Easiest way I've done it is by forming Italia, pressing neighbor kingdom claimants and vassalizing them, then withdrawing their titles and giving them to family, and setting them free. Was able to get it within a generation or 2
You don't need Ironman to get achievements anymore, that means you just need 10 kingdoms out of your de jure empire given to dynasty members and grant independence. After that just go back to your save file. Arguably the hardest if you do it the fair way though, without forming an empire.

I made the Pope a witch once, I did it as a punishment after he tried to claim the kingdom of Ruthenia from me in a crusade. Sadly didn't seem to do anything, but maybe that's changed since last I played.
I've built a lot of empires where I gave 10 kingdoms to family members, but I never bother with the decision because I don't want to grant them independence.
So lucky getting the witch event pop up so early. I got it 300 years into my playthrough when my dynasty had hundreds of members it's kinda imposible to get enough of them converted for that decision cause more kids just keep getting born faster then I can convert.
After a while tracking down your dynasty members and who you need to kill in order for them to inherit becomes impossible without using an excel spreadshit (or otherwise noting it down). Totally worth it though, as it's still the most enjoyable way to play the game for me (got the idea from your "ruling Europe from the shadows" video and trying it out currently with a tall Bohemia -and a cheat character along with tons of save scumming; can't help myself...).
This was easy for me when I played England, I conquered scotland, wales and ireland, and france will be next since I have a claim on it, I thought it was going to be a long siege since I don't have strong allies(my only ally was Leon), but when I landed my troops in normandy and regrouped them to siege Paris I luckily captured the King of france making my warscore from 11% to 100% real quick ending the war, I created the empire of france and britiannia in order for me to gain a claim on aquitaine, britanny, and arles, I conquered the 3 kingdoms again. Finally, my character was the emperor of france and britannia, I gave the kingdom of scotland, ireland, wales, britanny, aquataine, arles, and france to all my relatives, I was about to take the Iberian kingdoms but its was already 1453 which is the time the game ends since I'm playing ck2 without dlc. Imagine just because I luckily captured a king I became an emperor lol😂
I had a fun playthrough where i managed to get this, started in italy as the duke of tuscany, usurped italy, decided to move to Sicily, so i gave italy to a brother, formed sicily without all the dejure moved to napoli made the sicilian parliment then swore fealty to the Byzantines, warred italy back, usurped the byzantine empire, warred the papcy and stole the popes hat, formed reformist christanity and named myself the head of faith since i owned both rome and Constantinople, made it communion, armed pilgrims, and monastic, then crusaded for isreal. Holy war for alexandria, duchy holy war for tunis. Which allowed me to mend the schism and reform the roman empire. Afterward my vassels started conquering north through bulgaria and russia, and having mended the schism most of western europe bent the knee. Having done basically everything i decided to just destroy the empire titles which granted everyone independance allowing me to grab dynasty of many crowns since my family had become fairly wide with just shy of 200 members i was giving titles to telatives the whole playthrough so yeah almost a perfect run in my opinion i loved stealing the popes hat to then become the new pope
Crete, Cyprus and the Canaries are all kingdoms that are small and easy to conquer. I have in the past gotten the duchy of the aegean islands minus the county of Abydos (which is on mainland Anatolia) but plus the county of Euboea (who I granted to a count and transferred vassalage to the duke of the Aegean Islands) and grant the duke as a vassal to the King of Crete (I had an empire title by that point), and let dejure drift do the work. Then you're truly an island nation. Might as well try take that island duchy between yemen and somalia as well
There should be 2 decisions for securing the Mediterranean 1. Secure the Mediterranean (just take over Sicily, corsica, Sardinia, and Mallorca) 2. Complete Mediterranean dominance (rule over krete, cyprus, Aegean islands, Malta, corsica, Sardinia, Mallorca, and sicilly)
I did this on my Outremer run. Was super hard because all my kids I kept giving kingdoms to were morons who would lose their thrones within a few years.
I’ve just started my first serious near Ironman playthrough and I chose to be the basque ruler I can found the kingdom of Aragon but I also own Sardinia almost so perhaps I’ll take this decision I didn’t know it existed

so i was doing a similar kind of start got a claim on pisa from the pope which is a king level title then i swear fealty to the hre and they vote for me to be heir and i do a plot to kill the emperor he dies first try im emperor enemies everywhere everyone wants me dead so i give the pope all the lands he forms an emperor level title as the pope and after a bit the hre title gets destroyed and i swear fealty to the emperor pope 10/10 i dont think that would happen again if you tried a million times. it was a at point limit created character lmao
