The fact. NIH did Not shift money from other disease research to covid research
most likely due the money was already earmarked for other research, many studies eg. Alzheimer's research and cancer research are longitudinal and take a period of years to get data that's meaningful, yes more money should have been put in Covid research, but congress and both presidents placed their bets on vaccines. But the COVID virus became a moving target as all viruses do,
the backlash from antivaccine group did not help
The chasm between personal freedom is even wider now.

As a healthcare provider and educator, I have been continually frustrated at the lack of evidence based policy, the slow response to new evidence within policy, and now the utter lack of research on so many aspects of this pandemic. Corruption, political posturing, power plays, inefficiency, and ineptness should have no place within local, state, and national departments of health, yet it seems that's largely all we have.

I am a retired European professional maintenance and reliability engineer in heavy engineering. I would study projects for feasibility, safety and numerous other factors. So many projects appeared to have been based on emotional ideas formed on the golf course by totally incompetent top managers. I am not talking about little projects but multi million and multi billion dollar projects with multi national top level companies.
There was so often basically no proper thought put into the project ideas other than 'everybody was on board with it'. Therefore I have no trouble believing that the NIH would also think it was a good idea to spend more on rare diseases that almost no- one catches, than on Covid which almost everyone has caught.
What I would like to know is was was the NIH funding via an intermediary the Wuhan institute as reported, and if so why?

Dr. Moran, since this presentation is provided mid 2022, it reasons that there are trends that should be included with respect to the CDC and NIH budgets. For example, what were the actual expenditures in 2021 and what is the budget for 2022 (half of the fiscal year is complete). While I agree with you and the authors of the study that the institutions were slow to pivot, it would be a more complete discussion to understand where we've come. We see many missteps, but we also see (hopefully) opportunities to learn.

NIH funds research and anyone who has applied for a research grant knows that it takes a lot of work and a lot of time to submit an application and get it approved. NIH is not a rapid response agency. The US barely acknowledged the virus and it's presence in the US until March of 2020. This portfolio is the legacy of the past recent years of funding and you acknowledged that new funding in 2020 was not separated out. The NIH is also known for the high quality of it's research and, of course, it takes time to do quality research. Check back in 5 years to see if NIH has reallocated it's portfolio in response to the pandemic and check to see where Congress has placed this as a priority. Congress is currently reducing funding for the Covid response. The problem is not with the NIH. The problem is that the US does not have a pandemic response agency and does not have a political and regulatory structure to either advise, establish, and enforce whatever emergency response is deemed needed. Authority is distributed to the States and their public health officials. There is no national health system. There is no way to enact a unified response and American culture would resist and has resisted any attempts to do so.

Do you think that the revolving door between the NIH, FDA, CDC and pharmaceutical companies presents a large conflict of interest? What do you think about the head and deputy of vaccines, Dr. Marion Gruber and Dr. Phillip Krause, at FDA resigning? Do you think the doctors at the cdc should get royalties for drugs resulting from their research, as Fauci does? What is your view of Pfizer refusing to release test data for 75 years? Do you think lack of transparency and apparent conflict of interest significantly harms the credibility of these agencies and raises valid questions about accuracy of their data and the ethics of their decision making?

We have the non-emergency system of funding research that goes slowly on its way and we have little capability to pivot to short term research for emergency situations like COVID. I wonder how an agency like NIH would even identify researchers capable of conducting the research, have an expedited proposal and contracting process, and peer review the results in the timeframe that the answers are needed for policy decisions. It seems like we needed questions like mask effectiveness and effects of social distancing answered on day one. Many topics need time to pass for the disease to cause its effects. We did not know the age distribution and comorbidities until well into the pandemic. Now we need to know if the vaccines are still effective, if multiple boosters are appropriate, and if natural immunity will provide adequate protection for the unvaccinated. Is that research underway in the US or other countries?

