5 SKINCARE ACIDS | How to use, recommended brands, explained by dermatologist

    Skin acids are a group of molecules that can help a myriad of skin conditions.
    ALPHA hydroxy acids
    AHAs include lactic acid, glycolic acid, mandelic & citric acid. Glycolic acid is derived from sugarcane, lactic acid can be found in sour milk (oldest peel), whilst citric acid is derived from fruits; hence the blanket name of ‘fruit peels’. (For those who love organic skin care)
    Uses: Glycolic acid can be useful in the management of acne, blackheads, whiteheads. BHA are better as salicylic acid is both anti-inflammatory and oil soluble. AHAs can be useful in the management of sun damage, wrinkles, pigmentation & as a primer to aid delivery of other acids, & actives like retinol- retinoids.
    Concentrations: 2-30%, creams, gels, lotions, washes. Start slow, increase as tolerated.
    Careful: If you have rosacea, sensitive skin, dermatitis
    Azelaic acid
    This is an organic acid found in wheat, barley and rye, however this acid is manufactured in a lab environment
    Works by 1.Killing bacteria 2. Keratolytic 3. Reduces pigmentation - tyrosinase inhibitor 4. Anti-inflammatory
    Uses - Acne, rosacea, pigmentation.
    Concentrations: Typically 5-15/20%. Concentrations less than 15% are less irritating cf higher concentrations.
    Careful: High concentrations. Most benign acid to use. Good for inflamed skin, acne in pregnancy, mild rosacea, some forms of pigmentation (melasma).
    Ascorbic acid
    Also known as Vitamin C. pH 2.5-3.5 for highest bioavailability. Potential to cause skin irritation in higher concentrations.
    Works by
    Antioxidants reduce UV induced DNA damage. Free radical scavenger
    Anti-pigment works as a tyrosinase inhibitor
    Stimulation of fibroblasts to produce dermal collagen
    Caution: Sensitive skin, rosacea. For patients who have this skin type, start at 10% and SLOWLY increase as tolerated.
    Retinoid acid
    Vitamin A derivative. Arguably the most beneficial skin care active as prescribed by physicians & dermatologists.
    Works by 1.Decreasing sebum or oil 2.Anti-inflammatory 3.Anti -bacterial 4. Helps normalize skin shedding 5.Reduces wrinkles by stimulating collagen production.
    Caution: sensitive skin, titrate accordingly.
    How to start? Retinol over retinoids unless prescribed by your doctor. Typical retinol concentrations range from 0.5 to 2.0%.
    Kojic Acid
    Is widely used in Asia to improve skin clarity, by reducing pigmentation. This acid is a naturally occurring compound derived from #fungus. (Obviously purified so don't worry, your face won’t be taken over by fungi).
    Mode of action: Tyrosinase inhibitor, decreasing #melanin production.
    Clinical application: Melasma, post inflammatory hyperpigmentation, brown sunspots.
    Best applied: At night. If tolerated twice a day. As with all prescription skin care, start slowly, increase as tolerated.
    How to use: Best compounded by a pharmacist and prescribed by a dermatologist. Ideally Kojic acid works best with combination therapy.
    Caution: Kojic acid has a high sensitizing potential and hence used in caution in patients with sensitive skin, or inflammatory skin conditions such as #eczema, and seb #dermatitis

    My mum suffered from pigmentation all her life and was not aware that there was a simple drug store trick to get rid of it.     Would love to hear some possible full skincare routines from you. How do we use all these acids safely?
    I usually use SOME kind of acid 1-2 times per day. And in addition, I use .1% tretinoin every night. I still have acne, and I also think I’ve over-exfoliated because I can cause very dry and red patches, especially around my mouth and nose.
    But when I lighten up on the glycolic, my acne gets worse. I tried to swap out the glycolic half the week with lactic or a PHA toner, but again- it makes my acne worse. Thanks for all your expertise!

      From the Ordinary: AM peptides and Vit B; PM Vit C & A. Vit B and C together not advised, as Vit B may neutralize Vit C...this is what I was told. Any confirmation is welcome :)

      Cool choice. 4years ago, I used Neostrata 15 also 20, couples of bottles done and the results was bullshit. Since then I stay away from that brand, waste of money and fancy for nothing.

