@@jesuisellacho yes...it’s very confusing and there’s a lot of grey area. We have studies that show it is both good and bad. Concentration matters as does form. I hope to be able to update this soon with more info but the more I dive into it the more complicated it becomes. We just don’t have all of the answers yet!

Enjoying learning more about MBD & dry eye. My doctor has never told me any of this. . I have been using prescription tretinoin for about a year. Have now stopped. Have to be honest I have not seen the changes in my skin I expected anyhow. Now worried I have done damage to my eyes. I recently had a blocked meibomian gland & was told apply heat, as much as you can stand. That's it. Very frustrating but I am now looking up everything I can on MGD & dry eyes! Thank you!
I think the doctor makes a very good point here. I do think we all spend too much time looking into our phones and don't blink enough to keep out eyes moist. Fish oil or flax seed can help.
I was told by my optometrist that I have dry eyes. I’m 53 and I feel my eyes have deteriorated drastically in the last two years. But when asked about dry eyes she said there’s nothing I can do for it. I came across your video and I’m shocked they didn’t offer any advice like you have. I have allergies which doesn’t help. I’m not sure whether to change my optometrist now. Ty for the advice.

Hi! I have had chronic dry eye for about 15 years due to autoimmune diseases and my age 60+. I was on Restasis which was about $450/month last time I bought it. My insurance doesn't cover it. I started getting generic from Canada about 10 years go. It comes in a 3ml bottle of drops. The generic is STILL not available in the US . It is about $30 per bottle which lasts 2-3 weeks. Yiu CAN do something about chronic dry eye. Sure hope this helps missblondeappetite !!!
Hi, would putting vaseline in the under eye area help to stop the tretinoin reaching the under eye area and causing dry eye? Edit: I got a response from her on instagram she advised: “Most of the dermatologists I follow like Vaseline under they eye or at least think it’s ok, but I’m not 100% sure it would stop tretinoin. I have started telling people to not put any retinol inside the orbital bone, a good hydrated cream with hyaluronic acid inside the orbital bone is ok.” From what i can gather from looking into it if you’re already suffering from dry eye because of retinoids wether you’re putting it under your eyes or not stop using immediately as this can cause permanent damage to the glands resulting in long lasting/permanent dry eye

Dr. D, would you recommend using retinyl palmitate around the eyes, instead of retinol? Retinyl palmitate eye drops are used to treat dry eyes, so I guess it's safe to use it around the eyes too?
I have the same question. I uknowingly used a moisturizing product with it and wonder if it caused my constant dry eyes (put it under eyes). I want to test better but my eyes seem constantly dry with or without it anyway. I think it does make it worse but am not sure. I will ask my ophthalmologist on the 17th.
Concerning the patient who had been using a retinoid for years, did they have to cease use when they found the eye damage? Or did they continue to use it? I have been using retinol for some years and dont want to stop. I dont have eye issues. Thanks for the video!
Dr. D , I recently started watching your eye school. I just discovered it. I’m 66 yrs. Old female. For 2 years I have suffered with dry eye. July 2019 I went to opthamologist. Long story short. He NEVER explained Anything. 2020 he did cataract surgeries. First, I would love your personal opinion about cataract surgery with uncontrolled DED. After major, eye pain, and no answer, I got all my records. I’m dx. With MGD, DED, blepharitis, and on. I NEVER was explained anything. You have been my Blessing. I’m in Florida now. May I am going to new opthamologist. Now, I’m suffering. First thank you for caring and doing eye school. I live with my eyes feeling so full, burn, I wear no makeup nor skin care. I can’t. I’m moving to NC by Mid July. Close to Greensboro or mccleansville. I pray you are close and please take new patients. I. Desperately need your help. I don’t have social life, I can’t. I also feel my rx for glasses is wrong. When I wear them my eyes are on fire. I’m currently on cequa, refresh PF, wear mask when sleeping, I’m miserable. I am in fact in North Carolina in Salisbury, just down the road from Greensboro so I hope to see you soon :) I am taking new patients and I have an esthetician that provides complimentary skincare analysis for my Dry Eye clinic patients as well. Also, it depends patient to patient but I can tell you that I have prepped some of my severe dry eye patients prior to cataract surgery utilizing IPL, Rx meds and whatever is necessary for that particular patient and I DO believe this optimizes outcomes after surgery. I hope this gives you some hope + I sincerely hope you're able to stop and see us in Salisbury.

