Ben Bikman is a very polite individual. Even when he gets interrupted (quite often and unnecessarily) he stops talking, smirks and when allowed goes on with the subject. Very nice man! and super interesting. Too much witticism is not advantageous to a serious conversation.

I owe these two fine gentlemen my life! It was 12 midnight, January 7, 2022, I woke up to a very painful stomach. Went to the toilet, was writhing in pain on the toilet seat. I literally wanted to die from the pain. What's worse was nothing was coming out! After about 30mins of the most excruciating pain in my life, I started defecating. I breathed a sigh of relief and I started FBing on my phone, and I happened upon this video. I've never engaged in health videos before. Never heard of either doctors before. How FB's algorithm decided to show me this video is nothing short of God's divine intervention. This video jump started my journey to eating healthy (no processed food, no sugar, low carb, lots of veggies and whole food). I am obese (272lbs, 5' 8", 44yo, male) and overweight by 110lbs last Jan 12. Now, 3 months and 1 week later, I am only 220lbs! I lost 52lbs! And I won't stop losing weight until I reach my ideal body weight of 160lbs. Currently in day 4 in what I hope would be a 7 day water fast. Previously, my longest water fast was 5 days. I hope Dr. Hyman won't mind, but I would also like to mention the other doctors that helped me in my journey: Dr. Pradip Jamnadas, Dr. Jason Fung, and Dr. Eric Berg. You, guys, rock!!!!

    Tests:
    1. Fasting Insulin should be below 5
    2. Oral Glucose tolerance test ( at 30 mins, and at 2 hour)
    3. Triglyceride to HDL ratio should be less than 1.5 - no insulin resistance. If above 1.5 , there is insulin resistance
4 . Waist to height ratio - if waist circumference ( around navel) times 2 is higher than height , there is insulin resistance. If less, there is none.
    5. Skin signs
    Skin tags, acanthosis nigricans
    Lipid test
NMR- LDL particle no and size
CRP, uric acid , liver function test.
Management : (small fat cells just multiply and dont become insulin resistant , not inflammatory ; big fat cells, hypertrophy, insulin resistant , inflammatory).
    1. Exercise: the best is the one you do, particularly after the most starchy, spiking-insulin meal. This makes cells insulin sensitive,
    2. Supplements: berberine, cinnamon, vitamin D, alfa-lipoic acid, magnesium, omega 3 fats, biotin, chromium
    3. Medications: metformin ( is only half as effective as lifestyle changes)
    4. Nutrition: control insulin by control carbohydrates, prioritize protein , and dont fear fats. Eat when hungry, dont
Control carbs, eat them, dont drink them , dont eat them out of boxes and barcodes, Eat them from vegetables and fruits.
Get high quality protein, lots of it.
Focus on ancestral fat, from fruit fats and animal fats. - dairy fat, avocado , olives/olive oil, coconut oil, nuts ( seed oils cause fat cells to go into hypertrophy-inflammatory route; also saturated fats are stable but seed oils ( polyunsatirayed oils) are unstable and become easily oxidized which is phagocytosed by macrophages ( start of atherosclerosis).

In Australia, fasting insulin tests are commonly ordered. In fact, it picked up my high A1C and put me on the path to serious diet reassessment. A low-carb diet saw me drop from 84kgs to 56 kgs. My sore knee, which the physio said was caused by arthritis was really just inflammation caused by a high carb/sugar diet. Thanks to that Dr never telling me I was pre diabetic or ever at risk for diabetes, I am Type 2 with severe insulin resistance. It makes me very resentful.

While beneficial in many cases, I was unable to maintain a low carb diet. After 6 months, I could no longer sleep well, unable to handle stress, and developed a hormone imbalance. What also likely contributed to that was I consumed only 1400 calories a day. I was never hungry though and shocked at how little i was eating when i began to track. I feel much better with my daily carbohydrate intake (healthy of course!) at 30%, also making sure to eat high quality protein and fats. As a nutritionist, I believe there is never a one-size-fits-all approach.

However, I still kept believes in low carb diet and so I switched to ketovore and carnivore and I got my period back ever since, and I feeling more energized more ambitious and always in a good mood , also I don't really craved sweets and starchs.... by going through these I realized how important animal fats and protein to my body... ketovore and carnivore diets r much more sustainable to me, I'm on my way to the 6 months extremely low carb diet👍 and still feeling good

Exercise greatly increases your carbohydrate tolerance by way of glycogen. Post workout I will consume 100gm of sweet potato in my breakfast. Pre meal bg will start at 4.4 mmol. 1 hour postprandial bg never rises above 4.8 mmol. During lockdowns where I haven't exercised, that same amount of carbs can push bg to 5.7mmol.

The elephant in the room as to why the proper testing isn't done for fasting insulin, our practioners only do fasting glucose, as a rule is our insurance companies. It is outrageous - I have been asking our primary doctors to do fasting insulin abd the A1C test for over a year and they refuse to do so. I finally got one primary to admit they only do the A1C test once you have a fasting blood glucose of 130 two times, otherwise insurance won't pay for it. And that is just to get the A1C test - still won't approve the fasting insulin test at that level. Have to climb a mountain to get decent health care in this country. I know I can pay for tests out of my own pocket, but why should I - I already have high premiums. Tragic.

Surely they know that your fasting insulin will show insulin problems long before anything else
