A 26-year-old man experienced hair loss in the last four years which compelled him to undergo a hair transplant. Now he is receiving contradictory recommendations from two different dermatologists with regard to the prevention of further hair loss. One doctor states for him to take minoxidil, while the other doctor states that he should undergo platelet-rich plasma treatment instead.
New York hair restoration specialist Amiya Prasad, M.D. discusses his concerns in this video, and talks about the limitations of hair loss medications and platelet-rich plasma alone as a treatment for pattern hair loss. My question with finesteride post your proceedure was concerning if it extends or helps extend life/quality of hair restoration gains as maintenance long term before you need a booster. I understand it's unneccesary as an adjunct but as a complement or strategy say for someone with aggressive loss. Thanks
Hi DR; I am 44, and i have been considering to do hair transplants, but 2 clinics said am not a candidate for hair transplant, but rather treatment, they recommended prp, at least 3 sessions in a year, but now that i am watching your video, if i understood right, you said prp is not enough but rather in concretion with soothing else. That is what i am not clear what is the method that you are using. I know you were asked how much it cost and you answered half what it cost a transplant in the U.S but i am not familiar with prices over there but rather with prizes in Turkey. Hello again many thx for previous answers to my questions. Finesteride for 10 years has left me with thick but relatively sparse frontal and bald spot area hair, although it did stabilize existing hair: now nothing and appears to be slighly deteriorating. Looks a bit like a wig to be honest. I am hoping your technique will fill the gaps in scalp. I feel that though I thought it improved my thinning, did not address density/thickness. I am 41 being on finesteride from 30. I had/have diffuse thinning concentrated on back and line going from frontal peaks across. Is a consult by skype usual for foreign patients? Cheers.

I talked to a PRP treatment center here and I asked them about the frequency of treatment using Acell. They said it is 3 to 4 sessions every month followed by a follow up after 6 months-1 year. Yours uses Acell yet it is much less frequent.

Sorry, I'm not aware of Dr. Ball or the Maitland Clinic. I don’t administer PRP alone for hair loss because it has a limited effect on hair, usually just short-term hair growth. Also, the number of sessions needed does vary, and the longevity of results is unpredictable. Hair Regeneration is a PRP combination treatment where PRP is the activator of the main ingredient extracellular matrix by ACell. On the video, the Dr. said that Minoxidil does not stop hair loss. I used Minoxidil for 15 years on a regular basis and it sure seemed to prevent hair loss to me. Nobody would comment that my hair was thinning or that I was losing my hair. For some stupid reason, I got lazy and stopped using and my hair has thinned quite a bit and I started getting comments about losing my hair or my hair thinning. When I first started using Minoxidil, I could tell within 6 months that my hair was much fuller and I saw "new" hair grow in areas that were recessed or thinning. I always thought that I was a very good responder so results may vary.

Good to hear that minoxidil worked on you for 15 years, as for most people the effects wane long before then. Minoxidil appears to work by delaying the hair shedding cycle, and extending the growth cycle so you have more hair on your head at a given time. It sounds like that's the benefit you got. However, if you stop using it, you can shed more than usual, which also sounds like what happened to you. I hope this helps.
@@AmiyaPrasadMDI had a different protocol. I only used once in the evening but I would put up to 10 ml per application, making sure that my whole upper scalp was covered well. I would also cut back to 5 days per week after I used every day for a couple of week and would even reduce usage a little more once I started seeing results. The first sign that things were working would be shedding of hair that would last a week or so from what I remember. The first time that this happened it worried me but later I realized that the shedding was caused by dormant hairs being discarded causing more hair to enter the growth stage. Of course existing hairs would get thicker and stronger and I could easily tell that this was occurring.
I don't really thing that my hair is worse off than if I hadn't every used minoxidil. Given how my hair looked when I was 30 and started using Minoxidil, I actually think that my hair would be worse if I didn't use minoxidil. Of course I have no way of knowing. I haven't used Minoxidil for about 8 years but I am thinking of giving it a try for 6 months or so to see what kind of results I get. Just thickening existing hair would improve appearance.

