Blood clots are always a potential risk with contraceptive pills, but if you have a clotting disorder on top of that, the risks start to outweigh the benefits and an alternative contraceptive method would be safer. And yes, pregnancy is another counterindication. The pill wouldn't abort the pregnancy, because it is just pregnancy hormones, but it could cause serious adverse affects.

Had Dr Manning referred her to a gynaecologist or family medicine doctor or family planning nurse, instead of wasting ED time and resources, they would know to interview about her sexual history. A young healthy teen doesn't need lab work. You just need to ask about sexual history and maybe to a blood sugar test. Dr.Manning would lose her license over falsifying medical records or at least be reprimanded for misuse of the ED.
If they are giving a minor they don't have the medical records of some hospitals do require it to cover their buts. The girl is there anonymously and her father works there so accessing her medical records wasn't an option.

It's not perfectly safe if you're pregnant but it's less of an issue than an IUD and other implants. So they generally take your word that you shouldn't be pregnant and in this case the girl was implying that she wanted birth control to start getting intimate with her boyfriend. But as the doctor was doing a quick screen she would have also done the pregnancy test and of course gotten the shocking positive results that she redid to confirm.

This made me laugh as I'm highly fertile and get pregnant even on birth control, the pill, implant, injection you name it I've got pregnant on it !!
Also, hormonal BC can take months to become effective
actually birth control is not 100% efficient either, u could still get pregnant on some of those.
