How Doctors Can Predict Who Dies From COVID

  • How doctors can predict which patients are more likely to die of COVID
    Several studies have come out about the COVID prognosis. As a result, we now have a powerful way to predict which patients with COVID pneumonia are more likely to die and require longer stays in the hospital.
    We already know that less than 15% of people who get COVID have a severe disease to the point of requiring hospitalization. Less than 5% of people who get covid require ICU. For those with COVID pneumonia who require ICU, and for some of the other patients in the hospital who might not necessarily need ICU, these are the patients who have the cytokine storm that we keep hearing about, meaning the immune system is going haywire. There is a ton of inflammation in the body….especially the lungs. But now, we have a more specific way of predicting who is more likely to develop cytokine storm, and therefore more likely to die, and who is more likely to require a more extended stay in the hospital….and it's based on the bloodwork we get when patients are hospitalized.

    When we get bloodwork, we do various lab tests. It is most commonly a complete blood count, CBC, and a complete metabolic panel or CMP. We often check other things like troponin, d-dimer, LDH, ferritin, and CRP. A complete blood count is specifically looking at 3 things: hemoglobin levels, platelets, and white blood cells. We can use the total number and the breakdown of the percentages of the types of white blood cells, meaning what percentage are neutrophils, monocytes, lymphocytes, eosinophils, and basophils. Lymphocytes generally make up about 15-45% of WBC. This is very important as it pertains to COVID, and you'll see why in a little bit.
    The CMP stands for the complete metabolic pan looking at sodium, potassium, chloride, bicarbonate, blood urea nitrogen, creatinine, glucose calcium, bilirubin, albumin, and liver enzymes like AST ALT alkaline phosphatase. Some of these also have important implications for COVID disease severity. Because In the recent study done at Temple University, they analyzed over 500 hospitalized COVID patients, all of which had inflammation in their lungs seen on a CT scan of the chest (show). They determined the criteria for COVID cytokine storm. For patients who met these criteria, their length of stay in the hospital was 15 days, compared to 6 days if they did not meet the criteria.
    Mortality was 28.8% in the group that met the criteria, vs. 6.6% in those who did not. So if they met cytokine storm criteria, their likelihood of dying was 4 times higher. And how accurate was this prediction model? Pretty accurate, as this study had a specificity of 79% and a sensitivity of 85%, which is a lot of predictive power. Most of the patients who met the criteria for CS did so at the time of admission or shortly after. This suggests an early and rapid progression in these patients and that there is a low likelihood of developing cytokine storms after 10 days of hospitalization.
    So based on these specific lab parameters, this is how doctors can predict which patients are more likely to die of COVID. I find that this study correlates with what I've seen in my personal experience with hospitalized covid patients. And yes, the man whose CT scan I showed you earlier in this video met all of these criteria for cytokine storm, and he did so as soon as 24 hours of being admitted to the hospital.

      So what happens if you are on immune suppression when you contract COVID? Does it help or because you have your autoimmunity as a cormorbility is it considered a hinderance, or is there no science on it yet?

      Well presented. You didn't mention what effect if any we're seeing with vaccinated patients that still fit the criteria for a cytokine storm. Are they less likely to experience one? Or are the storms still developing but having higher survival rates?

    My dad’s friend passed away yesterday. She was 42 and had just adopted a child after years of trying...and now the child is orphan for a second time. He doesn’t have anyone to look after him and he is just 6. I am devasted. Please keep him in your prayers. The agencies here will not let us intervene.

      This story is just so sad in every way possible, 42 years old and passed away, so young and to pass away after after she finally was able to be a mama. Breaks my heart, keeping you, your dad, his friends family, and the little young boy who just wants to be loved

    I had severe COVID disease with ARDS and bilateral pneumonia. I was intubated for 3 weeks and in the hospital for 5 weeks.
    Thank you for these videos. I know I was close to losing my life. I am going to check my initial blood labs to check with this information.

    My dad passed away within a week of being diagnosed with Covid. He developed ARDS and we were asked to make a decision for a DNR because he was just so weak and wasn’t going to make it. When we approved the DNR, about two hours later we got the call that his heart had stopped. We couldn’t say goodbye. We couldn’t be there for him. This was the worst experience of my life.

