Start shooting practices from close in to the hoop, shooting with the correct form and then work your way out still using the same form only as far as your correct form feels comfortable. The problem you are likely experiencing is that you have shot so many three's probably with the incorrect form that you don't have a soft shooting touch that is consistent irrespective of where you are in your shooting range.
With correct techinque, appoach, patience and a lot of practice. Aim to have made at least 500 shots a day within an hour for a whole month. In that hour you want to work on form then progress to more game like situations and eventually light defence. In a month you will notice a huge change in you confidence to shoot and ability to hit those shots. The problem is that you then need to maintain the ability by practicing otherwise you will lose your touch.
Just to add, I stress using the board if at an angle, and like Gary, challenge yourself by counting a made shot where it is a swish. Keep track of your shooting workout with a diary.
