    Wow, a doctor who really knows his stuff! Thank you! I have yet (after 10 years) to find a doctor give me this information. I'm 71 now and was told to get a replacement 8 years ago. I refused, changed my diet, lost 25 lbs., and stretch, strengthen on the floor everyday and swim 4 times a week. I'm now 90% better than I was. I do still have bad days, and Tylenol pretty much does the trick. So glad to learn about PRP and Visco injections just in case I can benefit from them even more in the future. Thanks again!

    THANK YOU DOCTOR!. This is the BEST common sense advise video i've seen. After 5 years of not seeing my doctor, I went to see him about my VERY ANGRY LEFT HIP.. He immediately said that I would need HIP REPLACEMENT (wtf?). My HIP problem was what I believe caused by me working from home sitting in my uncomfortable chair in front of my monitor for 40-65 hrs per week for the past 4 years. Little by little, after a year of sitting for the 40--65 hrs, it started to be painful to get out of my chair and eventually my walking, bending, sleeping, getting into my car, driving and everything got very painful to do. But I will beat this tho. I WILL FOLLOW YOUR ADVISE and BEAT IT. NO HIP REPLACEMENT FOR ME. THANK YOU SO MUCH.

    This has to be the most enlightening video yet on options for hip osteoarthritis!
    About 8 mos ago I started with this pain. I'll just suck it up & it'll go away...not. Long story short I was sent to a surgeon; with 2 options: steroid shot or hip replacement!
    Not in my play book!
    It's good to know of the visco supp. & the PRP injection!
    Now I have the direction I knew was out there, but didn't know what it would be!!!

    Reading through the comments I had no idea how many people suffer with hip pain. I have the most excruciating pain in my right hip, groin, front of leg. But Xray shows severe osteoarthritis with bones grinding in left hip, not the right. Very disabling and depressing condition but I see I'm not alone in this.....

      How amazing that the side you experience the more severe pain, is not the side indicated in your x-rays. What I am learning from tremendous online sources like this one is that the nervous system, muscles, and tendons are what MOVE our bones. Muscle pain is the result in muscle atrophy but also muscle imbalance (tight muscles taking up the slack forweak muscles. Our pelvic tilts, rotation etc, are root causes. I have a condition called NICO that began in my jaw and spread through the neuropathways to my hips. The body is a magnificent and often misunderstood system of inter-relatedness. The majority of medical professionals are trained to look at parts and not the whole...

      Our bodies are amazing structures that we really do not understand, not even our doctors know all that there is to know.
      Since my original post, I now have severe pain in my arms, wrists and hands.... I have never in my life suffered the pain that I have gone through in the last 4 months. To make it worse, I'm unable to sleep with the pain. The new diagnosis for the top part of my body is Poly Myalgia but blood test need to confirm this, I'm having them today.
      I can't rely on doctors any longer and like you need to do my own research. I think much of my pain is stress related. And I wouldn't be surprised if 5g is another contributing factor. Our bodies aren't designed to be exposed to emf at the high levels and there must be a consequence to it...
      I wish you well and hope we're able to find a renewed healthier body soon 💚

      Same left hip is bone on bone I can hear popping sound every move I make.. Osteoarthritis has been determined and telling me only option 4 me is hip replacement which is scary because I also have MS.. I feel so defeated the pain is excruciating & debilitating

      @tinarossetti3939 I've just now been sent this reply that you wrote a month ago. I do hope you are on the road to recovery. Recently, I was advised to have a tooth extracted to facilitate healing in my jaw. I was apprehensive because I'd had this problem before an impacted wisdom tooth, which moved to the next tooth. I reluctantly agreed. The extraction could have been dangerous, but all went well... until days later, when the pain & ear ache returned. Long story a bit less long I finally discovered (thanks to a nurse cousin who armed me with medical terms to insist be investigated) that the problem is the nerve in my jaw which was damaged from over 20 years of dentist induced injuries, has spread from my jaw throughout the neurological pathways to other parts of my body. It's a mess & right now, I am on a three month "green" antibiotic course to effect a healing.
      I pray not to allow myself to wallow in anger toward the plethora of dentists who caused the damage and those who dismissed what their colleagues had done who blew me off telling me that my pain was all in my head.

    I was in bad shape about three weeks ago….limping around…thinking..this is it? Until surgery?? I felt like I aged so much so quick. I knew it was hip related. I was using pain relievers..crème..patches..even quit my sales job..too much walking. I finally did some excersizes…nothing that extreme…moved my ball joint in different directions..stretching…I cannot believe how much better I feel!! No more sharp pain w every other step. My leg does feel sore but not achey like it was. Thank you for educating us.

