Case study clinical example: First session with a client with symptoms of social anxiety (CBT model)

  • Case study example for use in teaching, aiming to demonstrate some of the triggers, thoughts, feelings and responses linked with problematic social anxiety. This section here represents the first 12 minutes of an initial therapy session, but in order to try and outline the vicious cycle, it moves faster than an average session might. The video features an actor (Malak El-Gonemy) playing the character of Hannah, but the dialogue is not scripted, and as such represents a natural therapeutic exchange. I myself am a qualified Clinical Psychologist, but would definitely not suggest that this is a perfect example of therapy!
This video was produced as a collaboration between the University of Leeds and Leeds Trinity University (Ricardo Barker).

The fact that she moved out despite her anxiety, is taking photo jobs and intuitively tried to ground herself by holding tightly onto her camera, and even came to therapy, shows a lot of strengths I wish the therapist reflected back to her. Those are behaviors that show she is so much more than her anxiety and her hearing that would be the beginning of possibly thinking differently about herself and feeling more confidant.

That's a big thing I had to learn. We label ourselves as awkward and undesirable, but it's Distorted thinking. We are not anxiety. It's as if my symptoms lessened, a different personality came out 🤷‍♂️

I used to have anxiety about people seeing my anxiety. The worse the anxiety got the more I froze up. But once i thought myself to simply not give a single shit about what people think, even if I was visibly nervous, why should I care what anyone thinks? I could by sweating and and shaking but I still don't care what they think. There's a root to why we feel social anxieties, it doesn't really come out of nowhere. Sometimes it's just us placing ourselves in other people's minds too much.

Send them a link to this video and tell them to read the comment section. They will get the point and approach it from a removed perspective rather than going into an automatic defense mode. If they can reflect by themselves rather than react while in the moment of conversation there is a much higher chance they will empathize and understand. Your parent’s LOVE YOU, but its very hard to face how many possible ways, as parents, we messed up. Remember, parenting doesn’t come with instruction manuals and regardless of what we think we know, we’re all effected by our “mental baggage”. And if they freak out, remember their not freaking out at you. They are freaking out at the prospect they inflicted damage to the thing they love most (whether they show it or not). If other approaches don’t Work, Try approaching them with sympathy, empathy, and forgiveness so that you don’t fear their response. It will remove the inherent conflict, give you the perspective to be more fragile with the words you choose, and disarm your reaction to their emotional response. To understand, is not only to forgive, but to remove the need for forgiveness. Kind of like “Judge them not, they know not what they do.” … Bc chances are, they really don’t.. Good luck and remember, your mental health is worth fighting for. There is value to be gained from all obstacles. Find a way

      here's several ideas worth trying
Work out why you are shy - the first step in solving an issue is to understand why you have it.
Be comfortable with yourself - this makes it less difficult when you are with others.
Ju?t do it - you will gain confidence by doing things - especially by doing activities which stretch your boundaries.
(I read these and more tips on sebs shyness tactic site )

I went home and I start blaming myself for unable to control myself in a social situation. The problem is whenever i go to therapist, I always try to assure her I'm getting better when its actually not true.

I have/had the same problem, mindful focusing helped me a lot. I haven't gone to therapy i'm just realizing now that I have moderate social anxiety. But when I focus on my surroundings it's more difficult to fixate on the negative thoughts (like omg this is awkward think of what to say next), and because I become more relaxed my mind just naturally thinks of something to ask/say. It's so strange but I always thought I was awkward/boring and had a bad personality but I'm starting to realize that constantly worrying about what to say next has repressed my personality because conversations flow well when you're relaxed and focusing on what they're saying not on what to say next. It's amazing how this little change has made such a huge difference. Not only am I starting to realize I'm not a terrible conversationalist, even if there are lulls in the conversation I feel at ease when they happen I don't feel that constant tension and pressure not to appear awkward. I'm also actually enjoying conversations with people which I really haven't in quiet a long time. I would suggest looking into it, it could help you as well.

According to the fact that this was her first session she did a pretty good job. She seems a little awkward and pretty nervous but at the same time this makes her very likable, because we all have our human flaws and insecurities. All those assumptions how we are being perceived by other people are mostly not true but they exist in our mind and we believe in them, which is by far the worst thing, because this holds us back from becoming and expressing our true self.

