Great video! Now I'm fairly certain I have an impingement, as tests 2 and 3 definitely agitated my shoulder. 1 - not so much. It was more while my arm was up at around 2 o'clock that I felt some discomfort, rather than at 4 o'clock or so as you indicated. Now...on to your "How to Fix" video!
Yaay!! So if it hurts deep in my shoulder to hang from a bar, or to do triceps extensions behind my head- would that indicate impingement? My shoulder clunks a lot, and I’ve said it feels like an impingement- without having any clue what that truly is- because it feels like something is in the way. It’s been like that for many years. Thank you for this information!!! 🙏🏽🙏🏽 I just can’t go to a doctor again, I’ve had 12 procedures and 13 rounds of PT in the last 6 years for other injuries, but UA-cam is more helpful than most doctors, sadly…
nice job, sports medicine doc here. Really good presentation for patients at home. Of course, a lot of these tests suffer from mediocre validity but there is no perfect set of shoulder exams, so given that is what we have to work with, I like your selection here. overall, well done!
Thank you! You are very good and your channel is excellent. I have researched for my shoulder pain and seen a lot of other videos. You are the best, the clearest explanations and tests and therapy instructions. Thanks very much. I really appreciate you posting all your videos and will share your channel with others.
Hey Dr. Saman, I want to thank you for this awesome video. I'am not able to afford to go and see a doctor. Now I have a better understanding of what I could be dialing with in my shoulder pain. I will be subscribing to your Channel. Thank you again.
This video helped me figure out it is shoulder impingement. Negative on all the rotator cuff exercises but positive on 2.5 of the impinge test (2.5 because the last test was barely an issue but still dull pain). Thanks! Big weight off my shoulders....lol
I really appreciate this video. I have had shoulder pain for years and went to see my doctor about it. He sent me to a specialist who did some different types of tests than you did here and said I had shoulder impingement syndrome. I never believed him because the tests were not explained thoroughly and I didn't have pain in all the movements. So I never believed him and was sure I had torn my rotator cuff or some other type of structural damage. This video helps bring me some clarity.
My shoulder started hurting about 3-4 months ago. I thought because of how long the problem has lasted it was more likely to be shoulder impingement than a tear but I had a scan and it is actually a tear. I am quite pleased about this because I think an impingement is more likely to be an on going or reoccurring problem. Once my tear is healed I should be okay again.
This pretty much confirmed what I already thought I was dealing with, but clarified really well the differences between partial tear and full tear. I have been dealing witih excruciating pain, specifically at night, trying to get comfortable in bed. I'll wake up every hour or so and at times its so bad I have to get up. Pretty sure I have some form of partial tear. I am currently waiting for the referral process to go through from my PCP to an Ortho specialist. Thanks for the info sir!
I think maybe a small rotator cuff tear and definitely impingement but found it a bit tricky to determine if the impingement thats causing the pain might be making it look like a small rotator cuff tear is there. Thanks though this will get me closer. Looks like a common problem that affects lots of people. I got mine from pretty high intensity workouts after months in bed. 500 push ups and 1000 lat lifts and some other alternative exercises. Didn't notice the pain until increasing the reps extra hundred too fast one day. Be very careful, don't overtrain. My routine was months to get there but neglected the corresponding stablisers.😊
A few things have helped me. So I started doing my Lateral raises, overhead dumbell press and dumbell bench press with a neutral grip. I had a grindy sore right shoulder. So yesterday I switched to the new grip. First I warmed up, did some doorway stretches for my shoulder. I hung on an elevated barbell with my feet on the ground. Every thirty seconds I did 10 scap pulls, thent went back to hanging position. I did my workout with the beginning excersises neutral grip. I ended my session with hanging from the bar. I felt this slightly weird pop in my shoulder, it felt good though. When I stood up from hanging, the shoulder pain I had for weeks was gone.
i can agree here…..i’ve been actively thinking about fixing my impingement and i think it’s working…i started by fixing my rounded posture because i figured my shoulders being rounded could very well be causing the impingement in the first place i’m also doing rotator cuff work evry time i hit the gym…also dead hangs to relieve some shoulder pressure…gotta change workouts around too i stopped workouts that bother my shoulder and i couldn’t skip chest so i changed my bench position and arm positions to where my shoulders don’t grind…HOPE THIS HELPS 🦾
Amazing video. Best I’ve seen on the shoulder. I have a recurring impingement, can anyone recommend some treatment or exercises to stop it It’s the bane of my life. Thanks for great video !
