@@drkopelman Love your vids Dr. Kopelman! Especially your good-natured debates with your colleagues! Question...I'm 3 weeks post-upper/lower bleph. Upper eyelids are terribly tight & they pull each time I blink. Also, when I go to close my eyes naturally, they stay open about 1/8" inch. If I force them down they will close all the way, but I can feel the pull/stretch in my upper eyelid when doing so. Did the surgeon remove too much skin? Is this upper eyelid/incision tightness & inability to gain total eye closure temporary/part of the healing process (and, if so, how long does it usually take before you can close your eyes totally without force & can no longer feel the tightness/pull with each blink)? Thank you, and keep the vids coming!
Pineapple takes away inflammation. I have a client who is an eye surgeon and he told me he has his client eat pineapple before and after surgery. You can also get pineapple extract in a pill form at the health food store. I got my brother on pineapple extracts for inflammation and pain in his whole body and he no longer has to take Vicodin.
As an oculoplastic surgeon, I can appreciate the benefits of pineapple and bromelain for reducing inflammation, especially when combined with other proven post-operative care strategies. In addition to natural remedies like pineapple, I typically recommend a few key approaches to help with swelling and discomfort after eyelid surgery. Applying cold compresses for 10-15 minutes several times a day can be very effective at reducing swelling and easing that tight feeling. Elevating the head, even while sleeping, helps minimize swelling by promoting proper drainage from the surgical site. I also sometimes suggest arnica supplements, which can assist in reducing bruising and swelling, though it’s important to discuss any supplements with your surgeon beforehand. For more significant discomfort or inflammation, over-the-counter medications like ibuprofen are usually sufficient, though in some cases, I may prescribe something stronger. While bromelain is a great addition for some patients, combining it with these post-op strategies will help ensure a smoother, more comfortable recovery.
As a fit 82 year old, should upper and/or bleph be avoided? If not, are there any special procedures to apply since skin is so much thinner at this age? I appreciate your videos.
Just had a lower blepharoplasty on Friday. Nice juicy plums under my eyes but no pain. Done by experienced eye surgeon so i hope the results will be great.
I had this done on my eyes years ago. I did not like the way it came out the first time. The dr had no problem doing the procedure again. But this time the skin from the incision and below was completely reddish and has remained that way ever since. The dr said he had only seen this happen once before. And because it’s the eye lid they could not correct it with a laser treatment. I researched this dr and he was a top surgeon here in MA. So even with the best there can be an unhappy outcome.
In regard to your upper eyelid incisions you may want to consult an aesthetic dermatologist who may? perform a pulse dye laser treatment for the residual reddish color. Unfortunately even in the best hands complications do occur.Any surgeon who claims that they don't get complications is not telling you the truth.

Hello Dr. Kopelman -- I am 76 & had "saggy eyelid" surgery in 2017. The lids are starting to sag, again. How many times can you do this procedure on someone? Thank you.
Nope. I do not think it is necessary. Especially since eyelid surgery frequently is done under local anesthesia where you are not being put to sleep but where they area is just numb. You can of course continue to take your laxatives if you need them before but this procedure will have no impact on your bowel movements after and eyelid case.

Hello doctor! I had a rinoplasty (for two old lesions giving me issues) and lower/upper blefaroplasty almost a year ago, inflamation and rye bags really getting smaller but after four months that I started using my contacts (I have very high prescription), I’m trying to take breaks and use my glasses but now I don’t really see a difference with the surgery, my eyes look very puffy again even more than before and smaller. What can I do? I was on arnica te for a while and it worked for me!
Compared to the dozens of other surgeries Ive had, this was a breeze, as you're under light anesthesia so other than a moment of stinging on one eyelid there was no surgery pain, a few Tylenol did the job afterwards. Interesting to find up to 2 weeks or more for the swelling to go down, been a week today and hope the temporary swelling is the issue making my vision even worse than before the surgery.
