  • Posterior capsular tightness is a common cause of shoulder impingement. Check out my explanation of "why" that is and how we improve it.

I would love to see if you could help me with my current shoulder deal. I’ve made major progress, but have massive amounts of scar tissue that we address weekly and I use a softball for daily, but this I may have this as My external/internal rotation is still restricted. Great video 👏

Internal and external rotation are hard to regain. Are you seeing a physical therapist? If so, they should be able to give you a lot of self stretch exercises specific for you. Thanks!

This must be what i need. I'm a pitcher and was up to 93 and have clean mri/arthrogram but feel the pinch in the anterior part of my shoulder in late cocking phase.

Hi Jake, Typically pitchers need more motion then that. If the imaging is clean then you could definitely try this. Best of luck! Paul

I think I have the same issue with a lot of biceps tendon problems occurring when I do pressing movements. A physician told me that my humeral has glided forward and told me to train my subscapularis. He also told me that too strong external rotators would create tension and shortness and would pull the humerus anterior like a rubber band which would push the humerus when it gets shortened. Do you think that makes sense and is training the subscpaluraris beneficial in this type of problem?

I’ve had a biceps tenodesis on my rig side about 7 weeks ago. Movement fine and generally pretty good in the day but very painful in the night usually about 3am. Anyway I can do posterior glides and release the capsule pain myself?

Hi Olga, Adhesive capsulitis and it can happen to anyone who has had a shoulder injury where motion is limited especially into internal rotation.

I wonder if these stretches are acceptable to perform on someone with a SLAP repair. I'm having a ton of tightness in my posterior capsule. Surgery was 4 weeks ago and they just had me start exterior rotation stretches.

Having issues reaching over head near my head while laying down, reaching across to my other shoulder, and behind my back to try to touch my midback or spine.
Think its this or AC joint, or rotator cuff or a slap tear.
Going on almost 2 months dealing with this.

I'm having a hard time conceptualizing why tightness in the posterior capsule results in anterior translation of the humeral head in the glenoid. My intuition keeps telling me that a tight *anterior* capsule would be the one pulling the humerus anteriorly when it's moving through these ranges. Shouldn't the posterior capsule help reduce movement anteriorly at the joint surface or what am I missing?

I totally know what you mean. Think of it as grabbing one rope on one side of a hammock and there is a beach ball in the middle. As you pull one side, the beach ball moves up.

Hi Chandler, the modified Sleeper stretch can be performed but there has been a little bit of controversy about it causing impingement. I've also had good luck with a towel stretch and some soft tissue mobilization of the posterior structures. Good question!

@@OrthoEvalPal Really looking forward to attempting what you've shown in the video. I had minor shoulder surgery about 6 months ago and am kind of hitting a wall when it comes to my capsule. Similar to what is in the video, but I think a bit more extreme.
My therapist has done the movement, as you showed in the video, where you push down but they didn't put nearly the amount of pressure that you did. I've been instructed to push the sleeper stretch as hard as I can and typically do it multiple times a day. After watching your video and reading your comment it seems like this might be causing more harm than good.
is the towel stretch just the pulling up with the opposite arm behind your back?

Don't use sleeper stretch esp if there's any chance of rotator cuff injury. This person has the smartest take I've seen so far and can be done at home so long as the patient is careful, educated, and can keep balance. Note that it's not a musclehead doc advising. ua-cam.com/video/YoZ4O09q6rg/v-deo.html

is there, if you know, a relationship between low trap and posterior capsule tightness?

    • @vertebral
      @vertebral 3 роки тому

maybe if lower traps are weak, you have anterior scapula tilt, leading to poor scapula position, you get limited shoulder abduction/ horizontal abduction, more strain with your shoulder external rotators leading to posterior capsule tightness?
