We would advise speaking with a doctor with a focus on dietary changes that may provide you with the energy required on a daily basis but also ensure weight loss. Following a low-fat, calorie-reduced diet can aid weight loss when taking Orlistat. If exercise is not possible, the rate of weight loss may be lower, but weight will still be lost nevertheless. I start taking orlistat and due to some medication I’m taking for my heart I only can take it twice a day. Would it still be affective ?? And I do exercise 4 or 5 times a day. This includes a lot of strength training and waiting on the thread mill for 60 mins to the highest incline.
If you're eating only once a day and want to take Orlistat with that meal, it could still be effective, provided that meal contains some fat. Orlistat works by blocking fat absorption, so it only needs to be taken with meals that have dietary fat. Taking it with a fat-free meal or on an empty stomach wouldn’t be useful and could increase the risk of gastrointestinal side effects.

It depends on the dosage - Orlistat 60 mg is available over the counter (we have it here at Pharmica too), but higher dosages require a prescription.

Is it okay if I will do fasting? I mean If I eat once a day, and take 1 orlistat, will that be okay?
