His addiction issues haven’t helped. Be it illicit substances or prescription abuse, both can cause significant cognitive decline with long term abuse.
We know he promoted the use of amphetamines and other stimulants by administration staff during his regime. The DOD report was damming and really sheds light on why his administration was so dysfunctional and erratic.
His abuse has caught up with him. He may not make it to November without a significant break from reality.

Severe stress from multiple directions(lawsuits, court appearances, money issues, political campaining, etc) and probable sleep deprivation can really be a problem in terms of maintaining cognitive function.