I didnt even consider makeup until I was 17 because I didnt know the first thing about it, and then I found out a skintone-(foundation? tbh, i kinda still dont know whether I like or dislike this video. Bc, hannah was really emotional, honest and clear. She showed affects. Yet, the therapist made her kinda repeat things later on, and (imo: it was simply too often straightforward asking), somewhat unnecessary. I missed active and reflective listening. I'd try to get a connection with hannah in the first place, by e.g. trying to really let emotions sink in first - for both of us. I mean you could see her tremble. So, watching a psychologist staring at her at these moments, and asking more and more details (only)... - this made me feel kinda awkward. U know? Nevertheless, I am curious to hear more about the continuation of that specific event; bc the cbt techniques were at least very accurate ;) seeya.

I used to have social anxiety. I watched until 1:32 and thought: That's way more than I could ever talk. I'd literally be giving one word/weird answers and shaking. Years on...I have better confidence than many.

Yes, SA is strange. In that you can be in 2 places at one time when in an exposure situation. Not only are you in your own body feeling the physical sensations and racing thoughts , but also it's as if you are able to stand outside yourself and see what other people can see when they watch you. It would have been a better experiment here if the therapist had brought a total stranger into the room to play the role of the customer. The therapist is known to the ( pretend) client and has developed a bond with her, which would have the effect of relaxing the client. A stranger would lead to a more realistic encounter.

Self-Consciousness is something that all people experience throughout their lives. Some learn how to deal with it, and some allow society to tell them how to deal with it. Unfortunately, society also tells you who and what you should be. Societal norms are the enemy of the self.

Even though you wrote this in May, I hope it helps you, and I hope you found a therapist that "clicks" (matches). Therapy is rough at first. It is emotionally exhausting. But it gets easier and you also learn to cope under high anxiety.
Wish the best for you, hope you are well, and staying well :)

This is how I act around people I don't know. My palms got sweaty from this. Jesus. I got social anxiety (undiagnosed due to parents unable to find a "reliable" psychologist outside of school hours and me needing to focus on studies) from an extremely controlling friend from year 2 to 8 so I didn't get the proper social skills. It's gotten so bad my physics teacher is sending an email to my parents to recommend me just taking a day off school to see a psychologist (one day off school over hours of lost work in his words) so I can go to olc (optimal learning center) and get exceptions to listen to music in tests (my body constantly tries to find a noise to block out and so during silence my body can't find it so I go into a state of panic, having quiet music gives me something to block out but not loud enough to distract me)

I like to turn around what a socially anxious person is saying about others.
Client: I just feel like people are staring at me and thinking I am weird.
Me: So if you saw someone behaving like you do, you'd stare at them and think they are weird?
Client: No, I don't think so.
Me: Why would people do that to you, then?
Client: Because people are always judging others.
Me: Are you always judging others?
Client: No, I just mind my own business.
Me: Why do you think others aren't doing the same thing?
Or If the client is absolutely convinced others are judging him/her, I ask so what?
Me: So people are thinking you are weird because you act anxious around them. How does that affect you?
Client: Well, it makes me more scared!
Me: Really? Are they doing anything to threaten you?
Client: No, just thinking I'm weird.
Me: Why does their opinion matter?
Client: Well, I want people to like me.
Me: Is it possible for everybody to like you?
    Client: Well, no.
Me: Right. No matter what you do or say, someone out there will not think very highly of you for any number of reasons, some of which might not be under your control.

Hi Judith great insight into what takes place. I would like to know I think I have SAD I think its abbreviated too, would I see a therapist or a councillor for this. I have been out of work as of late and I find it hard to talk to people as it never mind an interview its embarrassing. Its only certain type of people I am just not sure why.the funny bit about this my uncle and ant are practicing Psychologists but I don't tell my family anything as I don't like it all I want to do is get rid of going red and having hot flushes, the rest I can put up with. Another question how does this differ from AVPD?Any tips?Mat

Aw too bad it was obvious at the first 3 seconds it's an actor. Sure, it's in the description, so no wonder, just saying. It's a bit of a stereotype, it's not necessarily general nervousness like she tries to play out. But then again the point of the video is to show what a session looks like, so all good. I'll just search for an actual session if it exists.