I had a surprise blepheroplasty at a teaching hospital in California as a result of skin graft donor site for a BCC on my other eye. The sutures were horrible with a lot of bleeding for days. I was never given an ice pack or any covering over my sutures. Just sent out the door oozing blood. Scar is horrible. Why are medically necessary blepharoplasties treated with so much disregard to aesthetics? I’m sad with the results. I know I need a revision because my eye is now drooping but am scared to see the same doctor. Do you know any exceptional oculoplastic surgeons in California that take insurance? I admire your kind demeanor and attention to detail. Thank you for these informative videos. I knew nothing about blepheroplasty until my surgeon decided to give me one.
Are there any sleep angle recommendations? Would a person that has had a PVD for 3 months be disqualified from this surgery or would you recommend not doing this with a recent PVD of that timeframe? I worry that it would possibly make the PVD worse or increase the chances of a retinal detachment, I hope I'm wrong.
Dr. I have blepharoplasty 2months back now I have hypertrophic scar in the lateral border of the incision side where they tie sutures and there is mild pain in that area also. I was diagnosed hypothyroidism after the surgery and I'm under medication for that. is this hypertrophic scar related to my hypothyroidism? And will this hypertrophic scar reduce by itself.
It sounds like you might benefit from 5FU injections into the scar. I commonly use that if patients develop hypertrophic scars. You might need 2 or 3 rounds of the injection but this should soften it and help resolve the issue you are experiencing.
Yes a midface lift can be accomplished at the same time as a lower eyelid blepharoplasty. The midfacelift can be performed through a browlift incision, or the midface can be augmented with free fat grafting
Thank you Dr Kopelman, I want to do the surgery my concern is I have a baby 5 months old his 20 pounds after 2 days of surgery I don’t have a help. I need to taking care and hold my baby… is that ok to carry him?
I’d recommend using artificial tears 3-4 times a day. It sounds more like you might have chronic dry eye syndrome and I wouldn’t assume it is from the eyelid surgery. I haven’t had patients who had issues with dry eyes after surgery for that many months out.
@@drkopelman Yes, had dry eyes syndrome under control going in to surgery. Also had SOOF same time. My surgeon is unconcerned…says be patient, may take a year or more. Really regret the whole thing…minimal results for 4 months and counting extreme dry eyes. Another eye doctor has me taking steroid drops, doxycycline pills, preservative-free Systane, etc. Very painful…wearing sunglasses for light sensitivity. Bad decision doing this surgery.
@@jeankirkham5230 Dont know what SOOF is, I am Dutch. Thank you for telling your story; i am considering to have an upper eyelid-surgery and I am glad people who are not happy with the results tell their story too! It is so delicate.. I dont know if I should do it now; maybe I will make myself look better with make-up and wait untill it really gives me trouble looking because there is too much skin under my eyebrows. I look good considering my age; maybe I shouldn't fix what isn't really 'broken'. Thank you again, have a nice day hope you are feeling better by now!
@@sunnya.2665 Probably you will heal just fine doing only the upper eyelids. I really researched the surgeon and he hit all the high marks, but afterward when I had all the complications ( horrible painful dry eyes, sensitivity to light, cysts all around both eyes, terrible vision, etc.) he only said to be patient. Finally found a good ophthalmologist who prescribed meds, drops, treatments to help. It’s been 7 months now since surgery and there is some improvement but it will take a full year for it all to be normal. The SOOF is a procedure through the lower eyelids to lift up sagging fat pads & attach to cheek bones inside…the fat pads sag as we age. I think doing upper & lower lids & SOOF was just too much. I’m 66 & look 10 years younger (before surgery). I just thought I’d improve myself…never thought it could take a year to recover. Needless to say, surgeon never did take the “after” picture! Best of luck!
@@jeankirkham5230 Thank you so much for your response! I am 58; look ten years younger 😉 and would like to stay that way a few years more! The plan was only my upper eyelids. I think about it, here in Holland January/february is the best time to have this done; not match sunshine and it's dark outside early so.. i think i'm going to make an appointment tomorrow! Thanks again!!
Hi doctor, is it possible for your eyelids to look the same post surgery swelling? I’m Asian, had double eyelids already but wanted it bigger ( more western look ) I’m four months in now (your videos helped me a lot mentally prepare for it!) and I feel like my eyelids look quite the same before surgery. I actually didn’t kind it during the swelling stage as it gave my Asian eyelids a more western look! But now it’s gone back to my pre op lids. I cannot seem to find any information on this online.
