Dr Mathew Leonardi is an advanced gynecologic surgeon, sonologist, and endometriosis expert at McMaster University Medical Centre in Hamilton, Canada (experts.mcmaster.ca/display/leonam). He is the director of Specialized Ultrasound in Gynecology & Obstetrics (SUGO) (sugoclinic.com/). He is an Honorary Adjunct Lecturer at the Robinson Research Institute, University of Adelaide, Australia.

Hello Matthew, thank you so much for your conference. It was very interesting to learn more about endometriosis detection. I had one quesion. So i suspect i might suffer from this condition and so my doctor gave me an appointment for a pelvic ultrasound. The results came back negative. But, I did not drink water before my appointment. How much can the lack of water consumption have affected my results? Should i reschedule an appointment. Also, what is the best time during the menstrual cycle , to have the most accurate results possible?

An endometrial polyp
Is this a prolapsed lower endometrial polyp/fibroid into the cervical canal? The stalk appears to be coming from the uterine cavity and the pathology “slides up” ?
Negative sliding sign
The true sliding sign - for obliteration of the pouch of Douglas is not shown here and is not the focus Look at the intracavitary pathology and think of the sliding sign concept relative to what you are seeing there
Actually lower 1/4th of the screen is covered by your text. Also the last 2 lines of text is covered with another text which is your video title. I can only appreciate that you are demonstrating sliding for posterior compartment but cant really see properly due to text . Also i can see fluid with low level echoes in the cavity at the level of isthmus. Are you demonstrating Taurus? I suggest to put text in video description and not on the clip.

Endo is horrible! I've had it for 2 years at least with no help! The nausea is chronic and debilitating , I can't function! 😭 The pain is horrid, the nightly fevers, chills and everything else is absolutely awful! Ugh!!!

Hi great question! I will have to do a video on pitfalls! 1. If no videos are taken by the sonographers, lots of information is missing 2. If no 3D is acquired, lots of information is missing 3. Other pathology can make it difficult to visualize the IUD - large fibroids, adenomyosis
Distance of upper end of IUD from uterine fundus? Is that not an important measurement to take?

Focal adenomyosis

Training of person writing the reports is also chalenging. We don't have sonographers but we do our own scans and nurses write reports. They need to have an understanding about terminologies and also chemistry with the doc 😂....for example if my other hand goes to abdomen to mobilise a structure nurse immediately puts her hand on freeze button.

This may be a stupid question but, do you have to remove the second uterus? Or can you just leave it there, as long as it doesn’t cause the patient issues? I assume since you’ve gone in laparoscopically to get a look at it you probably took it out because it was causing problems for the patient.

How often do you find uteruses that seem to have one of everything? As a bit of a joke, I have a blackout bingo card of a uterus (gotta little bit of everything!) and in that case how easy is it to miss something?
We definitely find patients with fibroids, adenomyosis, polyps, etc! Sometimes people have PCOS, uterine pathology, and endometriosis!

You should be concerned enough to look for it if the patient has symptoms Some red flags on ultrasound that should make you look more for endometriosis: 1) anteverted, retroflexed uterus, 2) ovarian endometriomas, 3) adenomyosis, 4) hydrosalpinges or hematosalpinges

I am not clear how you make the extension between this being an ovarian cyst or a paratubal cyst
Few ways: 1) Paratubal cysts are usually very thin-walled, 2) Paratubal cysts are almost always anechoic, 3) Paratubal cysts move distinctly from the ovary with pressure from the probe or hand
