believe you should be able to apply for all these things in Bangladesh. I'm originally from Kashmir but did med school in Saudi Arabia and applied for my US Visa and completed other requirements in Saudi.

    Hi Omar--an IMG here.
    Due to COVID, I could only manage to get one clinical elective. However, I was able to get 3 LORs out of it.
    Do you recommend uploading all 3 of them?
    Or should I use 2 of them and upload 2 from my home medical school?

      Hi! Great question. US LORs will always be more valuable than outside LORs, given your US experience most closely reflects what programs want to see from you in residency. However, home LORs can be valuable especially if from some prominent, eg your clerkship director, or the Chair of medicine or the PD of the residency program at your hospital. Also if the home LORs are well written and truly highlight your best qualities, they may prove very useful. So I'd say 3 US LORs and 1 home LOR sounds reasonable, unless the other home LOR is much stronger than one of your 3 US LORs.
