So far I haven’t had any major issues with mine. The OM606 Diesel engine is very good. 96-97 gas models had head gasket issues (E 320) and throttle actuator issues (E 320 and E 420). All 96 automatics (722.4) have a glass jaw reverse gear. 97 - 99 automatics (722.6) have issues with bushings in the transmission. 97 - 02 automatics have an issue where it leaks trans fluid onto the connector which can wick into the TCM itself, so keep an eye on that. If you’re handy fixing things yourself they are relatively easy to work on, but expensive as hell if someone else has to work on it for you.
To be honest I have no experience with the W123, but I do know that the 123s have fully mechanical fuel systems, the W210s are electronically controlled. The OM606 is a DOHC 24 valve inline 6, fairly complex compared to the old OM615, 616, and 617.