The Hair Regeneration treatment can only work on viable hair follicles, so if your hair is thinning, or if you have dormant hair follicles, it can save and thicken that hair. If the hair follicles are dead, then there is nothing left to save.
At your age, hair transplants are not advised because hair can't be transplanted as fast as you are losing it, and expected hair density in the 20s cannot be achieved by hair transplants. If you try to keep up with hair loss at this age via transplant, you will have no donor hair left by the time you are 30, and you will still continue to lose hair. I have seen clinics separate the plasma from the blood and inject the plasma into the head. But this clinic I notice they injected the Platelet rich blood on the bottom of the tube with the separated plasma blood. My question is, which part of the draw should be injected into the scalp? Is it the plasma or the Platelets in the blood? The light yellow plasma or the blood? Thank you

The Hair Regeneration treatment is for viable hair follicles such as thinning hair and dormant follicles, but it can't bring back dead hair follicles. A hair transplant depends on your age, and the viable hair you still have. I would need to perform a physical exam and get the details of your loss before recommending something specific for you. Is it true that you can get off of Propecia and Rogaine, after getting Hair Regeneration from Dr. Prasad? I fear that stopping either, mainly Rogaine, will cause a bad shed, where hair that's lost from not taking it, won't grow back.

Yes, you can stop using finasteride and minoxidil after the Hair Regeneration treatment. Minoxidil doesn't thicken hair (neither does finasteride) but it does delay the hair shedding cycle, and prolongs the growth cycle, so you may get some excess shedding after you stop using it. However, the Hair Regeneration treatment enables those shedding hairs to grow back thicker. Usually, my patients do stop using finasteride and minoxidil once they notice the effects of Hair Regeneration working better than their previous medical therapies, which is about a month or so. Sorry to hear about your problems. While stress can cause temporary shedding known as telogen effluvium, the hair does grow back. Epileptic seizures or epilepsy are not associated with hair loss. You may simply be having pattern hair loss at the same time as other health concerns. I hope this clarifies things for you.

After the prp treatment she said that i can stop using minoxidil because i had some side effects. I still have my doubts about quitting minoxidil. I still use dutasteride every day. Dutasteride is more beneficial than minoxidil as it can slow down and stabilize hair thinning, and it can be used long-term more predictably. Minoxidil won't thicken your hair, or stop hair loss, but it appears to delay the hair shedding cycle, and extends the hair growth cycle so you appear to have more hair on your head. You will have to use it daily to sustain these effects, but these effects can also decrease with time even if you use it daily. Since minoxidil delays the hair shedding cycle, and prolongs the hair growth cycle, if you stop using it, you will likely have a significant hair shed. I hope this clarifies things for you.

Your doctor is right about shedding after stopping minoxidil. Our Hair Regeneration patients can stop minoxidil, or are weaned off minoxidil slowly to prevent sudden shedding. Finasteride for someone your age with early onset of male pattern hair loss would need to be taken continually. Treatments for PRP alone require constant treatments sessions, not just 3 in the first year. I have been suffering from Polycystic ovarian disease for last 5 years.& doctors told that I am experiencing female pattern hair loss.
when I went to the gynaecologist,she suggestes me to take OCP only for 3 months for irregular menstruation,& my cycle became regular as long as I took OCP.But hair loss continued.
then I went to a dermatologist who told me to take dutasteride,giane-35, spironolactone & to use topical minoxidil.
but an endocrinologist suggested not to take dutasteride for its side effects, told me to start Metformin 500 mg BD & OCP regularly & to use minoxidil as before.
I have taken one cycle of PRP recently.
should I use Dutasteride with Metformin?or should continue my treatment?

Sorry to hear about your problem. Without a physical exam and no further details about your issues, it does make sense to continue Metformin for your polycystic ovarian syndrome, but ask your doctor why they prescribed finasteride as it is a medication for male pattern hair loss and for male prostrate issues.
Hi doctor I had a fue transplant for my receding hair line back in July (5 months ago) I started taking finasteride on and off for like the first month and half during my healing process then I kept on the for 2 months now I finished the 3 month course I stopped. Should I take them again? Also similar issue with rogaine I've now been off for a month or so should I get back on it? Not sure if I'm being paranoid and I'm starting to thin on my transplant or not. But if I get back on rogaine will I shed again 😔

