I am very new to tretinoin, I’ve been using it for just about 5 weeks now, so I’m still learning the flow that works for me. I’ve only been using 3 times per week and I’ve been following your guidance on moisturizing while skin still damp and to “grease the orifices” haha, but it’s working on keeping irritation at bay. So far I believe I been having a small purge, but it looks like it’s starting to clear up now. The rest of my Face is looking better, although I still have a long way to go.

I used tret only for 2 weeks. Skin was doing fine but it made my eyes dry like sandpaper. I have it confirmed now by an Ophthalmologist. I stopped for a week now and they are still so bad. I hope it’s reversible. I am not the only one experiencing it and I am tired of some people saying topical can’t make your eyes dry.
Ps. I stayed far away from eyes. I also never had problems with dry eyes.

I recently finished the last of my Avene RetinAl 1% which, while I got on sale, it's still outside my budget, as we're retired and on fixed income. My dermatologist, thankfully, was ok with Rx tretinoin, which I pay less for through one of the discount Rx programs (GoodRx, Single Care, etc) as my prescription plan will not cover it. All that said, she Rxd the 0.025% and I was concerned it was too low a strength to do anything. So, I'm glad to hear you say it will work, though it may take longer.

Thanks Dr. Dray for all the info you share. It's been very helpful and useful. I started Tretinoin 0.025% last week and I've been using it every other day and will continue for 2 weeks and then ill go to nightly. I've seen videos on people purging while on this medication and it freaks me out because my acne isn't bad at all to begin with. I only break out on 1 cheek and on my chin. All else remains clear but I use the treatment all over my face. My face tolerates Benzoyl well. It loves it. Been using that during the day for over 20 years. I worry about the Tretinoin causing my face to break out all over (even areas that I never break out in). Can you please do a video explaining skin purging? If someone has severe acne does that mean someone like that will purge the most as opposed to someone with mild to moderate acne? Blessings~

My dry eyes beg to differ. Never had dry eyes until I started using topical retinol. It's probably true that there are no studies that link dry eyes to topical retinols, but that doesn't mean that it's a myth, simply research has not been carried out yet. And doctors should still be taking into consideration the experience of people they treat. Another dermatologist on UA-cam mentioned that up to 20% of his patients have suffered from dry eyes when they were treated with retinols.

Hai! Oke so no increase of a sunburn but the skin can FEEL more sensitive, maybe for example a tingling sensation. But if you experience that, does that mean your skin IS more sensitive? Because skin sensitivity can lead to more pigmentation, just a feeling of sensitivity can not.

Hello Dr. Dray! I've been thinking of starting a retinol in my nightime routine, but my staple cleanser is Acne Foaming Cleasing Cream by Cerave, but it has 4% benzoyl peroxide which I was reading may interact badly with retinol. Is this true? Should I switch to maybe the hydrating cleanser they have for nighttime and then use the acne one for mornings only? Or do you think I should be fine to just use the benzoyl peroxide together with retinol? I do have pretty sensitive skin. Thank you so much for all your advice always! 💕

I would definitely switch to the gentle cleanser in pm and bpo in am 😊

Warning when I started a retinol I had to stop all exfoliating/active cleansers for awhile so I would definitely switch to am only with the BP cleanser but you may find you need to skip it all together or alternate days initially while you’re adjusting.

What about using benzoyl peroxide during the day- should you wash it off before using dapsone at night? I am nervous that there will be interaction between the two product and my skin would turn orange as a result.

I've been struggling with acne for a little over 5 years now. I've tried many treatments and my acne seems to fade and then will pop up months later. I've been on winless and a tretinoin regimen for 6 months and my acne has completely cleared, but I've been struggling to get rid of blackheads and congestion mainly on my chin and forehead as well as my scarring. I'm not sure what to use and I've had to stop using panoxyl and things like Paula's Choice BHA because they really made me burn and breakout in the past. I don't know what to use and it feels like my skincare journey has stopped progressing. Thank you for all your tips so far but do you have any suggestions that would work well with these two products to target my specific concerns?

I’ve been using retinol by L’Oréal (serum) once a week for about 4 months. In my first week now of Adapolene. Do you get a new wave of purging when you go up ? Is this a thing, or does my past usage of retinol help in any way. My acne was terrible but I didn’t think the L’Oréal helped me. I didn’t use it as often as I was starting super slow to not cause irritation and my eczema to flare so my purge in the 4 months wasn’t really a thing. I didn’t have a bad purge per say … so now I’m wondering if a purge is on the way 😂

wondering how long term tetrinoin use affect psychiatric health, as it is suppose to have an effect. Same with the non stop exfoliation over 10-20-30-40 years.

Would be really appreciated if you could make a very in depth video on that "does tretinoin ages skin part". Bc I don't completely understand, if tretinoin increases skin cell proliferation, why would that for example then not exhaust basal cells or affect telomere length? Telemore length is influenced by skin cell proliferation, and also if new skin cells could be made from basal cells indefinitely, why does skin age then? Retinoic acid does reduce telomerase and it causes epigenome modifications and it also reduces chaperone metiated autophagy. Personally when I read the studies available on these things, it seems like it is not really that clear how these things may impact skin over a long period of multiple decades. So if you could make a very in depth nerdy video on this, so I and others can understand it, would be really appreciated..

Yaassss! It is not a competition for "going up in strength". I found my happy place with tretinoin 0.05%. I can tolerate it daily with zero irritation.

Hello Dr. Dray, I have an inquiry. I have eczema on hands. Previously my eczema is flaky dry eczema but now this is my first time having pustular eczema and when I googled it’s dyshidrotic eczema. I’ve tried to looked up what can trigger this, and I’ve observed perhaps this could be after I’m starting lifting weights and perhaps the weights contain metal that can trigger my eczema. Hence, this week I’m hiatus from strength training workout. However Dr. Dray my body feels sluggish because I haven’t exercised like I used to and only limited to brisk walk. Hence, Dr. Dray do you have any recommendations for me. I really miss lifting weights but I feel like my eczema hindered me