      I can’t blame my doctor..the first thing he did was print out several pages of exercises..which I didn’t do. And schedule physical therapy but took forever to get in!

    Best hip treatment ? PST (pulsed signal therapy) it saved my hip. I was diagnosed with Ostheoarthritis level 3. They wanted to put me on surgery. No way. After research I found PST and nowadays I play bball 3 x week, jump rope 6 x week and strength training.

    Playing hockey my entire life, got diagnosed with hip arthritis over a year ago, and I’ve been told I need a replacement so I’m doing my best to avoid it!!

    Excellent video! While an MRI exam established a degradation of the cartilage in the hip joint, there is still some cartilage left. So I am far from hip replacement. Pressing on my hip joins does not hurt and neither does weight lifting. But I often feel pain while citing or driving the car. I do get periodic and persistent pains at different parts of my inner thigh and pelvis. The focal spot of my pain migrates once every couple of months to a different spot along my inner thigh and groin. My biggest problem is mild but persistent pain at night that wakes me up. I also have some buzzing sensation in my groin that creates a desire to frequently urinate. In the past I had intestinal surgeries such as hernia repair in my groin. So perhaps some scarring tissue impacts my nerves. Many of my symptoms seem to stem from a nerve compression, but perhaps I do have arthritis. I wonder how to diagnose and treat the root of my problems (i.e. arthritis, or compressed nerves).

    I had the cortisone injection and now am regretting it. My doctor never told me about any of these other options like PRP.

      Whenever a PA or doctor tells me what they plan to do going forward, I always investigate the pros & cons on my own, 1st.
      I am 67, have Osteopenia & certainly don't want to make it worse. I just recently found I have arthritis in my hip. PA just does what my Medicare Disadvantage plan wants him to do.
      When you have to rely on western medicine only & cheap insurance, you don't get the proper care you really need.
      Insurance companies like to delay and deny. It just makes the physical pain you have worse. So much stress.
      Our healthcare in the US stinks.

    I had PRP 3 years ago and it did not work in the longterm. Plus it requires a yearly booster after your first 3 treatments. Its also very expensive and NOT covered by insurance. I swim and do PT but still in pain. Its really hard to walk, run or do even bike riding when you are in pain. I cannot imagine it is beneficial to walk when you have bone on bone hip arthritis.
    Question: Are Hyaluronic Acid Injections effective when you have bone on bone hip arthritis?

    Great info.
    I really have to watch my hips, and the left one particular.
    I've done too much heavy Squats, Bulgarian Split Squats and Stiff Legged Deadlifts in the past.
    Heavy weights might be cool to build some strength.
    But the joints aren't that keen on dealing with heavy weights on a continuous basis.
    I'm now looking for safer ways to keep the leg muscles relatively strong , functional.
    Like static squats, bodyweight only. Like the Sumo Wrestler do.

    I notice now that a high wall sit, and a sumo squat static hold, is the way to go for me, regarding the quadriceps .
    Those one legged exercises, even with bodyweight only gives me hip pain.
    Especially in the left hip.
    I got to play it smart= safe . Working the muscles without aggravating the joint.

      Yes, in general, lighter weights more reps are easier on the joints but still effective in maintaining / building muscle strength

      I also read about it.
      That even 30% of a 1 rep max can build quite some muscles mass.
      Without the joints being smooth, life gets less pleaserable for sure.
      If I knew what I know now.
      But this internet wasn't that much a thing in 2000 and before.

      I'm in exactly same boat, your suggestions are precious, but first I have to cure edema, ie bleeding and swelling in my right hip before I can put even body weight load on that right hip, that is what the doctor says

      Did you have an accident?
      The one legged exercises I still do. Bodyweight only. No weights.
      The wall sit gave me too much pressure on the knee joints/caps.
      You have to adjust and readjust until you find what works for you.
      And that might be only for the time being. It might be, that in the future you have to change exercises and/or weights again.
      This I found out.

    I have focal chondral loss in the left hip. At 51 years of age. I am going for a consultation for mesynchymal Chondracyte injections. I will let you know if my cartilage replacement therapy is successful.