I am so curious about the question. Why do my eyes burn BAD, especially with my glasses on while TRYING to read on phone? This morning is without glasses. I’m extremely disappointed that I went to the most reputable eye clinic in Orlando for exam. I’m now (2years latter) in agony. I’m changing doctors due to situation until I get to you. The opthamologist md NEVER took time to tell me dx. , make sure DED was under control before going forward with surgery. I would have never had cataract surgeries. I'm still investigating Bakuchiol, but I see many dermatologists recommending it. I personally use an LED mask for the eye area by Dr Gross which helps to thicken that skin and reduce fine lines. I am also researching creams and have a video coming about that!
@@EyeSchoolwithDrD do u use red or IR Light or any other type of light there? If I understand correctly we need to avoid blue light due to possible pigmentation?
@@Sky10811 yes I use red only. I’d have to look up the wavelengths. It’s 1/10th as strong as in office red light therapy (LLLT) but good for daily use. Not dangerous that I have been able to find, especially at the level that is sold OTC. There was a mask recalled a few years back due to eye concerns but the newer ones keep the lights closer to the skin.
I won't get into the details, but I can honestly say that Accutane ruined my life. (My dermatologist was very careless, and had me on it even when I started to complain of some pretty severe issues.) Among many other things, I now suffer from perpetually dry eyes that haven't responded to any type of treatment (including punctal plugs and prescription eye drops). At least this video put a name to the dry eye issue - Meibomian Gland Atrophy.
Good video. But my concern is that if I use a retinol on my face at night actively avoiding the eye area, that the retinol will migrate into my eye during sleep during the night. This is why I think it would be better to use the retinol during the day so that there is no chance of it entering the eye area/socket. What is your theory on this Dr D?
Wow. This is informative. Can I use face cream and face wash that has retinol in it? Does the retinol have to make contact with the conjunctiva to cause damage to the meibomian glands or can ot penetrate through the eyelids???
Hi, I've been using retinol for 3 months, once per week, and I have been diagnosed with MGD, do you think the retinol triggered the MGD or it could have happened by something else? I'm someone who stopped Accutane early due to dry eyes/MGD (it's been about 8 months now, and my MGD has actually improved tremendously after visiting an opthamologist regularly and I luckily had no signs of atrophy, just super clogged glands). Unfortunately, my acne has come back and I'm not sure what to do. I got prescribed tretinoin, but I'm obviously scared because I now have dry eyes and want to be careful. I'm considering using it with putting vaseline all around the eye area AND only with short contact therapy (basically only leaving the product on the skin as a mask for a fixed amount of time and then washing it off). Do you think this would be a safe way to start if I have no other options to control my acne?
OMG I can't thank you enough for this amazing video! I have ocular rosacea and the biggest issue for me is the fat content of my tears, it is very poor and my eyes water constantly, eyelid skin irritated, blepharitis... It's much worse in the winter because the cold makes the oil in the glands thicker. Lately my eyes have been worse, and we're in March already so it's not as cold as these previous months and I was not understanding what was going on...but I stumbled across this info of correlation between meibomiam glands and RA and then I found you. Guess what, I've been trying on retinol under the eyes for the last month, like in a month maybe I applied it 4 times. I don't know if that would even make that an immediate impact but just thinking that I was risking my already sparse oil without knowing! So I guess I can't use retinol...having rosacea really is a pain...
I also have rosacea (and ocular rosacea/MGD/aqueous layer deficiency) and have used Retin-A daily for a year without it worsening. Maybe I got lucky? It really isn't doing anything as far as aging goes so I'll talk to my derm about it (I'm on the lowest dosage still). As you have ocular rosacea, my best tip for you is to get IPL treatments for rosacea, especially under your eyes and on your cheeks to zap any telangiectasia and to heat the skin, and then get to an eye doc within 24 hours to express your glands while the oil is still soft (warning, both will hurt but it's worth it). I used to get this all done every 6 months for hundreds of dollars at a big eye clinic with little improvement, but my optometrist said he could do it if I found an esthetician to do the IPL and I now do this every 2 months and it's actually only $75 for the IPL and a co-pay for my eye doc for the expression. As the IPL is stronger and targeting my under eye area more, the improvement in my tear film is actually noticeable for the first time in nearly 10 years. I also use an eye drop through my optometrist's office called RegenerEyes as Restasis, etc., did nothing for me. This one helps, but it's not cheap. I have multiple dry eye issues going on and a degenerative corneal disease/transplant so if it helps me, I figure it will help anyone with dry eye. Lastly, don't ever use warm compresses with ocular rosacea in case you didn't know. I was told to do that for years and it made things worse and left me with purple circles under my eyes due to all the telangiectasia forming, which then cause more inflammation and round and round we go. I do get those from rosacea regardless, but the IPLs get rid of them so I don't look like a racoon anymore. Best wishes.