      I'm so sorry for the loss of your Dad. My Dad died alone after dinner in a nursing home one week after I told him I was scared about him getting Covid. My heart is broken so I know how alone you must feel. I told Dad about what Heaven would be like from the Bible. He chuckled when he heard about the streets of gold...Who knows if it's true but I know that he's in no pain now and that gives me peace.

      I had to sent an audio message thru WhatsApp saying goodbye to my mom. My cousin works at the hospital so she was able to deliver it to my mom. The hardest thing i had done in my life.

    My father contracted COVID and was in the hospital for a total of four months. At the beginning his doctor said he wasn’t going to make it so there was no point in doing a tracheostomy. We had to fight with them to do the surgery - now he’s home with us and recovering well.
    Always get a second opinion people, or even a third if you think you need it. It’s not over till it’s over

      How old is your father ? Did he have any other health issues ? Mine is in icu on ventilator. He had a stroke and was not moving . He also had bad lungs cus of not moving. Other than that he was healthy . Super good diet . No blood pressure , a blood sugar at 86 and strong heart. They tell us the prognosis is not good. He is unvaccinated and 77 yo.

      @@PADELATOR I'm so sorry to hear about your father. My dad is 67, he's diabetic and has fatty liver disease (which we didn't know haha.) He got the latter condition with age, he doesn't drink. Prior he was in overall great health and was/is very athletic. Yeah lying in bed for so long can make the lungs stiff - the same thing happened to my dad. Do you know what his vent settings are? Did they also perform a tracheostomy? I'll say a prayer for you and your family mate. Hope he pulls through - give him some time

      I'm sure it was not easy, but unless you had a serious comorbidity your chances of surving with the right care was similar to surving the flu. I hope they did not scare you into thinking that it is common to die from COVID, it's really not.

      @@burkeyatm The COVID survival rate is incredibly high, the vast majority of people survive COVID. You only need to be scared of COVID if you have a comorbidity, are over 70, or your doctor denies treatment. And, actually there are plenty of peple over 70 that survive COVID with therapuetics. Is most of the world scared they are going to die from the flu? no...but, if you are older or have certain conditions the flu is actually very dangerous and can kill someone. However the vast majority of people survive the flu.

      @@vade137 My pick ‘‘twas that this is an order of magnitude more deadly than the flu. 1% is ten times more deadly than 0.1%. Even in the worst year for flu over the past decade, flu killed around 50,000 Americans. Typically it’s half of that at best, Covid makes that look like a game.

    My dad was in the hospital for over a month. He developed a blood clot in his heart which dissolved due to the heprin drip. Doctors say its a miracle he survived considering he's a diabetic. He signed to have his story put in a medical journal.

      Heparin does not “dissolve clot(s)”. I prevents platelet aggregation decreasing the chance that an existing clot will get worse and decreasing the chance of new clots forming, allowing the body to (hopefully) reabsorb a clot.

    I had Covid pretty bad in February. I did go to the ER but never got pneumonia. Folks please add Vitamin C, D and Zinc, especially Zinc to your diet. That is what the Doctor told me. I added Zinc and in just a few days I started feeling better. Not saying it's a cure but, it helps. I forced myself to eat and drink after my daughter told me I had not eaten all day. I suffered from brain fog and didn't even know I wasn't eating. I lost 8 pounds in about a week. I couldn't taste anything or smell. When I tried to eat I could only taste salt. I am well now and very thankful.

      Is he talking about the levels for ALT/AST before getting sick with Covid as a risk factor or after getting Covid? Along with all of that other blood work??? I was looking at my blood work from last year and my liver enzymes were elevated a bit. I need to get new bloodwork done and a checkup....unfortunately I don't have a medical background but I do read as much as possible. I don't understand everything though.

      Asmita....glad you made it through cov. and your parents too. Please tell your friends and family that Dr. Vladimir Zelenko and now many other doctors are suggesting people take Vitamin D, Zinc, Vitamin C and Quercetin. But make sure you tell them to check with their doctors first if they're not already taking any of these. If you look up "Dr. Vladimir Zelenko" on the internet, you'll probably be redirected to one of his follower's pages as the government and different internet social medica groups are banning him....silencing people is not a good thing....(It's feeling more and more like China here in the USA). I take my Zinc and Quercetin in the middle of the day together (as suggested by some doctors online). and not with my other vitamins. Also, Ivermectin is becoming more and more used to help people recover from cov. Several hospitals in Houston, Texas are using Ivermectin to help cov. patients recover. It works fabulously. Some doctors still won't prescribe it because the administrators are digging in their heals to approve as they are part of the "Deep State"....or....they're just scared. Well, anyway---glad again that you all made it!

