5 Things Patients Should Know About Small Intestinal Bacterial Overgrowth (SIBO)
Вставка
- Опубліковано 29 тра 2023
- Yes, you have bacteria in your gut. Yes, that’s normal. But small intestinal bacterial overgrowth (SIBO) is a gastrointestinal condition where there is an excessive amount of bacteria in the small intestine. SIBO can lead to uncomfortable symptoms including bloating, diarrhea and nausea, which overlap with other gastrointestinal conditions, making SIBO diagnosis a bit tricky. SIBO can also cause unintentional weight loss and malabsorption. Mark Pimentel, a leading gastroenterologist and SIBO expert from Cedars-Sinai Medical Center joins us to explain what’s really going on inside our gut with SIBO. He also gives us some insight on the causes, risk factors, and how SIBO is diagnosed and treated. Dr. Pimentel, who is the author of the book, The Microbiome Connection: Your Guide to IBS, SIBO, and Low-Fermentation Eating, also shares some surprising insight on how artificial sugars can affect our gut microbiome.

When I hear bloating is a symptom of SIBO if you are a female you need to know that bloating can also be a symptom of Ovarian Cancer. My wife was diagnosed with Ovarian Cancer because of bloating. After a 30 month battle she lost the battle. Please if your doctor says your IBS and has not tested for Ovarian Cancer please get it checked.
I was told there is no test for ovarian cancer
Gosh that's scary, I have a cyst on my ovary, showed up a few months ago. But the blood was clear for cancer. As I got tested at a cancer clinic due to a friend having treatment there. I had an MRI and blood work done. He did say there is a potential for it in the future after menopause. I will do whatever I can do to get rid of the cyst. But I have some spiritual issues as which I'm working removing. So a whole lots of bunch of problems. Mind u I get constipated and have had IBS since 14 years and that time my reproductive organs were absolutely fine
i’m sorry :( i’ll pass on the info to anyone for you and your wife.
No way bro
@@beckyphillips7888my partner survived Ovarian Cancer. There is a blood test CA125 that can indicate cancer. It’s not definitive, but it’s an excellent marker to look further. Today her CA 125 is 5, when she was diagnosed it was over 10,000. Ask for it. It isn’t standard.
Straight to the point! Great video.
Thank you for this, Jacqueline!!!
I want to stress how incredibly important it is to ADVOCATE FOR YOURSELF. Odds are you will not be given the correct medication(s) for SIBO if you don't. Actually the odds are not good you will get offered the test if you don't even ask for it. I actually served up my symptoms to my doctor on a silver platter and he kept telling me to have fiber. So i went online and learned of SIBO. So i asked my doctor for the test and that's how i was able to find out. But that's not where it ends. I wasnt getting relief like i said and i looked at my own results to realize i have IMO which is similar but NOT the same as SIBO. Hence its own diagnosis. SIBO is specific to the small bowel whereas IMO is found anywhere in the digestive tract. Another reason to needing a systemic antibiotic. Rifaximin is not a systemic antibiotic. Anyways, when i brought this to my doctors attention, his response was: the test just said Hydrogen. What he meant was that the 6-word sentence from the lab just said "hydrogen sibo detected in this patient"
The distressing part is that my doctor didn't look at the actual numbers. Just the finding. It's traumatizing bc you think over and over to yourself "what if i hadnt looked?" "What if i hadnt looked up what the numbers mean?" And not only does it create stress, but fear that something else is going on and if YOU don't find it, you will suffer. So it leads to even more googling of symptoms and how to treat what you know you have and what you might also have and such. And it wont limit itself to SIBO. It will instill that fear across the board.
How did you treat yourself after?
Well said I feel the exact same way
What was your treatment?
You are absolutely 💯 correct. Your story is my story. You stated it so perfectly. I diagnosed myself, with little to no help from the physicians using a FoodMarble Breath Analyzer. I am treating the condition and symptoms myself. I’m methane dominant. Fodzyme enzymes combined with Atrantil was my saving grace. No symptoms of Sibo when taken with meals, however eating foods not LowFODMAP is a risk that’s easily controlled by the enzymes mentioned above. I’m living fairly normal with no bloating or other IBS symptoms. Thanks for sharing your story. I know now that I’m not alone. I hope this helps others on this journey treating this condition.
@@handsomed104 So many of us with SIBO/IMO have experienced this same thing. Please know that low FODMAP is not meant for the longterm. Malnutrition is the result of that. Dr. Pimentel's Low Fermentation Eating is very helpful and can be done longterm. I, too, am methane dominant but mine was so bad I could not heal it on my own. Check out Dahlia Marin of Married to Health. She is a GI certified dietician and knows more than most doctors. During our very first visit (she sees patients virtually) she told me immediately that she thought I had SIBO, when after years not one GI suggested that! I am now being seen at the GI Motility Clinic at Cedars Sinai, the very one started by Dr. Pimentel and I'm getting ready to have an endoscopy. Dr.Lim will be doing things that aren't usually done during routine endos, such as withdrawing fluid from my small intestine, along with a couple other things. I have the FoodMarble, too, but question its accuracy. Dahlia has helped me immensely and I am thrilled that I got in with Dr. Jane Lim at Cedars. She trained with Dr. Pimentel and his partner, Dr. Rezaie. Lastly, I don't mean to be picky, but this girl in this video keeps mispronouncing 'SIBO' and Dr. Pimentel's name! OMG! It's really bugging me! 😂