I specifically mean products from Avene with retinaldehyde and La Roche Posay with retinol. They are formulated for sensitive skin around the eye area. Do you think they would be bad choice as well? Well then what the hell is the point of using retin-A if you're going to have smooth beautiful skin everywhere and an eye area that looks 20 years older. Bright firm supple skin except for the under eye and hoods. I'll take the risk of dry eyes. Plus if I wear it while I sleep then how damaging can that be?
Ultimately you get to make your own decisions and evaluate the risk/reward for those decisions. I'm just here to give info. Knowing what I covered in the video, if you do start to suffer from dry eye, consider getting yourself to a specialist with meibomian gland imaging capability and in-office treatments to help. :)
If you develop dry eyes, you'll frown non stop. The light will bother you, working on a computer or even looking at a mobile phone will become impossible. Not to mention driving a car. You'll end up with more wrinkles than you started with and the quality of your life will plummet.
I was suspecting that retinol has something to do with my weird vision but wasn’t sure and I have told my ophthalmologist that it might have an effect but denied if it was for topical use. I don’t want to stop using it because I would have acne but also I want my vision back :(

I must say, I am so disappointed as the wrinkles under and beside my eyes age me terribly and I was hoping tretinoin might help. Can you suggest an eye cream that might help this issue? Also, you recommended keeping retinol away from the orbital area. Does this include the "elevens" (located between one's eyebrows) and the outer corners of one's eyes? (I am not sure if these two areas are within the orbital area or not.)
Your orbital area can be found my running your fingers around the somewhat circular bone around the eye. Starting just under the eyebrow, follow the bone down and to the outer corner of your eye and keep following that around the whole eye. You'll want to keep retinol/retin-a/tretinoin about an inch away from the opening of your eyes.
I've given up trying to find an eye cream. Most seem not to work and / or leave me with stinging eyes. And, I'm not sure a "non-retinol" eye cream helps at all with eye aging.
I used accutane when I was 15 twice and it sort of made everything drier, skin, scalp, mouth, eyes. They sort of just got drier gradually through my 20s, especially my eyes in the winter.
Mine too. Went on Accutane at 25, after the course dry skin got slightly better. Now 4 years later my skin is even more dry then when I was on treatment.

But what can you use in retinol's place? Believe me...having dry eye is so painful I regret my use of tretin-a around my eyes for years. But I don't have wrinkles even in 70s. What can I do instead? Also...I don't want to stretch my eye skin with that massage. Hopefully I could use Vaseline or something for massage.
my esthetician uses a gel based product for facial massage i believe. bakuchiol is an alternative but the eye studies are not there yet to support its use around the eyes specifically.

Hi Dr D, I’ve been getting chalazions / stys, I’m not sure if it’s from tretinoin or the moisturiser I use. The tretinoin has fixed my rosacea and made my skin look so much better. Even when I try to keep the creams away from my eyes I get the stys- maybe because it moves against my pillow? Or is it possible that as my rosacea heals the issue moves to my eyes? If I kept the cream very far away from my eyes do you think it would be fine or do you think I need to stop using it? I don’t know anyone else who knows about the correlation between retinol and eye glands so thank you so much for addressing this!!
@@ekb0127 hey! I stopped using tretinoin until my eyes got better and then started using the weaker 0.025% instead of ,5 and am keeping it well away from my eyes. I use it only every 2-3 days. I don’t get the eye problems anymore 👍
Hi loving your chanel! I use VitA-POS at night as I have severe MGD and rosacea. Just saw your video and wondered if this product was any good. It contains retinol palmitate 138 mcg/g. Is it the same as the retinal you are talking about? My eye specialist recommended it and I have been using it for over 10 years. Thank you! 🙂
Interesting as that's actually approved for dry eye and it's all vitamin A, so why would it be an issue? I also have severe MGD and rosacea, but have never used that ointment (I use the OTC kind). I do use a low dose of Retin-A nightly on my face and lids and a year later, my eyes are not any worse. I've had severe dry eye for 15 years now. I'm a little skeptical about this as everything irritates my eyes so why wouldn't the Retin-A? I have an optometrist/lens fitter and a corneal specialist and no one has said anything. My derm knows I have dry eye, as well. I asked for Botox between my eyebrows but was worried as that can cause dry eye, so she gave me Retin-A instead. I can't say it's done much for my face, but at least nothing is worse eye-wise.
I got dry eye once, nothing would help my tears to go away , until I warmed some water , q tip and coconut oil saved me ! Idk why nothing would help? But I probably used some product that messed me up , since you explain here well

Hi. Mid 60’s and have worn rigid lenses since middle school in 1969 .. noticing a slight drop in left eyelid. I know nothing short of surgery someday may help but I’ve been using retinol on eyelids for years. Is there something else you would suggest to firm up my lid a bit? Ps. Don’t have dry eye issues. I stopped using the tretinoin cream recently because my eye doctor asked me to stop using it for my dry eyes. I want to use it but i am worried. Is it safe for the dry eyes if I put it on except eye area? Thank you
It really is best to avoid, especially if your doctor has identified it as a contributor to your dry eye. Your doctor may be ok with you restarting and avoiding the eye once your dry eye is better controlled. That’s typically how I handle it. Definitely reach out to your eye doc because they have better knowledge of your particular situation and can guide you better. Good luck!

