This is misleading. This process indeed was fully supported on Windows Server up to 2019. However on Windows Server 2022 it's no longer supported. It's shame you didn't try to launch the IIS 6 console again and open properties of your SMTP instance as this would show the actual symptom on Windows Server 2022 which is the MMC crashing.

How come this is no longer supported?

@@JapyCorporation The Windows Server SMTP service support was dropped after Windows 2012 R2... it's still working, but in the next Windows Server release it is gone completely. Use hMailserver or Mailenable (free) as a replacement.

Can't believe we still rely on IIS 6 for SMTP in 2022.

It's not true what some people write here. The SMTP server works on Windows server 2022. Only a minor modification is required. This procedure will get it up and running on your server, we've now solved it:
We found that the following worked, and now we can modify the settings in II6 Manager at any time without errors (and without failing to apply the settings):
Stop SMTPSVC [Display name: Simple Mail Transfer Protocol (SMTP)]
Stop IISADMIN [Display name: IIS Admin Service]
    Edit "C:\Windows\System32\inetsrv\MetaBase.xml"
    Find:

Reported as mis-information. Support is gone in 2022. Only works on 2019 or earlier.