I had upper/lower bleph early April. About 3 weeks ago I suddenly developed multiple styes, one on one upper eyelid & multiple ones on the lower lids. Opthamologist prescribed drops and ointment, but still have some on one of my bottom lids. I was fine for 3 months. Just curious if you've ever heard of this as a complication from bleph. Maybe the oil glands being disrupted or something during the procedure that causes them to not work well? My eye doctor has never heard of that..just wondering if you've ran across that before.
Dear Doctor - is it normal that 19 days after blepharoplasty below, one eye is still swollen and the other is not? Is it the fault of the internal seam? What to do?
Could be asymmetric tissue removed or a response to the suture material. If you want me to take a closer look you can book a 1:1 video consultation through my website and I’ll be able to figure out your issue and how to resolve it.
silicone gel can be used after two weeks but be careful especially on the upper eyelid that you don't get it in your eyes because it can irritate your eyes
Hi Dr. I had upper and lower lid surgery 3 weeks ago and I’m concerned with the healing process? Waited 10days to take stitches out which was torture as my body was fighting the stitches and bumps formed around it. Upper left lid has a visual scar above inner crease, lower lids are healing but one eye seems smaller than the other, as one has more fat underneath so the eye looks weird. Also it seems as if the lower incision is directly in the outer corner of the eye and not out past the end corners and this makes my eyes seem really tight. Won’t see my surgeon until a month out. Should I be concerned? Thanks for any feedback you can give.
Would applying slight pressure with a warm compression reduce swelling after 4 days or no? The swelling us pushing on the levator muscle. Should I do eye exercises or how can u reduce the levator muscle swelling?
I had blethroplastic surgery (ectropn) of the upper eyelids. It has been 5months since the first surgery. I have excess flesh on the edge of my lid, with the eyelashes appearing in the middle of the lid. Is this a normal outcome and will it go away?
I think you had an upper eyelid entropion and some times they intentionally over correct because they anticipate that your eyelid will revert slightly to your original state This May correct in self with time
Thank you for sharing your video. Im thinking to have my surgery. My eyes is uneven one is big one is small. The right eye is big and have fat and I am not happy about it.
My experience having this surgery was horrific. Restasis controlled my dry eyes but after surgery, eyes were unbearable. It’s been over 4 months with little improvement. Couldn’t read, watch TV, or look at any screens until just recently. My vision is soooo bad. Every ophthalmologist has a different idea….antihistamine for allergy, steroid pills and drops, antibiotics, warm compress, cold compress, surgeon ignores my complaints. Very scary & frustrating. Definitely wasn’t simple recovery for me!
@@tinamartin450 No. I am seeing an ophthalmologist who has prescribed steroid eye drops, PF moisturizing eye drops, bruder mask, humidity, no fans, sunglasses, no watching screens, etc. for past 4 months. He said vision and dryness has improved a bit, but need to continue all this for at least a year. Very unexpected outcome and rotten deal for sure. It’s just terrible not knowing if it all will ever resolve. Good luck!
I had upper blepharoplasty done 6 days ago and my sutures are out. However, my eye is “folding” lover than my incision. Also, the scar one eye is very visible. My wedding is is 2 weeks and I am worried since incision is not where eye is folding, my scar will be visible even more with makeup on. Also, the lid is so small now. Will this change? Thank you 😢
Having cosmetic elective procedures prior to a major event is not a good idea. If you are planning on having cosmetic surgery don-t do it for at least 8-12 weeks prior to your special day to give yourself enough time to heal.
I am assuming you just had eyelid surgery. If so yes, I recommend to my patients to switch from using a cold compress to a warm compress after 48 hours.
Hi Doc, is it normal that after your upper and lower bleph the under eyes gets sunken or hallowed? Cause mine is more noticeable a week then after a week its more noticeable. I’m worried 😢
I had upper and lower eyelids surgery last October of 2022 with one of the plastic surgeons here in Orlando,Florida. I was not totally satisfied with the results. I’m thinking of getting a consultation with another Md later on .