      Yeah I tend to stick to TO because I know across the board they're fragrance free and cruelty free. I used to pick through Revolution's serums (copied heavily from TO) and get some of them but some of theirs have fragrance so it's just easier...plus TO has a $30 free shipping limit which is amazing. I gotta spend $50 to get free shipping from major Australian places like Priceline or Chemist Warehouse.

    Dr. Lim - How would you assess Murad skin care products? Also, does a prescription of retinol-a micro expire? A have a tube, but rarely used it because it irritated my skin. Meanwhile, thank you for explaining the various acids. The product recommendations are very helpful too❗️

    Hi dr Lim, I’ve recently discovered your channel and although I’ve been doing a lot of my own reading up on dermatology recommendations as well as some scientific studies, I just wanted to tell you how amazing and simple your vlogs are! If I’m in QLD - will make sure I come for a consult! 👏🏼 I’m using CE ferulic acid daily and want to add in the ordinary Hyaluronic. I use a prescription retinoid at night. How do I add in Hyaluronic into this regime? Also, can you please discuss the use of oral supplements like hydrolysed collagen and Hyaluronic acid for skin care? I still can’t make up my mind based on what I found myself

    Hello Dr Lim, Thank you for all your videos they are great! I have one question regarding body acne, KP and enlarged blocked pores on entire body.
    Dr, is it possible and beneficial to apply the double cleanse method using a all over cleansing oil that emulsifies to help clear clogged pores on the body?
    Also, is it safe and beneficial to add crushed aspirin ( salicylic acid) to a body wash/shower gel to gain a better skin texture and exfoliate? (cant afford expensive treatments)
    If not ,what about using aspirin mixed into a paste to form a body scrub? Lots of questions I know.

    if you've got dry acne-prone skin and struggle with acne scarring mandelic acid works wonders. My skin always looks so much better after using it and I can even notice the difference in my skin after a few hours.

    The negative side of glycolic acid is that it makes de skin more sensitive to the sun, more exactly to the UVB rays. In other words, it can increase the amount of sun damage from UVB rays if you dont use sunscreen like a vampir around 1g 3 times per day. Instead of AHA i prefer to use PHA, you can use it everyday, day and night and even under retinol or ascorbic acid or other anti age ingredients.

    I use Retin A .5 nightly with no issues for the last 8 months and have subscription boxes so have tried a lot of skincare over the last 18 months. I have fair skin but not sensitive skin. Never had an issue with any products which is why I was surprised when The Ordinary AHA peeling solution a few weeks ago that made me peel like a snake for weeks. Then I tried The Ordinary vitamin C which broke my skin out with an acne like rash all over that has taken almost two weeks to fade and it’s still not completely healed. But they were more like water blisters than acne. I’ve learned my non reactive skin can react! I might add I am in my 50s and my skin has always been a bit dry.

    Brilliant info again thank you! You mention to put on a moisturiser half an hour after using a prescription retanoic acid if you have sensitive skin. I don't have this and I put on moisturiser straight after putting on the Retrieve cream. Does moisturiser dilute Retrieve? Thanks :)

    This was so very educational and did not put me to sleep! Lol. I am a new subscriber and find your presentations fascinating. I am 68 and have great skin but have severe wrinkles on the right side of my face bc I am a side-sleeper. I recently bought a dropper bottle of 100% pure Hylauronic acid and the results are staggering. IS IT SAFE IN THAT FORM ? Also I have a bottle of 100% Retinol (?) coming from the same pharmaceutical company...IS USE OF THAT PRODUCT SAFE? TY so much for all you do for us here !

    Kojie San is a Filipino brand, not Japanese but the brand logo is a Japanese woman. The soap doesn’t work that great for pigmentation but it does lighten and brighten the skin. It’s great for acne-prone oily skin actually.

    Also, you bring up differin gel/cream in america because I believe that's now over the counter there atm. Is it still something that needs to be prescribed by a dermatologist here in Australia, or could I get my general practitioner to prescribe it?

    Dr Davin, Is it correct that glycolic acid is not suited for skin of colour? My/my parents skin is white, however my heritage is middle eastern. And thank you for all the great information you so generously provide.

    I have all of these in a box. I’d like to learn how to layer them with moisturizers and each other. Also, can these be used I the summer with frequent beach visits if proper amount of sunscreen is used??

    So, other than the retinoic acid, the rest are safe to use in SUMMER and when exposed to sun?
    Also, how effective are they when included in products like toners, foam cleansers etc. or should we only look at creams and serums?