I actually did take vitamin C (2 times a day), Vitamin D (once a week), Zinc 10mg (2 times a day) and a multi vitamin Tab ( one tab a day). This is openly suggested by many doctors/physicians here. I dont think the ones you suggested are available here but hope people from the US will see your comment ❤️

      ​@@asmitamanandhar349.....Hi! It's so nice to touch base with you, Asmita. I think one supplement you might have a hard time getting could be the "Quercetin". But the other ones are so so important too like Vit. C, D and zinc. Can you get Hydroxychloroquine? I heard in some countries, HCQ (it's initials) is easy to get at a pharmacy without a prescription. HCQ and zinc are super important to take in the early stage (first 3-5 days of symptoms and the fifth day is pushing one's luck) of covid. Dr. Vladimir Zelenko explains how to use HCQ and zinc together. After the fifth day, it doesn't help much. The Ivermectin is even better as it can be used even if a person is in the hospital...not sure if it helps with a person who is almost dead though. Maybe Ivermectin was one you thought you couldn't get as well. It's a prescription in America. This is harder for many to get as the bureaucrats are pushing the vaccine and the don't want us to get well with alternatives. It's a money and control issue---I think---my own opinion. Well, keep in touch and I hope you and your family stays well and healthy. Oh, just out of curiosity---what do people in India think about Bill Gates? I heard he pushed vaccines in India and a lot of children either died due to his vaccines or they got very ill. I'd love to know from someone from India about their opinion of Bill Gates there.       As a nurse, He seems like he's fantastic to work with. Some doctors are great, and when they are, I ask questions and have learned a whole lot. Currently not working d/t COVID, being 68 yrs and smoker. (tritrating down with goal to quit) I actually miss taking care of patients 😕 I can't understand why folks don't follow guidelines. I sincerely hope with Biden coming in we can eradicate this horrible deadly disease.

    I was diagnosed with it 3 weeks ago. I had a fever for 8 days straight. I really felt like I was going to die and I was not even in the hospital. I am 43 years old and my white blood cell count was a little bit low, so my immune system is not at its best. Maybe that's why I got a bad case. My lowest oxygen number was 84, but it went up to 95 if I focused on my breathing. I feel like I have a special bond with those Covid survivors.

      @@pierrecurie Go to 1:24. The patient is laying on his back and you can think of this image as if you have sliced through the chest and are looking down from above. The large dark gray area on either side is the anterior part of his lungs. Underneath the two dark gray areas you can see it is a light gray. That is the posterior part of his lungs and should be the same dark gray. Restart the video and you can see as you move further down this chest that pattern continues.
      Now go to 1:53. This is a a reformatted image made by the computer from the raw scan slices that were obtained. In this image it is as though you are looking at the patient who is facing you. His left side is on the your right. His right side is on your left. Once again the darker gray areas on either side are the lungs. The large light gray area between the lungs is the heart. The lighter gray area arching up under his right lung (your left) is his liver. Now go to 1:57. You should be seeing both lungs really well here (the darker gray). Instead you can see that the majority of the lungs on both sides is the lighter gray. Now if you start the video again you are moving toward the back of his body. The further posterior you go the worse this gets.

    My dad and I live in the Central Valley, CA which was hit super hard by covid-19. My dad is 68 years old and got the rona and gave it to me. He fought it at home for 12 days until I dragged him from bed to the hospital. I thought it was just a super bad flu and he would be discharged in a few hours. I was extremely wrong! We have a family friend who is a physician and had him tested immediately and he came out positive. They scrambled to get him a icu bed and fortunately got him one in a few hours 🙏🏼 my dad fought for roughly 4.5 weeks in the hospital connected to a ventilator thankfully he was never intubated 🙏🏼 after roughly 35 days in the hospital he was discharged to in home care where I took care of him for 3 weeks. Checking his blood sugar, oxygen levels, checking his ventilator, checking up on him every 30 minutes throughout the night. I also had to inject blood thinners twice a day. Thank God my dad recovered! It was crazy tough seeing my dad barely able to walk 10-15 steps without collapsing. COVID-19 is real and people should be cautious. My dad is cut from some tough cloth and it bent him but couldn’t not break him 🙏🏼

      My Dad is obese has diabetes smoker heart problems only has 17% kidney function on one kidney the other one doesn't work. Has hypertension. Has a bag to pee in. He got the rona! Tested positive. He's urologist would not see him again until he tests negative. So, I gave him some antibiotics and 500mg the Z-Pak he recovered and tested negative. He eats really unhealthy. We never let him enter the hospital because the will give you the remdesivir it has sianiad in it. Once you take it your organs fail and your lungs fill with fluid so then your oxygen levels drop and that's the excuse to put you in ventilator after that your almost dead.