Among many other research avenues unexplored were those relating to long covid. There are many who might have benefited.
One problem with such research was that likely therapeutic programs involving, for instance, drugs like ivermectin, could not be performed except in vitro because ivermectin has been effectively outlawed in the US/UK.
It's not just that funding was inadequate. Such areas of research were actively discouraged. Doctors/scientists attempting to perform such research were and still are subjected to various forms of personal and professional disparagement.

Dr Moran should do a video on the influence of political thinktanks and how much influence they had on the pandemic. You shouodnstart with the AIER and Brownstone movements and track that activity through lobbying efforts around the world. Its odd people objected to the usa spending money on corona virus research when it had anything to do with lab in wuhan and politicised the entire notion of gain of function being something nefarious. (Which it isnt) But now its the governments fault again. American people, and more than half of your listeners , chose to turn their back on science because of political bias and the willingness to believe some youtube influencers helped to notoriety by thinktanks. Yet now they're upset we didn't waste more money on science they would ignore anyway. Can't have it both ways. Seems people hate socialism and capitalism yet dont trust experts working for universities or trust big companies. So who do you actually want to make vaccines and conduct research??

Bureaucracy and politics each on their own are not well suited to handle emergencies let alone the combination of both - this is what these organizations have become. Add to that the complete disregard for practical experience and prior scientific knowledge to be replaced with all emphasis on start from scratch pure science with all its careful hesitancy in coming to actionable conclusions and it gets worse. It took weeks for hospitals to allow the use of dexamethasone in critical patients.

Non pharmaceutical interventions attracted 5%, 1/9th that of Pharmacies. When will Americans realize they are being forced to ONLY look at expensive pharmacies when natural treatments have merit and deserve more publicity. This applies to a range of common disease processes. If 40% of dead covid patients were diabetics - DIET MATTERS.

Your comment about preparing for next pandemic is sound- and some benefit for this pandemic is still possible. Ongoing monitoring of death rates in unvaccinated versus various levels of vaccination (adjusted as much as possible for risk level) should be happening, but it is difficult to find such data.
Valuable lessons can be learned from routine monitoring of this type, which might not require a research grant application.
Mask studies had been done prior to Covid on other pathogens; transfer of that knowledge to Covid would have been sensible in the early phases, especially as Covid masking studies would have had difficult ethical issues.
Messaging needs studying too; many people use the term ‘doesn’t work’ to mean ‘not 100% effective’ and that has been very unhelpful throughout the pandemic. Pre-existing mask studies were quoted as showing 40% reduction of transmission- that was however for a single contact; real life involves chains of contact so one could be looking at 0.6x0.6x0.6 and so on - which means a potentially larger impact in wider society.
I don’t know if you have tried to tackle the problem of design of an RCT for mask wearing , taking into account the number of difficult factors (rate of wearing, prevalence of virus, risk levels of participants, compliance levels to name just a few; I am sure you can come up with others). The 40% figure was a 1:1 laboratory figure, with volunteers and a different pathogen.
    Finally; R number 1.25 = steady increase. R number 0.75 = steady decrease. The difference is 40%.

Seems everything we hear about this whole covid mess sounds like it came out of a three stooges movie. Its just sickening finding out for sure no one is watching out for us. That not one responsible person is in charge. Maybe its good we found out before the next killer wave of something else finds us so we can learn we must fend for ourselves because those in charge are asleep at the wheel.

Why research when you already have the cure? The treatment was already developed before the problem was released.

Hi Doctor I hope you are keeping well, would you do a video on monkeypox? I had chickenpox in 1987 when I was 7 & my 2 daughters ages 6 & 7 recently had it & they're better now, if you had chickenpox in your life does it give a bit of immunity against monkeypox? Probably not 100% but would it give a more milder sickness of monkeypox if been infected with chickenpox before.

7:13 Public trust in a bad medical profession is not important.
A skilled well educated independent medical profession is important even if not trusted.
Similarly if I am a passenger on a plane my trust in the pilot is nothing, his ability as a pilot is everything. 6]