That was brilliant thank you for a wonderful interview ❤
This was amazing. Wonderful interview!
Thank you for your comment/
I am always fascinated that the stomach pulls off the trick to digest the food without digesting itself.
Thank you. This is fantastic. I am a 4 year suffer. Ordering the book now.
Save your money it’s useless. Dr. Pimentel is a sham.
OMG I'm in love with Dr. Pimentel. Finally, someone who knows and understands!!!
Is the most useless guy when it comes to Sibo
Great info!
Glad it was helpful!
All my symptoms are completely gone including bloating, abdominal pains/discomfort, chronic fatigue, weakness, diarrhea & persistent foul smell.. I successfully cured my SIBO symptoms & Halitosis/Bad breath from stomach due to GERD with alternative treatment and natural diet protocol from DR.UWENBOHERBALHOME On UA-cam..🎉🎉🎉🎉
What book are they talking about? How do you get the book?
Wow, I am so glad I watched this! I have suffered from IBS on and off most of my life. I recently had a Camphlobacterium colitis infection and had a great resurgence in my symptoms. I was going to talk to my Primary Care doc into getting me a GI consult to see if maybe this is SIBO. Now I am not going to wait til my physical next week but will contact her on Monday. Thanks so much!
You want to do the TrioSmart Breath Test. It was designed by Dr. Pimentel and is the most accurate AND tests for methane whereas the others don't. Also, if you have a PPO I highly recommend Dahlia Marin of Married to Health. Dahlia is a GI certified dietician with a strong medical background and is very experienced with treating SIBO/IMO. She has helped me more than any doctor and was actually recommended by a GI. Since she sees patients virtually, location is not an issue. I hope you're getting the help and relief you deserve!
When i fast i have no issues lol
What’s the insurance code so we can see if our insurance covers his test
a question hopping from the other side :D what's the song playing in the background at the beginning ?
But rifaximin isn't even necessary. You must get your migrating motor complex working properly again, or the bacterua come back! This is why there's a bacterial overload, because it's stuck and fermenting in the small intestine, leading to histamine intolerance, chronic fatigue, food intolerance, pots, duodenitus, etc. It's literally poisoning our bodies.
The cheapest way is by taking ginger and artichoke supplement together. This stimulates the motor complex, pushing the food down. Focus on the migrating motor complex!
Can't Sibo inhibit the MMC function? I've heard a lot of people's MMC works better once the Sibo is cleared.
@sizbowl there are many causes of slow mmc. Mine is stress. Then slowed mmc, then sibo. If you can address the migrating motor complex, it's a cheap ginger artichoke supplement, and it will move the sibo out of your small intestine, back down to the lower intestine.
Most people still need a kill phase for the bacteria (or methane archaea if you have methane overgrowth), then after that prokinetics can be used but prokinetics alone dont kill the bacteria in small intestine
The underlying issues of gut problems : congenital malrotation/ malattachment of the gut. Hard to diagnose. Possibly connected -off midline fusion of the filum terminale found in 3 out of 27 normal cadavers according to neurosurgeons Hansasuta, Tubbs Oakes.
The off midline fusion stresses the cauda equina and can put a torque in the entire neural tube. So there can be a lower or upper motor neuron bowel. Expect higher rates in Chiari and Ehlers - Danlos people.
Why do you speak in this ridiculous language and expect people to understand???
Are those 2 bacteria the same culprits that produce methane as well?
Oh geesh! I think I’m the Stevia queen! 😩
I have a lot of really good experience and knowledge on this. Wonder if i should come out with a little youtube channel.
How about if you were told you have Ulcerative Colitis & IBS-C? What is the management for that? It seems to be tough to treat cause antibiotics for SIBO can flare the UC. I was told I have SIBO in 2005 then was dx’d with UC some years later. I actually feel my SIBO was caused by having a chronic anxiety/depression disorder.
All my symptoms are completely gone including bloating, abdominal pains/discomfort, chronic fatigue, weakness, diarrhea & persistent foul smell.. I successfully cured my SIBO symptoms & Halitosis/Bad breath from stomach due to GERD with alternative treatment and natural diet protocol from DR.UWENBOHERBALHOME On UA-cam..🎉🎉🎉
Wouldn't a bacteriophage specifically designed to taget the exact klebsiella and e coli species be the best way to treat the sibo? And other gut bacterial infections/overgrowths as well? I dont know how much research is being done with phage therapy, but i would think in this era it is possible to genetically engineer bacteriophages for these bugs.
The killing of the bugs is relatively straightforward. The difficulty is in getting the antimicrobials/antibiotics in contact with the bacteria. They often hide in biofilms so biofilm busters are needed. And the small intestine has the surface volume of a tennis court so there are many nooks and crannies where the bacteria can hide out. That's why it takes so long even despite these substances working nearly instantly in a petri dish.
Then you also have to fix the root cause which is usually related to a disturbance in MMC function.
Hi
When you have bad odor in the poop is this the same problem with the 3 gases ?
Thank you for your comment. It's best to speak with your healthcare provider about your specific situation. Here a link you may find helpful www.mountsinai.org/health-library/symptoms/stools-foul-smelling
Are there different diets/food you eat when you have methane or hydrogen or both avoid the same?

