  • In this video, Dr Sanjay Gupta, consultant cardiologist, discusses how cocaine use can effect the heart. Cocaine’s main effect is that it increases our sympathetic activity (flight or fight system) by increasing the amount of adrenaline, noradrenaline, dopamine and serotonin in the blood and therefore causing an exaggerated and prolonged activation of our sympathetic system. With regards to the heart, cocaine causes an increase in the heart rate and the blood pressure. Because it has the effect of narrowing the arteries, it can increase the narrowing in the heart arteries. This is also why the blood pressure goes up. What this means is that the user has a horrendous double whammy - the heart is made to ask for more blood and the blood flow is choked at the same time! Imagine trying to pressing down on the accelerator of your car with all the force that you can and blocking the pipe that takes the fuel to the engine at the same time. Imaging how damaging that is to the engine and how easily then engine can break down in that scenario. Another problem with this scenario is that it promotes stagnation of blood and therefore blood clots can form within the vessels which can then make everything worse. In this case, the heart can start suffocating as it does so it can go into funny heart rhythm disturbances which then make everything worse can prove fatal. In addition cocaine can also with interfere directly with the sodium and potassium channels in the heart and cause QT prolongation thereby predisposing patients to dangerous dysrhythmias.

    I used cocaine for 10 years. After 3 years I had a heart attack. I was never able to weight train again . Still I went back and did it for 7 more years after I recovered. now I am 75!! I'm building up my aerobic stamina to where it was decades before.I would like to have the heart health I had before coke! im blessed to be alive. I will keep you informed to how I am recovering my heart health as it is a miracle!! I love your channel!!

    youre doing a great job by describing how this drug works purely on the physical systems...next do amphetamine, ecstasy and alcohol! And also the big hidden drug that ruins natural energetics of the heart and often causes ectopics: caffeine - guys vote up if you want Sanjay to see this!

    I remember DARE classes in middle and high school and they had very little of this type of explanation. I just remember them saying it was addicting and will make you unhappy. I hope they have gotten more in depth and include this exact type of presentation in schools. Very concise.

    My boyfriend took cocine for 15 years , he was a professional , active guy but would do drugs at times . He died at 40 from a heart attack right after he did cocaine .

    Thank you doctor. Your videos are clear & your advice is very helpfull indeed. Alas, I have CHD & following a 4 way CABG was diagnosed (some time ago) with Grade A, heart failure. Following the CABG, I got an infection due to being allergic to the steel wires & so I was co-operated on to remove the wires & infection, I had so much post-operative pain for so long, that I ended up becoming addicted to opiate painkillers. I am now clean after 3 years, following Buprenorphine maintenance & then titrate, off the opiates but was wondering how much damage I may have done in terms of exacerbating my early heart failure? Many thanks.

    im a young man and i needed to hear this. no more for me even once in a blue moon bc i want heart health when i enter my later years. its true you get euphoria, but as you alluded, "never a feeling of total satissfaction. Thanks doc

      Aye some people never touched a drop of alcohol or nicotine and still die of lung cancer. Some people can be abusing cigarettes till there 110 years old. You never know your own body as good as you think you do when using substances like coke.

    Weird fact, I'm a recovering alcoholic who has never done cocaine. I have no idea how, I was offered it regularly back in my drinking days. For some reason I always refused, even thought I was often close to blacking out. Shame my self control wasn' so impressive when it came to whiskey, lol. But I'm sober now, so its all good.

    I had very noticeable ectopic heart beats that were getting worse and worse this year. I started getting them last February. I have been a long-term alcoholic and decided to quit in April. The ectopics continued to get worse up until July when I quit taking my blood pressure medication (Diltiazem). My blood pressure was normal after quitting drinking, although it wasn't super high before, (130/90) at night and (120/80) when waking up. Sometimes much higher if I had anxiety.
    After stopping diltiazem, I haven't noticed any ectopic heartbeats since, and my blood pressure sits around 110/70 at rest.
    EDIT: I started Diltiazem July of 2018. Ectopics started in February and stopped after ceasing the medication.