Yes, this was very helpful. I was going to walk my new dog (big, strong dog). She took off before I was ready. She yanked my arm up and forward, I started to fall and even though my shoulder was screaming, I had to pull back on the lease to stop her from dragging me down the driveway. Thqt was 4 months ago.i waited a few weeks to see if the pain lessened and when it didntI saw the doctor and got a sterlid injection. It halped for about 2 weeks.I qm going in next week for a follow up and 2anted to learn more. I am going to do all this tomorrow morning and see what I have going on. Thank you!!!
Thank you for an informative video. I did the tests you suggested and I am comfortable I probably don’t have a rotator cuff tear or impingement. However, 3 years ago I took a cortisone injection for my right shoulder as the pain was unbearable. Couldn’t sleep for 3 nights and 4 days. It was awful.
Thank you for showing me the difference between small, medium, large and full tears on a piece of paper. Also the way you describe the amount of pain of each tear is very educational and scientific. Who knew that a medium tear really hurts and a full tear would hurt a lot. Now can you me the difference between A1, A2, A3, A4 size papers.
I fell and jolted my shoulder back hard. Very painful in beginning…less now but I can’t raise my arm on the side or even reach back enough to put my hand in a pocket. Feel like it’s locked. Got an X-ray. Not dislocated. Going for MRI next. Very frustrating injury. Excellent video. Thank u
You are really a competent therapist.I have been facing this issue for last nine months. No doctor was able to locate the problem. Please advise me which doctor should I go to a physio therapist or an orthopedic doctor or to a neurologist
I started having shoulder pains a few weeks back, I suspected it was over use but despite taking it easy, the pain didn't go away completely. Dr Google said it was a tear but your video now suggests an impingement. One of the symptoms I get is a clicking sound as I rotate in various angles along with a sudden sharp pain. I now need to look up some therapy. Thank you for the video and the simple tests.
Saw a physio and thought it was impingement. Gave me exercises which has helped it a lot. I have almost all movement, but sleeping on the right side will bring on sever pain. He advised me that these sort of injuries can take up to a year to heal.
Thanks much! I woke up (after sleeping on my left shoulder and the previous day working out) and the shoulder hurt. I failed the last rotary tear test and failed all 3 impingement tests. So definitely, I seem to have shoulder impingement. I just got some X-ray tests done, so waiting on results.
Best way to relieve this condition quickly as possible without surgery is what Im currently doing myself. Dead hangs, yes dead hangs, with head forward and down. Even better close grip dead hangs. This has helped me stretch the tendon back out so to speak. Basically anything that actually engages the pain is a way to remedy it. As crazy as that may sound. If you DON'T move your arm at all and you're always resting it thinking this will make it better IT WON'T. And no I don't recommend bands or any type of strength movement. Just stretching the faulty arm a lot. Take it from me I am no longer feeling that excruciating pain anymore. Its still a bit irritated now but not agony like a couple weeks ago.
This is the most AMAZING video on the subject. I could not understand what other doctors and PTs were saying about shoulder/arm pain. Nothing seemed to line up with my issues. But now I understand I probably have a partial tear. I did fall 2 weeks ago and hurt my shoulder/upper arm trying to break the fall. Thank you SO MUCH!!!
it's so annoying for me. over twenty years I've had an impingement or partial tear. it will. sometimes go years and I can lift and everything no pain. then it comes back and I can't do anything without popping a bunch of ibuprofen. I don't want to do surgery but I know I need to do something to fix it or I'm just going to end up with a full tear eventually.
