  • Emily Jerry endured a grueling, but successful, battle with cancer. During her final chemotheraphy session, Emily Jerry was tragically killed due to a medication error resulting from an inadequately trained pharmacy technician. Christopher Jerry shares the story of Emily's death, which spurred him to become a patient safety advocate. "I believe that we can get to ZERO deaths in the United States from preventable medical errors because they're just that, preventable," says Christopher Jerry.

    What kind of hospital runs out of IV fluids??? And if they didnot have it they could have just asked the parents to get it from another pharmacy............this is unforgivable

      No what should have happened is that the 0.9% NaCl should have been ordered for immediate delivery. No excuse. And this should have been checked by the pharmacist before it left the pharmacy door for the unit. Again totally preventable and a mother fucking error cost the life of his daughter angersome.

    How very sad. Thank you for sharing what must be a horrific pain. I want you to know that your story and efforts have not gone unnoticed. My son, who is a caring and wonderful Professor had shared your video to his class of students, who are studying to be Pharmacy Techs. The lesson he was trying to show was how very important it is to be proficient in Math. And your story made them understand. Thank you. You made a difference, again.

      What is the need of Pharmacists FINAL CHECK then. Though whatshe did is wrong,No Technician can handle a pharmacy alone without a Pharmacist,they spend almost 6 to 7 years in the field while some technician spend 8 weeks.Honestly, they should always inspect the pharmacy tech in what ever they do to avoid had I know.May the humble soul of the little girl rest in peace. Its very sad.

      Thanks Chris, even worse is the situation that New York Presbyterian Hospital took over the place and is enforcing stipulation of confidentiality, seal and Temporary restraining order against me because I blew the whistle and I have knowledge of this horror. They are trying to prevent patients and their families to know what took place and how they were harmed by egregious hospital staff. I cannot use any of my documents , everything is sealed and confidential even though I never approved confidentiality. To protect the hospital egregious practices and their corrupted bosses, my constitutional rights are taken away from me , while the hospital is covering up patients injuries, deaths, retaliation, falsification of medical records etc. Basically with the court decisions , influence , power , politicians, the hospital tied my hands behind my back and they are punching me directly into my face, obstructing my defense and prohibiting my exculpatory evidence hidden under cloak of stipulation of confidentiality that I never consented. In the mean time, patients will never learn what happened to them.

      The distinctly unpalatable truth that medicine, for all its lifesaving accomplishments, can also cause harm. Doctors were understandably desperate to help their patients, but resultant frantic prescribing, probably caused more harm than good…

    What an incredibly sad story I'm so sorry for your loss ! What a cruel turn of fate to learn your beautiful child had beaten the beast of cancer only to be killed hours later by a pharmacy error 😔 Your daughter would be incredibly proud that you have taken the trajectory of her loss and not allowed it to be a waste 🥰

    as long as humans are administering health care nothing is preventable. people like christopher jerry while i feel for his loss, encourages toxicity in the health care field with these goals. striving to increase safety and such should be the goal.

    So very sorry for your loss. I lost my daughter in an accident (negligence of another). I know all too well how you feel. This is why it is so important not to give healthcare providers immunity through tort reform (wrongdoer immunity). We can get to zero deaths if we continue to hold for-profit medical facilities accountable for their negligence. They will continue to understaff and under supply themselves unless their bottom line is impacted.

    My heart is heavy right now, but someone had to speak out. These deaths are preventable, but we all make mistakes. however, that is no excuse for a professional to cause such a horrific death. My heart goes out to Mr and Mrs. Jerry.

    For no apparent reason I did the calculation and discovered what the tech did wrong. By using 3 vials of hyper tonic, the 'Normal' saline created was 21.5 times too strong.

      sorry ur math is wrong :/ NS is 0.9% and the hypertonic solution used was 23.4 making it 26 times the concentration of sodium chloride. hope that helps :-)

    I wouldn't so much blame the technician for the error, more or less I'd blame whoever let them do it alone when they clearly were new to their job.

      Hi, Certified Pharmacy Technician here. I know this person will never forgive them-self for what they did, but that is an egregious error that should never have happened, whether they were new to the job or not. Also, the technician cannot send a medication to be administered without a Pharmacist checking the product, so two people failed that little girl that day. Two people should have caught the error. Plus, with this being a chemo, there wouldn't have been a huge rush to get it up to the patient. It's not like it was a patient coding needing a life saving drug, if that makes sense.

    No reason this should have happened. All hospitals run out of fluids and even drugs but a second set of eyes could have helped. Words won’t even describe what this had caused to you and your family. As a parent I am sorry you had to go through this.

    This happens when incompetent people do not know how to do their job properly. Very sad that Emily had to die after having beaten the cancer. The mistakes are only preventable if medical professionals start being more competent and are held accountable for what they do.

    hurt so much to see baby no fault facing cancer. Saline is salt! The pharmacy tech is trained by Govt if has enough money 💰.
    THE pharmacy tech is Manipulated by pharmacist if there wasn't enough ingredient for. The mix why did the pharmacist approve it. and he keep the prescription pay in the pocket. A technician doesn't make any decisions without approval at pharmacy. It sad to talk.

      The ratio of Pharmacists to tech is that bad! This is the reason why the work of most techs are not well checked because of the workload. Unfortunately, most of our doctors and pharmacists should be punished in cases like this. There is absolutely no excuse to kill a beautiful young girl.

    All the medical staff:you have very very import job when you are caregiver from kitchen staff, housekeeping , you are all doing something very impt, and have peoples loved ones in your hand, imagine it’s your own family. Then you will do your job better and be more careful as hospital worker.

    This Pharmacy Technician SHOULD have had criminal charges filed against HER TOO. Yes, the Pharmacist is the last check , BUT she should have questioned the SALINE solution if not sure. I am a Tech and ask questions all the time. Hope this young woman is NO LONGER in Pharmacy and that this "error" haunts her until her dying day.

      The pharmacist should have asked her what vials she used instead of assuming she did it right if the pharmacist knew it was from hypertonic saline vial it would have been easily avoided may Emily Jerry RIP

      She was charged but later the charges were dropped. She is currently working at a pharmacy.
