Ok so while I don't know what in particular caused it a train station jam was definitely involved in some way. Had trains outside the city clipping with my intercity trains with everything just getting stuck and 1000+ people in the station waiting for a ride. Noticed this sky people thing going on after figuring out the train station screwup, the NPCs were definitely walking out from the direction of the station with the jam, and yeah some of those NPCs decided to change direction at a random point over a lake.