I am to have endoscopy and colonoscopy
My doctor is trying to figure out why I have been suffering from the last 2 months and it has gotten worse since the last month.
Is there anyway I can speak to this doctor
Thank you again for your comment. Here is a link to his Cedars Sinai provider profile. He is based in Los Angeles. www.cedars-sinai.org/provider/mark-pimentel-887112.html
You can deal with sibo by yourself herbaly it’s a lil tricky but it’s doable low starch diet and herbals and high probiotics ..
@@saffronsl thank you for you comment but I caution listeners/readers to please consult their healthcare providers who can best guide them to the proper treatment plan thst is right for their individual condition. What works for one person may be harmful or ineffective for another.
@@GastroGirl yes I would in a perfect world be able to tell everyone to go to a dr but regular drs don’t help and even naturopaths are not very knowledgeable and very expensive.. It’s incredibly difficult for us with very little money

Every book I read and video I watch makes wildly different claims about sweeteners!?!? Now Aspartame is OK?
No it is not Ok, The doctor was referring to the effect of sweeteners on the gut microbiome and explaining what the research shows. He is not saying Aspartame is Ok. Hope that helps.
How do you get the Tri Smart test?
Can order on line
So what about sweet fruit
I'd say not much. It's fiber + sugar= fermentation.
For me, when I eat right, I feel great. When I don’t I have loose stool, cramping, horrible gas and sometimes nausea along with lethargy. I absolutely know dairy and sugar are my worst. If I could get my eating under control I could get a handle on this. But I love to eat and all the wrong things!! This is all on me though. I need no medication just discipline!! I also have rheumatoid arthritis and it really flares when I eat the wrong things.
I feel your pain! I am such a foodie ! Especially the bad food! Sugar , dairy , gluten . I have a hard time staying away from it even though it has been making me so sick. I instantly bloat , get gasey, pain, nausea , burping , headaches , vertigo. I hope that you can get your eating under control ! Feel better God bless ❤️🙏
I had the breath test but unfortunately only for hydrogen and methane not hydrogen sulphide. The results showed extreme methane prepoonderance, almost normal hydrogen yet I have IBS D. Presumably I have diorrhea with methane and cant see the H²S. Is the trio breath test available in Germany and am I right in thinking that it is possible to have too much H²S and diorrhea?
An answer would be MUCH appreciated :)
Thank you for your comment. We are looking into this for you.
I’m also in Germany and wanting to know if this test is available here before a go to the doctor.
It seemsas though they have been "looking into" it for two months now. So far as I know, the triple test is not available in Germany@@mxdebi
@@MrYorickJenkins thank you for your reply. I figured that was the case. At least now we know about this information so we can ask our doctor about it. For me,if mine says he’s not aware of it then off I go to find one that is. May have to be in another country.
do keepme updated on that and Ill do the same if I get to have the triple test. I think having the extended test is les simportant than finding out if the cures based on these tests actually work@@mxdebi
Thank u so much for this video