Great question Sophie. The answer that I give my patients is immediately you can wear either glasses or contact lens after eyelid blepharoplasty surgery.
Hello, doctor. How soon after the procedure can you resume physical exercise, and what are the specific risks associated with performing too intense physical exercise too soon during recovery?
I recommend a two week vacation from intense exercise where your blood pressure rises quickly this can lead to late bleeding. You must avoid getting an exercise induced hemorrhage. To play it safe you can walk or use a stationary bike for two weeks following surgery then you can resume your normal routine
People of color tend to scar more easily. If you have a history of hypertrophic scars or keloids then yes the probability to scar following surgery increases
I had my blepharosplasty and pitosi done since Oct 31 of last year but my surtures and upper lid is still reddish and itchy, my doctor just told me to continue the ointment and i even took steroid for it but still the same aftr 2 weeks of taking the steroid, what do i need to do?
No Aquaphor is not compatible on your eyes You could use some Aquaphor on your lower eyelid away from the margin of your eyelid to prevent it frm contacting your eye.A better option is to use topical bland eye ointment which will not irritate your eyes
It sounds like you’re experiencing that common post-surgery feeling where your upper eyelids feel tight, or like there’s glue pulling on them. This is a typical part of the healing process, especially after upper eyelid surgery (blepharoplasty), and is usually related to swelling and tissue adjustment as everything settles into place. That sensation should start to improve within a few weeks, and most patients notice a significant difference by 6 to 8 weeks. In the meantime, it might help to use cold compresses or follow any specific aftercare advice you were given to manage the swelling and tightness. If it continues longer than expected or feels particularly uncomfortable, it’s always worth discussing it with your surgeon to ensure everything is healing properly. Each case is unique, so don't hesitate to reach out to them if you have any concerns!
@drkopelman the problem is it's been 60 days. Also, the bottom part of my eye , whatever it's called that holds my eyeball up i's not tight. My surgeon did say he goes back in after 3 months to tighten it up but I hope that's normal as well. I had my top and bottom done. I do thank you so much for responding.
@@thelucabrazzi If you want to book a virtual consultation for me to take a closer look at you, I am happy to. You can book through my website drkopelman.com
Yes because when you get blepharoplasty surgery you are not having surgery under the eyelids where the shunt would be. My recommendation is that it would be safe to proceed.
Initially we want cold compresses to constrict blood vessels to inhibit capillary bleeding .After three days you can switch to warm compresses which dilate blood vessels and help to accelerate the resorption of bruised tissue.
Ok..thanks...tell me please when I have to put cream like Dermatix silicon gel or Strataderm gel on my scars?Have I do massage on my scars and with what? Now I am two weeks after surgery and have thickened scar under the left eyelash which is hurt me...what I shall to do?thnx

For ptosis repair or a lower eyelid transconjunctival blepharoplasty this is absolutely the correct method. An upper blepharoplasty requires an extenal incision.
Hi. I had external lower Blepharoplasty. Not sure if that's how my plastic surgeon chooses in general or if it was in my case. I did not know to ask him ahead of time and he did not discuss. Now I'm wondering if it was the wrong way to go. In addition, I had pain for way more than two days and I'm still having pain on my scalp from my brow lift. Severely numb on my forehead and pain and numbness on the top of my scalp. Also was an unknown prior to the surgery. I am afraid that the nerve damage will be permanent. Thank you so much!
Oh, and I had ptosis repair along with upper and lower Blepharoplasty and brow lift. So now I really wonder why the incision is on the outside under my lower lashes. I don't want to be rude or sound challenging. If I ask why he didn't go inside the lower lid. But I am curious as it's for my own notification.
If you have a ptosis issue with your eyelids then yes. I would encourage you to watch my ptosis video to understand the difference between that and a blepharoplasty. A blepharoplasty though is about removing excess skin from the eyelid and is considered cosmetic. Therefore extremely rare for a blepharoplasty to be covered by insurance.

Very helpful video. Would you recommend blepharoplasty at the same time as a mid facelift? Or would it not matter to do separately? Thank you.