I DO have a "Cuff Tear," but I suspect there might be an Impingement issue, as well, because, when my arm has been at rest for awhile, every time I try to raise it at, or, above my head, I feel a little, "Snap," that causes a bit of pain. I have no problem going through the Rehab,' because I've been involved in fitness my entire life. It just takes time, Patience, and dedication. Don't give up, guys!
Thank you for the tests. I made a hard tackle in a rugby match in March 2023 and injured my shoulder but still feeling the discomfort stil in December 2023. Seems like an impingement based on the test
Thanks for the thorough description of the Painful Arc test, it confirmed I have an impingement as it was painful exactly in the angles you descirbed, and then went away as I went full straight up. Thank you!!
Thank you! I had a bad fall from a horse few weeks ago where I landed on my shoulder and head from high speed. I did get an rtg and nothing was broken, but even weeks later it's still really painful to use it. Though I don't think I have any tear(or I had and it's been healing past few weeks on it's on. It was really painful and limited for first few weeks), this helped me understand the anatomy of my shoulder and roughly where my issues are. Now I just have to figure out physiotherapy that will help me improve motion and ability to carry weight with the arm. 1st test was ok(bit more painful than it should be but managed to do it ok). 2nd test it was more painful but still managed to do it. 3rd test was a fail as I can barely get my hand behind my back, not to even talk about lifting it behind. I really hope I can recover my arm to it's old functionality. I can't really lift anything with my left arm, It's hard and painful to get on and off a horse as my arm is forced to make painful motion. I can't sleep on my back or my left side, and I have to use my knee trick to get up from bed(I broke a bone on my left shoulder once before so I learned this right hand to knee and abs trick). I'm a person with active life style and physical work so I truly wish I can recover my arm.
Almost all of these tests hurt my upper arm! My injury is 7 months old from stretching the arms back overhead but now I'm unable to cross my left arm across the torso, both at the back and front. But now stretching the arm straight forward or up, even full shoulder rotations in yoga don't hurt at all! It's just the crossover.
Thank you, I have a shoulder impingement. Pain in every test. Sucks , but it great to know what has been going on. Injured in since high school. The hit made me quit FB, lol. There's your sign!

Just stumbled on this video and it’s very helpful understanding the involvement of the muscles involved. I’m not sure whether I have a shoulder impingement or slight tear. I will definitely seek some professional help.
Finally i know for sure! 😅 Definitely an impingement! Dang it hurts 😩 but thabks to you, i can find the information i need! Tysm for your thorough video!
Thank you for the information. I know you get paid for what you do as you should so I don’t expect any free advice but I’m in a lot of pain so I’m gonna give it a shot. i’ve been lifting weights for a long time incorrectly flared out elbows during benchpress. I’m in my 40s. For the last couple of months my left arm has had obvious impingement. But I could still easily lift up pick up things all that stuff. Last week I’ve been doing many things to try and help it and I was doing hanging just for about 7 to 10 seconds I didn’t feel anything special. About an hour after that I couldn’t move my arm an inch from my side. That night I couldn’t sleep for one minute and still haven’t for three whole days starting to lose my mind. It hurts all the time it’s worse when I’m laying on my back or laying down at all obviously I wouldn’t lay on it but I thought I could lay on my back but nope feels like somebody’s sticking a Knife in my arm and it radiates all over. But it just won’t stop the pain is constant it’s not throbbing it’s like tooth pain it’s like a nerve pain I took some pain pills I never take stuff like that I was desperate it did nothing. I work as a dog groomer which is very physical believe it or not and to be honest I’m just scared. Could I have torn it doing the hanging I thought that was kind of a benign thing obviously not. I guess I don’t even really have a question if you have any thoughts please give them to me. And due to my many poor choices throughout life I don’t have much money and I have no insurance completely my fault no victim here. People have been telling me I should go to a chiropractor that sounds like a terrible idea to me I can’t imagine somebody pulling on it or pushing on it. Anyway I’m probably not making much sense I’m kind of out of it getting kind of desperate. Thanks man

Great explaining! Thanks. Just confirmed what I was thinking. I have partially torn mine.