The safest course of action is to avoid applying any retinol inside of your orbital bone. But it would depend on your situation. Do you have dry eye? Rosacea? Known meibomian gland dysfunction?
I've used Tretinoin .025% since I was 35 years old; at 57 years old, I have no wrinkles or sagging. Never felt the need to go to a higher dose where there's lots of redness and flaking. I'm seeing my eye doctor in two days, and will ask about warm eye compresses.
If you already had dry eyes in the past and it has gotten better (my case), would it be justifiable to use Retin-a (topical) only on the lower cheeks and forehead (with an extra space to the eye area)?
I think it’s a reasonable ask… if I were your doctor I’d monitor your meibomian glands with imaging as well as all the other dry eye markers and have you stop if I saw a change
I was trying to figure it out what’s happening with my eyes… have got 5 styes in my eyes in 3 months! I remember I got one 35 years ago and never again until now! Styes and little dandruff like in my eyelashes line are my nightmare.I didn’t suffer dry eye or anything like that before. I got eyelash extensions for 2 years and I was perfect, until I notice I was losing my natural eyelashes so I decided to stop it 6 years ago. Last year I started using Tretinoin 0.05 % cream to treat some dark spots in my face! I stopped it in summer and started again last year in November. So I started to investigate on internet and I found out Retinoids may produce or be a potential cause of dry eyes and blepharitis. Because of my almost bald eyes I started on February Latisse to get my eyelashes back again. Now my big question is could be Tretinoin cream the cause of those annoying symptoms in my eyes? 😵💫👀👁😓 Any suggestions? advice please? I hear honey, coconut oil, tea tree oil and aloe Vera are great natural options to help with this condition + good hygiene+ lubricant drops and heated pads help a lot. Try adapalene or retinol instead of tretinoin. They are less more potent. Or use a strong tretinoin 0.1% every sunday and wash off before sleep. About eye area
Did you say increase hyperpigmentation? So many people are trying to reduce hyperpigmentation. Is it ok for the eye if we put it on the forehead and cheeks only?
If they cause irritation that can lead to post inflammatory hyperpigmentation, but as a whole my understanding is that retinol doesn't itself cause hyperpigmentation. I have started recommending patients not put retinal inside the orbital bone. If you keep it outside of that area, i.e forehead and cheeks it should be fine. But if you have Dry Eye consider getting your meibomian glands imaged at baseline and then once a year to make sure you're not losing any glands.
This is informative yet frustrating since using 0.025 tret on my entire eye skin for a few months I've seen the clock rewind nearly 7 years. I think I'll just back off from daily to once a week instead of stopping completely. Until there's an equally effective solution that's not eyelid surgery.
Hello, I tried using Olay Retinol 24 avoiding the eye area but I guess it migrated to my eyes and I had swollen upper eyelids. Can u suggest how I can use retinols without this happening in future? thanks in advance
Hi Dr Is it common to see people who went on Accutane with minimal dry eye symptoms during treatment (I only had a few days of grittiness) develop dry eye years later? (mine only started showing painful symptoms 4 years after accutane). I also had 2 months of recurring styes after I stopped Accutane (but never any during the treatment)
YES, there’s no studies I know of on this yet but clinically I find that either you will have dryness right away while actually on the medicine OR it can present years later
Heat therapy and massage will certainly help to make sure the glands you have are working well. However, the harm from the retinol will most likely outweigh any good you're doing with the heat and massage.
Pls can u do a video of dermatitis or eczema's reasons on eye lids, specifically from some components of eye-shadows? I started getting it and it s awful! I think mostly it s from sparkling eye-shadows. What to do if one still wants to use eye-shadows?..
There are a ton of different iterations, lots of different concentrations of retinol. The story on this is evolving, there is a lot of grey area and nobody knows definitively right now what is ok and what isn’t. To be very conservative avoid all retinoids. I’m actively trying to figure this out and when I do will post update video!

i've started using the inkey list retinol for eyes and i started yesterday night and it really burned and now i have a red area around my eyes and there a lot of wrinkles and it still burns and feels dry what should i do :(
Stop use, rinse the area and keep it clean. Put a cool compress on it. Consider using the Cerave healing ointment to restore skin barrier. No makeup or skincare actives until it resolves. Call your eye doctor and let them know what’s going on and get their advice.

Could you please update if they improved? the same thing hs happened to me. Ive been using it a week and a half and now my under eye wrinkles are much worse and look old and dry. Did they recover? Did you stop using retinol and if they r better how did you help them?

There are so many different forms of retinol and the data is definitely continuously evolving. There's a lot of grey area. If you do have Dry Eye already or Meibomian Gland Dysfunction I think avoiding them entirely is the absolute safest. I plan to update this video soon because there have already been more studies on Vitamin A, retinol, and the eye!

