Theoreticaly, it is actually possible to have this happen in vanilla NES Tetris. The pieces are chosen purely by random, not like how they are chosen in modern versions of Tetris.
its not completely random, every time it needs a new peice it rolls 1-7, the seed being the frame counter, if it gets 7 or the peice you got last, it rolls again, this time 1-6 and it will stick with its choice no matter what

I messed around with this in Tetris DS with a cheat code. I would always stack them end-to-end as high as I could go without ending the game, and see how well I could do. I would always fill out the bottom first, so that way I would be less likely to hit a condition where because the game is sped up I wouldn't be able to move my piece out of the way in time as it spawned at the top

apparently their implementation of rng had some weird quirks making getting all the i pieces like this impossible without other glitching