    Hello Dr. Hansen, I have been a subscriber of this channel for quite some time. I just want to say thank you for your ability to make sense of the chaotic virus. It seems to change and/or mutate as time goes by. So kudos to you and your entire team for doing all of the work that do to help those with Covid. I am sure each of your patients feel they are getting the best of care with you as their doctor. I am a 58 year old woman who has survived a nearly fatal car accident when I was 42. My body was traumatized from head to toe and the doctors were shocked I was still alive. A year later they finally figured out I had become a Type 1 Brittle diabetic. I had symptoms for months but we never thought to look at my bloodwork nor a urinalysis. Finally my liver gave a shout out for help as I became jaundiced and ended up in the ER. The doctors were confused that I was still coherent and not in a diabetic coma. They also asked me why I had not been taking care of my diabetes? I said, what diabetes it doesn’t even run in my family? I truly thought they were in the wrong room. That is when they explained that my numbers showed that I should be in a diabetic coma. Anyway, now I live with it and it’s many complications as it has wreaked havoc since the proper diagnosis due to the trauma of the accident. I stay safe, and do the best I can each day as I watch this pandemic nightmare play out each day. It is like one very long sci-fi movie and it keeps getting worse rather than better. Again, you are one to quiet the storm of fear that I feel for all who are living out this nightmare. Thank you again, your voice and presentations are so very soothing (actually, I am glad you are a doctor, but you would be great as a Voice Over also!!). May you continue to stay strong and healthy, and you are quite generous to make these videos to help us understand what we are dealing with!! Happy Thanksgiving to you and your loved ones!!     I'm one of the lucky ones. I had COVID-19 with pneumonia and respiratory failure over the later part of summer. I was in the hospital for 43 days, including 2 weeks of that time in ICU. They wanted to ventilate me but I refused. I just had a really bad feeling about it and I think for me, I was right. I was on high levels of oxygen but I kept fighting. While I was in ICU, I was given steroids for 10 days. During this time I was also on antibiotics and upon admission, I was given rendesivir. I was completely coherent during the entire time in the hospital and I talked the whole time I was there. I guess I figured that if I kept talking, I didn't have time to give in to having covid-19. Prior to my admission, all I had was a runny nose and well controlled asthma. In fact I went to the ER for an infected bug bite so I was very surprised to learn that I had COVID-19. However, I do have the risk factors for this virus. I'm asthmatic and I have type A blood, but I'm a female.
    When I was finally released, I lost all my strength, including standing and walking. I was very active before all this happened and it's frustrating for me to still be recovering. My doctor has assured me that I'll eventually regain my strength but she's keeping me off work until the end of January. Hopefully by then I'll be well enough to go back to my usual level of activity.
    I've listened to quite a few of your videos and have found them to be very informative, so thank you. You've answered a lot of my questions about this virus that caught me completely off guard. I think one thing I've learned is to not compare myself to other COVID-19 patients. I don't know why I was in the hospital for so long other than the fact I was very critically ill, I guess. So keep up with the videos; you're reaching so many people with important information.

    Wow. Nurse here for quite some time and when I first 'met' you, didn't have patients with Covid-19.
    Now, let me tell you who perished: those not on Vitamin D therapy. Even our 'at risk' patients had mild cases: in each case, they had been on some type of Vitamin and Vitamin D therapy. Don't want to disclose more information but what an experience. I didn't sleep well for a couple weeks.