I love Dr Dray too! I diverge from her just a little bit when it comes to not only retinol but parabens, phenoxyethanol, and formaldehyde donating preservatives as well. I actually already filmed an eye cream video that will be coming out soon! She rightly talks about how the clean beauty movement should die and talks about these ingredients as being ok for the skin. But when it comes to the eyes, I am a little more conservative. The thing is, studies are showing the toxic effect of these ingredients on the meibomian glands and epithelial cells of the cornea. And while yes, for the dermis and epidermis they may be ok, I want to get the story out there about their effect on the eye. Now, since you don't have Dry Eye that decision is up to you. I just want you to have all the info. I definitely think you will like the eye cream video coming out soon! Thanks again for watching! :)

I used retin-a 0.05 on my cheekbones and I woke up with my under eye are swollen!! What can I do? I've been swollen all day... I have hyalluronic acid fillers under my eye circles (I did it less than a year ago)... maybe it was a reaction?
I have a tendency to dry eyes hence my contacts tend to annoy me after a few hours. I have started tretinoin two year ago and recently my eyes have gone even drier and i ended up with a little cornea, ulcer. I stopped contacts for a couple of months but I realised tretinoin might have played a part as I’m applying it quite close to my eyes. I think ill keep using tretinoin but definetly not around my eyes. Also i noticed a little darkening inside the corner inner lid But ophthalmologist said its just pigmentation, do you agree?
See if you can get your meibomian glands imaged, if you have glands dropping out or shortening that would tell you a lot. A dry eye doctor can tell you for sure. I’m not sure about the pigment, I’d never be able to comment on that without actually examining.

Hey everyone! At one point in the video I state that retinol INCREASES hyperpigmentation, but I misspoke! Retinoids actually work to DECREASE hyperpigmentation. Sorry for getting that confused and thank you to those who pointed it out!!!
Retionol is vitamin A. But i saw many dry eyes products that has vitamin a.... is therefore difference...? Retinol on our eyes vs. in our eyes???
@@jesuisellacho yes...it’s very confusing and there’s a lot of grey area. We have studies that show it is both good and bad. Concentration matters as does form. I hope to be able to update this soon with more info but the more I dive into it the more complicated it becomes. We just don’t have all of the answers yet!

Enjoying learning more about MBD & dry eye. My doctor has never told me any of this. . I have been using prescription tretinoin for about a year. Have now stopped. Have to be honest I have not seen the changes in my skin I expected anyhow. Now worried I have done damage to my eyes. I recently had a blocked meibomian gland & was told apply heat, as much as you can stand. That's it. Very frustrating but I am now looking up everything I can on MGD & dry eyes! Thank you!

I think the doctor makes a very good point here. I do think we all spend too much time looking into our phones and don't blink enough to keep out eyes moist. Fish oil or flax seed can help.
I was told by my optometrist that I have dry eyes. I’m 53 and I feel my eyes have deteriorated drastically in the last two years. But when asked about dry eyes she said there’s nothing I can do for it. I came across your video and I’m shocked they didn’t offer any advice like you have. I have allergies which doesn’t help. Menopause also aggravates dry eye! Estrogen therapy could help if that's the cause.
Hi! I have had chronic dry eye for about 15 years due to autoimmune diseases and my age 60+. I was on Restasis which was about $450/month last time I bought it. My insurance doesn't cover it. I started getting generic from Canada about 10 years go. It comes in a 3ml bottle of drops. The generic is STILL not available in the US . It is about $30 per bottle which lasts 2-3 weeks. Yiu CAN do something about chronic dry eye. Sure hope this helps missblondeappetite !!!

I got a response from her on instagram she advised: “Most of the dermatologists I follow like Vaseline under they eye or at least think it’s ok, but I’m not 100% sure it would stop tretinoin. I have started telling people to not put any retinol inside the orbital bone, a good hydrated cream with hyaluronic acid inside the orbital bone is ok.”
From what i can gather from looking into it if you’re already suffering from dry eye because of retinoids wether you’re putting it under your eyes or not stop using immediately as this can cause permanent damage to the glands resulting in long lasting/permanent dry eye

Dr. D, would you recommend using retinyl palmitate around the eyes, instead of retinol? Retinyl palmitate eye drops are used to treat dry eyes, so I guess it's safe to use it around the eyes too?
I have the same question. I uknowingly used a moisturizing product with it and wonder if it caused my constant dry eyes (put it under eyes). I want to test better but my eyes seem constantly dry with or without it anyway. I think it does make it worse but am not sure. I will ask my ophthalmologist on the 17th.