Did you have the surgery? Interested in how it was while taking care of a baby
Very informative video, thank you ❤
Hello Doctor, Can we use eye drop after blepheroplasy for dry eyes?
Yes. Definitely 💯
I had upper and lower blepharoplasty and still “on fire” eyes due to terrible dry eyes 4 months so far!
I’m Asian, had double eyelids already but wanted it bigger ( more western look )
I’m four months in now (your videos helped me a lot mentally prepare for it!) and I feel like my eyelids look quite the same before surgery.
Fantastic information thank you so much
Dear Doctor - is it normal that 19 days after blepharoplasty below, one eye is still swollen and the other is not? Is it the fault of the internal seam? What to do?
Could be asymmetric tissue removed or a response to the suture material. If you want me to take a closer look you can book a 1:1 video consultation through my website and I’ll be able to figure out your issue and how to resolve it.
I am the same I am 4 weeks on and one eyelid is swollen at the waterline
@@tracyr365 it is normal - take youre time :)
I have a question. Does this procedure cause blood clots that travel to the heart, brain and lungs too? I know about the hematoma part.
hi Doc. when can i use silagen after remove sutures? thank you Doc.
silicone gel can be used after two weeks but be careful especially on the upper eyelid that you don't get it in your eyes because it can irritate your eyes
I have thyroid eye. I wonder if there extra steps in doing this kund if surgery
I'v seen many top surgeon on UA-cam stating dissolvable stitches should not be used due to healing process
Can someone with dry eyes have this procedure Doctor? I think that the Miebomian glads would be affected.
6 weeks in and weeping eyes. The surgeon prescribes eye lid exercises. What's your call?
Is it normal to have little red bumpy color on your eyelids 2 weeks after surgery?
What about chemosis post blepharoplasty? How can it be treated??
This was great thank you so much 🎉🎉
You are so welcome! I hope your eyelid surgery recovery is smooth.
Will it be more painfull sir ?
Hello ,这个手术全部费用是多少?
I had blethroplastic surgery (ectropn) of the upper eyelids. It has been 5months since the first surgery. I have excess flesh on the edge of my lid, with the eyelashes appearing in the middle of the lid. Is this a normal outcome and will it go away?
I think you had an upper eyelid entropion and some times they intentionally over correct because they anticipate that your eyelid will revert slightly to your original state
This May correct in self with time
How are you doing now?
Can I go on a sun vacation after a month or two of the surgery? thanks a lot, Dr. Joel
Thank you for the info .
Thank you for sharing your video. Im thinking to have my surgery. My eyes is uneven one is big one is small. The right eye is big and have fat and I am not happy about it.
I have dry eye issues and am wondering if this surgery could damage Mybomian gland production?
No. Generally no impact.
@@jeankirkham5230
@@jeankirkham5230 I am 2 months out and having the same problems with blurry vision. Did you find a solution?
@@tinamartin450 No. I am seeing an ophthalmologist who has prescribed steroid eye drops, PF moisturizing eye drops, bruder mask, humidity, no fans, sunglasses, no watching screens, etc. for past 4 months. He said vision and dryness has improved a bit, but need to continue all this for at least a year. Very unexpected outcome and rotten deal for sure. It’s just terrible not knowing if it all will ever resolve. Good luck!
Do you have to take antibiotics for this surgery or can you get by without them?
You don't necessarily need them but I have my patients on antibiotics for just a few days.
Thank you 😢
You did cosmetic surgery three weeks before your wedding?!?! 🙄
Having cosmetic elective procedures prior to a major event is not a good idea. If you are planning on having cosmetic surgery don-t do it for at least 8-12 weeks prior to your special day to give yourself enough time to heal.
If I may ask? Dr would you consider this surgery for yourself?
Believe it or not. I do! My son will be performing my blepharoplasty in the coming months and I will share the experience here on UA-cam.
@@drkopelman Phenominal! Good Luck
So... after 48 hours of applying the cold compresses (frozen peas), I should begin placing warm compresses on my eyelids?
I am assuming you just had eyelid surgery. If so yes, I recommend to my patients to switch from using a cold compress to a warm compress after 48 hours.
