My guess is those with runes have a perceptible aura. That those with some degree of discernment can register the divine power of those that carry the runes. It is not something most seem to be able to do. Many just seem to register you are tarnished and then fall back on whatever bias they have about that. About the only people who comment about quantity are those with some familiarity with the matter.
To be fair to From Software, if they wanted to make a lore accurate change to the base game post DLC, the ENTIRE Haligtree area would need to be changed, and there wouldn't even be a reason to find Malenia here in the first place. When Miquella broke his rune, his charm fell off, so everyone who was previously charmed no longer was (until he return to Radahn/Mogh's body and charms them again). So everyone in the Haligtree would be fighting each other, and Malenia would probably just f-off to the Erdtree, and that can break the entire game. Not saying it couldn't be done, but From is really good at setting their limits and not over-stretching. So lore-wise this just means that the DLC is canonically set after all of these fights.

Sif recognizing you from his past is not a problem, it doesn't change anything else about the game, but Miquella's actions are way too impactful. I too like those nods to the player, but it just doesn't fit this situation.
How would she even know what runes you have unless you told her? All she knows is you are a divine bounty hunter who has come to collect her head.

When you acquire multiple great runes you don't tell anyone that you have multiple great runes somehow they just know that you have it