Very well explained! Been looking for videos on this subject and yours was, by the far, the most well explained, easy to understand.
nice job, sports medicine doc here. Really good presentation for patients at home. Of course, a lot of these tests suffer from mediocre validity but there is no perfect set of shoulder exams, so given that is what we have to work with, I like your selection here. overall, well done!
I can't lift my arm up to the side either without almost dying
I'm screwed
So torn rotator cuff and impingement?
@@kellybragg2068 you ok now
Thank you! You are very good and your channel is excellent. I have researched for my shoulder pain and seen a lot of other videos. You are the best, the clearest explanations and tests and therapy instructions. Thanks very much. I really appreciate you posting all your videos and will share your channel with others.
Thank you
Very well explained, thank you. If you could put the links to the videos about the exercises in the description, that would help many people. Thanks.
Hey Dr. Saman, I want to thank you for this awesome video. I'am not able to afford to go and see a doctor. Now I have a better understanding of what I could be dialing with in my shoulder pain. I will be subscribing to your Channel. Thank you again.
That video helped me to finally know I have a shoulder impingement
This is a fantastic job!! You take the mystery out of the shoulders! Amazing!
Best of luck.
Hi, I have a question, did the doctor tell you it will heal by itself? I have definitely tear in RC due to calistenics workout
.
it's my understanding that a full tear can not heal without surgical reattachment, but a partial tear can heal on its own.... sometimes.
I had surgery on my tear and now 3 years later I have an impingement in the same spot. good luck. ill just deal with it now
This pretty much confirmed what I already thought I was dealing with, but clarified really well the differences between partial tear and full tear. Thanks for the info sir!
same. i sleep until it gets painful and im up until i can sleep again
Any update?
Very helpful video. After watching the whole video I realize I have a shoulder impingement
Eleanor always seems to walk the line between comedy and drama with such subtlety-she is uniquely talented and beautiful.
Great, informative video. The issues are so well-explained. Thank you.
Looking forward to seeing the rehab videos now...
Taking a screenshot. Will try these in my next session.
Thx!
@@Master_Chief_532 hope it helps
@@alexarmstrong1134 all the best
i can agree here…..i’ve been actively thinking about fixing my impingement and i think it’s working…i started by fixing my rounded posture because i figured my shoulders being rounded could very well be causing the impingement in the first place i’m also doing rotator cuff work evry time i hit the gym…also dead hangs to relieve some shoulder pressure…gotta change workouts around too i stopped workouts that bother my shoulder and i couldn’t skip chest so i changed my bench position and arm positions to where my shoulders don’t grind…HOPE THIS HELPS 🦾
Amazing video. Best I’ve seen on the shoulder. I have a recurring impingement, can anyone recommend some treatment or exercises to stop it It’s the bane of my life.
Thanks for great video !
Thank you Doctor for the video.
Best thing about the presentation was its simplicity. Keep up the good work.
Regards.
Thank you so much!! Thankfully I have a basis to go off of and let my doctor know what might possibly wrong with my shoulder
Huge help!! Impingement for me for sure. I was so worried it was rotator cuff tear. Now I'm going to watch the next video for exercise tips to repair.
That was a better explanation than my local doctor. Now, I know that I have an impingement, not a tear.
CGP Grey has a good video on A1-A4 paper! Here on YT
@@mangos2888 I have contacted Mr. Schrute from Dunder Mifflin Co, Inc and all my paper enquiries have been answered.
This is exactly what I've been asking myself.
Thank u
Any update? How it went?
You are really a competent therapist.I have been facing this issue for last nine months. No doctor was able to locate the problem. Please advise me which doctor should I go to a physio therapist or an orthopedic doctor or to a neurologist
I started having shoulder pains a few weeks back, I suspected it was over use but despite taking it easy, the pain didn't go away completely. Dr Google said it was a tear but your video now suggests an impingement. One of the symptoms I get is a clicking sound as I rotate in various angles along with a sudden sharp pain. I now need to look up some therapy.