Dr. D , I recently started watching your eye school. I just discovered it. I’m 66 yrs. Old female. For 2 years I have suffered with dry eye. July 2019 I went to opthamologist. Long story short. He NEVER explained Anything. 2020 he did cataract surgeries. First, I would love your personal opinion about cataract surgery with uncontrolled DED. After major, eye pain, and no answer, I got all my records. I’m dx. With MGD, DED, blepharitis, and on. I NEVER was explained anything. You have been my Blessing. I’m in Florida now. I am in fact in North Carolina in Salisbury, just down the road from Greensboro so I hope to see you soon :) I am taking new patients and I have an esthetician that provides complimentary skincare analysis for my Dry Eye clinic patients as well.
Also, it depends patient to patient but I can tell you that I have prepped some of my severe dry eye patients prior to cataract surgery utilizing IPL, Rx meds and whatever is necessary for that particular patient and I DO believe this optimizes outcomes after surgery.

I'm still investigating Bakuchiol, but I see many dermatologists recommending it. I personally use an LED mask for the eye area by Dr Gross which helps to thicken that skin and reduce fine lines. I am also researching creams and have a video coming about that!

@@Sky10811 yes I use red only. I’d have to look up the wavelengths. It’s 1/10th as strong as in office red light therapy (LLLT) but good for daily use. Not dangerous that I have been able to find, especially at the level that is sold OTC. There was a mask recalled a few years back due to eye concerns but the newer ones keep the lights closer to the skin.

I won't get into the details, but I can honestly say that Accutane ruined my life. (My dermatologist was very careless, and had me on it even when I started to complain of some pretty severe issues.) Among many other things, I now suffer from perpetually dry eyes that haven't responded to any type of treatment (including punctal plugs and prescription eye drops).
At least this video put a name to the dry eye issue - Meibomian Gland Atrophy.

Good video. But my concern is that if I use a retinol on my face at night actively avoiding the eye area, that the retinol will migrate into my eye during sleep during the night. This is why I think it would be better to use the retinol during the day so that there is no chance of it entering the eye area/socket. What is your theory on this Dr D?

I'm someone who stopped Accutane early due to dry eyes/MGD (it's been about 8 months now, and my MGD has actually improved tremendously after visiting an opthamologist regularly and I luckily had no signs of atrophy, just super clogged glands).
Unfortunately, my acne has come back and I'm not sure what to do. I got prescribed tretinoin, but I'm obviously scared because I now have dry eyes and want to be careful. I'm considering using it with putting vaseline all around the eye area AND only with short contact therapy (basically only leaving the product on the skin as a mask for a fixed amount of time and then washing it off).
Do you think this would be a safe way to start if I have no other options to control my acne?

OMG I can't thank you enough for this amazing video! I have ocular rosacea and the biggest issue for me is the fat content of my tears, it is very poor and my eyes water constantly, eyelid skin irritated, blepharitis... It's much worse in the winter because the cold makes the oil in the glands thicker. Lately my eyes have been worse, and we're in March already so it's not as cold as these previous months and I was not understanding what was going on...but I stumbled across this info of correlation between meibomiam glands and RA and then I found you. Guess what, I've been trying on retinol under the eyes for the last month, like in a month maybe I applied it 4 times. I don't know if that would even make that an immediate impact but just thinking that I was risking my already sparse oil without knowing! So I guess I can't use retinol...having rosacea really is a pain...
I only used it a couple weeks around my eyes and it made my dry eye 10x worse.
I also have rosacea (and ocular rosacea/MGD/aqueous layer deficiency) and have used Retin-A daily for a year without it worsening. Maybe I got lucky? It really isn't doing anything as far as aging goes so I'll talk to my derm about it (I'm on the lowest dosage still). As you have ocular rosacea, my best tip for you is to get IPL treatments for rosacea, especially under your eyes and on your cheeks to zap any telangiectasia and to heat the skin, and then get to an eye doc within 24 hours to express your glands while the oil is still soft (warning, both will hurt but it's worth it). I used to get this all done every 6 months for hundreds of dollars at a big eye clinic with little improvement, but my optometrist said he could do it if I found an esthetician to do the IPL and I now do this every 2 months and it's actually only $75 for the IPL and a co-pay for my eye doc for the expression. As the IPL is stronger and targeting my under eye area more, the improvement in my tear film is actually noticeable for the first time in nearly 10 years. I also use an eye drop through my optometrist's office called RegenerEyes as Restasis, etc., did nothing for me. This one helps, but it's not cheap. I have multiple dry eye issues going on and a degenerative corneal disease/transplant so if it helps me, I figure it will help anyone with dry eye. Lastly, don't ever use warm compresses with ocular rosacea in case you didn't know. I was told to do that for years and it made things worse and left me with purple circles under my eyes due to all the telangiectasia forming, which then cause more inflammation and round and round we go. I do get those from rosacea regardless, but the IPLs get rid of them so I don't look like a racoon anymore. Best wishes.