    I know this an old video, but due to cocaine overdose constricting the blood vessels would taking emergency heart attack medicine like ASA or Nitroglycerin be of any effect (opening the blood vessels) when someone is experiencing overdoes?

    I was a heavy cocaine user in my late teens to mid 20’s. I first started getting heart rhythm disturbances back then. Dial it forward 15 years I have major problems. I did let my cardiologist know about my previous use and she shluffed it off like that didn’t matter. It sure does. Maybe she didn’t care because there isn’t anything that could be done now. This is a case of if I knew then what I know now, I probably wouldn’t have abused by body so badly back then.
    Thank you for this video it answered the questions I’ve had for a very long time.

      Its terrible the price one has to pay for mistakes whose long term cosequences are not known at that time.feel bad 4 u.but u could try yoga breathing techniques & exercises to at least start recovering some of the whole or maybe full.all the best.

      @smurfmobile H what kinda of problems do you have. I used to use cocaine about 7 years ago I used cocaine again for the first time in 7 years about a year ago hope that makes sense. Amd recently have started having bad ectopics!!!

      Chandra V yes I have been treating things for a few years now. Lots of yoga, meditation, breathing, I am also very active. I do cardio and strength training plus I try walk after dinner every day. Unfortunately I still have problems :( Although they did somewhat improve over the last 3 years when I made major lifestyle changes

    Can you do a video on the heart and theuroputic use of ADHD medications such as Ritalin or Adderall. I know that Ritalin is similar to cocaine but does it still habe the same effect on the heart? Worried because I take Ritalin for my ADHD but my heart feels funny yet my doctors all say it's anxiety...even after several checkups and an EKG.

    You explained the reason why I was given pharmaceutical cocaine as an anesthetic while in the Air Force back in 1979, right before all four of my wisdom teeth were extracted. The drug was administered in nasal spray form.

    Thank you Sanjay. I have always been curious to try cocaine with my friends but as I suffer with regular ectopic heartbeats and palpitations, i've always been too hesitant to join in as I was anxious that cocaine would make it worse, therefore I have always avoided it. You have just re-assured me that i've made the right choice.

      Took me 30yrs to try it. Never had a problem with it but its "the party drug" for a reason. If you have the money you will chase that coke high more than you should. My honest opinion of someone who never touched it til i was 30, its EVERYTHING they show it is in the movies! Yes its amazing but dont even try it once! Period! Truuust me you dont want to go down the rabbit hole

      Yes, you definetely did. I occasionally have mild palpitations and tachycardia and cocaine makes it 20x worse and it lasts until evening the next day. That is why i started to avoid cocaine.

    I have been taking cocaine since my 20s. I am now in my late 30s. I can now feel the damage from taking cocaine. My chest feels weird. I have no energy. My weight has gained. The worst thing that nobody tells you about cocaine is the gastrointestinal problems is also causes.

    How would you know you damaged your heart I’ve gone to the hospital they say no damage was done but I’ve gain premature beats and flutters from cocaine use

    Ive just turned 39 and decided to leave it alone the heart definitely feeling the effects of it. The body gets so tired i mean it takes about a week to recover after a two day binge of alcohol and coke use.

      @@MarioMartinez-tt9ly off and on since early 20s but id say through out my 30’s mostly on weekends party friday, Saturday sleep all day Sunday than back to work really didn’t faze me till got to the mid 30s thats when it was taking a toll on body and mind more. Last time i did it few months ago even a day or two after the heart felt like it was still a lil off im thinking i may have hi blood pressure now. Im eating healthy and sober now hopefully body heals itself and not to much damage has been done.