Thank you for the video and the simple tests.
im also getting that clicking sensation when i lift my arms up
Update? Cured?
Saw a physio and thought it was impingement. Gave me exercises which has helped it a lot. I have almost all movement, but sleeping on the right side will bring on sever pain. He advised me that these sort of injuries can take up to a year to heal.
just fantastic instruction on how to assess - best guidance I have seen -
thankyou
Did the rotator cuff test. Past through. Hurt my shoulder but glad it might not be torn
absolutely enlightening. According to you description I´ve got an inpinchment but can´t better even though it has been more than a year since then....
it's so annoying for me. over twenty years I've had an impingement or partial tear.
it will. sometimes go years and I can lift and everything no pain.
then it comes back and I can't do anything without popping a bunch of ibuprofen.
I don't want to do surgery but I know I need to do something to fix it or I'm just going to end up with a full tear eventually.
Thank you, very simple and easy to define the problem.
I DO have a "Cuff Tear," but I suspect there might be an Impingement
issue, as well, because, when my arm has been at rest for awhile,
every time I try to raise it at, or, above my head, I feel a little, "Snap,"
that causes a bit of pain. I have no problem going through the Rehab,'
because I've been involved in fitness my entire life. It just takes time,
Patience, and dedication.
Don't give up, guys!
It really does suck though. Injured my left in highschool now my right is faulty. Shoulders are sensitive I guess
Thank you for the tests. I made a hard tackle in a rugby match in March 2023 and injured my shoulder but still feeling the discomfort stil in December 2023. Seems like an impingement based on the test
Im happy,,,i learned from shoulder prob ,,,,now i know i have impeachemnt,,,,Im subs now,,,loving from Philippines
Just had a MRI which showed partial infraspinatus tear and AC tendinosis and LOTS of pain
Thank you, I have a shoulder impingement. Pain in every test. Sucks , but it great to know what has been going on. Injured in since high school. The hit made me quit FB, lol. There's your sign!
FANTASTIC VIDEO! Thank you1 gonna go see what exercises you recommend for shoulder impingement now. ;) You did a great job here - appreciaste all the explanation and the use of skeleton and animation. VERY helpful.
Thanks for this well presented video!! I am scheduled for surgery.
Thank you. I understand more about my shoulder pain.
Will wearing a shoulder brace help a partial tear? PS...Doctor, you did an amazing job explaining this type of injury and testing for it. Thank you!
Incredible video, easy to follow and very helpful.
The video was easy to understand and extremely helpful
Just stumbled on this video and it’s very helpful understanding the involvement of the muscles involved. I’m not sure whether I have a shoulder impingement or slight tear. I will definitely seek some professional help.
Can’t even put arm behind my back, pain so bad! Waiting to see consultant for diagnosis
This dr is fantastic
Thanks man
Go to the doctor.. you have a torn rotator cuff
I can help with that please send me your credit card information for payment
You better see a sports medicine rehab doctor - not a chiropractor
Between 20yrs of working as a navy diver and all the lifting we had to do on our jobs, both shoulders have been diagnosed with multiple injury sites. I simply stretch, do light lifting ( SUSPENSION TRAINER ) and a daily dose of pain killers. Beyond that I just live with it.
Most informative video I've found. Thank you. Question: what makes your arm pop in and out of socket?
Thank you for the video, I have an impingement from the suprapinatas, don't know if I spelled it right 🤔 anywho I was pitching a bucket of 🥎 to my 9 year old granddaughter, she's in a pony league team and she can hit the ball big time 👏🏼👍🏼 I'm so proud of her ❤ but the next day my goodness the pain was unbearable. This occured Spring break week and I'm starting to get movement 😞 before I couldn't hold a toothbrush, comb my hair.... Thankfully I don't have to go to the VA hospital and wait all day. Bought some capsicum patches and it has helped along with gentle exercises 👍🏼👏🏼
Thank for excellent explanation of the problem. Yeas is no tear is impingement.
Thanks a lot I'm sure now that I have shoulder impingement ❤❤
Thankyou Sir very well explained.God Bless you
