- #MonitorandLive. Leah Coufal’s mother, Lenore Alexander, recounts the tragic story of her 12-year-old daughter’s preventable death in the hospital - a victim of opioid-induced respiratory depression due to a lack of continuous postoperative monitoring which could have saved her life.
I'm a nurse of 21 years, specializing in private duty pediatrics. I have never given 2mg ativan every 2 hours (iv at that) to an adult let alone a child!!! Prayers to this family. I am so sorry for your lose.
Exactly. I have chronic anxiety and depression when I’m not medicated and one to two Ativan at that age would calm my body down for the entire night.
Also a nurse. That dose is unheard of.
100% agree. I once had a physician order 2 mg Dilaudid, 2 mg Ativan for a 100 pound patient in their teens who rated their pain as 6/10. Nurses need to question these orders and show some sense.
yeah wtf, isn't that overdose
Wow I see why legit pain patients have to be left to suffer because of these kinda mistakes. It’s still not right though. When I was in hospital I was given 2 mg Ativan after a reaction to steroids but it didn’t make me a bit sleepy.
This is so sad and so preventable. I am so sorry for your daughter's death.
Monitoring every two to four hours??? For Fentanyl and Ativan??? Where is this “the law”?!
I have been administering Moderate Sedation in adults for fifteen years and has the standard has been vital signs including ETC02 and 02 Sat every three to five minutes.
Poor darling, she’ll never suffer again, bless her Mom for her warning.
The Dad knew. So sad, Always advocate for your family and do your own research.
I'm so sorry for your loss.
That's an insane dosage for an adult let alone child😳! It's extremely concerning that not one member of staff questioned that dosage the dr that wrote it ..the two nurses that checked it none of those trained professionals questioned it?! The Ativan alone was a overdose never mind the fact she was seedy saturated with fentanyl and the lack of motioning is another completely unforgiverble factor
The daily maximum dose for an adult is 10 mg/day for lorazepam. Knowing this because I have worked in psychiatric nursing, I would have questioned this order. Now working in medical-surgical nursing with mostly geriatric patients, physicians only prescribe very low doses for the elderly, usually in the amount of 0.25 mg every 4-6 hours typically. I know with children, medication dosages are based on weight in kilograms. Post-operative monitoring is a simple, non-invasive means to catch opioid-induced respiratory depression.
she didn't die of an opiate overdose she was Kill by a benzodiazepine overdose , and I am sorry for your lost.
I hope you sued the living shit out of these people. Ativan is NOT something you just hand out like candy, especially every two hours! Good lord.
Yep on-top of what was likely already a overdose of fentanyl 😳. Not only did they heavily overprescribe they did it even bother to monitor her
oooaaa me sirve pa estudiar enfermeriaaa
YOU DID NOT FAIL HER AS A MOTHER!!! GODSPEED!!
What amazing parents, looking out for their child and her future. Thrown away by negligent doctors, patients are people with entire lives not just another chart.
I am so sorry for your loss. Thank for you your efforts for patient safety.
I am so so sorry. This breaks my heart.
Wow Lea would be 30 now.

I’m one of those patients and they make us jump through hoops b/c of these medical mistakes and drug seekers
I am so sorry