    I have heart palpitations that I’ve never had before right after I inhale cocaine they happen once or twice but it seems to be correlated on how deep or hard I inhale and I’ve never had this issue before it happened the other day I can’t tell if it’s from the cocaine or my anxiety it happens even when I’m not doing it

    ive tried this 3 weeks ago, ive been feeling quite fatigued the past week and im now feeling my heart ache. The left side of my heart seems to be the side where it aches, and breathing takes more effort than before. Also realised that sometimes my heart does beat faster but not all the time.     Great video. Can you do one on marijuana? I’ve used it very infrequently in the past and it always caused tachycardia and palpitations. Friends say that’s not the case for them. One even advocated it should be good for my Afib!

      What a frighteningly dangerous drug to be addicted to! But then again, they are All frightening and dangerous........
      But with Fentanyl, from what I've learnt, it only takes a very tiny amount in doseage to over-dose and die.

    Young people are so uninformed. If you'd tell these details to younger people who don't see cocaine as a dangerous drug... they might rethink again. Because just telling them "It's dangerous... you can die from it" is obviously not nearly enough. A brother to a childhood friend of mine died last night from a heartattack after the use of cocaine. He was 22.

    My sister in law take medicine for ADHA, she is very thin 168 cm, 43 kg. She walk for 2 hours day.
    Still she had her first stroke at 41 and the second stroke at 44.
    I know that she take very strong medicines for pain too.she's addicted to them.
    Wonder if her stroke's had something to do with her medicines.

    im 31 iv been doing cocaine since i was 17. probably twice a week since i was 17....now lately when i do it i feel like my heart is going to blow up. I went to the ER in aug and they did all the tests and said i was fine but iv done it 3 or 4 times since I was in the ER and i feel like SHIT the week after. like super paranoid for a week after that im going to have a heart attack. It sucks

      if it makes you feel better, I've had had the exact same symptoms as you with much much lower usage...basically a kind of "diminishing returns" to taking it where all the positive aspects are completely overshadowed by the very bad side-effects..

    I'm 28 and have used it for an evening every 2-3 months over the past 3-4 years and have developed some pretty bad symptoms in the months following a single usage (random palpitations at rest and long recovery periods for heart rate to relax after basic exercise). I must have a predisposition that makes it not agree with my heart. Have just flushed what I had down the loo as it's just not worth shortening my life... hoping any damage I have done will repair itself.

    Tried it once on a Stag Party, was already inebriated which is why I went for it. Long story short, I felt amazing initially after taking it, the urge to do so manifested every 30mins or so to get back on the optimum feeling. Later that night I was sitting on a toilet and my heart was pounding with a feeling of pressure behind my eyes, couldn't get to sleep. Hopefully didn't shave off too many years of life.

      @@MrJetMango It was an experience that had dizzying highs and bone crushing lows. Not sure if it was worth it, for now it feels like I broke even, but if I lay dying of some cardiac malfunction at some point I may feel like I paid to high a price, although that feeling of regret may only bother me briefly given the circumstances.

    I'll never understand the hype around cocaine, it's very expensive, short in duration and it's extremely difficult to do in moderation, I've maybe bought it a handful of times in my life and regret it each time, never really enjoyed it, just puts you on edge and makes you talk absolute garbage.

      I loved it the very first times I used it. But I do feel that (like most things in life) the good effects realyl wear down over time with use. And then you start feeling it on the heart...but I will always remember the highs I had at the beginnig, even if short. Hard to parallel.

      Depends. Some people use it a couple of times and they die. Some people use it for 30 years and nothing happens to them. It's crazy.

    I know someone who has been abusing coke for over 20 years, in massive amounts.
    They now have high blood pressure that won't go down even with medication
    Is it too late for them to get better?
    No damage showed up on a recent heart scan

    I smoked a large amount of crack one night(was using a lot of drugs at the time)...my heart suddenly started beating over 200pbm...felt like a massive crushing weight was on my chest...couldn't breath that I could tell...felt like I wasn't getting any oxygen...half of my body went completely numb...lasted about an hour...just laid there wanting to die...called my parole officer the next morning and demanded to be put into a program

      Any drugs will affect the nervous system, which can cause digestive issues. Also, drugs can increase chances of getting ulcers. Not worth it.
