There is a reason why biting is in most cases considered felony assault. The human mouth is full of pathogens, it involves breaking skin and exposure to bodily fluids, and it is nearly impossible for it to be accidentally.
A human mouth is also significantly more infectious than say, a dog's mouth, they are harder to treat and take more time to recover from in comparison.

Bite from human to human would allow human-adapted bacteria to go straight into the bloodstream, thus higher chances of infections.

The answer to why Karen’s never lose their voice and can yell and scream all day. They have practice, their vocal cords are maxed out in strength from a lifetime of screaming like a spoiled brat.
Even some non Karens have that kind of vocal cord capability due losing their shit over something that causes them extreme levels of frustration I should know an example anyone can relate to is getting cut off in traffic you may not be a Karen but you'll definitely be yelling your ass off like one in that situation

A karen in there natural habitat. They are hostile, rarely calm, and they act like psycho's, they may bite, they may yell, they may act like the victim, but at the end of the day, they don't see that the only one who there embarrassing is themselves.

Behold a karen in her natural habitat, they commonly appear in neighborhoods, walmarts, gas stations, and more. Karens have the ability to fake injuries, they may play the victim, they may say your the jackass in the situation, they may hit, scratch, bite, but at the end of the day they still can't seem to figure out through there thicc skulls that there only embarrassing themselves.

When I see these Karens screeching orders like they're in charge, i'm always reminded of the movie Falling Down. Specifically the burger joint scene. As well as this quote from Freeman's Mind: "I have a gun, so I'm in charge! Many governments around the world function on this principle, AND SOME OF THEM LAST FOR MONTHS!"
Imagine, if you will, a Karen with a gun. Queen for a day.

The monopoly of force is a hallmark of a sovereign entity, but there isn't anything that prevents a government from not being sovereign. Haiti and Sudan are some good examples. Somalia is another.
Alternatively, if you take a look back in time, you can find interesting examples of power structures where this was subverted. Usually, in some kind of religious culture. So it might even be more accurate to note that government relies on the perception of the monopoly of force.
If you believe god will smite you, actual mundane enforcement might not be necessary.
