I am not in favor of dedicating a day or an hour for the poor because that marks them as someone less important. But dont worry even patients I am seeing with full fee are a kind of community work for me. No one can suggest that I am seeing patients in Pakistan for money, such assumption would be ridiculous. So, I try my best to adjust poor patients in our daily schedule.

So far best video i have seen on Vitamin D Supplement.Dr you have explained it very well. Just to ask you my Acr level is 16.8 mg/mmol. would it be reduce by taking Vitamin D with Valsartan 80 mg Please advice.
Thanks a lot that it helped you. Your proteinuria is almost 150mg per day which is not too bad. Yes, Valsartan will help it bring it down to 30 or less but vid D I am not sure how much. So talk to your nephrologist and increase the dose of valsartan to manage your proteinuria.
Amazing information regarding vit. D3. One question, if you spare some time to answer, plz. Do you recommend taking vitamin k2 with vit.d. It is learnt that taking k2 with d can be helpful in reducing calcification.

In short, I am not targeting the population who wants instant gratification, and neither 2 million such subscribers would serve my purpose. You can take VitD with Vit K. I am usually not in favor of taking supplements. Vit D is the only exception because we can confirm its low levels. If you take a decent amount of salad in your diet, you dont need to take Vit k2
Another fully loaded video with good and understandable explanation. I have been taking based on my test results. I have had one bad experience when my previous nephrologist prescribed me vitamind+calcium supplements, even though I didn't need calcium. My current nephrologist took me off of them and put me on vitamin D only supplements. My calcium normally keeps Borderline high even though my egfr is 33. Those calcium supplements caused me heel spurs, and I have to use silicon insoles now in my shoes

I think you are the top nephrologis of the world.... i am taking pork meat 3/4 time a week. 2 month before i ckeck my whole body. my creatinine 1.43 lab value (0.7-1.5) uric acid 8.13 (not gout attack) Weight 86 Age 22 I have no high bp ( only it go to 130 to 140 when i worry about these raise creatinine) other time 110/70 No sugar Protine nil Blood nil All other are normal After that i stop eating pork and less protein diet. Eating lemon water 2 time a day Drinking water 2 liter a day Exercise 20 min a day.. Now after 1.5 month My creatinine 0.93 lab value (0.5-1.3) Uric acid 5.8 So i have ckd ??? Sir you say not sure in previous video ... Why are you not sure... I have no any sign symptoms I check my whole body without any compecation... No bp , no sugar ....... I don't know why are you not sure... Google say (high protein diet increase creatinine temperarory but persistent high mean ckd ) my doctor also say it is normal ..he is also see only these report as i write above... My creatinine naturally down by same diet (except over consumption of meat).3 month before 1.43 then 2 month before (0.93) and today also same (0.95) But i don't know why are you not sure.. Google also say gfr above 90 is good kidney without protine loss ... But gfr above 90 with protein loss is stage 1 ... But i not loss my protine in urine and gfr also 119 in egfr calculator But why you not sure i have ckd.... Google say ckd cause are high bp, diabetes, medication, family history of ckd , iga nep, polycystic ckd...I don't have these of the above cause.... My hemoglobin also 16... No high cholesterol No thyroid.. But i don't know why you not sure i have ckd.....??? I think you sure i have ckd 50% and not sure 50 % What is the reason of not sure....... I am extremely sorry if i have ever given the impression of being best nephrologist of Pakistan or the world. I am just a nephrologist, one among thousands, who are trying to help patients with kidney disease. I am just going out of the way to increase the awareness of kidney disease by making educational videos, which all other nephrologist can make if they will. Sorry please scratch this impression that I am the best nephrologist. Then, you are right, by decreasing the consumption of protein your creatinine comes down because more protein we eat more creatinine in our body. I am not sure because just like your creatinine of 1.43 is not real, your creatinine of 0.93 is not real. These are one dimensional values that may not be true for the long term. However, if you creatinine is remains at this level of 0.93 over many months, say at least 6 months, with your regular lifestyle, which should be healthy, then we can say with some confidence that you have no CKD. Then, I am not sure because as a nephrologist we go through some basic work up to make sure that you dont have any risk factor for kidney disease, and I am not going into details of that on social media. Lastly, google is good to increase your understanding, but it has not become intelligent enough to take care of your health and disease. Neither you can question it if something goes wrong. So, do use google to educate yourself but not to diagnose yourself. It may tell you that your protein would decrease with low protein diet, but it is not going to tell you what kind of nutritional deficiencies you would run into if you drop your protein too much.

You can take VitD with Vit K. I am usually not in favor of taking supplements. Vit D is the only exception because we can confirm its low levels. If you take a decent amount of salad in your diet, you dont need to take Vit k2

I have had one bad experience when my previous nephrologist prescribed me vitamind+calcium supplements, even though I didn't need calcium. My current nephrologist took me off of them and put me on vitamin D only supplements. My calcium normally keeps Borderline high even though my egfr is 33. Those calcium supplements caused me heel spurs, and I have to use silicon insoles now in my shoes

I have no high bp ( only it go to 130 to 140 when i worry about these raise creatinine) other time 110/70
No sugar
Protine nil
Blood nil
All other are normal
After that i stop eating pork and less protein diet.
Eating lemon water 2 time a day
Drinking water 2 liter a day
Exercise 20 min a day..

So i have ckd ???

I am extremely sorry if i have ever given the impression of being best nephrologist of Pakistan or the world.
I am just a nephrologist, one among thousands, who are trying to help patients with kidney disease. I am just going out of the way to increase the awareness of kidney disease by making educational videos, which all other nephrologist can make if they will. Sorry please scratch this impression that I am the best nephrologist.
Then, you are right, by decreasing the consumption of protein your creatinine comes down because more protein we eat more creatinine in our body. I am not sure because just like your creatinine of 1.43 is not real, your creatinine of 0.93 is not real. These are one dimensional values that may not be true for the long term.
However, if you creatinine is remains at this level of 0.93 over many months, say at least 6 months, with your regular lifestyle, which should be healthy, then we can say with some confidence that you have no CKD. Then, I am not sure because as a nephrologist we go through some basic work up to make sure that you dont have any risk factor for kidney disease, and I am not going into details of that on social media.
Lastly, google is good to increase your understanding, but it has not become intelligent enough to take care of your health and disease. Neither you can question it if something goes wrong. So, do use google to educate yourself but not to diagnose yourself. It may tell you that your protein would decrease with low protein diet, but it is not going to tell you what kind of nutritional deficiencies you would run into if you drop your protein too much.