My eyelids become dry even when I avoid the eye-area when using Retinol. I suppose the ingredients travel up to the eye area.
Any tips for managing milia if retinol can't be used near the eyes? Or is it far enough from the eyes to be safe?
Well then what the hell is the point of using retin-A if you're going to have smooth beautiful skin everywhere and an eye area that looks 20 years older. Bright firm supple skin except for the under eye and hoods. I'll take the risk of dry eyes. Plus if I wear it while I sleep then how damaging can that be?
Ultimately you get to make your own decisions and evaluate the risk/reward for those decisions. I'm just here to give info. Knowing what I covered in the video, if you do start to suffer from dry eye, consider getting yourself to a specialist with meibomian gland imaging capability and in-office treatments to help. :)
If you develop dry eyes, you'll frown non stop. The light will bother you, working on a computer or even looking at a mobile phone will become impossible. Not to mention driving a car. You'll end up with more wrinkles than you started with and the quality of your life will plummet.
I was suspecting that retinol has something to do with my weird vision but wasn’t sure and I have told my ophthalmologist that it might have an effect but denied if it was for topical use. I don’t want to stop using it because I would have acne but also I want my vision back :(

I must say, I am so disappointed as the wrinkles under and beside my eyes age me terribly and I was hoping tretinoin might help. Can you suggest an eye cream that might help this issue? Also, you recommended keeping retinol away from the orbital area. Does this include the "elevens" (located between one's eyebrows) and the outer corners of one's eyes? (I am not sure if these two areas are within the orbital area or not.)
Your orbital area can be found my running your fingers around the somewhat circular bone around the eye. Starting just under the eyebrow, follow the bone down and to the outer corner of your eye and keep following that around the whole eye. You'll want to keep retinol/retin-a/tretinoin about an inch away from the opening of your eyes.

I've given up trying to find an eye cream. Most seem not to work and / or leave me with stinging eyes. And, I'm not sure a "non-retinol" eye cream helps at all with eye aging.

I used accutane when I was 15 twice and it sort of made everything drier, skin, scalp, mouth, eyes. They sort of just got drier gradually through my 20s, especially my eyes in the winter.
Mine too. Went on Accutane at 25, after the course dry skin got slightly better. Now 4 years later my skin is even more dry then when I was on treatment.
My skin is super oily and I have bad acne but I'd rather just deal with it

But what can you use in retinol's place? Believe me...having dry eye is so painful I regret my use of tretin-a around my eyes for years. But I don't have wrinkles even in 70s. What can I do instead? my esthetician uses a gel based product for facial massage i believe. bakuchiol is an alternative but the eye studies are not there yet to support its use around the eyes specifically.

Hi Dr D, I’ve been getting chalazions / stys, I’m not sure if it’s from tretinoin or the moisturiser I use. The tretinoin has fixed my rosacea and made my skin look so much better. Even when I try to keep the creams away from my eyes I get the stys- maybe because it moves against my pillow? Or is it possible that as my rosacea heals the issue moves to my eyes? If I kept the cream very far away from my eyes do you think it would be fine or do you think I need to stop using it?

Interesting as that's actually approved for dry eye and it's all vitamin A, so why would it be an issue? I also have severe MGD and rosacea, but have never used that ointment (I use the OTC kind). I do use a low dose of Retin-A nightly on my face and lids and a year later, my eyes are not any worse. I've had severe dry eye for 15 years now. I'm a little skeptical about this as everything irritates my eyes so why wouldn't the Retin-A? I have an optometrist/lens fitter and a corneal specialist and no one has said anything. My derm knows I have dry eye, as well. I asked for Botox between my eyebrows but was worried as that can cause dry eye, so she gave me Retin-A instead. I can't say it's done much for my face, but at least nothing is worse eye-wise.

It really is best to avoid, especially if your doctor has identified it as a contributor to your dry eye. Your doctor may be ok with you restarting and avoiding the eye once your dry eye is better controlled. That’s typically how I handle it. Definitely reach out to your eye doc because they have better knowledge of your particular situation and can guide you better. Good luck!

The safest course of action is to avoid applying any retinol inside of your orbital bone. But it would depend on your situation. Do you have dry eye? Rosacea? Known meibomian gland dysfunction?

Me too 😭 i have only been using it for a couple months. I had dry eye before and had no idea this would contribute.
I've used Tretinoin .025% since I was 35 years old; at 57 years old, I have no wrinkles or sagging. Never felt the need to go to a higher dose where there's lots of redness and flaking. I'm seeing my eye doctor in two days, and will ask about warm eye compresses.

