MSE Expander and Vivos DNA and Homeoblock can Work Together

  • It is not always jaw surgery versus dental expanders. Often times tooth borne expanders and orthodontics can be used in combination with various jaw surgeries including maxillary skeletal expansion. The most conservative treatment should be tried first which is Vivos DNA appliances and Homeoblock appliances and Invisalign. These modalities of treatment can in many cases be all someone needs to correct TMJ pain sleep apnea and posture problems. In the event surgery becomes necessary people will still need the other treatments such as Invisalign braces and expanders. These treatments truly can be used together and it is not one versus the other.
    Vivos DNA Appliance Versus MSE and SARPE
    60% of adults have crowded teeth and facial and jaw bone deficiency. In-other-words most adults have small mouths compared to the size of their tongues. Often adults with jaw and facial deficiency suffer from TMJ pain, sleep apnea, bad posture and head and neck tension. All of these problems are linked and related which is why you are reading this page.
    How do we Grow the Mouth and Fix Jaw Bone Deficiency?
    Both maxillary skeletal expansion (MSE) and surgically assisted rapid palatal expansion (SARPE) are surgical procedures used to increase the width of the upper jaw called the maxilla. MSE and SARPE are very good at surgically expanding the width of the upper jaw. Most people undergo these procedures to get more space for the tongue to fit in the palate. And also to correct teeth crowding and cross bites. These treatments only grow the upper jaw wider. Most people who have small mouths need all their jaw bones to grow wider, more forward and taller. Not just the maxilla. The jaws and facial bones consist of many other bones that need to change size, shape and re-orient if treatment goals are to be accomplished.
    Homeoblock and DNA Orthodontic Expander Appliances
    The Homeoblock and the Vivos DNA appliances are non-surgical facial growth devices. They look like retainers and are worn at night only. The appliances are custom designed per the patient to change the size and shape of the mouth and jaws. The DNA appliance uses light pressure, biting and tongue forces to grow the jaw bone. Depending on how the appliances are designed, they are capable of growing the jaws in all 3 dimensions. There is an upper and lower appliance to control the growth of the upper a lower jaws. The appliances are worn at night and the treatment usually lasts 12-18 months.
    What is MSE?
    MSE stands for maxillary skeletal expansion. The MSE device is screwed into the palate bone with titanium screws. The expansion mechanism is slowly widened over time by the patient or a helper. It is surgically installed into the palatal bone after numbing the patient with local anesthesia. The same numbing process as used during filling procedures. The MSE is also called a “bone borne” expander. Or the action of the appliance is at the level of the bone. This is in contrast to braces that are connected to the teeth or are “teeth borne.”
    What is Surgically Assisted Rapid Palatal Expansion?
    SARPE is basically a procedure where an oral surgeon cuts various facial bones to separate them. Once the bones are cut, the SARPE expander is placed and the jaws will expand very quickly. The SARPE Video above clearly shows how this works minus the unpleasant visual details of the bone cutting.
    DNA or Homeoblock Appliance can be used with MSE
    The orthodontic appliances are more comprehensive treatment appliances. The DNA and Homeoblock can grow jaw bone in all 3 dimensions whereas MSE and SARPE only increase the width of the palate. Cases only start with MSE and SARPE. Other treatment modalities such as braces and forward growth appliances will be necessary. Notice the large space that was opened between the front teeth in the after MSE treatment photo above. MSE only expands the palate, so a forward growth appliance such as the AGGA (Anterior Growth Guidance Appliance) will be necessary. The bite is very important. A problem with expanding the palate is now the upper jaw is wider than the lower jaw. Something will need to be done to correct the difference in size between the upper and lower jaws after MSE therapy. The best course of treatment is to use the non-surgical expander first and if the palate growth is inadequate, the case can be finished with MSE.
    DNA compared to Homeoblock Appliance
    As you can see, the DNA and Homeoblock look very similar. They are both capable of growing the mouth in all three dimensions. Using these devices is a matter of training and experience, but the appliances have comparable growth potential when in the hands of a knowledgable doctor. One major benefit of these appliances is they rarely need to be followed by other treatments such as braces or other growth appliances. Unless they are followed by MSE, in which case braces or Invisalign will be necessary.

    Would you generally recommend expansion to patients who have undergone 4 premolar teeth extraction?

      Generally expansion appliances is a good place to start. The DNA and Homeoblock appliances can sometimes completely fix bicuspid extraction retraction case problems. It is case by case, but why start with surgery as it will also commit you to years of braces.

    What is the difference between the Vivos and the Homeoblock? Can one of these be used for someone who doesn't have orthodontic problems but to cosmetically change the shape of your face?

      Certainly facial asymmetry can be improved, but that is not the primary purpose of these appliance. I think some headway will be made with that kind of treatment goal, but I would not make big promises to my patients about that. The primary difference between the DNA APPLIANCE and Homeoblock is the kind of spring. Most of the differences lie in the difference of the design one doctor may prescribe versus another. The other big difference is the manner in which the various providers will manipulate and adjust the appliance throughout treatment. Most notably the bite plane.

    Hi Sir i got some few questions when you use mse too grow the maxilla it will grow forward right? And when the maxilla moves forward will the mandabile (lower jaw) move with it?

      mse will not move maxilla forward, just transversally (left and right), and i dont think you want forward growth as there is nothing to do for the mandible(just biting a bit forward with lower jaw will also not work as your jaw joint is set already and it would then pop out of joint when biting.

    I believe you said titanium implants are better than Zirconia for a full mouth implant (I dont need that) but why is that? I want to replace 2 adjacent root canals with implants. Can I still use Zirconia or do I have to use Titanium?

      You can use zirconia for individual teeth and 3-4 tooth bridges, but not a full mouth one piece bridge. You can do two individual zirconia implants for your case.

      @@SereneOrthodontics After speaking to my Oral Surgeon he said that he doesn't recommend getting a Zirconia molar implant. How easily would a molar implant break?

      @@angelicannihilator I’ve never seen one break. I place both zirconia and titanium. They both work. The OS probably has never done a zirconia. But my 1st choice for a molar is titanium unless some has metal sensitivities or autoimmune disease.

I will be coming to see you for a zirconia implant and/or a DNA device. I forgot to mention my top left lateral incisor is rotated slightly. Should I get that fixed with braces or will the DNA fix that?

      I think if you are asking this question you are focusing too much on palatal expansion. That is only one of many problems that can be traced back to why people even have the teeth removed to begin with. The maxilla is only one of several bones in the head face and jaw spaces. Making one of these bones wider is not going to get a lot accomplished considering all the other related problems. Anterior growth or forward progress is a major thing that needs to happen. Balance of all the different jaw and facial bones is super important. Tongue and airways performance. how the teeth fit together top to bottom and the relationship between the upper and lower jaw. Everything in three dimensions needs to be totally optimized. I think you need to take a look at the bigger picture. A surgical increase of the size of the palate alone is not going to get anyone better. There’s no question that orthodontic appliances may get 3 to 9 mm of expansion depending on the person. But I would a whole lot rather see 3 mm of expansion and all the other things get corrected, than 9 mm of expansion and nothing else done. What an adult appliance can do is have a fighting chance at starting and finishing a case as appropriate appliances are capable of getting growth in every direction and they are also capable of balancing the bite and all the jaw and facial bones.
