I may be wrong about PHF being the spin-off in question, but I'm very certain I've heard it said before that Araki regretted ditching the traitor idea. Pretty good read imo, definitely recommend. I think the novel you're thinking of is Golden Heart Golden Ring though
In the novel Purple Haze Feedback, Fugo kills Tonio's brother, so if this animation took place in the same universe as PHF, Tonio would have seen how the guy who just entered his restaurant killed his brother, it would have been fun to see that

She was actually cured! He needed the abalones to cure her, so when Rohan got them during the chapter, Tonio cooked up the dish and cured her! Tonio actually got a good ending

Pearl Jam has only ever been seen healing physical conditions Sure, a proper diet and exercise can help manage and minimize the effects of mental health issues but they're only short term solutions. Therapy and facing your issues will be far more effective in circumstances like Fugo's. I imagine that Pearl Jam in this case could only provide a temporary relief, not a full blown cure, which might cause an unhealthy dependency on the stand's affect. And Tonio might understand this and thus refrain from using it, seeing that it would cause more problems than it would solve But hey, that's just a theory. A FILM THEO-
It's a shame Fugo completely disappeared after the first half of the arc, considering how truly deadly and useful his Stand is. In fact, in the novel "Golden Heart, Golden ring", Fugo (while confronting the book's female protagonist Coniglio) kills along the way more people than a serial killer. Bro is truly a threat.
Now that I think about Fugo is so strong that Araki cut him out of the story if he mastered his stand and used it he probably could have killed Diavolo with Giorno’s help