    Thank you for the information I have cerebral palsy.. I've had corrective surgery when I was 11 and 19 and I'm 54 now I just got diagnosed with traumatic arthritis. Osto arteritis. I'm in incredible amount of pain like on an 8.. I can't sleep it hurts so bad. My orthopedic doc they want to refer me to the hospital in Portland for surgery it's about 5 hours away from me. I really don't want surgery at all so I'm hoping when other referrals go to Portland I hope when I get the consultation they will have other options other than therapy cuz that's a lengthy surgery out and in so I definitely don't want to have surgery and they also recommended me to go to PT.. I did that PT cannot help this problem either they tried. Any suggestions please for this other than surgery??? This is extremely overwhelmed at CP pain on top of traumatic arthritis!

    is it ok to moderately train legs with seated leg curl and seated leg extensions machines to build lower body muscle? or should leg workout be avoided at all costs? I’d like to get leg muscle but i have hip arthritis,is it possible to do so without making arthritis worsen over time?

    Thank you Dr Peng for posting this informative video I have one concern as recommended to stay active but in my case suffering from osteoarthritis on my hip and knee after activity I get pain on my hip which after resting gets quite better so does that mean activity makes my hip arthritis worse, if u can comment on this.

    Thank you for this valuable information. I have wanted prp injections for my SI joints. The current hospital I am seeing does not use prp in that location. I have a bad hip replacement making my right leg longer and it has caused stress and bone on bone on outer part of my left hip. I feel that I need my SI joints need to be repaired no matter what is done to my hips. Where can I get treatment for my specific needs?

    Thank you very much My Dr in Germany got really anti when i refused a OP . Running is out of the qeustion even walking 2 kilomters is painful cycling is ok.Abo done.

    Great info thank you. I was just diagnosed with severe osteoarthritis bone on-bone stage in my hip would you still recommend The PRP injections? I am about to start physio and would like to learn as much as I can to make the most informed decision.

      Unfortunately once you are bone on bone, there is little we can do. PT and injections are only palliative. They can help delay a surgery but eventually it may still be needed. Good luck!

      Oh dear, I know how you feel as I have been diagnosed with the same just three weeks ago. The pain is excruciating, especially in the night. This has come on suddenly and I feel completely disabled. What a miserable existence. I'm hearing that the hip replacement is a way forward. I'm such a believer in self healing but I don't think I'm able to do this one.

      @@tinarossetti3939 I have been controlling my diet eating food that is not processed forcing my self to exercise daily. I eliminated all painkillers as advil nearly killed me. My heart started doing funny things and had a really bad case of angina. Thought I was dying. It's been 3 months since my diagnosis about 5 years of pain but I do feel better I lost 50 lbs and quiet smoking and drinking. My doctor said I can come of the heart meds in 2 months and was suggesting that I take an antidepressant for my pain and depression. I opted to take psilocybin mushrooms instead on a personnel recommendation of .5 to 1 gram every 3-5 days before bed. It's given me up to 70-80% pain-free days 🤗 and my mood has improved. I hopefully will be talking to a surgeon in the next 12 weeks and surgery is expected to be within the next 2 years so pain management has become my number 1 focus

      Oh my goodness poor you! I got my xray results back three weeks ago. Severe Osteoarthritis in hip and lower spine. Night time is awful day time more bearable. I've a three month wait to see orthopaedic surgeon then who knows what treatment I'll be offered. It's great you've lost so much weight and are able to exercise. The osteoarthritis is showing up on the left hip but it's my right hip, groin and leg that has the excruciating pain. I'm scared to attempt stretching exercises as I might cause more damage. I can't bear to think of going through life with this as I can't make plans as one day might be bearable and another I stay in bed with hot water bottle. I'm on co-codamil I've been given naproxen too but suffer with Atrial fibrillation so haven't taken any as yet. It's all fun and games from here on :( now all the things I should have done and didn't, I want to do now, typical.....

    ( M.R.I) Calcification of Tendon on HIP. Pain starts Only when Sleeping. Tried Tablets, Cushions, Electro Therapy. OK Sitting Up,And Walking. Pain Goes Into Groin. Not Told Why ! Thank You From England.

    I'm 67fit and not overweight I have had an active life working on a farm but early this year I started with pain in my leg and thought it was my sacrolliate joint so went to a physiotherapist and he couldn't find a problem then I had an x-ray and they say I have no cartilage around my hip joint so need a hip operation 🤔...is there anything I can do..I'm in pain when I walk now and can't take my dogs for walks??

    one day i end up football trained in beach in 2021, after 2 weeks later I felt pain in my groin area which is annoying me while playing football or sitting in bus and change my side in bed, I went to a doctor with x-ray but he said nothing shown there so he gave me a pain killer and muscle flex capsule and cream + 1 month rest after that I recovered 80 percent almost and by the time everything was ok, in 2023 last month I did trained again in beach though everything is alright , after 1 month of training in beach I felt the same pain again while playing a football game in grass ground , so I decided to take again the described drugs and muscle cream + rest so in this case what should I do eliminate this injury forever in your opinion ?