Hair transplants do not treat progressive hair thinning at all, so treatment is needed after a hair transplant. Finasteride needs to be taken continually to block DHT-formation to slow progressive hair thinning. I do find finasteride or another DHT-blocker more effective and important than minoxidil. Minoxidil appears to delay the hair shedding cycle, and extends the hair growth cycle so you appear to have more hair on your head. You will have to use it daily to sustain these effects, but these effects can also decrease with time even if you use it daily. Finasteride can be used long-term more predictably. None of these injections stop production of DHT because sensitivity to DHT is the problem - people with thick hair still have DHT in their body, but they are not sensitive to it. Hair Regeneration overcomes the effects of DHT so hair grows thicker instead of thinner. Hair Regeneration is a one-time treatment that lasts at least 3-5 years. I use rutexil and it stopped my hair loss. If I stop using it then will my hair loss will be back. because i have heard something like that for minox but not for rutexil

Rutexil is a supplement, and is not an FDA-approved drug like minoxidil, so they can't really be compared. I don't prescribe or advise my patients to use Rutexil.

Sorry to hear abut your problem. At 20-years-old with that aggressive a hair loss pattern, you are most likely highly DHT sensitive. You do need a DHT-blocker like finasteride to slow your hair thinning. Biotin is vitamin B7, which does help with normal hair growth, and health, but it doesn't treat the genetic causes of pattern hair loss that causes hair thinning. I hope this clarifies things for you.
Minoxidil will MAYBE increase hair growth. But to combat hair loss, you need ANTIANDROGENS, such as Finasteride or Dutasteride to inhibit DHT production and stop hair thinning/loss.
I take 1.5mg Finasteride every morning (most effective to take it in the morning) and after 2-3 months, results really started to show nicely. There was a lot of shedding the first couple of months so that was really off-putting, but it decreased over time.
So far, I haven't suffered from any side effect from the finasteride. In fact, my libido has probably more than doubled. I suspect since that one's body isn't converting testosterone into DHT, there's just a surplus of testosterone in one's blood.
I hate minoxidil, and the stickiness it leaves in your hair and therefore avoid it. As long as you keep on using an antiandrogen, you don't need minoxidil.
I have read published scientific articles that state that hair growth increased most when combining minoxidil with finasteride, but I have personally experienced very nice growth with finasteride alone. Just be patient, and watch your hair get thicker from the finasteride alone. Interestingly enough, that same research paper stated that while hair growth and thickening was accomplished through finasteride alone, minoxidil by itself did not really accomplish anything...

If you are smooth bald with hair not growing in a particular area for 5 years or more, then you hair follicles are already dead, and a transplant is your only option. Treatments like finasteride to slow hair thinning; minoxidil to extend the hair growth cycle and delay shedding; and Hair Regeneration to thicken thinning hair can only be applied if you still have viable hair follicles. 10% minoxidil is a custom formulation, and the higher percentage doesn't necessarily mean it works better, but it can work after PRP treatment alone, along with finasteride daily. If you do have PRP combination treatment like Hair Regeneration, minoxidil is no longer needed, and the majority of patients don't need finasteride, but it may benefit some patients. It is normal to shed 50-100 hairs a day, but they do grow back. Hair loss happens when the hair grows back thinner. I suggest you do not use a straightener as it can damage your hair follicles. If Dutasteride is 0.5mg finasteride for prostate treatment, but to slow thinning hair, finasteride should be 1mg taken daily. Minoxidil delays shedding of hair, and extends the growth cycle, so it works differently from finasteride. You will need to take finasteride longer than 6 months if you want to slow your hair thinning for the long-term. If you want to reverse hair thinning so your thinning hair grows back thicker, without the need of all these other treatments, look into the Hair Regeneration treatment featured in this video. Hello Dr Prasad. I would like to ask a question. Is the function of PRP and minoxidil the same? They both are supposed to bring blood to the follicles, they both are good at slowing down the thinning, they both elongate the growing phase of the hair, they both have to be used lifelong to keep the hair and they cant stop MPB as a standalone treatment. You are right that PRP and minoxidil does have a similar effect as neither really thicken hair, but can provide short-term hair growth. The best option to retain your existing hair, aside from Hair Regeneration, would be finasteride. Finasteride does not completely stop or reverse hair thinning, but it does not slow it down. Finasteride stops the conversion of testosterone to dihydrotestosterone (DHT), with people sensitive to DHT having hair thinning in the scalp. It also needs to be used daily like minoxidil to sustain its effects, but you can stop taking it after the Hair Regeneration treatment.
Having a hair transplant done in your frontal hairline will not be affected by PRP. In fact PRP will help your grafts heal, but graft healing is better with two wound healing materials being ECM and PRP. I hope the additional information helps you, and I look forward to seeing you soon.