Hi Doc, is it normal that after your upper and lower bleph the under eyes gets sunken or hallowed? Cause mine is more noticeable a week then after a week its more noticeable. I’m worried 😢
My cosmetic dermatologist does upper bleph under local anesthesia and uses dissolvable stitches, what are your thoughts on that?
Thank you.
AS long as you go to a surgeon who has experience with the surgery and the management of potential complications you should be fine
Great info
Glad it was helpful!
Hi i open stictes yesterday my eyes look pull down coz my surgeon cut to much skin n he put back skin can my eyes become normal like before
I had upper and lower eyelids surgery last October of 2022 with one of the plastic surgeons here in Orlando,Florida. I was not totally satisfied with the results. I’m thinking of getting a consultation with another Md later on .
Is it possible to remove stitches 5 days after surgery and fly after a week.
Yes

I had uppers n lowers and 6wks after I still have 1 lid larger than other

Can you wear glasses during recovery? When can you start using contacts again?
I had this done 3x worst decision ever. Eye looks worse. The upper lid is now lower. Wish there was a way to fix it 0:18 0:18
Can I still get blepharoplasty if I have mild blepharitis
Yes you can. That should not stop you.
Dr, I’d love to come and see you from Las Vegas, do you find darker skin have more scarring
I would probably stop the ointment because you may be developing contact dermatitis and I would consult a dermatologist for further recommendations
What is the cost?
Can I used (aquaphor healing ointment advance therapy)to hydrate my eyes 2day after my surgery to hydrate my eyes
No Aquaphor is not compatible on your eyes You could use some Aquaphor on your lower eyelid away from the margin of your eyelid to prevent it frm contacting your eye.A better option is to use topical bland eye ointment which will not irritate your eyes
arnica works! i had a hugeeee bruise and it was the only thing that helped it go down quicker
Does the end incision line that extends past your outer eye flatten out and heal or will this be a scar line forever?
Usually the incision line will heal very well and will fade with time but it may take a year
I developed eye bags after upper eyelid surgery the following day. Is this normal and does it resolve?
You could have simplify developed swelling post surgery. I would give it a few days and this should resolve completely.
@@drkopelman thank you. I’m starting to see improvement
How long for the feeling of upper eyes to stop feeling like I have glue or maybe soc made the top too tight .....for lack of description to go away ?

Can upper blepharoplasty be done on a patient with a glaucoma tube shunt in the eye
Yes because when you get blepharoplasty surgery you are not having surgery under the eyelids where the shunt would be. My recommendation is that it would be safe to proceed.
@@drkopelman thank you doctor
Cold compress freezes the skin arterias etc... it is better warm compress because we need the blood to flow
Initially we want cold compresses to constrict blood vessels to inhibit capillary bleeding .After three days you can switch to warm compresses which dilate blood vessels and help to accelerate the resorption of bruised tissue.

Doctor I have a question ❓ please 🙏 and this is the question, I have Droopy eyelid then when I get headache the side of Droopy eye is always pain thank you
Sounds like you might have ptosis. Not sure why you are experiencing pain though
Hi dr.kopelman
I’v done my eyelid surgery 2 years ago and till today i have a bump in my eyelid area this only happened to me after the surgery.. I am not sure how can get rid of it, it’s like a fat under my skin. i have been to many clinics and dr’s and they all agreed that this will never goes and i even with a surgery it might come back again 😢😢 please tell me if u ever had a case like main and what was the solution.
Hard to say you may require a revision
Where are you located?
How far in advance should we stop Omega 3’s?
I like my patients to stop all blood thinners at least two weeks in advance of their procedures.
One dr. I went to said he makes the incision in the inside of the eyelids. What’s you r take on this option?

Does Medicare pay for your upper eye lids ? My upper lids skin blocks my vision
I had upper lid surgery 4 days ago and I’m very swollen and red my eyelids are fat with liquid and my under eyes are swollen as well is thus normal? I’ve been following all instructions but feel like the first day swelling doesn’t subside at all.
Swelling and "redness" occurs commonly following eyelid surgery. If you are diabetic? you might experience more swelling than the average patient.
How are you doing now?
