For the issue you had at 4:18 you can weld objects of the same creation together while said creation is on the lift. this should help for whenever you use barring in the future. just a tip, I think the way you did it was cooler.

Since the survival update most mods have been broken. Majority of gear mods can be very glitchy too. Pipe pieces is about as close as you might get. Quick edit: use the thicc pipes not the noodle pipes
Some small tips: 1. "Generator pipes" look like thick pipes but actually are quite a lot smaller, so if you'll need smaller hitbox you can use these 2. Talking about gears, sm while having quite small blocks, works best with big objects in that regard

Instead of expanding the pistons you could just weld them directly to the other crankshaft. Place a bearing on opposite sides of a block, put blocks on the bearings, put another block on the side of one of the blocks you just placed, place a block on the piston and weld that block to the other block. ⬛️l⬛️l⬛️ ⬛️ ↔️ ⬛️

If you put the creation on a lift you can weld "child entities" together, so you don't need to slot pins in to rotate. You may also have less problems of friction with pipes (not really 100% sure though) Since scrap mechanic did an update that messed up friction between blocks
There’s a mod on the workshop that adds actual gears that work… would love to see you remake this with that mod. I think it can also solve the whole piston thing to spin wheels too

You could make a clutch to keep the engine running even when the vehicle is stopped, this would also allow you to get it rolling without relying on a bump start, as when you extend the clutch pistons to make contact with the flywheel, it will still be able to slip to keep running, and the vehicle would gather speed off of the leftover friction until there is no longer any slip. You could try various block combinations for different levels of drag, who knows what the right combo would be as too much friction would stall it while too little would hinder performance.
i love to make piston engines powered by spud guns, and it works pretty well, managed to build a V8 with a shifting gearbox powering a car (also made an I4 engine, powered by 4 spud guns on each cylinder and managed to make a car with a clutch out of it too)
It's interesting to see how you solve problems your own way. There are usually easier/more efficient options people figured out already but doing things your own way can be interesting too. I made a helicopter in Scrap Mechanic that uses a "realistic engine" and a working swashplate once with the wings mod and it works very well.

You can edit the games code for the terrain height by changing literally 1 value. It’s a perfect method to get otherwise static grids off the ground and into your lift!
instead of extending pistons, you should use suspension, since they have the same no-collision property but are covering 3 or 4 blocks of distance with no collision, and like other comments say, pipe pieces would help as well
Advice - make the camshaft bigger for higher torque. Its 3 blocks big now, if it was 5, that would make a huge difference. Could even go for 7. Thrusters shine not at torque, but at speed, so, using them in a system which expects less torque out of them, will make the system much better
you should try the game "instruments of destruction"! it's a lot like besiege, but with a cleaner aesthetic and new mechanics like gravity wells and magnets!
you can make this much much much better by using the aforementioned pistons as the actual pistons that move to make the crankshaft spin instead of using thrusters
The moment you got it sorta working, but the engine wasn't producing enough power, I thought "You need to put a gearbox in!" Engines almost always gotta be moving faster than the wheels they're connected to, after all. :3 But yea, gears in the stock game aren't very good.

i remember making a car with en engine including crankshaft and pistons. a drive shaft. a differential and realistic stearing and realistic suspension in the same space that he used just for his engine

Bro, I dunno how to tell you this, but realistic cars don’t have the engine extending half the width of the chassis out the left side attached to a fifth wheel.
Piston engine: exists* What the guy made is effectively a piston engine, rotating the axis and transfers rotation to wheels, still can't figure out myself how do you turn the wheels, seems difficult af
I think you might have less friction problems if you use pipe pieces, since scrap mechanic hates cube blocks being in contact with eachother

You dont need an engine for steering, if you just connect a seat directly to a bearing it will act as steering when you press A and D in the seat.

The new scrap mechanic update has some optimization like less lag and probably less unpredictable physics. Some new things also
You should try doing survival mode! you can make your mechanics clothes look better if you progress far enough
You should make a video on you beating the new chalenge mode levels that got aded in the update