I was trying to figure it out what’s happening with my eyes… have got 5 styes in my eyes in 3 months! I remember I got one 35 years ago and never again until now! Styes and little dandruff like in my eyelashes line are my nightmare.I didn’t suffer dry eye or anything like that before. I got eyelash extensions for 2 years and I was perfect, until I notice I was losing my natural eyelashes so I decided to stop it 6 years ago. Last year I started using Tretinoin 0.05 % cream to treat some dark spots in my face! I stopped it in summer and started again last year in November. So I started to investigate on internet and I found out Retinoids may produce or be a potential cause of dry eyes and blepharitis. Because of my almost bald eyes I started on February Latisse to get my eyelashes back again. Now my big question is could be Tretinoin cream the cause of those annoying symptoms in my eyes? 😵💫👀👁😓 Any suggestions? advice please? I hear honey, coconut oil, tea tree oil and aloe Vera are great natural options to help with this condition + good hygiene+ lubricant drops and heated pads help a lot. Try adapalene or retinol instead of tretinoin. They are less more potent. Or use a strong tretinoin 0.1% every sunday and wash off before sleep. About eye area

If they cause irritation that can lead to post inflammatory hyperpigmentation, but as a whole my understanding is that retinol doesn't itself cause hyperpigmentation. I have started recommending patients not put retinal inside the orbital bone. If you keep it outside of that area, i.e forehead and cheeks it should be fine. Yes both can cause problems, accutane orally as well as topical derivatives on the eye area
This is informative yet frustrating since using 0.025 tret on my entire eye skin for a few months I've seen the clock rewind nearly 7 years. I think I'll just back off from daily to once a week instead of stopping completely. Until there's an equally effective solution that's not eyelid surgery.
Hello, I tried using Olay Retinol 24 avoiding the eye area but I guess it migrated to my eyes and I had swollen upper eyelids. Can u suggest how I can use retinols without this happening in future? thanks in advance

Is it common to see people who went on Accutane with minimal dry eye symptoms during treatment (I only had a few days of grittiness) develop dry eye years later? (mine only started showing painful symptoms 4 years after accutane).
I also had 2 months of recurring styes after I stopped Accutane (but never any during the treatment)
YES, there’s no studies I know of on this yet but clinically I find that either you will have dryness right away while actually on the medicine OR it can present years later

Heat therapy and massage will certainly help to make sure the glands you have are working well. However, the harm from the retinol will most likely outweigh any good you're doing with the heat and massage.
Pls can u do a video of dermatitis or eczema's reasons on eye lids, specifically from some components of eye-shadows?
I started getting it and it s awful! I think mostly it s from sparkling eye-shadows.

I use Tretinoin but I apply it very carefully not to apply it around the eyes and neck. However I still find that it really dry around my eyes. 😓

There are a ton of different iterations, lots of different concentrations of retinol. The story on this is evolving, there is a lot of grey area and nobody knows definitively right now what is ok and what isn’t. To be very conservative avoid all retinoids. I’m actively trying to figure this out and when I do will post update video!

Definitely run it by your doctor, he’ll be able to help you make better decisions than I can here. Rinse your eye if you get some cream in it!

i've started using the inkey list retinol for eyes and i started yesterday night and it really burned and now i have a red area around my eyes and there a lot of wrinkles and it still burns and feels dry what should i do :(
Stop use, rinse the area and keep it clean. Put a cool compress on it. Consider using the Cerave healing ointment to restore skin barrier. No makeup or skincare actives until it resolves. Call your eye doctor and let them know what’s going on and get their advice.

Could you please update if they improved? the same thing hs happened to me. Ive been using it a week and a half and now my under eye wrinkles are much worse and look old and dry. Did they recover? Did you stop using retinol and if they r better how did you help them?

There are so many different forms of retinol and the data is definitely continuously evolving. There's a lot of grey area. If you do have Dry Eye already or Meibomian Gland Dysfunction I think avoiding them entirely is the absolute safest. I plan to update this video soon because there have already been more studies on Vitamin A, retinol, and the eye!

I love Dr Dray too! I diverge from her just a little bit when it comes to not only retinol but parabens, phenoxyethanol, and formaldehyde donating preservatives as well. I actually already filmed an eye cream video that will be coming out soon! She rightly talks about how the clean beauty movement should die and talks about these ingredients as being ok for the skin. But when it comes to the eyes, I am a little more conservative. The thing is, studies are showing the toxic effect of these ingredients on the meibomian glands and epithelial cells of the cornea. And while yes, for the dermis and epidermis they may be ok, I want to get the story out there about their effect on the eye. Now, since you don't have Dry Eye that decision is up to you. I just want you to have all the info. I definitely think you will like the eye cream video coming out soon! Thanks again for watching! :)

I have a tendency to dry eyes hence my contacts tend to annoy me after a few hours. I have started tretinoin two year ago and recently my eyes have gone even drier and i ended up with a little cornea, ulcer. I stopped contacts for a couple of months but I realised tretinoin might have played a part as I’m applying it quite close to my eyes. I think ill keep using tretinoin but definetly not around my eyes. Also i noticed a little darkening inside the corner inner lid But ophthalmologist said its just pigmentation, do you agree?
See if you can get your meibomian glands imaged, if you have glands dropping out or shortening that would tell you a lot. A dry eye doctor can tell you for sure. I’m not sure about the pigment, I’d never be able to comment on that without actually examining.