need to know if monk fruit sweetener feeds sibo!
Monk fruit is mostly just erythritol. It caused my husband to have high blood pressure. A week after he stopped taking it, he had a normal BP again.
@@pamelajean1968 I found a monk fruit sweetener that is only monk fruit!

you can find monkfruit without any other added sweeteners! It’s the erythritol that cause gastrointestinal upset, headaches etc.
I took trio smart. methane sibo. been low fodmap. antibiotics didn't work. gastro gave me a new drug that gave me terrible diarrhea so I stopped taking it. still on mirilax, magnesium, biofilm disruptor /acv, oregano, and fiber, and triphala. nothing has worked yet. my gastro is useless. anyone?

On gee .. sooo has there been further investigation…. Low stomach acid . Inflammation markers and leaky gut ? CIRS , MCAS . Mouid Illness ? Test MSH hormone , leptin . Tick bite bacteria ? Candida
Did you fast overnight 12 h and take prokinetics & no dairy elemental low carb diet ,
Yikes, Trio-Smart is $350 USD. Can't afford that. The book isn't released until April 2024, which feels like lightyears from now.
Test is useless. Took it 5 times. Different results everytime
You need to deal with your gut motility, called mmc, or motor migrating complex. The way to do that is by taking; get this: ginger and artichoke supplement. This will get your stomach working properly again, and it will therefore help eliminate harmful bacteria in your small intestine by moving them out of your digestive tract. If this doesn't help, sibo may not be your issue.
I took the Trio-Smart test and received positive results for SIBO. Do whatever you need to do to save money for the test. Save money by not eating out, you’ll be amazed how much money you can save by eating at home. No take out, just buy groceries. While saving for the test, don’t purchase anything you don’t absolutely need.
@@rconach I took that trio smart test, five times and each test result is different. One of the test says I don’t even have Sibo. I go based on symptoms. These tests are useless.
@@rconach
Go watch all the research on Sibo tests even people who don’t have Sibo took the test and shows they have Sibo and they have no symptoms. It’s a gimmick.
Gosh i can't have anything sweet now
Is it only an intestinal issue?
SIBO is found in the small intestine. However, some symptoms can also be indicative of (or overlap with) other GI issues so it is important to see healthcare provider/gastroenterologist who can make an accurate diagnosis. Here is some additional info from the Mayo Clinic www.mayoclinic.org/diseases-conditions/small-intestinal-bacterial-overgrowth/symptoms-causes/syc-20370168
Rifaximin and neomycin did not work for my sibo. Took in 3 times. Did it work for anyone else?
I’ve read this frequently on comments on Reddit. Maybe it just takes 6 months it could need a longer course but I’m not a doctor.

Conventional drs I have talked with don’t believe in it.
What options does a person like me have?
Oh my gosh!
I am 68. I have been suffering since I was 16. I have been through so many tests with no positive solutions. The money I may as well just thrown out the window. I remember reading Dr Mark Pimental’s research when he was a man with dark black hair.

Mucolytics on the rise.
Rediculous… people whine and cry to their doctor … as he shakes his head when they go home and eat chicken wings/ cheese/ pizza/ boxed potatoes/ pop tarts/ corn flakes etc etc etc …..
But if they don't know that they have SIBO when they are whining and crying, it is NOT their fault.
Everyone starts somewhere. For me- this news is hopeful!! I never heard of this before.
The drs. In Australia that discovered h.pylori way back in the early 80's stated that in the future they will probably find other pathogens.
Keep in mind dr barry marshall had to infect himself to convince the medical community to believe him...that's how tough it is to make change in the boys club. We've yet to see another pathogen found by these Australian researchers.gotta make a person ponder
I wish you would of asked this guy what he eats on a daily basis