      I have many patients with bone on bone who still get 5000+ steps per day. They key is to control the environment of the joint first with injections. Then work on strengthening exercises to build up surrounding muscles. This takes many months but is very effective.

      My orthopedic doctor claims stem cell therapy for hip is a patient rip off, it doesn't work and he doesn't recommend it.
      Dr Cheng probably doesn't recommend it for the same reasons

    I get frustrated at advice like loose weight, exercise, and eat well. My BMI is great, I exercise regularly, I eat pretty dang good. I still have OA. I’m still loosing ROM. I know this is good advice for most people but it leaves me feeling confused. I’m doing everything right yet here I am. I am considering PRP. I’d love a video that is made to give advice to healthy athletes who are suffering from arthritis. A friend of mine (personal trainer) is going through the same thing but all videos seem to talk about like it’s not something that happens to active people when the advice is “get active”. I know that’s not the intent but wanted to put it out there. I guess that just leaves options like PRP for people like us?

      High level athletes (ie. pros, olympic levels, etc) have VERY high rates of osteoarthritis. And that's because of the wear and tear and "pushing through pain" that is causing more damage. PRP is an excellent option for high level athletes that want to continue to stay active. A rehab program is just as important to make sure you body can withstand the loads you are putting through it.     PRP Is not paid for by insurance so it takes it off the list for many due to cost, most insurance companies consider it experimental unfortunately

I am a soon to be 72 year old ( birthday next Monday) recently diagnosed with OA after right hip locked up after sleeping in a chair for 6 nights next to husbands hospital bed. Otherwise healthy. Walk 4 miles a day, attend yoga classes twice a week, on zero meds till the OA diagnosis and now was told meloxicam for 30 days then steroid injection inti right hip next week. I wish I had a doctor line you here in Texas! I am sure there are some and thinking of trying to find another doctor that understands a less invasive approach. I am one week into the melixicam (could walk after 3 days on the med w/o hip locking up). Soooo I want to skip the injestion for now and try just meloxicam, appropriate exercise, drinking lots of water and seeing if this works for me. I have Medicare ( which seems to rule the roost and a plan g supplement that pays everything Medicare does not pay. Do you think my plan is reasonable and I should search for another doctor here in Texas that favors a less invasive approach at this point in time. Maybe even laying iff the walking and finding another firm if exercise may help. Please respond with your thoughts as I am ready to cancel my injection scheduled for next week.

    I mean, thanks for the basic common sense advice. But it's real, easy to have a smile on your face. Talking about these treatments when you don't actually have arthritis. I have chronic Arthritis in my right hip. And sometimes it hurts so bad I wanna die. I have no way out. I have no other way. I can make money other than the job that I do. And for me to look for another job, I'd have to quit working and not have money coming in. So I don't know what I'm gonna do. Thank god he told me about tylenol. Lol

    Hi, I'm new to all this. 2 weeks ago, I was walking up stairs and there was a grind/grab/awkward sensation at the front of my hip. It slowly progressed to burning stiffness upon standing and almost unbearable nerve irritation/pain sensation when sitting. This is when I turned to watching videos on YT while awaiting an xray.
    I started doing some of the recommended stretches but it has made things 10 times worse. I now cannot drive my car or sit at all without feeling agonising burning/irritation which only decreases after I lay down for hours. I realise now that I may have overdone the stretching/strengthening exercises in the first week post injury but there was no great fall/accident to indicate crippling injury was on its way. Xray says I have hip degeneration/narrowing and likely OA.
    When you google acute hip injury or how to treat bone on bone acute hip injury/flare etc. no one is talking about an acute situation on YT. It appears to be just long-term suffering and potential solutions/treatment plans.
    I would imagine I'm not the only one out there who is in this window (just injured in last few weeks etc.) of injury with subsequent imaging indicating arthritic changes, so could you please provide some assistance as to how best to treat an acute injury like mine. Also, when and how to know when it is time to transition into an arthritis/FAI treatment program?
    Treatment summary:
    Mistake No.1 - Stretching/ROM exercises
    Mistake No.2 - Warm epsom salt baths (felt so good initially and I could do full ROM afterwards but then a world of pain and stiffness.
    Mistake No.3 - Driving/sitting in spite of increasing pain/irritation (this was a HUGE mistake!)
    Massaging - short term relief.
    NSAI's and lying around in bed all day - So far this is the only thing that is keeping me sane.