      So what is the proper of safest amount of Vit D Pill or Supplement yiu should be taking. I just know mine is low. Dr put me on 5,000 mill tab a week, did another trst & now a 50,000 tab per week. Says she diesnt understand why still after doing blood work, its still low. Now something about a low T arm patch.       @@jimroushing7534 It could be because you're on D2 (synthetic source) instead of D3 (natural source). D3 is about 500 times more bioavailable than D2. Also a *daily* therapeutic dose over a number of months may be more effective than a weekly 50,000 IU dose of D2. Depending on how low your D levels are, your beginning theraupeutic D3 dose could start at, for example, 10,000 IU daily for a number of months then be decreased to a maintainance dose of 5000 IU daily depending on how much sunlight is available in your area year round. Oh and supplemental D works best with daily outdoor sunlight exposure (7 to 15 minutes for light-skinned individuals and 45 to 90 minutes for people of color or dark-skinned individuals).

      @@orisatalabi All I know is that she just did the blood workup twice. I then went to a different dr... a Heart Dr. He dud some sort of lung scan because of a little shirtness or breath. He nentiobed seeing a Spot/nodule of about 4 mm but went iver it with 3 other drs immediately. He stated they were 95% sure it wasnt cancer, but I didnt jniw about these other lubg test & they werent done. He then sent me within about an hour to a pulmonilogist, who had me do the breathing test. He explained that I had slight copd 80/20 & put me on Albuterol & Spiriva. I still dont know why this came on all of a sudden or why the other test were nit done. I did have Triple Bypass in 2009. Could I actually have the Covid from that time, & not know it, & the slight copd show up 8 years later. And as far as my heart dr, as stated, that nodule is what got me sent to the pulmonologist the very same day.,if its not cancer, what is it? Coupd it sctually be a piece of scar tissue left over or started from the open heart sugery in 2009. I remember af that time (because it was not a heart attack), afterwards wanting me to take some sort of New Phemonia Shot right after the bypass that I declined. Tgeyvsaid itvwould lower my case of catching phemonia during recovery if the bypass! Perhaps I have a touch if the H1NI & should certaintly have the antibody test included when Im tested fir thus Covid, as Ive not been tested as of yet?

    My brother was 37 years old. A musician, father, and loving brother. He contracted covid-19 on Aug 20th. He was hospitalized on Aug 22nd. ICU since the 23rd. His kidneys failed. He was on ventillator and dialysis. He unfortunately passed away on September 4th 2021. I will forever love you Daniel. God bless you for the love you shared with the world.

You have an amazing vast knowledge of medical terminology. I’m hoping you have someone helping you update all those medical words on your subtitles as I noticed they are highly precise. I know that takes a huge amount of extra effort because every word and scientific terminology will change if an alphabet is spelled wrong. Even like blood type for example. 🩸 I just want to say thank you. I am praying for honest doctors to share straight facts.     I am a 59 yr. old white female. Tested on 11/11 for the virus. Learned on 11/14 test result was positive. Which I already knew because of the symptoms that I was experiencing. I never had a fever, I never had any of the signs we are told to look for. What I did have was a scratchy throat, runny nose and eyes. I did eventually developed a cough. I was uncomfortable but not at all nothing I couldn't handle. I ate very healthy foods, excercised, and did breathing excercises to keep lungs clear. I did have phlegm in my lungs, but I was able to cough out. Here it is 11/21 and as of yesterday my health has pretty much return to normal. I remained calm, and proactive during the time I was ill. Yes people are getting this and not surviving. Which is just incomprehensible. But do the right things for you and the rest of us, wear your mask, and try not to let "fear" overcome you. God Bless

      Germany has studied this. 80% of hospitalized patients who recovered are seen to have lingering heart and lung problems, and possibly liver, kidney and brain side effects. kindney

    Son in ICU with Delta unvaxed since July 21. The ECMO didn’t help. His lungs are full, receiving dialysis and blood pressure meds, insulin, blood thinners and now antibiotics for an infection in the blood. The wifey gets the info, but nothing has been said of his prognosis. He was intubated about 10 days ago. Praying but prepared. Thank you for the explanations on all your videos. 🌼

    I know this comment might get lost, but can you please suggest what we can do to protect ourselves and not let this situation happen?     First off I want to thank you for all of your hard work and dedication to these educational videos. They have been a tremendous help for my family and friends. I do have to say your production quality is top notch. Unfortunately You do have one problem with your videos The audio level is consistently low. If I had to guess I would say it averages minus -12vU to -14vU I watch your show on a Samsung 82-in 4K television Volume level for your show is around 70 setting to be normal, as a comparison most other shows on UA-cam average around 30 on my volume setting. Minus this one flaw your production quality is excellent keep up the good work.