Multivitamins may help with hair quality and texture, but they will not stop thinning as it's a genetic, not a nutritional cause. There is no permanent cure for hair loss yet.

If by losing you mean shedding, this is normal, and shed hair grows back.
Pattern hair loss is not hair shedding, but progressive hair thinning. Hair thinning of native hair, and grafted hair prone to thinning still continues after a hair transplant, especially with FUE transplants that take hair grafts outside of the permanent zone (hair genetically resistant to hair thinning). Finasteride is generally not prescribed to women. If you don't have active hair thinning, and your transplant was only to lower your hairline, treatment is not needed for your native hair. In fact, many of my Hair Regeneration patients were using PRP alone for hair loss prior to seeing us. They felt they weren't seeing the results they wanted with PRP, and/or couldn't keep up with the schedule of regular shots.

If you started thinning at your crown just recently, you are at a border of medium DHT-sensitivity, so further examination is needed to determine if you need a DHT-blocker like finasteride with the Hair Regeneration treatment or not. Minoxidil can also help the crown, but it doesn't treat the cause of hair loss. PRP alone only stimulates short-term hair growth,so I would recommend a treatment like Hair Regeneration and possibly a DHT-blocker. I hope this clarifies things for you

Sorry to hear about your problem. You should first see a doctor to diagnose if you have male pattern hair loss, or another cause of your hair loss. If it is confirmed you do have pattern loss, get a prescription for finasteride to slow your hair thinning. You will need to take finasteride daily at its full dose to slow hair thinning. Minoxidil can also delay the normal shedding of your hair. Keep in mind that hair shedding is normal, but hair loss occurs when shed hair grows back thinner with each growth cycle, until it eventually stops growing back. Finasteride won't stop thinning completely, but it will slow it down. Best of luck with your diagnosis and treatment.

You need to discuss this with your prescribing doctor as the usual daily dosage for oral finasteride in male pattern hair loss is 1mg. Oral finasteride is more effective than topical as it blocks DHT-formation in the bloodstream, not just once it reaches the scalp. I hope this clarifies things for you.

1mg a week is too small a dosage to be effective. As I said previously, the standard dosage for finasteride is 1mg daily. The side effects of finasteride are rare, and if you have them, you can stop taking the drug, or use an alternative like Hair Regeneration featured in the video. Finasteride is a systemic drug to block the conversion of testosterone to DHT, and needs to be taken daily for life to be effective in slowing hair thinning - if you only take it for a few months, the effects will wear off, and your hair will start to thin again. You won't necessarily experience faster hair growth if you take additional medications. The reaction time for Hair Regeneration, as well as minoxidil and finasteride, including ru58841, depends on where you are in the hair growth/shedding cycle. If you are planning on getting the Hair Regeneration treatment, and not currently on hair loss medication, I suggest you have the treatment done first as you may not need additional medication. Hair loss drugs require daily maintenance, so it is better for you if they can be avoided.

Scalp massages can help, but hair growth is fairly slow so it won't make a significant impact on speed of hair growth.

Sorry to hear about your problem. Minoxidil won't thicken your hair, or stop hair loss, but delays the hair shedding cycle, and extends the hair growth cycle so you appear to have more hair on your head. You will have to use it daily to sustain these effects.
You need to consider slowing down your hair loss progression with finasteride, or stopping and reversing hair loss progression with a treatment like Hair Regeneration. Both these treatments can do more than minoxidil. I hope the added information helps you.

The Hair Regeneration treatment is primarily for hair that is still present, but thinning. Thinning hair still has the hair follicles in place, so as long as the follicle is still there, the hair can be saved and thickened. If the hair follicle is no longer there, there is nothing left to save.
Hair Regeneration is a one-time treatment which lasts at least 3-5 years, although the first patients from over 5 years ago still haven’t experienced hair thinning again. Only time will tell how long the treatment really lasts. Some patients with advanced hair loss get a second injection in the second year to build on results, but the vast majority of patients only need one treatment. I hope this clarifies things for you.