    I think more people would be interested in PRP injections if insurance paid for it; or the cost was not so high. Unless you have the money to pay for it yourself; most patients are left with options like steroid injections even though they might choose something else if they didn't have to worry about money. I very much agree with multi-modalities, but for many it is not always possible.

      Thank you forasking about the shots. I will check that video as well. My left knee had 3 tears in the meniscus, and just a hint of arthritis. We went for cortisone shots. First was great results. 2nd not so good but it could be my Achilles ruptured.3rd shot was awful. My insurance did approve HA Gel injections, so we did the series of three weekly in December. My follow up is next week and my ortho doctor stressed it is important because the insurance companies are trying to assess the cost/efficacy ratios. So maybe there will be more of these alternatives available sooner!

    I was very fit and flexible, then I developed a slight limp in my right leg. Have had back issues B4, but fixed that with exercises. Went to physio, he suggested exercises and use a cane. Worse thing I ever did, because I have became reliant on aides. Now I use a walker😭 Specialist said I have tight hips, how did this happen, when I was very active? I do exercises with a stretch band, and was doing hydro therapy, but the pool was dangerous to get in and out😢 Please help me. I'm 68, and was active up to 3 years ago😢My legs are weak, and I can't walk on my own😢 I've read by being flexible, doing splits, touching toes ect can cause tight hips, I've done all this, all of my life, from a very young age. I even did ti Bo .

      Hi Abbey, I feel for you. For nearly 2 years I've been dealing with a series of issues with my left leg due to long COVID. Some reports showed last fall that it will attack the musculoskeletal system, which confirmed my own determination.
      After my right leg having to pick up a lot of the weight bearing function I started having pain in my right hip about 4 months ago. An xray showed absolutely no arthritis in my left hip, but my right is bone-on-bone and shot. The left leg had so many issues my orthopedic surgeon thought it best to finish treatments there before doing anything with the hip.
      That was about 3 months ago now, and the pain has gotten steadily worse. Until about a week or so ago. I've found a few PT channels and started looking for exercises and treatments to avoid hip surgery.
      There are several. I've got several I've tried, and found one so far that was specifically helpful. The hip joint is one thing. I also have deep pain in the front of my thigh just below the groin, and pain in the rear and up into my buttock. My right knee is starting to be unstable a d painful as well.
      Besides finding exercises can stand and that have descriptions that indicate they should be helpful, I got a Recommendation to try TheraWorks, a rub in foam. I also bought a massage gun, which I use on the hip pointer, along my thigh muscles and around my knee. One good trick is to use the gun, rub in the TheraWorks then kick back in my recliner with my heating pad covering as much of the affected areas as possible.
      Stairs are still mostly off my schedule. Other than a bit of a setback today it's showing some promise. Just keep looking for things that will work! Saying a prayer for you!

Sitting upright is my worst. Thankfully laying down and resting in my recliner I’m good. My walks are short but I do them most days. I’ll use two canes on my walks/hikes but only one walking to stores. Both my hips are bad and before I decide on hip surgery, I’ll Need to increase my rom as well. Thinking about applying for SSDI to focus on my rom, flexibility and strength. Definitely focus on the glutes to balance the thigh/quads. All the best to you…

      Best you discuss with your healthcare provider about your individual situation. In some cases, we use injections (ie. PRP, hyaluronic acid, etc) to help calm down the arthritis symptoms to allow patients to exercise more.

      I'm with you! I don't understand why so many YT physical therapists and drs like this keep pushing to exercise when it hurts like hell.
      While I did manage to find away to exercise, it's VERY limited and I miss being able just to walk an hour with no pain.
      I sympathize with you.
      If we need a hip replacement we should be able to get it. There is only so much you can do within your own insurance or budget. Our country's healthcare stinks!

      I used to jog a lot t, now my both knee are bad.. still doing bike that help me to walk...now I'm getting hip pain, I'm 63 now. I started running at 27 years old

    I broke my hip at 16 now at 18 I have arthritis out the wazoo

    what to do with doctors that work hard to make sure you loose your hip so they can make money operating you?
