- Tirzepatide (brand name: Zepbound) is the newest injectable medication for the treatment of adults with obesity, approved by the FDA on Nov 8, 2023.
Based on the results of the SURMOUNT trials, tirzepatide is the most effective obesity medication ever. This once-weekly injection produced an average weight loss of 20.9% of body weight at 72 weeks.
Prior to Zepbound, Wegovy (generic: semaglutide) was the most effective weight loss medicine, with an average weight loss of 15%.
In this video, Dr. Christopher McGowan, MD, MSCR, of True You Weight Loss, explains how tirzepatide (Mounjaro) works, and how it compares to Wegovy.

7 months mounjaro were amazing! Lost 35 quick with no side effects. Just lost all interest food. Then the coupon ran out so switched to Wegovy which barely stopped the food noise. Didn't gain it back but didn't lose any either. I had to be careful. Now Im one month back on Zepbound and staring to lost again finally. This Zepbound is MAGIC. But Wegovy didn't do it for me.
I stalled out after 80 pounds lost on Ozempic and it's creeping back. So starting Mounjaro now.
Good that you did'nt gain any. How long were you off of monjauro b4 taking Zepbound? I'm impressed that you didn't gain. Wegovy must have helped somewhat?
Thanks for that helpful comment
My first month on Zepbound. 22 lbs down, zero effort. You just don’t want food, and when you get it it’s hard to eat. It’s just like…I just don’t want this anymore,
Very true you just don’t care about food at all
How much did you weigh before?
Praying 🙏 for this outcome. I remember as a teenager when I was skinny through my 20s and 30s I just didn’t want food and then all that changes as I got older and insulin resistant and now I just want to get rid of the fat and the high blood sugar I’m praying that I will get that feeling again on Zepbound.
Nice share!
Fantastic quick summary!
Down 92 lbs since August on wegovy just switched to zepbound this week got 48lbs to go for my target. Anyone on the fence just try it yourself
Wegovy didn’t work for me
I’m with you on Zepbound.. 30 pds on two months of Mounjaro. Off 3 months because of insurance. Now on Zepbound and down an additional 10 total 40 pds down. Very few side effects, stay mindful of constipation.
I am thanks for your comment. So expensive but worth it if it works
Do your mind sharing your starting weight? I seen most people start in the 200s im in the high 3s, im wondering if you weigh more you lose faster?
I am so scared of the side effects… I have a month’s dose of Zepbound sitting in the fridge because I’m scared to use it smh…
This was such a nice, compact, and educational video.
Hope so much those drugs to be autorized in France soon 🙏 for my patients
I'm in France (Lyon) and my doctor has prescribed Ozempic at least. But the newer medicine is not here yet I guess.
Been on zepbound for two months. Now its gotten so popular canr find it and dr is going to switch me to wegovy nsonce it seems its more in stock st higher doses.
Do you offer telemedicine
How do you compare the dose escalation done in clinical trials to the current trend of keeping people on a low dose for a longer period of time? Does it result in greater ultimate weight loss? The current chatter is to "stay low and keep it slow", but the Surmount studies would indicate that keeping the dose at 5 mg will lead to less weight loss.
I recommend following the FDA-approved dose escalation schedule, whenever possible. Some patients may need to titrate at a slower pace due to side effects. Otherwise, we should follow the protocol that was studied in the clinical trials.
Been on zepbound for 2 months down 22 lbs.. but now we cant find it. Its so popular it has disappeared.
Hello, if possible please can you talk about EMSCULPT NEO, is it effective for fat reduction as they marketing it, Thank you!
I was on Wegovy for 6 months I only lost 10 pounds 😢I don’t know if it’s because I have hypothyroidism but I will start Zepbound tomorrow so hopefully it will work better.
TLDR: Zepbound and Wegovy are both effective weight loss medications, but cost and insurance coverage are major factors to consider when choosing between the two.
00:00 📈 Zepbound and Wegovy are both effective weight loss medications that work by boosting naturally occurring hormones in the body, specifically increasing glucagonlike peptide 1 levels.
00:55 💡 Zepbound (tirzepatide) targets both GLP1 and GIP receptors, while Wegovy (semaglutide) only impacts GLP1, making Zepbound more effective in managing appetite, satiety, glucose levels, and weight.
01:37 💉 Both Zepbound and Wegovy are weekly injections with a gradual dose increase over 4-5 months, but their milligram strength should not be directly compared, and they both impact PCT glp1.
02:00 💊 Both Zepbound (tirzepatide) and Wegovy (semaglutide) have similar side effects, with rare potential risks including pancreatitis, gallbladder disease, and gallstones, as well as a blackbox warning for thyroid cancer.
02:35 💡 Zepbound (tirzepatide) showed greater weight loss compared to Wegovy (semaglutide) in individual randomized trials.
03:28 💡 Zepbound (tirzepatide) and Wegovy (semaglutide) are both effective for weight loss, but cost is a major factor due to limited insurance coverage.
