He could maybe use chocolate bars to heal, and since some of the opponents seem old, maybe doc could battle younger versions of them, but others may not be there because they were too young. Maybe in an ending cutscene Doc would retire and then it’ll show a short bit of him training Mac.
I think part of why that is, Is because this guy plays it and makes it look easy, but then again, Being able to actually hear the game does help, Most of the time The game grumps are just that loud XD

My first time ever beating King Hippo I didn't know how to jab. I thought the intended way to beat him was to carefully chip away a small chunk of his health with regular attacks and make sure he never hits you so you can win by decision.

I like to imagine he did it specifically because, if I remember right Bear Hugger has a special property where body blows immediately end combos against him so the fight encourages you to focus on uppercuts, thus making it so you have to really try to knock him down with a hook.
