For so many years I have looked at my face and while I knew what some issues were there was something else I couldn't quite pin point with my mouth. I felt like I looked like a turtle and hated how my top teeth didn't show. I realized the area above my lip was too long but didn't think anything could be done. This is amazing work, Dr. Linkov! I have never had the opportunity to focus on the lip lift independently, excluding all other procedures on a face. This made me realize that your patient looks 8-10 years younger with your work, and she looks doesn't look over done on the profile, as is the case for many lip lift patients. Her swelling was minimal. Her scar area looks fantastic in a super short amount of time. You're a true gifted and talented artist and master of you craft. Dr. Gary, Thank you for sharing how you developed your own techniques for minimal scarring! Many POCs like myself worry about scarring. I myself suffered hypertrophic scarring after a laparotomy :( Because it was an emergency in a rural hospital I had no choice about my surgeon or team, I could only requested they suture me up nicely and apply surgical glue as measures to minimize scarring. Will you be publishing your findings? Because I would love to keep refer it to my team when if I can get ''cosmetic'' work done on my huge scar. If you specialized in abdominal reconstructive work, I would have definitely come for a consultation^_^

is there a reason why botox isn’t used to help immobilize the upper lift for recovery? i have been getting botox for the “lip flip” and also to reduce my gummy smile, and botox in those locations definitely immobilizes my upper lip to some extent.
Never thought about how my front teeth naturally showing like that is a desirable trait. I've always thought it was cute on pictures, but after learning about it I've noticed on video that it's also showing a lot when I speak and such lol. Finally something making me feel better about my face lol!
I'm so glad I watched this. Once again, a very helpful, informative video, with excellent examples. You touched on several points that I've been wondering about. I am looking forward to having facial feminization surgery, hopefully in the not too distant future, and as mine is a rather complex case, I have some concerns about what can be done and what to expect. I suffered a very severe jaw injury 17 years ago that drastically altered the planes of the lower half of my face. The swelling from the initial injury was so severe that my lips separated from my face all the way around my vermillion border, leaving it with a very jagged outline, not unlike a watercolour painting. I have wondered if there is a permanent solution for that, other than tattooing, but in the meantime I have found that a lipliner matched to my vermillion border helps immensely. Lastly, I found your advice about lip lift recovery to be very helpful and your patients' results to be remarkable. I only wish I could come to you for mine, but I don't know if you accept Medicaid. Thank You for the wealth of information you continue to provide!

This is something I’ve been dreaming of getting ever since I found out it was even a procedure lmao! The space between my lip and my nostrils is so long and my lip curls in all the way when I smile. So, I literally have no top lip :( Like actually noneeee. It really makes me feel more masculine and it’s something I’ve always been insecure about. I’ve gotten the lip lift Botox but it only helps just a bit. I feel like I’m really the perfect candidate for this procedure! I’m still very young (20) so I might have to get it when I’m a bit older and richer LMAO!

This is good that you continue to improve on what is the one feature that can make or break facial appearance, the lips. It used to be the eyes but I have seen what a huge difference nice lips can make on an otherwise average face - lips matter. First, I wish I could post my drawing, it would have made this so much shorter. 1. So that said, I want to share this. You have probably seen this article in Aesthetic Surgery Journal 2014, Vol. 34(3) 457-468. On page 461 - where is mentions that care is taken to de-epithelialize the nasal sills in order to keep them intact when the choice is made to anchor the skin flaps more superiorly. This is for the type of anchor that would take place ABOVE the sill INSIDE the nose, not below the sill. Still my preference. 2. On page 460, left column first paragraph it says for "skin being elevated above the sill; for the latter, we suggest thinning the flaps to avoid the unnatural appearance of intranasal philtral extension." Here again, there are some people who need that bulk and NOT below the nose but INSIDE the nose on the sides of a thin columella where the columella creates a sort of triangular pad against the face. So for them it might be good NOT to thin the skin being elevated above the sill and buttress that flap against the sides of the columellar septum inside the nose closest to the face. So the vector of that flap would be such that it's not vertically but placed diagonally towards the middle of the nose. 3. And one more thing, anchoring to the firmer septal cartilage... this I really wonder about for many reasons. One, can one undermine or separate the epithelium from the inside lateral areas of the columella so that the flaps can then become the new covering??? This is why, so the flaps can be pulled higher and attached at the level of the septum. I am not saying the septum will provide great anchor strength by itself but that the skin flaps become part of the new inner nose up to that point provide that stability. Only a U-shaped NARROW incision would be required under the columella for this. Not a wide one like is seen with so many lip lifts. The incision for the other aspects of the lip lift would create an inverted fan so that as the skin is lifted upward, it leaves no spaces. 4. New topic: I like that you mentioned the flaring of the nostrils which occur with the less optimal variations of the bullhorn excision. Particularly the wide crescent shaped excision lateral to the nostrils. That area has NO anchor point, is flimsy/soft, and what so often happens is the tension to close that excised area pulls the nostrils laterally. Ideally, when the flaps are anchored at their highest point, that excision area should look and be approximated even before they are sutured - no tension whatsoever. The sutures in that area should not be for the purpose of pulling an excised area closed but to only close two already approximated skin edges - that's it. This can happen with a superiorly placed anchor inside the nose, which takes up all that slack as well as the right excision (inverted fan at the sill area, NARROW U-shape under the columella). Next time: Suturing to the floor nasal vestibule increases the nasal labial angle, how does one prevent that unattractive, sucked in depressed look at the base of the nose, a duck lip rather than upward rotated lip? Upward rotated lip (good) does not always have to mean duck lips (bad). I need to find some good drawings or figure out a way to post my drawings here. I look forward to your filming the procedure with the new device you mention. I, like so many others here like your explanations and videos.
Thank you to your patient for sharing her experience. She's looks amazing! Did she get cheek and chin filler or implants as well? I happened to notice the increase in fullness in those areas and it made me curious. If she did, it looks great too. Id like to have a lip lift but I already have a prominent cupids bow and I worry it would distort the upper lip too much. I guess I need a consult to see.
Is there a better age to get this kind of surgery? If the lips get thinner by age, by age 70 for instance would it make a difference whether you had it done at 40 vs 60?
I have a big mole about 1.5 cm below my left nostril . I love this mole since it’s part of my identity much like how Madonna has hers. If I get a lip lift ( already 63 and had noticed my lips are lower due to the aging process ) will I lose my beloved mole or will go up higher where the left nasal opening is?