03:55 💰 Both Zepbound and Wegovy cost around $1,000 per month, making them unaffordable for most people without insurance.
04:12 📈 Insurance coverage and prices for Zepbound and Wegovy may improve, but it's best to consult with an obesity medicine specialist for guidance on the best option, and adding support from a dietitian or health coach can maximize effectiveness in managing obesity.
Do you practice in NY state?
In looking at the Surmount trial results, there appeared to be a cap of weight loss at 20%, resulting in a plateau. Are you seeing this in patients?
Yes, all patients will reach a nadir weight. The weight loss is not continuous. Typically, this is reached between 1 and 1.5 years after starting the medication. Those weight loss percentages are averages, so keep in mind that an individual’s weight loss may vary from that number. Some patients may lose more than 20%. Some patients may lose less (up to 20% of patients will lose less than 10% of their weight).
I have a prescription for Zepbound but am hesitant to try it. I was previously on Rybelsus but had to stop because my insurance refused to pay for it. My concern is the constipation. With the Rybelsus, it was pretty bad. My doc told me to take a probiotic and that didn’t help. What else can I do to help keep it in check?
Zepbound works!! But unfortunately its a shortage right now
The side effects are meant to be less with tirzepatide as it’s a weaker GLP1 agonist than Wegovy and this is what causes the side effects
Yep. That's the assumption. Thanks for clarifying! There haven't been head to head studies (yet) but it appears slightly better tolerated overall.
I lost 60 lbs on Wegovy in 4 months, but shortly after i hit the highest dose i could feel all my old habits and appetite creeping back and ive now regained 8 lbs. Thinking of asking my doc if i can switch to Zep
Have you switched yet? Are you strength training during your weight loss or after?
Hi Dr McGowan. I’m thinking about talking to my Dr about trying Zepbound but before I call and schedule an appt. I’m wondering if it’s worth it because my family has a history of thyroid cancer. Do you think it’s worth talking to my Dr?
The GLP meds are contraindicated (black box warning) if you have a personal or family history of MEDULLARY thyroid cancer, which is a less common form of thyroid cancer. If you cannot confirm what type is in your family, it may best to avoid the medication.
Thank you so much! That is very helpful 👍
Will I lose more switching to Mounjaro from Ozempic which I have stalled on?
Is a stall different than gaining? Cuz a stall may be great so one doesn't lose too fast?
My Doctor has prescribed Tirzepatide (Mounjaro) for me. I've completed the 4 week intro on 2.5mg and 2 weeks on 5mg. Nothing, no side effects at all, but no weight loss either. My concern.....my body hates moving off Stasis. I never eat junk food, I live on a ranch in the Idaho Mountains and cook for my wife and I.. No deep frying, only bake, roast, boil and air fry. I also move around a lot, yet no weight loss yet.
My question, according to statistics, people who finally get to the 15mg does lose from 21-26% of body weight. Does this work on everyone, or are there those like me whose bodies are stubborn. Just wondering?
@jimjohngirard any update?
@@MomofSeven123 Hi Janet...tomorrow I will take my 2nd injection of 12.5mg Mounjaro terzepatide. No weight loss yet...my body resists anything that takes it of stasis (maintenance). I'm frustrated as can be. Sorry it took so long to reply, as, I just saw your post.
@@jimjohngirardany updates?
@@nihadbalushi5138 I've lost 27 lbs in 6 months on Mounjaro. I eat a lot less, but my body is fighting me at every turn. I hate it. The good news, my A1C has gone from 6.3 to 5.8....a full half point down. My body just can't handle any form of sugar. I don't eat any carbs at all and yet my glucose was a bit high....go figure. That how these poor bastards get to 1,000 pounds.... because their bodies go haywire....I can tell you from experience, it's not just move more eat less....it's f'ing genetics.
The medicines do not increase GLP-1 and GIP levels. They just mimic them and bind the receptors.
1:33 GLP and GIP-1 :)
Of course. Hence the “receptor agonist” name. Many people may not be familiar with what an agonist is, or how hormone receptors work, so consider this a slightly simplified explanation!
By mimicking them it causes an increase 😅😅😅

What are your thoughts on gastric balloon?
Gastric balloons are a good option for someone seeking a low-risk, reversible weight loss tool. The average weight loss is approximately 15% of body weight in 6 months - similar to Wegovy, but a little faster. Different options for different people, that’s how I view it.

@DrMcGowanMD didn't know you had a video on it thank you so much for your time 😊
Haircut 💇♀️. Haha..couldn't get just hair. I'm about 7 days into my journey
Im on Zepbound compounding pharmacy, hope its real …lol
Is it working for you? I'm considering that route.
Do people who don’t respond to Wegovy respond to Zepbound?
I really hope so
I am praying. Because Wegovy didn’t do anything for me
Some say yes but don't switch if you are doing ok.
Please lose the background music - it's unnecessary and distracting
Dnp is the real goat haha
