Well, the whole Romeo and Juliet concept works because the song is about being in love with drugs or alcohol, and it slowly killing you. Romeo and Juliet died from being in love with each other. Even though the drugs are killing the Patient, he still does them knowingly, which fits "lather the blood on your hands".

( this was an artistic choice as gerard, being a rockstar who underwent a drinking problem could relate to) The patient was inspired by the marching band when he was young, leading to his life of music, which hasnt treated him well at all. He follows the black pardade in the afterlife because the parade is very meaningful to him.
