As a prof, I don't advise trying to schedule outside office hours. Just come to office hours, you'll be one of the few there and you'll be heard. Profs are super busy which is why we schedule specific times. All the other tips were great. I may show this to my students.
I had great Professor office hour yesterday for final preparing for next week! It was awesome and it is my second time visit him than any other my professor.
It depends. Many offices are small, but in my experience there is a seat for the student. If a student has a quick question, then he or she might choose to stand, but especially if you have made an appointment with the professor, it is normal (in America) to come in and sit down.