I totally agree on relaxing the muscles after surgery, but will drinking or eating disturb that?

This is something I’ve been dreaming of getting ever since I found out it was even a procedure lmao! The space between my lip and my nostrils is so long and my lip curls in all the way when I smile. So, I literally have no top lip :(
Like actually noneeee. It really makes me feel more masculine and it’s something I’ve always been insecure about. I’ve gotten the lip lift Botox but it only helps just a bit. I feel like I’m really the perfect candidate for this procedure! I’m still very young (20) so I might have to get it when I’m a bit older and richer LMAO!

These videos are so interesting. Dr. Gary, do you work on any adults that had a cleft lip/palate when they were born? My daughter has an amazing pediatric surgeon and her first 2 surgeries have yielded amazing results but I am wondering if adults opt to have further rhinoplasties or possibly a lip lift once their face has matured after childhood.
I have been thinking about a lip lift for quite some time now. I had a revision rhinoplasty a few months ago and was wondering how long you should wait until a lip lift. Also can a lip lift have any negative results on my rhinoplasty? I have heard some people mention it can pull down nose tip?

This is good that you continue to improve on what is the one feature that can make or break facial appearance, the lips. It used to be the eyes but I have seen what a huge difference nice lips can make on an otherwise average face - lips matter.
First, I wish I could post my drawing, it would have made this so much shorter.
1. So that said, I want to share this. You have probably seen this article in Aesthetic Surgery Journal
2014, Vol. 34(3) 457-468. On page 461 - where is mentions that care is taken to de-epithelialize the nasal sills in order to keep them intact when the choice is made to anchor the skin flaps more superiorly. This is for the type of anchor that would take place ABOVE the sill INSIDE the nose, not below the sill. Still my preference.
2. On page 460, left column first paragraph it says for "skin being elevated above the sill; for the latter, we suggest thinning the flaps to avoid the unnatural appearance of intranasal philtral extension."
Here again, there are some people who need that bulk and NOT below the nose but INSIDE the nose on the sides of a thin columella where the columella creates a sort of triangular pad against the face. So for them it might be good NOT to thin the skin being elevated above the sill and buttress that flap against the sides of the columellar septum inside the nose closest to the face. So the vector of that flap would be such that it's not vertically but placed diagonally towards the middle of the nose.
3. And one more thing, anchoring to the firmer septal cartilage... this I really wonder about for many reasons. One, can one undermine or separate the epithelium from the inside lateral areas of the columella so that the flaps can then become the new covering??? This is why, so the flaps can be pulled higher and attached at the level of the septum. I am not saying the septum will provide great anchor strength by itself but that the skin flaps become part of the new inner nose up to that point provide that stability.
Only a U-shaped NARROW incision would be required under the columella for this. Not a wide one like is seen with so many lip lifts.
The incision for the other aspects of the lip lift would create an inverted fan so that as the skin is lifted upward, it leaves no spaces.
4. New topic: I like that you mentioned the flaring of the nostrils which occur with the less optimal variations of the bullhorn excision. Particularly the wide crescent shaped excision lateral to the nostrils. That area has NO anchor point, is flimsy/soft, and what so often happens is the tension to close that excised area pulls the nostrils laterally. Ideally, when the flaps are anchored at their highest point, that excision area should look and be approximated even before they are sutured - no tension whatsoever. The sutures in that area should not be for the purpose of pulling an excised area closed but to only close two already approximated skin edges - that's it. This can happen with a superiorly placed anchor inside the nose, which takes up all that slack as well as the right excision (inverted fan at the sill area, NARROW U-shape under the columella).
Next time: Suturing to the floor nasal vestibule increases the nasal labial angle, how does one prevent that unattractive, sucked in depressed look at the base of the nose, a duck lip rather than upward rotated lip? Upward rotated lip (good) does not always have to mean duck lips (bad). I need to find some good drawings or figure out a way to post my drawings here.

Thank you to your patient for sharing her experience. She's looks amazing! Did she get cheek and chin filler or implants as well? I happened to notice the increase in fullness in those areas and it made me curious. If she did, it looks great too. Id like to have a lip lift but I already have a prominent cupids bow and I worry it would distort the upper lip too much. I guess I need a consult to see.

I have a big mole about 1.5 cm below my left nostril . I love this mole since it’s part of my identity much like how Madonna has hers. If I get a lip lift ( already 63 and had noticed my lips are lower due to the aging process ) will I lose my beloved mole or will go up higher where the left nasal opening is?
